We are seeking enthusiastic and motivated professionals who hold registration with a recognised professional body (NMC, HCPC, or SWE) to join our supportive, dynamic, and multidisciplinary team.
This role is ideally suited to an experienced clinician with advanced clinical expertise, strong leadership capabilities, and a passion for delivering high-quality, person-centred care. The successful candidate will be able to inspire, support, and motivate colleagues while driving excellence in service delivery and outcomes for those accessing our services.

You will be supported to further develop your leadership abilities, expand your clinical expertise, and achieve your professional aspirations.

Main duties of the job
1.To support the service manager in the effective operation of the Frailty, Later Life and Dementia service line.2. To ensure the service operates in a safe, effective & efficient way as part of the whole Runnymede community neighbourhood team.3. To manage complex cases, providing leadership and supervision of staff and ensuring high quality evidence-based patient care.4. Being part of the community neighbourhood clinical leadership team -working collaboratively to ensure people's needs are met and a reduced need for repeat assessments.5. Assess patients/clients; plans, implements care; provides highly specialist advice; maintains associated records.6. May liaise with other agencies in planning programmes of care and/or health and education programmes

Job summary

We are seeking two exceptional individuals to act as Enhanced Care Team leaders, who will deliver effective and efficient operational management and clinical oversight of enhanced care within the organisation to our most vulnerable patients. The postholder will work closely with emergency department and inpatient ward staff, providing expert assessment and allocation of resources, and providing motivational leadership to a team of junior staff, role modelling a psychologically safe and trauma-informed approach.
Main duties of the job
The enhanced care team leader will be responsible for working within the set budget, clinical governance, and staff development and training. The post holder will be expected to provide professional leadership within the clinical area and provide direction, and inspirational leadership to all members of the multi-disciplinary team working alongside the enhanced care team. The post holder will be involved in the development of protocols/guidelines and patient information.
The post holder will be responsible for ensuring that there are systems and processes for the planning, provision and review of enhanced care. The post holder will ensure that the enhanced care team deliver therapeutic engagement and interventions to patients with mental health conditions, cognitive impairment or those experiencing psychological distress whilst working in collaboration with internal and external stakeholders such as the Liaison Psychiatry Service.
They will provide regular reports on the effectiveness of the team in delivering quality care by reducing restrictive practices and incidents of harm.

Main duties of the job

We are delighted to introduce occupational therapy across our all age Home Treatment Teams as part of the service delivery model designed to enhance comunity based crisis care and improve outcomes for people over 18 years who are experiencing an mental health crisis.

We are looking for a motivated, organised and enthusiastic Team-Based Administrator to join our friendly and supportive Children's Dietetics Service. This is an exciting opportunity to play a key role in supporting dietitians and ensuring children, young people and their families receive high-quality care. Job summary
The Trust is seeking a consultant psychiatrist to join Walton CMHT based at Norris Green Hub, Falklands Approach, L11. The vacancy has arisen as a result of the previous substantive post holder retiring. This post is one of three CMHT general adult consultant psychiatrist posts (the other 2 are also 1 WTE each) within Norris Green hub.
Norris Green Hub serves a catchment area of around 120,000, covering the Mid to North areas of Liverpool for adults of working age. The current post encompasses the Walton catchment areas. This is a demographically mixed area overall, but with some areas of social deprivation and some drug and alcohol- related difficulties in an urban environment.
The post holder will carry no responsibility for inpatients.
The staff at Norris Green Hub are organised in three distinct CMHTs. This CMHT consists of:
1 whole time equivalent (WTE) consultant psychiatrist (this post)
1 WTE higher / core Psychiatric Trainee.
0.5 WTE dedicated medical secretary, Band 4 and a full time band 3 shared with the other 2 teams.
1 Band 7 team leader
5 WTE community psychiatric nurses (band 6 and band 5)
7 WTE social workers shared between the 3 teams
1 WTE senior occupational therapist, Band 6 shared between the 3 teams
1 WTE psychologist.
4 WTE support time and recovery workers, Band 3 shared between the 3 teams

We are looking for an Administrative Assistant to join our Adult Mental Health Services based in Severn Fields Health Village, Shrewsbury. The Service offers mental health care and treatment across a variety of different age ranges and presentation. The role is fixed term for 10 months and would require you to work 37.5 hours per week (Monday to Friday 9.00am to 5.00pm). You would predominantly be based in Severn Fields, although you may be required to travel to work in other Trust locations to meet service need.
In the role you will be working as part of a team along with Administrative Assistants and Medical and Clinical Administrators. You will be expected to be able to use your own initiative and judgement, although you will be well supported by other Mental Health Admin staff and through the clinical and professional networks accessible through the community teams. The administration offices are busy, and the role is challenging but it is alsorewarding.
Main duties of the job
- To accurately maintain and keep secure patient health and staff records/systems in line with policy and standard operating procedure, to ensure data quality, confidentiality and compliance with the current Data Protection Law.
- Maintain strict confidentiality regarding information concerning patients, staff and sensitive Trust Business at all times.
- To carry out reception duties including meeting and greeting service users, carers, relatives and visitors, as requested by the Admin Support Supervisor and/or the Admin Team Leader.
- Handle incoming and outgoing telephone calls in a polite and efficient manner. Receive and deal with telephone enquiries referring telephone queries to a more senior colleague where appropriate. Taking accurate messages and passing them to the appropriate person in a timely manner.
- Type a range of documents including emailed requests, copy typing and audio transcription (via T-Pro digital dictation system) generated by the mental health pathway teams.
- Prioritise own workload and meet deadlines in order to meet the requirements of the post.
- Regular data entry and booking of appointments for service users on the clinical patient administration system, RiO.
- Take / transcribe minutes as required and at the request of the Admin Support Supervisor and/or Admin Team Leader.

Job summary
This team will provide an active involvement in the assessment, development, implementation and evaluation of service user care which focuses on ensuring that the service users are supported to remain in their own homes wherever hospitalisation can be safely and appropriately avoided. Our service will deliver home treatment 24/7 to an identified group of children and young people in Crisis by responding quickly to those referred with urgent and high-risk mental health needs, and provide collaborative, effective, evidence based interventions at the right time with the right person.
There will be scope for creativity in this team, to develop evidence based interventions, to think systemically and work collaboratively with children, young people and families, informed by the local Transformation Plan, Future in Mind and the THRIVE framework. It really is time to think differently and creatively about how we can help more children and young people with their emotional wellbeing and mental health. You will be supported by some very lovely people in all parts of our service and the wider partnership
If you are currently working at an AfC Band 5 position, you are encouraged to apply for this role as part of a development programme.
You must be a fully registered Nurse, Social Worker or Allied Health Care Professional with appropriate mental health experience to apply. Working 37.5 per week you will be required to cover a 24 hr rota on a shift basis.
Main duties of the job
- Provide specialist assessment and care planning in conjunction with service users and carers, evaluation of individual treatment plans and discharge planning
- Contribution to the delivery of home treatment through nursing care to an identified community client group and within a multi-disciplinary team..

Job summary
The Community Children's Nursing Specialist and Family Services team has an opportunity for a Band 6Community Children's Nurse (CCN) to their team in Fenton.
The Community Children's Nurse (CCN) will manage a defined caseload of CYP with a range of acute and complex health care needs, including life limiting and life-threatening conditions requiring palliative and terminal care.
The service aims to support families in caring for their sick children at home, in order to avoid hospital admission and/or facilitate early discharge. This is done by the assessment of children and young people's healthcare needs, and the planning, development, implementation and evaluation of programmes of care which facilitate care at home and in other community settings.
In the role you will have responsibility for the management of a defined caseload and responsibility for the team caseload including out of hours/on call when required. The team provides a 7-day week service to include 24 hours on call for children receiving palliative care.
You will work in partnership with health colleagues and other agencies to provide potentially complex packages of care.
Main duties of the job
- Plan and organise own caseload in order to meet the needs of the children and families.
- Develop, assess, implement and evaluate plans of care working in partnership with children and families
- Provide specialist knowledge and skilled nursing care to children and families in their own home or community setting
- Be able to work flexiblyand participate in on call rota to provide 24 hour cover if needed.
- Establish and maintain links with professionals and agencies involved in the child's care, possibly taking on responsibility of key worker when appropriate.
- Work in partnership with all concerned to ensure smooth transition between hospital and home to include discharge planning.
- Identify and report any cases of suspected child abuse in accordance with Staffordshire Safeguarding Children Board (SSCB) guidelines and 2004 Children Act, and attend core group and case conferences, preparing statements and reports as appropriate.
- Assist in planning and organisation of individual therapeutic programmes of care for children and young people by attending assessment, discharge planning and safeguarding meetings, ensuring reports are submitted on the child's progress if unable to attend.

Main duties of the job
The Adult Community Nursing Service provides care to patients in their own homes, covering care homes and working closely with our GP practices and integrated services such as Physio, OT and Podiatry. As a Community Staff Nurse you will deliver a high quality service, working closely with GP's and multi-disciplinary teams in addressing a range of care issues across the spectrum of health conditions, and preventing avoidable hospital admissions. You must be a qualified nursing practitioner with current NMC registration. Previous experience of working in the community is not an essential requirement, however, you do need to be able to demonstrate a commitment to delivering high quality targeted personalised care.

The role of the band 4 outreach support worker is to support vulnerable people who have entered the criminal justice system. The service has a focus on supporting adults who have been released from police custody and may have additional health needs. Main duties of the job
As a Complex Discharge Tracker, you will review all daily Home First referrals with the Locality Teams and ensure that all patients are allocated the appropriate care calls within the discharge to assess appropriate referral timescale.
As part of this role, you will be expected to:
- Ensure that all Home First referrals from the acute portals are immediately sourced and prioritised throughout the working day.
- Attend all CDT training and briefing sessions as the system evolves.
- Ensure that all updates recorded in CDT are accurate before entering them. Ensure that any identified mistakes are corrected immediately.
- Provide care/support to community patients in their own homes on a adhoc basis.

An exciting opportunity has arisen within our Pay Services Team for a Payroll Team Leader to join our friendly, supportive and customer-focused team.
Working closely with the Payroll Manager and Senior Payroll Team Leaders, you will support the delivery of an accurate and timely payroll service while helping to develop and lead a team of Payroll Assistants and Payroll Advisors.
This hybrid role combines home and office-based working, with attendance required for training, meetings and service needs. Main duties of the job
1. Lead on the coordination of patient flow across the Lewisham CAMHS community pathways, ensuring timely access and discharge planning.
2. Work collaboratively with multidisciplinary teams to identify barriers to discharge and implement solutions.
3. Support and advice teams on recording care pathways effectively on epjs and monitor recording reports.
4. Monitor caseloads, length of stay, last contacts and service capacity to support effective resource allocation and reduce waiting times.
5. Liaise with external agencies and services to facilitate smooth transitions and continuity of care.
6. Provide clinical oversight and support to practitioners managing complex discharge cases.
7. With senior colleagues, evaluate and review the therapeutic strategic outcomes for the team and set aims and objectives and the monitor practice to assure quality standards of care.
8. Establish and maintain evidence-based practice in the assessment and treatment of young people in crisis or needing intensive and/or crisis intervention.
