Our Integrated Neighbourhood Teams are made up of nurses, therapists, paramedics and support workers who work in an integrated way to support service users on their healthcare journey. We would love for you to come along for the ride.
As a Community Nurse you would be welcomed into one of our busy nursing teams to support service users who have nursing needs in their own homes.
Main duties of the job
As a Nurse in one of Sirona's INTs, you will work within a multi-disciplinary team to provide holistic and specialist skills supported by a robust competency framework.
You will have the opportunity to develop additional competencies within the requirements of the multi-disciplinary approach to the INT.
Due to the diversity of need in our localities, this is a brilliant opportunity to develop your skills through training and in-team support in a range of nursing interventions. Working in a community setting gives you the unique ability to really get to know your caseload and tailor your interventions in a holistic and person-centred way. This could be working with them to manage their health needs or supporting them through education and health promotion.

Job summary
The Wound Care Service has an exciting opportunity for a Band 6 Tissue Viability Nurse to join their committed and passionate team. This is a varied role providing expert assessment for patients with complex wounds in a variety of settings and via remote triage.
The service also supports the Integrated Network Teams (INT) in becoming confident in wound care and lower limb management by education, caseload reviews and a supportive in-reach model. The overarching aim is to enable patients to receive best practice care in a timely manner and improve outcomes for them. The role also plays a key part in the prevention of pressure injuries across the organisation.
The service currently runs Monday to Friday 0800-1700. This role is for 30 hour a week.
Main duties of the job
- Work as part of the wound care service providing an efficient and effective service to patients and colleagues within the Sirona care and health community.
- Provide specialist tissue viability assessment for patients with complex wounds in a variety of settings and via a rota of remote triage and develop appropriate treatment plans.
- Contribute to development and review of evidenced based tissue viability guidelines, protocols and pathways and contribute to embedding these in practice.
- Contribute to the pressure ulcer prevention strategy across Sirona in working to embed Sirona protocols in best practice and reduce pressure ulcer incidence.Provide accurate verification of pressure ulcers.
- Provide tissue viability education in a variety of ways to clinicians within the Integrated Network Teams (INTs).

Job summary
An exciting opportunity has arisen within Pathway 3 for a motivated and compassionate Rehabilitation Support Worker with an interest in supporting the assessment of individuals following hospital discharge.
The successful candidate will be confident working independently as well as within a team. You will possess excellent communication skills, demonstrate initiative and contribute to ensuring service users and their families receive a positive experience.
You will work alongside Physiotherapists to assess service users who have been discharged from hospital into assessment beds within local nursing homes. You will assist with gathering information, supporting prescribed activities and reporting relevant findings.
The role offers an opportunity to work closely with individuals during their assessment journey. You will help clinicians build a comprehensive picture of a person's needs and potential support requirements, contributing to informed decisions about future care and discharge planning.
You will be part of a friendly and supportive team committed to delivering high-quality, person-centred care. Regular training, supervision and opportunities for professional development will be provided to help you develop your knowledge and skills.
The post is based within nursing homes throughout South Glos and requires flexibility to work across multiple sites.

Main duties of the job
Promote optimum independence through encouraging rehabilitation techniques.
As a rehab support worker you will be a key figure in completing the day to day therapy input with patients under the instruction of the lead therapists. You will be required to provide feedback on the service users' progress and help to ensure that the service users achieve their maximum potential.
Undertake a range of delegated clinical and therapy led activities to support the service users on the caseload.
To provide safe, patient centred, effective and evidence-based clinical activities.
The post holder will work under direct supervision and be responsible to senior therapists providing one to one and group rehabilitation.
You will be part of an integrated multi-disciplinary team (MDT), working with partner organisations which includes nurses, physiotherapist, occupational therapists and Social Workers.
The Service operational Hours are between 08:00 and 18:00 Monday- Friday (flexible working can be discussed) with future possibility for weekend cover.

We are seeking an experienced Therapy Clinical and Operational Lead to join the new Burren Intermediate Care Beds Service.
The Burren is a newly commissioned service, currently funded for one year. This is a unique opportunity to play a central role in establishing the therapy service, developing the team and embedding effective rehabilitation pathways from the outset.
Working as part of the service leadership team, you will provide professional, clinical and operational leadership for the therapy workforce. You will help shape a high-quality, responsive service that supports rehabilitation, recovery, safe discharge and effective patient flow across BNSSG.
The Burren Intermediate Care Beds Service provides a dedicated step-down pathway for people who no longer require acute hospital care but continue to need rehabilitation, recovery support, assessment and multidisciplinary discharge planning.
The service comprises 32 intermediate care beds, alongside an additional bariatric pathway. It works closely with acute hospitals, community services, local authority partners and voluntary sector organisations to promote independence, reduce unnecessary hospital stays and support safe and timely discharge. The service operates seven days a week, with an initial aim of providing therapy input six days a week.

Join our dynamic Musculoskeletal Physiotherapy Service and play a key role in delivering high-quality outpatient care across Bristol, North Somerset and South Gloucestershire. This is an exciting opportunity to build on your existing experience while developing advanced clinical skills within a supportive and experienced multidisciplinary team. Working as an autonomous practitioner, you will assess, diagnose and deliver evidence-based treatment plans for patients with varied and complex presentations, with access to support and specialist advice from the wider MSK and Interface teams when needed.
Our MSK service is part of the specialist services delivered under the community NHS contract and works closely with GPs, consultants and other healthcare professionals across primary care. Care is delivered through face-to-face clinics as well as telephone and video consultations.
Core service hours are Monday to Friday, 8am-6pm (excluding Bank Holidays).
This role is based across BNSSG, with current vacancies in South Gloucestershire, Bristol and North Somerset.
Main duties of the job
- You will be highly competent in assessing a wide range of MSK conditions based on comprehensive theoretical knowledge of anatomy, physiology, and pathology.
- You will need to recognise potentially sinister signs and symptoms and contra-indications to treatment and manage these appropriately. This may involve referring on for further investigations.
- Carry out a skilled physiotherapy intervention drawing upon wide ranging evidence base and problem-solving professional knowledge and skills, including specific techniques and tests, gained through specific training and practical experience.
- To evaluate relevant information regarding all aspects of patient care (e.g. test results, assessments from other health professionals, along with the results of own assessment) to develop a clinical diagnosis, prognosis and expected response to treatment
- The post may involve assessing and treating paediatric patients, depending on post location, but appropriate training and support will be provided to enable the candidate to deliver this.
- The post holder will supervise junior physiotherapist, students and physiotherapy assistants and provide education and training to groups of staff and colleagues as required.
- To keep accurate and comprehensive written records, to include reports to medical referrers, adhering to Physiotherapy department and Chartered Society of Physiotherapy standards to comply with professional and legal accountability.

Job summary
Elton Rehabilitation Unit is a 8 bedded facility that is located in Clevedon, North Somerset. The unit is one of a number of community units providing support and active rehabilitation for those people being supported on Discharge to Assess pathway 2 (please see below).
Following a stay in the acute setting, the ethos for all people being discharged is 'home is best' and a large number of people are supported to return home utilising the robust and established ~Discharge to Assess Pathway 1. For those people that are identified as having on-going rehabilitation needs, they are considered to 'stepdown' into a Pathway 2 Rehabilitation, Reablement and Recovery bed at the Elton Rehabilitation Unit.
On the Unit, they will receive therapy intervention and support around complex discharge planning, to enable a timely and safe discharge to their usual residence or to a more suitable location if appropriate.We actively promote a multi-disciplinary model on the Unit, where nursing staff work closely with therapist, social workers, GP's with Special Interest in Older People's Care as well as other Acute and Community colleagues, to ensure the person we are supporting is kept at the heart of every decision
Main duties of the job
The Health Care Assistant role compliments and forms a key part of the multi-disciplinary healthcare services within the organisation, implementing and providing high standards of individualised care for the service user. You will be part of an integrated multi-disciplinary team (MDT), which includes nurses, occupational therapists, physiotherapists and other allied healthcare professionals.o You will provide holistic, person-centred care and support for service users of all ages in a variety of settings. Main duties of the job

Management of complex dysphagia, including triage, prioritisation, assessment, risk assessment and the development of individualised care plans as well as care and support staff education to support understanding and implementation of guidance given.
Your work will be in the community seeing people in their homes or other familiar and appropriate environments. You will also be working collaboratively within our multi-disciplinary team which provides a holistic approach to meet the service users health needs. You will receive support from clinical specialist SLTs and the SLT Clinical lead to develop in this role We actively encourage skill development and case discussion to ensure the team deliver a high-quality service that is innovative, safe and continuously improving for a positive and meaningful service user experience.

Job summary
This role is to provide leadership to the ICSS team and clinical specialism in Speech and Language Therapy. It involves working with other team leads in the MDT to support the day to day running of the service, monitor outcomes and performance and to supervise junior colleagues. You would also provide Specialist intervention to people who have communication and/or swallowing difficulties after stroke in their homes or local community.
The role is based in North Somerset but involves working with others to look at Speech Therapy provision and the running of the service more widely across Bristol, North Somerset and South Gloucestershire along with the service lead.
Main duties of the job
- Provide clinical intervention for Speech and Language therapy as part of an MDT including those with complex needs
- Provide leadership, clinical support and supervision to motivate team members and develop effective team working and personal development.
- Triage referrals & be a key point of contact for the community stroke service
- Involvement in performance reporting and workforce management
- Providing advice and support in specialist field for the MDT and other community teams
- Support the service lead in developing policies and processes and auditing performance
- Responsible for the principles of clinical governance, health and safety, governance, health and safety,
- Responsibility for HR processes such as performance management, recruitment, complaints and incident reporting and incident reporting with direct reports
