About us
Our ICU has recently been rated 'Outstanding' for patient care by the CQC and the Trust overall has a 'Good' rating. The unit currently admits nearly 1100 level 2 and level 3 patients per year into 23 beds. The case mix is predominantly medical with trauma and surgical patients making up about 20%. Around 90% of our admissions are emergencies with the remainder being high risk surgical cases including, colorectal & urological procedures. There are about 60 paediatric admissions and stabilisations a year, the vast majority of which are retrieved via the Southampton and Oxford Retrieval Team (SORT), but some remain for the initial stages of their stabilisation and treatment. The Trust is a tertiary referral cancer centre and includes all specialties except neurosurgery, transplant, vascular and cardiothoracic surgery. The trust has a primary coronary angioplasty service with the fastest door to needle times in the country as well as the fastest door to needle times for stroke thrombolysis. Job description
Job responsibilities
The Care Coordinator will manage a defined caseload of patients, acting as a consistent point of contact for patients, unpaid carers and professionals involved in their care. The post holder will play a key role in ensuring patients receive coordinated, timely and person-centred support, helping them navigate health, social care and community services to improve their overall health, wellbeing and independence.
Working as part of Windsor Primary Care Network's (PCN) multidisciplinary ProActive Care Team, the Care Coordinator will collaborate closely with GPs, Practice Nurses, Social Prescribing Link Workers, Health and Wellbeing Coaches, other Care Coordinators and wider health and care professionals. They will support the development and implementation of personalised care and support plans, ensuring that patients' individual needs, preferences, circumstances and agreed outcomes are considered throughout their care journey.
The post holder will proactively identify patients who may benefit from additional support through clinical searches, population health management initiatives, practice referrals and multidisciplinary team meetings. This will include supporting people with long-term conditions, complex health and social care needs, frailty or other circumstances that may affect their ability to manage their health and maintain independence. They will help coordinate appropriate interventions, facilitate referrals and follow up actions to ensure patients are connected with the services best placed to meet their needs.
An important aspect of the role will be supporting preventative and anticipatory care, including assisting with care planning, coordinating support following hospital discharge and identifying opportunities to prevent avoidable deterioration in health. The Care Coordinator will encourage patients to take an active role in managing their health, accessing appropriate community resources and making informed decisions about their care.
The post holder will also support the identification and recognition of unpaid carers, ensuring their caring responsibilities are appropriately recorded and that they are offered information, advice and signposting to relevant support services. They will work to identify barriers to accessing care, including practical, social and digital barriers, and help patients and carers access services in a way that reflects their individual circumstances.
Accurate and timely record-keeping will be essential. The Care Coordinator will maintain confidential patient records using EMIS Web and other approved clinical systems, ensuring appropriate clinical coding, documentation of interventions and recording of agreed actions. They will work in accordance with organisational policies, confidentiality requirements, GDPR and Information Governance standards.
Through effective partnership working with primary care, community health services, adult social care, mental health services, hospital teams, voluntary and community organisations and local authority services, the post holder will help deliver seamless, integrated care across organisational boundaries.
The Care Coordinator will contribute to the ongoing development and improvement of PCN services by participating in audits, service evaluations, quality improvement initiatives and the implementation of new ways of working. They will support the PCN's commitment to improving patient outcomes, promoting independence, reducing health inequalities and delivering high-quality, proactive and personalised care to the local population.

Job summary
"The next of kin are at the heart of everything we do"
We are seeking to appoint a proactive and self-motivated doctor to join our team of medical examiners at North & Mid Hampshire Medical Examiner Service.
Since 9 September 2024 when the service became statutory, all acute, non-acute and community deaths that do not require a coroner's referral must be scrutinised by a medical examiner before the death can be registered.
Main duties of the job
The successful applicant will
- provide independent scrutiny of non-coronial deaths
- ensure the appropriate referral of deaths to the coroner
- contribute towards a better service for the bereaved, including an opportunity for them to raise any concerns
- improve the quality of death certification
- support local learning and improvement by identifying matters that require escalation to local clinical governance and other processes
- improve the quality of mortality data
The ME service interacts with many clinical specialities from both the acute and non-acute setting, strengthening links between the health services, coroners' offices and register offices.
Medical Examiners are senior doctors from a diverse variety of clinical specialties in secondary and primary care. Relevant governance experience would be a strong bonus. This could be mortality or other case record reviews in secondary care; or actively considering similar issues in the context of team meetings in primary care. You will work closely with our team of Medical Examiner Officers, who are vital to the efficient running of the service.
About us
Hampshire Hospitals NHS Foundation Trust is the major provider of hospital services to the population of North and Mid Hampshire and surrounding areas.
