Phyllis Tuckwell is looking to appoint a full time Rehabilitation Assistant. The role supports high quality rehabilitative palliative care for patients of Phyllis Tuckwell Hospice.
The successful applicant will be an integral member of our community team and wider multi-disciplinary team. They will work across our Phyllis Tuckwell sites in outpatient and group settings as well as in the community, and work with patients living with advanced disease or a terminal illness, their carers and their family. The successful applicant will use a range of interventions to provide high quality rehabilitative palliative care, enabling a patient to develop the skills, knowledge and confidence to actively manage their condition. The role will empower patients to maximise their ability to engage in activities that are meaningful to them, promoting quality of life according to their preferences, needs and choices.

About You:
A successful Rehabilitation Assistant will have:
- A level 3 qualification in Health or Social Care of equivalent.
- An understanding of palliative care.
- Be committed to person centred, high-quality, rehabilitative palliative care for patients, their relatives, and carers.
- Experience in and be committed to working as part of an effective multi-disciplinary team.
- Well developed communication skills.
- Good information technology skills across a range of platforms.
- Be self-aware and resilient.
- Have the skills required to work with groups of patients and carers.

Post: Community Rehabilitation Assistant
Accountable to: Living Well and Therapies Manager
Job Purpose:
- To support high quality rehabilitative palliative care for patients referred to Phyllis Tuckwell, as an integral member of our multi-disciplinary team on the in-patient unit, on our sites and in the community.
- To work with patients living with advanced disease or a terminal illness, with their carers and their family. The role will involve working in the community, one to one and with groups of people using a range of interventions to enable a patient to develop the skills, knowledge and confidence to actively manage their condition and maximise their ability to engage in activities that are meaningful to them. This will empower patients to live fully and enjoy the best quality of life possible according to their preferences, needs and choices.
Responsibilities
1.0 Clinical
- To work to support patients, carers and families as individuals or in a group with others for peer support, to support the development of their skill, knowledge and confidence in managing their condition and to help to maintain functional ability.
- To assist colleagues to assess a patients functional ability and establish what is important to them and their goals.
- To support a rehabilitative palliative care approach.
- To manage own caseload of patients as delegated by the Living Well team lead in consultation with therapy colleagues.
- To motivate patients to be as independent as possible and support their ability to engage in what is important to them including personal care, movement, domestic, social and leisure activities.
- To be responsible for reporting any changes in the patients condition, treatment plan or progress to members of the multi-disciplinary team.
- To understand the physical symptoms, emotional and the psychological, social, spiritual and cultural needs of the patient and utilise these to support improvements.
- Following training and supervision, assess and teach patients to use mobility aids and minor pieces of equipment as required.
- To develop knowledge in falls prevention.
- To support a patients autonomy by using a range of solutions including information, assistive technology and skills in motivating and facilitating behaviour change.
- To know own limitations and seek advice from the appropriate professional when required.
- To be able to work alone in the community whilst understanding own competency levels and scope of practice.
- To ensure equipment is clean, well maintained and stored appropriately reporting faults to appropriate colleagues.
- To liaise with the multi-professional team including attendance at MDT meeting as required.
- To read and contribute to patient clinical records to effectively manage delegated caseload.
2.0 Administrative
- To support team working by answering telephone calls, making appointments and room bookings, scheduling, filing, taking and relaying messages, photocopying and printing.
- To monitor stock and ordering supplies as required.
- To clean and maintain equipment and maintain infection control standards in the working environment.
- To have good IT skill across a range of platforms.
3.0 Communication
- To communicate effectively with patients and their families and carers with compassion and by utilising skills that help to motivate and empower.
- To utilise effective communication skills to work with a range of people living with communication difficulties including hearing, sight and speech and language impairment.
- To gain consent and act in accordance with a patients choices.
- To work within the multi-disciplinary integrated team communicating appropriately at team meetings.
- To maintain accurate and up to date patient notes in line with professional standards and Phyllis Tuckwell policy.
4.0 Professional
- To work within established standards and policies to deliver high quality patient care.
- To be responsible for ensuring a safe working environment in all care settings.
- To participate in surveys and audits as directed by your line manager.
- To be responsible for time management and organisation of daily workload and schedules as allocated by senior member of staff.
- To maintain confidentiality according to the Caldicott principles.
5.0 Training and Development
- To participate in statutory and mandatory training.
- To participate in clinical supervision, one to ones, appraisals / performance review and look for opportunities for self and professional development ensuring skills are developing.
- To contribute to service planning.
- To support HCA colleagues to develop competencies in falls prevention, provision of walking aids and in engaging patients in and providing opportunities for meaningful activities.

We have vacancies for weekday Housekeeping Assistants to join our busy, friendly and supportive housekeeping team, as we look towards growing our team in our new hospice in Farnham.
Our Housekeepers are an essential part of Phyllis Tuckwell, not only for housekeeping but for also maintaining a positive and friendly atmosphere for our patients and their families. Whilst previous experience of similar work in an in-patient environment is preferred, this is not essential, and in-house training will be provided for the successful candidate.

Phyllis Tuckwell has vacancies for bank housekeeping assistants to join our busy housekeeping team, on our in-patient unit, which is based in Farnham, Surrey, but also with the flexibility to cover our Guildford site when needed. Our housekeepers are an essential part of our team, not only for housekeeping, but also maintaining a positive and friendly atmosphere for our patients and their families. Previous experience of similar work in an in-patient unit environment preferred, but not essential. The role provides in-house training for the successful candidate.

Phyllis Tuckwell is looking to appoint a Bank Clinical Administrator within our friendly and supportive Clinical Administration team, based at our sites in Farnham and Guildford. We work between 08:00 and 18:00 Monday to Friday and will require flexible cover during these periods. We are also required to work occasional weekends and public holidays 10:00-14:00.
The role provides administrative support to our clinical teams and liaises with external healthcare professionals, as well as patients and their families.

We have vacancies for evening Housekeeping Assistants to join our busy, friendly and supportive housekeeping team.
The In-Patient Unit vacancy is patient and family focused and consists of washing up patients' crockery, replenishing patient water jugs, serving teas to patients, replacing stock items and cleaning and emptying bins in the family room.
The Office & Clinical Cleans vacancy consists of cleaning the inpatient corridor floors, therapy rooms, toilets, staff canteen and emptying offices bins.

Our Housekeepers are an essential part of Phyllis Tuckwell, not only for housekeeping but for also maintaining a positive and friendly atmosphere for our patients and their families. Whilst previous experience of similar work is preferred, this is not essential, and in-house training will be provided for the successful candidate.

- Identifying and ensuring key internal EDI performance indicators are produced internally

Accountable to: Team Lead, or designated other lead
Job Purpose and Summary:
To work as a proactive member of the Phyllis Tuckwell (PT) multidisciplinary community team. The postholder will maintain the highest standards of nursing care with patients, their families and carers in the community setting. This role will involve providing holistic assessment, identification of patient needs, care planning and care provision jointly with other multidisciplinary team members. They will work alongside all community partners and with the Phyllis Tuckwell multi-professional team to ensure that the patients experience is seamless.
The role requires a commitment to enabling choice and providing respectful and dignified support for people as they approach the end of life and to their families and carers. The postholder will contribute to a safe, effective, and efficient culture by participating in risk assessment and by working in accordance with Phyllis Tuckwells (PT) policies and procedures.
Opportunities exist to rotate from Hospice at Home onto the In-Patient Unit to further develop your knowledge and skills in palliative and end of life care and enhance awareness of the patient and family experience of end-of-life care.
Responsibilities
1.0 Clinical
1.1 To assess, plan, co-ordinate, implement and evaluate palliative care interventions based on an agreed plan of care that supports quality and timely care delivered to the patient and family or carer. Assessing the patients changing needs and liaising with other PT specialists or external partners.
1.2 To ensure that care is centred around the person, family and their carer and that effectiveness, clinical safety and a joint approach with community nursing colleagues underpins PT practice.
1.3 To work closely with PT colleagues, the District Nursing teams and other partners in the provision of personalised holistic care to patients, their families, and carers.
1.4 To ensure quality standards are maintained and constantly re-evaluated, in order to provide the highest standards of holistic care.
1.5 To ensure that clear and concise reports of clinical and other activity are maintained and monitors record keeping standards in accordance with NMC guidelines, community and PT policy.

Post: Hospice at Home Registered Nurse
Accountable to: Team Lead, or designated other lead
Job Purpose and Summary:
To work as a proactive member of the Phyllis Tuckwell (PT) multidisciplinary community team. The postholder will maintain the highest standards of nursing care with patients, their families and carers in the community setting. This role will involve providing holistic assessment, identification of patient needs, care planning and care provision jointly with other multidisciplinary team members. They will work alongside all community partners and with the Phyllis Tuckwell multi-professional team to ensure that the patients experience is seamless.
The role requires a commitment to enabling choice and providing respectful and dignified support for people as they approach the end of life and to their families and carers. The postholder will contribute to a safe, effective, and efficient culture by participating in risk assessment and by working in accordance with Phyllis Tuckwells (PT) policies and procedures.
Opportunities exist to rotate from Hospice at Home onto the In-Patient Unit to further develop your knowledge and skills in palliative and end of life care and enhance awareness of the patient and family experience of end-of-life care.
Responsibilities
1.0 Clinical
1.1 To assess, plan, co-ordinate, implement and evaluate palliative care interventions based on an agreed plan of care that supports quality and timely care delivered to the patient and family or carer. Assessing the patients changing needs and liaising with other PT specialists or external partners.
1.2 To ensure that care is centred around the person, family and their carer and that effectiveness, clinical safety and a joint approach with community nursing colleagues underpins PT practice.
1.3 To work closely with PT colleagues, the District Nursing teams and other partners in the provision of personalised holistic care to patients, their families, and carers.
1.4 To ensure quality standards are maintained and constantly re-evaluated, in order to provide the highest standards of holistic care.
1.5 To ensure that clear and concise reports of clinical and other activity are maintained and monitors record keeping standards in accordance with NMC guidelines, community and PT policy.
1.6 To be able to demonstrate an understanding of issues relating to the ethical aspects of palliative care and their application in the workplace.
1.7 To act as a resource and support to palliative care colleagues through giving advice, information, support, and education.
1.8 To record clinical activity as required by the organisation and practice with due regard to confidentiality and data protection.
