Job summary
Embrace Medical Centre and Martlesham Heath Surgery are seeking a Lead Practice Nurse toprovide clinical leadership and day-to-day management of the practice nursing team, including Practice Nurses and Healthcare Assistants (HCAs).
Working as a senior member of the practice multidisciplinary team, the post holder will provide expert clinical care alongside leadership and oversight of the nursing service. They will support the development of the nursing team, ensuring appropriate standards of clinical practice, competency, training and professional development are maintained.
Main duties of the job
- Providing day-to-day leadership, supervision and support to the nursing team.
- Supporting staff development, competency assessment and training.
- Coordinating nursing rotas and clinical cover to support safe service delivery.
- Delivering clinical care including long-term condition management, health promotion, preventative care, immunisations, screening and treatment room services.
- Supporting care for patients with complex and long-term conditions, including older, frail and housebound patients.
- Leading and supporting Infection Prevention and Control (IPC).
- Supporting clinical governance, audit, quality improvement and evidence-based practice.
- Maintaining appropriate clinical standards, policies, protocols and procedures.
- Working with the Practice Management Team to support CQC, NHS and professional requirements.
- Working collaboratively with GPs and the wider multidisciplinary team to provide coordinated patient care.

ACE Medical Partnership is looking for an enthusiastic, motivated and experienced Healthcare Assistant (HCA) to join our friendly and supportive team. This is an excellent opportunity for someone who is passionate about providing high-quality patient care and enjoys working as part of a busy and forward-thinking GP practice.
The successful candidate will be based primarily at our Birmingham Heartlands Surgery branch, working closely with GPs, nurses, PA, healthcare assistant and our administrative team. The role will involve a range of clinical duties including phlebotomy, ECGs, blood pressure monitoring, NHS Health Checks, wound care and supporting the management of long-term conditions, depending on experience and competency.

Main duties of the job
The Healthcare Assistant will support the clinical team in delivering safe, effective and high-quality care to our patients.
The role will include a varied range of clinical activities, depending on the successful candidates experience, training and competencies. These may include phlebotomy, ECGs, blood pressure monitoring, NHS and new patient health checks, wound care, spirometry, peak flow testing, diabetic checks, COPD monitoring, Learning Disability Health Checks and Smoking Cessation.
The post holder will also support the practice in the monitoring and management of long-term conditions and contribute towards achieving relevant QOF and practice targets.
The successful candidate will be expected to manage their workload effectively, maintain accurate clinical records using EMIS Web, follow practice policies and clinical procedures, and maintain strict patient confidentiality at all times.
You will work closely with GPs, nurses and the wider practice team, contributing to the efficient running of the service and providing a professional, caring and patient-focused experience.

Martlesham Heath Surgery has an opportunity for an additional care co-ordinator to join our team, in thisgreat opportunity to work with an innovative and expanding surgery alongside current members of staff. This role could be offered on either a full-time or a part-time basis.

The Advanced Medical Practitioner (AMP) will provide high-quality, safe, effective and person-centred clinical care to patients registered with West Heath Surgery who reside in designated care homes.

Main duties of the job
The post-holder will contribute to the delivery of proactive, personalised and coordinated healthcare for care-home residents, with an emphasis on:
- prevention of avoidable deterioration;
- early identification and management of acute illness;
- proactive management of frailty and long-term conditions;
- personalised care and support planning;
- medicines optimisation in collaboration with the appropriate prescriber/pharmacy professional;
- reducing avoidable hospital admissions;
- facilitating timely access to secondary and community care;

Job summary
This will be a full time, permanent role with a 37.5 hours per week Monday - Friday contract arrangement. The job location is at West Heath Surgery, 194 - 196 West Heath Road, Birmingham, B31 3HB. Pay / Salary is dependent on experience level.
The Mental Health Advanced Clinical Practitioner (ACP) will deliver high-quality, patient-centred mental health assessment, triage, brief therapeutic and biopsychological interventions and care navigation within a GP practice setting. Working autonomously within their scope of practice, the post holder will be an integral member of the multidisciplinary team (MDT), providing specialist mental health support to patients presenting with a wide range of mental health needs.
The post is designed to improve timely access to mental health services within primary care through comprehensive assessment, clinical risk management, care planning, signposting and referral to appropriate statutory, community and voluntary sector services. The post holder will work collaboratively with the wider primary care team and external partner organisations to ensure patients receive coordinated, holistic and person-centred care.
Main duties of the job
The ACP will provide both face-to-face and virtual consultations, delivering evidence-based assessment and intervention across the practice population. Experience in assessing and managing the mental health needs of both adults and children is desirable.
An Independent Prescribing qualification is desirable but not essential. Where held, the post holder will be expected to prescribe safely and effectively within their professional scope of practice, in accordance with national guidance, local policies and regulatory standards.
