Job summary
Tonbridge PCN is seeking to recruit an Advanced Nurse Clinical Practitioner to complement our growing PCN team of ANPs, paramedics, pharmacists, and care coordinators to support our practices in the delivery of urgent care. We are looking particularly for an ACP (Nurse) who is confident in triage, management of 'on the day' primary care and communication with GP and MDT colleagues in the ongoing follow up and treatment of patients as required.
This role requires working in multiple sites (PCN practices) and working some evenings and Saturdays.
This is a 12 month fixed contract, with a view to extend.
We would welcome applications from individuals looking for full-time and part-time roles.
Main duties of the job
Job summary
Tonbridge PCN are seeking to recruit an Advanced Clinical Practitioner to complement our growing PCN team of paramedics, pharmacists, and care coordinators to support our practices in the delivery of urgent care. We are looking particularly for an ACP who is confident in triage, management of 'on the day' primary care and communication with GP and MDT colleagues in the ongoing follow up and treatment of patients as required.
About Tonbridge PCN
Serving approximately 70,000 patients; Tonbridge PCN strives to develop collaborative working across Tonbridge GP practices and local health and care services. Tonbridge recently won a Kent and Medway award for its Integrated Home Visiting Service, jointly staffed by PCN GPs, paramedics and local community nursing teams.
The PCN aims to support and strengthen delivery of primary care, offering high standards of patient care centred on the needs of our population, whilst working to support a happy and resilient workforce through the development of robust multi-disciplinary teams. A Same Day Access PCN Hub service with GP supervision using ACP clinical skills is currently in development, and we hope this will not only support patient access but provide a greater range of clinical experience for ACPs working at PCN level.

Clinical practice
The role will mainly involve working in our Same Day Access Hub seeing patients for urgent on the day issues. The role will involve working alongside PCN team of GP supervisor / Paramedic Practitioner in the PCN Same Day Access Hub seeing patients on the day with urgent issues.
The post may include cross cover with the award-winning Frailty Visiting Team visiting patients who are housebound for urgent on the day issues.
Work as part of a multi-disciplinary team to deliver safe and high-quality care.
Carry out clinical practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with acute, long term and complex health needs.
Manage undifferentiated undiagnosed condition and identify red flags
and underlying serious pathology and take appropriate action
Triage and treat patients, including same day triage, performing assessment of their needs and makes referrals as necessary to other members of the primary and secondary health care teams as appropriate.
Interpret investigation findings to determine effective treatment plans & discuss with GPs where a second opinion is required
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition, using both non-pharmacological and pharmacological treatments.
Co-ordinate the planning and delivery of care ensuring patients and carers or relatives are involved where required. Actively take a personalised care approach and population centre care approach to enable shared decision making.
Prescribing and reviewing medication that is appropriate for patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan.
Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.
Assess, diagnose and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency.
Refer patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary.
Communication
Uses and adapts communication style to meet individual patient needs to ensure patients are fully informed about treatment.
Communicates effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating

Main duties of the job

Successful candidates are encouraged to sign up to the hospital bank if desired to help fill occasional out of hours locum work (long days and/or weekends). We are seeking to appoint an experienced, proactive, dynamic, motivated Sickle Cell nurse who has an interest in service development.
The successful applicants will be providing nursing care within a new pathway and will work alongside the emergency department and the haemoglinopathy service. They will provide triage, assessment, and care delivery to Sickle Cell patients who present to the Royal London Hospital Emergency Department. As such the role requires a high level of enthusiasm, drive, organisation and a willingness to learn.
Main duties of the job
In conjunction with the haemoglobinopathies teams across Barts Health NHS Trust the post holder will be responsible for the ongoing development and delivery of a high quality sickle cell service across the Trust.
Barts Health NHS Trust has been designated as a Haemoglobinopathy Coordinating Centre for sickle cell disease, thalassaemia and other rare inherited anaemias. The Royal London Hospital is the host site within the Trust and this post.
The postholder will be an experienced nurse responsible for triaging sickle cell patients presenting to the Royal London Emergency Department
They will be expected to provide excellent customer care for patients, which may include communication with distressed and anxious patients and relatives, treating them with tact and empathy, ensuring the professional image of the Trust is always maintained.
They will support the on-going development of multidisciplinary team working in collaboration with the red cell team and the wider clinical haematology nursing team, as well as other nursing colleagues on wards and in emergency care.
You will work as a highly specialised member of the red cell team to assist in enabling the patients to live as a productive member of society and reducing protracted inpatient stays.

Main duties of the job
The post holder is responsible to the Operational Lead of the site and is required to carry out/ supervise technical and scientific work in the section. They will practice as a HCPC registered Senior Biomedical Scientist providing a patient focused, cost effective and efficient service.Duties will include the management of Biomedical Scientists (BMS), Associate Practitioners (AP) and Medical Laboratory Assistants (MLA) within the section involving the training and supervision of unqualified staff, whilst taking charge of the day-to-day operations of the section within the laboratory.Flexible hours including weekends and bank holidays will be required within the contracted 37.5 hours per week in accordance with Agenda for Change (AfC) Terms and Conditions handbook. Job summary
We are looking for an enthusiastic and motivated Radiopharmaceutical Technologist with experience in aseptic dispensing to join our dynamic and expanding Radiopharmacy team. Experience of Radiopharmacy is desirable but full support and training will be provided to candidates who meet the other specifications.
Consideration would be given to a less experienced applicant to join the team as a trainee.
Main duties of the job
Applicants must have the ability to work in an aseptic unit under current GMP guidelines and local rules. Participates in the programme of environmental and microbiological monitoring of the Radiopharmacy and blood labelling suites, to include:

4. Regional blood cell labelling service
In the absence of the Senior Radiopharmaceutical Technologist, to be responsible on a day to day basis for the operation of the Regional blood cell labelling service, including:
4.1 Aseptic preparation of radio-labelled red and white blood cells, in multi-stage patient-critical procedures, prior to re-injection of the cells into patients for diagnostic scanning purposes4.2 Liaison with customer departments for scheduling, transportation of blood to and from the customer department, and other arrangements
5. Management, supervision and training of staff
5.1 Deputises for the Senior Radiopharmaceutical Technologist in their absence.
5.2 Assists in the training of new staff and supervises the work of more junior members of the department
6. Stock control
Responsible for stock control and ordering of pharmaceutical products, including:
6.1 Pharmaceutical products, chemical reagents, and disposable items used in the radiopharmacy and associated areas6.2 Consumable items for the Clinical Isotopes Department6.3 Orders stock items for Radiopharmacy and Clinical Isotopes from Pharmacy
