Job summary
You will provide a comprehensive administrative service to support the Neighbourhood Team. We are a multidisciplinary service consisting of Community Nurses, Physiotherapists, Occupational Therapists and Advanced Nurse Practitioners, providing care and support to patients with health needs in their own homes across the local community.
This is a busy and varied role requiring a flexible, organised and motivated individual who can work effectively both independently and as part of a team. The successful candidate will demonstrate a calm and professional approach, excellent communication and interpersonal skills, and the ability to prioritise a varied workload while maintaining high standards of service delivery.
The role requires a good working knowledge of Microsoft Office, an understanding of confidentiality, the Data Protection Act and NHS Code of Conduct. The post holder will liaise with patients, families, GPs,external agencies and other healthcare professionals, ensuring enquiries are managed efficiently and appropriately.
The post is part time, working 24 hours per week across a seven-day service, including Monday to Friday working, rotational weekend cover and late shifts (10am-6pm).

Main duties of the job
The successful candidate will be a highly organised, adaptable team player with excellent problem-solving skills and the ability to manage a varied workload.
You will provide essential administrative support to Community Nursing, Physiotherapy and Occupational Therapy teams, including telephone duties, monitoring inboxes, maintaining records and liaising with patients, carers, families and healthcare professionals.
This role requires a positive "can do" attitude, flexibility and the ability to prioritise tasks, meet deadlines and deliver excellent customer service in a fast-paced environment.
We are seeking an organised, compassionate, and proactive individual to join our team. The successful candidate will need to have excellent written and verbal communication skills, along with strong organisational abilities, enabling them to effectively liaise with a wide range of professionals, patients, and their families.
Ideally, candidates will have NHS experience, including direct patient contact and experience of providing appropriate support and signposting to services. The post holder will need to demonstrate initiative, a practical approach to problem-solving, and the ability to work effectively as part of a multidisciplinary team.
Applicants should be confident using IT systems, with experience of databases and Microsoft Office applications. Accuracy, attention to detail, and the ability to manage a varied workload are essential.

We are currently looking to recruit a Podiatrist to join the CPFT Podiatry Service working in the community in Cambridgeshire. The successful applicant will work primarily in their locality podiatry team but will be required to support the service across Cambridgeshire and Peterborough when needed. We operate out of key sites in Oak Tree Centre/ Doddington Community Hospital/Princess of Wales Hospital/ North Cambs. Hospital/ Chesterton Medical Centre/ City Care Centre.
We are open to Podiatrists from various professional backgrounds including newly qualified staff and staff looking to join the NHS from private practice. We have developed a comprehensive preceptorship program for newly qualified Podiatrists, and a structured competency framework for more experienced Podiatrists to ensure those joining the team experience a structured, supportive onboarding experience.
There is scope for the positions to be full or part-time alongside potential for compressed hours (e.g. full-time hours over 4 day weeks or 9 day fortnights).
Main duties of the job
The post-holder will be competent across all core areas of Podiatry and have a strong team work ethic and sense of accountability. The successful candidate will work within the Podiatry team to support our core cohort of patients, with scope for specialist practice dependant on the individuals skillset.
We are keen to support and develop our staff through a clear career pathway and encourage promotional opportunities within the Trust. Within the service there is ample opportunity for professional growth and development across the variety of specialisms we offer; from diabetic and high risk foot management, in both the community and acute settings, to assisting in our community-run Podiatric Surgery unit. There are opportunities to develop line management and mentoring skills through supporting of Podiatry students, our apprentice Podiatrists and Footcare Assistants.

To provide high-quality nursing care to the practice population, promoting health and

We are a well-established training practice located within the North East London Primary Care Network, serving 9,700 patients, and maintaining strong links with Newham Hospital.

We are a unique dispensing practice in a picturesque Cotswold town with excellent railway connections to both Oxford and London. Attractive Cotswold Stone single site premises with good on-site parking. Easy access to Charlbury Station, which is only a 15 minute journey from Oxford Station. 2. With oversight prepare and assist in the setup of new patients up on Non-invasive ventilation, this also includes explanation into therapy, mask fitting and trial of NIV equipment and downloading Compliance data and taking appropriate action as necessary.
3. To deal with clinical queries from new and long-term CPAP and NIV users over the telephone and by email.
4. To provide Home sleep studies and oximetry on an outpatient basis.

This is an exciting opportunity for the right candidate to join our existing team of Pharmacists and Pharmacy Technicians, working within the GP Surgeries that form Wakefield North Primary Care Network (PCN). The current team, led by our Lead Pharmacist and Lead Pharmacy Technician, have built strong and valued working relationships amongst themselves, other roles within the PCN and our GP practices as well as colleagues from a neighbouring PCN.
Main duties of the job
Manage repeat prescription processes, including resolving queries, maintaining accurate medication records, and working from prescribing ledgers in a timely manner.
Undertake medicines reconciliation following discharge or correspondence from secondary care, community services, and clinics.
Maintain and optimise repeat medication lists, including synchronisation, review date management, electronic repeat dispensing, and appropriate amendments under agreed protocols.
Support care home and nursing home patients through medicines optimisation, prescription changes, and liaison with clinicians and community pharmacies.
Deliver patient-facing clinics where appropriate, including medicines optimisation, secondary prevention, cardiovascular disease (CVD) support, and frailty reviews.
Undertake clinical support activities including observations (e.g. blood pressure), urine dip testing and, if required, phlebotomy (training provided).
Support antimicrobial stewardship, opioid de-prescribing, safety alerts, and the implementation of national and local prescribing guidance.
Contribute to prescribing safety through audits, quality improvement activity, monitoring of recalls, blood tests, and medication reviews.
Support effective stock management, prescribing data analysis, cost-effective prescribing, and waste reduction initiatives.
Promote safe use of medicines, report and prevent medicines-related incidents, and escalate safeguarding concerns appropriately.
