Are you an experienced, autonomous nurse ready for your next challenge ? Cornerstone Practice and Healthcare is looking for a Same Day Consultation Nurse to join our friendly, forward-thinking team, caring for a large and growing practice population across four sites in Blackburn and Darwen. You'll be the clinician patients see on the day they need us most - assessing, diagnosing and treating a wide range of undifferentiated presentations, by telephone and face-to-face. You'll also support our Extended Access clinics, and help the practice achieve its QOF and Enhanced Services commitments - real variety, genuine autonomy, and real impact on your local community.

Main duties of the job
You'll see patients booked into your same day sessions, by telephone or face to face, using your clinical skills to assess undifferentiated presentations and decide, there and then, how best to manage each one.
Day to day you'll examine, diagnose and manage a broad mix of acute illness, minor injury and flare-ups of long-term conditions - treating and prescribing where your NMP qualifications allows, safety-netting appropriately, and referring on where needed. You'll travel out to patients at home when a visit is the right call, and a day in our extended access clinics.
You'll take shifts in our Triage Hub too, supporting our Care Navigators with clinical advice on the right pathways for a patient, and stepping in to triage patient when needed.
You'll keep accurate, timely records and work closely with our GPs on complex or urgent cases, and use audit and reflection to keep improving your own practice and ours.

Job responsibilities
The post holder will be an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.They will demonstrate safe, clinical decision-making and critical thinking, working collaboratively with the general practice team to meet the needs of patients and support the delivery of practice policy and procedures. NMC requirements for advanced practice must be met.
In addition to the Same Day Consultation duties, the post holder will work a day within the Extended Access clinics and carry out home visits for patients where this clinically appropriate, providing the same standard of assessment, diagnosis and treatment in the patient's home environment.
The role involves cross-site working, and the post holder may be required to work across more than one practice locations as agreed with the Practice Managers and Partners.
The post holder will also support the practice in achieving its Quality and Outcomes Framework (QOF) targets and its Enhanced Services commitments, contributing to a service that delivers excellent healthcare to our patients and the wider community.
Main Duties And Responsibilities
1. Clinical Practice
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
- Clinically examine and assess patients needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Diagnose and manage both acute and some mild chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
- Where NMP is achieved, prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
- Undertake home visits for patients where clinically appropriate, carrying out history taking, clinical assessment, diagnosis, treatment and evaluations of care in the patient's home environment, as part of the Same Day Consultation.
- Support Care Navigators in the Triage Hub by providing clinical advice on the most appropriate care pathway for a patient, and directly triage patient where clinical assessment is required.

- Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
- Communicate effectively with patients who are receiving 'bad news.'
- Communicate effectively with patients and carers, recognising the need for alternative methods of communications to overcome different levels of understanding, cultural background and preferred ways of communicating.
- Maintain effective communication within the practice environment and with external stakeholders.
3. Delivering a Quality Service
- Recognise and work within own competence and professional code of conduct as regulated by the NMC.
- Produce accurate, contemporaneous and complete records of patients consultations, consistent with legislation, policies and procedures.
- Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
- Deliver care according to NSF, NICE guidelines and evidence-based care.
- Support the practice in achieving its Quality and Outcomes Framework (QOF) targets and its contracted Enhanced Services commitments, contributing to a service that delivers excellent healthcare to our patients and to the wider community.
4. Home Visits and Lone Working
- Hold a full, current UK driving license and have access to a vehicle insured for business use, in order to travel between sites and to patients' homes.
- Exercise clinical judgement and manage risk independently while working alone during home visits, without immediate on-site support.
- Work alone in the community in accordance with the Practice's lone working policy, including appropriate use of any lone-worker safety measures and check-in procedures.
5. Confidentiality and Record Keeping
- Act at all times with regards to the confidentiality and sensitivity of issues to do with patients, their care and treatment.
- Ensure all contract with patients, and the results of any test or certain actions may also require a paper record.
- Work within the recommended guidelines of the NMC Code of Conduct and Confidentiality.
6. Training and Development
- Carry out all duties in line with the protocols and guidelines in use at Cornerstone Practice and Healthcare
- Participate in Continuous Professional Development (CPD) and maintain own professional development in accordance with post-registration education and practice guidelines, recording evidence required to maintain registration.
- Produce and update a Personal Development Plan (PDP), and participate in the development of practice clinical protocols.
- Support students of any discipline undertaking a work placement at the practice.
7. Working Environment
- Regular use of VDU equipment and computer-based clinical systems, with frequent interruptions from a variety of sources.
- Travel by car between practice sites and to patients' homes, in all weathers, and exposure to variable and uncontrolled home environment (e.g. pets, smoking households, limited space) while carrying out home visits.
8. Additional Responsibilities
- This role will be adapted, from time to time, to suit the changing needs of the practice and the service. Main duties of the job
At the heart of our administration team, you will keep patient records accurate and up to date - scanning and coding clinical documents, blood results, diagnoses and other data onto our clinical system with precision and care.
You will summarise the medical records of newly registered patients, recording significant history, diagnoses, medications and allergies onto EMIS Web. You will also read incoming hospital letters and clinical correspondence, code the key information using SNOMED and pass anything that needs action to the right clinician. Previous summarising experience is welcome, but full training will be given.
You will coordinate call and recall letters for patients with long-term conditions including diabetes, heart disease and respiratory conditions, supporting our clinical team in delivering annual reviews and meeting Quality and Outcomes framework targets - ensuring no patients falls through the net.
You will also assist in preparing third-party and patient report request, processing Subject Access Request, and accurately redacting medical records before disclosure - all in line with GDPR and NHS guidelines.
This is detailed, responsible work that sits at the intersection of administration and patient safety. Every entry you make and every record you handle has a direct impact on the quality of care our patients receive.

Administrative Duties
- Scan hospital letters and related documents into patients medical records in accordance with practice protocols
- Perform accurate data input including diagnoses, blood pressure readings, spirometry results, blood results, and other coding as set out in the coding protocols

Main duties of the job
As Administration Team Leader, you will be the go- to person for our Medical Administrators - organising the team's day, sharing out the work and making sure nothing gets missed.
Your week will be a mix of leading and doing. You will plan rotas and cover, hold one-to-ones and appraisals, support new starters and help each member of the team grow. Alongside this, you will keep a close eye on our key workflows - incoming hospital letters, summarising of new patient records, call and recall for long-term conditions, and Subject Access Request - spotting backlogs early and keeping everything on track.
You will be our quality champion for patient records, checking the accuracy of coding, summarising and redaction, and reporting on how the team is performing. You will work closely with our GPs, nurses, Care Navigation Team Leaders and the Quality Managers, and be a calm, supportive point of contact when things get busy.
This is a developmental Band 3-4 post. Full training will be provided in our clinical system and in summarising, coding and records processes, with a clear path to progress as your experience and confidence grow.

Main duties of the job
As Administration Team Leader, you will be the go- to person for our Medical Administrators - organising the team's day, sharing out the work and making sure nothing gets missed.
Your week will be a mix of leading and doing. You will plan rotas and cover, hold one-to-ones and appraisals, support new starters and help each member of the team grow. Alongside this, you will keep a close eye on our key workflows - incoming hospital letters, summarising of new patient records, call and recall for long-term conditions, and Subject Access Request - spotting backlogs early and keeping everything on track.
You will be our quality champion for patient records, checking the accuracy of coding, summarising and redaction, and reporting on how the team is performing. You will work closely with our GPs, nurses, Care Navigation Team Leaders and the Quality Managers, and be a calm, supportive point of contact when things get busy.
This is a developmental Band 3-4 post. Full training will be provided in our clinical system and in summarising, coding and records processes, with a clear path to progress as your experience and confidence grow.

Job summary
Can you read a hospital letter, identify what matters and make sure the right information reaches the patient record and the right person?
The Cornerstone Practice is looking for a Clinical Coding Administrator to provide maternity cover within our clinical administration team. This is careful, focused work that directly supports safe patient care.
You will read incoming hospital letters and discharge summaries, identify diagnoses, procedures, results and follow-up actions, and record the relevant information accurately in SystmOne. You will also highlight medication changes, appointment or referral requests and anything requiring GP review, then send it to the correct person.
You must be comfortable working with detailed clinical information, following clear protocols and knowing when to stop and ask rather than guess. Previous experience of clinical coding or processing medical correspondence would be a strong advantage. Training will be provided on Cornerstone's local procedures.
This is a fixed-term maternity-cover post running until 30 June 2027. The post is for 8 hours per week. The working pattern and rate of pay are to be confirmed before publication.
We care for approximately 9,500 patients and use SystmOne. An immediate start is available.
Main duties of the job
You will process and code incoming clinical correspondence accurately and safely. Your work will include:

Would you like to use your diabetes experience to make a real difference to patients who need more than a routine annual review?
The Cornerstone Practice is looking for an experienced Diabetes Practice Nurse to join our friendly and supportive team.
A key part of the role will be helping patients with poorly controlled type 2 diabetes achieve better control. You will independently assess patients, identify what may need to change and agree individual management plans.
You will review and optimise treatment, initiate and manage GLP-1 medicines, review and adjust insulin within your competence, and monitor each patients progress. Experience of initiating insulin is desirable.
Applicants must already have the knowledge, experience and clinical competence to manage poorly controlled type 2 diabetes, as this role goes beyond routine annual reviews.
Alongside the diabetes work, you will provide the usual treatment-room nursing service, including advanced wound care, compression therapy, HRT and menopause reviews, cervical screening and vaccinations.
This is a varied role offering 22 hours per week at £26.00 per hour. We are happy to discuss the working pattern.

Treatment room and wound care
Assess and manage simple and complex wounds
Develop and review wound-management plans

Job summary
Some patients arrive feeling nervous about having blood taken. A calm voice, a steady hand and a little patience can make all the difference.
The Cornerstone Practice is looking for an experienced phlebotomist to join our treatment room team. This is a replacement post working 20 hours per week, 4 hours each day, Monday to Friday.
The hourly rate is £13.00.
You will run planned blood-taking clinics, make patients feel at ease and make sure samples are labelled and handled correctly. You will work closely with our clinical, reception and administration teams.

Main duties of the job
Your main responsibility will be to provide a safe, reliable and friendly phlebotomy service for our patients.
Take blood samples from adults in line with practice procedures and your training
Confirm the patients identity and obtain consent before each procedure
Label, record, store and prepare samples correctly for collection
Keep the clinical room clean, safe and ready for the next patient
Maintain stock and report when supplies need ordering
Recognise when a patient is unwell and seek clinical help promptly
Record activity accurately in the patients clinical record

We look after approximately 9,500 patients and use SystmOne. Our clinical team includes GP partners, salaried GPs, advanced practitioners, treatment room nurses, healthcare assistants and a mental health practitioner. We also work with pharmacy, physiotherapy and social prescribing colleagues, supported by an experienced reception and administration team.
We are based in March, a Fenland market town with direct rail services to Ely, Cambridge and Peterborough.

Main duties of the job
You will provide general medical services to registered and temporary patients as part of the practice clinical team.
Your working week will include a mixture of:
Face-to-face and telephone consultations
Assessing new, ongoing and undiagnosed health problems
Reviewing test results and clinical correspondence
Prescribing and medication reviews
Referring patients to other services when needed
Home visits where clinically appropriate
Maintaining clear and timely patient records

The Cornerstone Practice is looking for a salaried GP to work 6 sessions per week. This is a new post funded through the Additional Roles Reimbursement Scheme (ARRS). Applicants must meet the ARRS eligibility requirements in place when they start.
The salary is to be confirmed.
We are open to discussing how the 6 sessions are arranged. An immediate start is available, but we are happy to agree a later start date for the right person.
We look after approximately 9,500 patients and use SystmOne. Our clinical team includes GP partners, salaried GPs, advanced practitioners, treatment room nurses, healthcare assistants and a mental health practitioner. We also work with pharmacy, physiotherapy and social prescribing colleagues, supported by an experienced reception and administration team.
We are based in March, a Fenland market town with direct rail services to Ely, Cambridge and Peterborough.

Main duties of the job
You will provide general medical services to registered and temporary patients as part of the practice clinical team.
Your working week will include a mixture of:
Face-to-face and telephone consultations
Assessing new, ongoing and undiagnosed health problems
Reviewing test results and clinical correspondence
Prescribing and medication reviews
Referring patients to other services when needed
Home visits where clinically appropriate
Maintaining clear and timely patient records
Taking part in clinical meetings and practice learning
Sharing duty and clinical cover fairly with GP colleagues
Contributing to audit, service improvement and patient safety work
There may be scope to develop a particular clinical interest, depending on the needs of the practice and your experience.

The Cornerstone Practice is based in the centre of March and looks after approximately 9,500 patients. We use SystmOne.
Our team includes GP partners, salaried GPs, advanced practitioners, treatment room nurses, healthcare assistants, a mental health practitioner and an experienced reception and administration team.
We also work with pharmacy, physiotherapy and social prescribing colleagues.
We want this to be a practice where people can ask for help, speak honestly and have a life outside work. We cannot promise quiet days in general practice, but we can promise colleagues who understand the pressures and support one another through them.
