Job summary
Substance misuse prescribing nurse, providing access to medications that both manage withdrawal from substances of abuse and offer Detox in clinical setting or domiciliary setting.
Encourage Service Users to work towards recovery whilst maintaining stability.
Promote engagement with treatment plans and offer access to treatment promoting an every contact counts approach.
Provide management support for lower grades, supervision and appraisal.
Encourage long term condition compliance.

We are seeking an experienced and motivated Practitioner to join the Sussex Health & Justice Liaison & Diversion Service.Working within Police Custody settings across Sussex, you will assess adults, children, and young people who may be experiencing mental health difficulties, learning disabilities, substance misuse, or other social vulnerabilities. Your interventions will help ensure individuals are identified early, connected with the right services, and supported to achieve better outcomes.This is a varied and rewarding role where no two days are the same. You'll work closely with police, courts, healthcare professionals, and community partners, using your clinical expertise to influence decision-making and ensure people receive the right care at the right time.We're looking for an experienced, compassionate practitioner with excellent assessment and risk management skills, a collaborative approach, and a passion for improving lives.Please note you will be required to meet the travel requirements of the community role.
Main duties of the job
o Deliver high-quality assessments for adults, children and young people in Police Custody settings across Sussex.o Identify mental health, learning disability, substance misuse, physical health and social vulnerabilities at the earliest opportunity.o Develop and implement person-centred care and support plans to meet complex needs.o Make timely referrals to appropriate health, social care and community services.o Work closely with police, courts and criminal justice partners to support informed decision-making.o Provide accurate information regarding an individual's health needs and vulnerabilities to relevant agencies.o Use a trauma-informed approach to minimise distress and avoid individuals having to repeatedly share their experiences.o Manage complex cases autonomously, using advanced assessment and risk management skills.o Provide consultation, advice and clinical supervision to colleagues and partner professionals where appropriate.o Promote positive outcomes by helping vulnerable individuals access the right support at the right time.

Main duties of the job
The Speech & Language Therapist will work as a part of a multi-disciplinary team, to meet the needs of patients referred to Mildmay Hospital, ensuring the highest possible standards of practice.
To provide a highly specialist Speech & Language Therapist input (assessment, advice and treatment) to clients referred to the team.
To provide clinical leadership to SLT students, and any junior/senior SLT posts.

We're looking for a motivated and compassionate person with lived experience of the criminal justice system to join our Liaison and Diversion team part-time. You'll play an important role in supporting people as they move through police custody, court and prison, helping them access the support they need to improve their health, wellbeing and future.
You'll use your lived experience to manage a caseload of people using our service, supporting them for up to 12 weeks for 'Liaison and Diversion' cases and up to 6 months for 'Reconnect' cases.
You'll work alongside each person to understand what's important to them and help them access services that meet their needs. This may include support with; Mental health, Learning disabilities, Neurodiversity, Housing, Social isolation, Education and training, Drug and alcohol support and Employment.
Building positive relationships is a key part of the role. You'll work closely with service users, their families, police, courts and a range of partner organisations to ensure people receive joined-up support.
Main duties of the job
- To hold a caseload of service users for up to 12 weeks (for Liaison & Diversion cases) and up to 6 months (for Reconnect cases) to support them to access a wide range of agencies and services that can provide help and advice.
- Tofacilitateengagement withappropriate services, byidentifyingunmet needs including, but not limited to, mental health, learning disability, ASD, ADHD, housing, social isolation and exclusion, education, debt management, drug and alcohol, and employment problems.
- Support service user who are typically hardfor services toengagewith, to access and utilise community servicesin order tomeet identified health and social needs.
- Build on professional and supportive relationships, including family, police, court staff, and other partner agencies etc.
Working pattern and service operating hours:
This role is based on Monday to Thursday, 9:00am to 5:00pm.
Our wider Liaison and Diversion service operates across police custody suites between 7:00am and 8:00pm, seven days a week, and in courts between 9:00am and 5:00pm, Monday to Friday.
You'll work across sites throughout Hampshire, so you'll need to be able to travel. Please note that some locations are not easily accessible by public transport.

Main duties of the job

If you don't already have a Level 3 qualification or equivalent experience in a relevant subject, RDaSH are offering this post alongside the opportunity for you to complete a Level 3apprenticeship qualification [relevant to your role].

Main duties of the job
The post holder will support patients, carers and families to share their experiences of care and ensure their voices contribute to service improvement across the Trust.

- Supporting the delivery of the Patient Advice and Liaison Service (PALS) function alongside the Patient Safety, Carer & Community Team

SCOPE AND RANGE
North Lincolnshire CAMHS forms part of the Children's Care Group. The Trust provides a comprehensive Adult, Child and Older Adult Mental Health Service to Rotherham, Doncaster and North Lincolnshire.
The post holder will be working within a large multidisciplinary team of doctors, nurses, social workers, psychologists, psychotherapists and creative therapists. The service receives referrals for young people up to the age of 18 from General Practitioners, Social Workers, Education and Paediatricians.
The post will be working in both generic CAMHS and ADHD.
Most of the clinical work will take place at the central clinical base. However, the post holder will be required to undertake assessments elsewhere, for example, Paediatric Wards, A &E, schools, GP surgeries, occasional home visits. Expectation is that 60% or more of work time is spent onsite.
The postholder will cross cover for non-Consultant doctors for annual leave etc.
The postholder will also contribute to writing repeat prescriptions and checking blood results.
There is a dedicated ADHD Team, led by an ADHD Specialist Prescribing Nurse. You may spend some time each week supporting the ADHD post diagnostic nursing team in prescribing and monitoring ADHD medication.
The postholder will attend one of the weekly Clinical MDTs, chaired by the Consultant Psychiatrists.
Main duties of the job
Principal Duties of the post-holder
o The post holder will be expected to provide appropriate care to patients within the North Lincolnshire area mainly through working closely with the multidisciplinary team through the assessment and treatment of appropriate patients.
o Provide consultation on individual cases where psychiatric opinion is needed to members of the multidisciplinary CAMHS team.
o Attend a weekly CAMHS MDT Clinical Meeting, in which the above patients are discussed.
o Attend multiagency meetings and Team around the Child etc as required.
o Provide first line psychiatric cover for urgent cases
o Provide medical support to the Nurse-led ADHD team. This will include prescribing and monitoring ADHD medication, alongside the ADHD nurses.
o Patient contact will take place mainly in outpatient clinics but may also occur in other community settings including A&E or paediatric wards. Appointments may also occur remotely via MS Teams or sometimes by telephone.
o Provide on-going monitoring and pharmacological treatments in ADHD, depression, OCD, etc.
o All practice will be supervised by a senior doctor or a specialist prescriber.

The Integrated Mental Health teams work as part of a 'neighbourhood' offering place-based provision of mental health services in partnership with other local organisations. Clinicians within the IMHT will offer interventions and treatments which are concordant with NICE guidelines, and allocation within the team will be based on the needs of the service user, matching clinician skills to service user needs.
Main duties of the job
A key feature of this post is to promote individual person-centred recovery by drawing on the peer support workers own personal lived experience of mental health challenges and of accessing mental health services.
As a pivotal and highly valued member of the team, peer support workers will have a case load of identified patients who they will work with on both an individual basis and by delivering group interventions, working in collaboration with the wider multi-disciplinary team.
This formalised peer support will allow the post holder to provide a safe space for individuals to talk and make sense of their experiences whilst sharing wisdom from their own recovery to inspire hope and belief that recovery is possible.
They will promote choice, self-determination and opportunities and assist the individual to set and achieve their own unique recovery goals and recognising that everyone's recovery is a distinctive and deeply personal process.
Their work may also involve providing practical assistance such as encouraging healthy lifestyles, helping individuals to build confidence, and offering information, advice and support to form connections within their local community resources.

An exciting position has arisen within our Lichfield Mental Health Support Team. We are looking for an experienced & enthusiastic Education Mental Health Practitioner.
Youwill continue to build on the fantastic work carried out within your allocated schools. The role is varied & no two days are the same, you may be setting up and delivering group work & assemblies one day to delivering assessment & interventions the next, as well assupportingschools to develop their whole school approaches to mental health & wellbeing.
You will be skilled & experienced in conducting mental health assessments of children and young people (CYP) presenting with common mental health difficulties such as anxiety and low mood. You will conduct risk assessments & identify the right level of support for CYP you come into contact with.
Alongside the clinical aspect of your role, you will play an integral part in supporting schools with their whole school approaches. There is scope for creativity and opportunities to work alongside CYP & parents/carers to drive the whole school approach agenda.
This is an opportunity to be involved in an established team within the wider CAMHS structure, as a joint initiative between health & education. You will work across the Staffordshire locality as well as supporting other MHSTs across South Staffordshire. You will work deliver parent, staff well-being, 1:1 & group work sessions.
Main duties of the job
MPFT will offer a welcome bonus of 5% of first year salary- at 50% on commencement of employment and 50% following 12 months employment.

An exciting opportunity has arisen for a Occupational Therapist to be part of the Home First service in South Staffordshire.
This is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs within Midlands Partnership University NHS Foundation Trust (MPFT).
The Home First Team prides themselves on the delivery of quality care to community patients, many with complex needs.
The team also work to facilitate safe hospital discharges for patients leaving acute care and to offer them nursing support in their own home environment.
Thisis a dynamic post which will be evaluated and adjusted to reflect the needs of the service, local population and feedback from users and staff. Your feedback and input will be essential to the development and shaping of the role.
The Home First covers South Staffordshire, Burntwood, Lichfield, Tamworth, Burton, Uttoxeter and surrounding areas, providing clinical support for patients in the community to either avoid potential hospital admissions or to support people returning home through the Discharge to Access Pathway 1 following a period of acute care.
The role will require working 5 days over 7 including Bank Holidays.
Main duties of the job
The post holder will be part of the Allied Health Professions (AHP) service providing Occupational Therapy and will work in a variety of settings.
This is a dynamic post that requires the post holder to work closely with uni/multi-disciplinary teams/agencies in addressing a range of care/support issues across the spectrum of conditions, preventing unnecessary hospital admissions and improving the health and wellbeing of the population served. We are seeking a passionate and dedicated Assistant Therapy Practitioner to join the Home First in the North of Staffordshire. You will be responsible for delivering high quality rehabilitation to patients with a range of physical health conditions including MSK, Stroke, Parkinson's disease, Neurological conditions, Spinal Injuries, Cancer, Post Surgery, General Frailty, Amputees, plus a wide range of other conditions which provides you with a varied and exciting experience within our service.
You will work jointly alongside qualified therapists with complex patients and will work autonomously within their scope of practice. Thisis a dynamic post which will be evaluated and adjusted to reflect the needs of the service, local population and feedback from users and staff. Your feedback and input will be essential to the development and shaping of the role.
You will be required to travel independently and efficiently over a large geographical and will attend planned loan visits to individual's homes.
Main duties of the job
The role involves managing a caseload of adults in close liaison with the Home First Team. Many of these patients will have multiple complex health needs. Within scope of practice this role provides the opportunity for you to undertake independent, holistic assessments with service users in their normal place of residence using manual assessment techniques, analytical and clinical reasoning skills.
TheAssistant Practitioner role supports the work of a range of different professions ensuring that the individuals' physical, psychological, and emotional needs and preferences are met.
- To carry out and progress rehabilitation programmes set by qualified therapists
- To complete secondary assessments following hospital discharge, including triage, subjective and objective assessments for appropriate patients. Within the scope of practice and competency for the role, responsible for identifying clinical need for input from MDT. To set and review SMART goals for individual patients.
- To discharge appropriate patients from the service, following appropriate pathways, completing appropriate onward referrals, completing timely discharge summaries and to signpost to community and voluntary groups.

Job summary
The post holder will work as part of a Community Nursing Team to provide nursing care without direct supervision for patients registered with GPs within Midlands Partnership NHS Foundation Trust, covering Cannock Villages PCN. Main duties of the job
The postholder will hold responsibility for a defined caseload of patients drawn from a GP practice population. The post holder's main duties will be to provide administrative support to the Directorate of Estates and Facilities management team covering a wide range of services.

The team provides holistic nursing care for pupils with a wide range of learning disabilities, physical disabilities and neurological conditions. The successful applicant will work within the Stafford Hub supporting Marshlands and Walton Hall special schools and Greenhall Nursery.
Working as a member of a friendly, highly skilled team you will have the opportunity to make a difference to the lives of children and young people who have special needs and their families. You will be responsible for their healthcare needs during the school day and work in partnership with education colleagues in the school and with parents.
