Job summary
Transcend is a newly commissioned service established to explore how Gender Incongruence can be best supported and treated within a Primary Care setting. We are seeking a Specialist Nurse to help deliver this care. The postholder will work closely with our MDT, local GPs, relevant partner agencies, and of course directly with service users.This is an exciting opportunity to contribute to the development of not only a new service, but a new model of care for a disadvantaged and underserved service user group. The role will involve travel across the locality, therefore being a car user is essential.
Main duties of the job
To provide direct clinical nursing care to the service users of Transcend. This will include both direct and indirect interventions to support individuals accessing the service. The ability to work autonomously, as well as part of a wider multidisciplinary team, is essential. The postholder will be responsible for managing a caseload of patients and supporting them throughout their journey with the service.

Job summary
Stratton Medical Centre is a busy town practice on the outskirts of Bude.
We are a well-organised, forward-thinking practice led by two GP Partners and an experienced Practice Manager. We pride ourselves on our ability to respond quickly to challenges whilst maintaining a supportive environment where staff feedback is valued and encouraged. We are committed to ensuring that Stratton Medical Centre remains a positive and rewarding place to work.
Communication and teamwork are at the heart of what we do. There is a whole team morning huddle to ensure everyone is aware of key issues and priorities. Mid-morning GP coffee breaks provide an opportunity for informal peer support, and regular team meetings ensure all staff have the opportunity to input into decisions, raise concerns or for information sharing.
The GP Day is carefully managed, with a balanced mix of routine and urgent appointments, triage sessions and follow up telephone consultations. Workload is continually monitored and our wider disciplinary team, including home visiting clinicians, pharmacists and administrative team, helps reduce the pressure on GPs and supports efficient patient care.

About us
Stratton Medical Centre is a busy town practice on the outskirts of Bude. We have over 10,700 patients who are looked after by an integrated team of 9 GPs, 2 Nurse Practitioners, a Paramedic, 2 Nurses, 3 Health Care Assistants and a Phlebotomist.
As part of a wider Primary Care Network (PCN), our GP team also benefit from a clinical pharmacist, a home visiting paramedic, a social prescriber and health and wellbeing coach. We also have a team of staff who work with more vulnerable adults including diabetics, learning disabled and the frail elderly. All these staff contribute to managing our patient group.
We are a well organised team that is led by a Practice Manager and 2 GP Partners. We benefit from being able to respond quickly to situations and to listening to our staff to ensure that we remain a happy, well functioning team.
We all meet every morning to ensure we are aware of the issues for the day and pick up any concerns that may be present.

Job summary
The Rapid Response Co-ordinator provides operational co-ordination of Patient Transport and therapeutic support for individuals on a Section 136 of the Mental Health Act within designated places of safety and patients waiting within health care settings for mental health support. Job summary
An exciting opportunity has arisen within Sunderland Talking Therapies to appoint a Qualified Psychological Wellbeing Practitioner. The position is full time hours (37.5 per week) for either a qualified PWP who holds a PWP accreditation or a Trainee PWP who is nearing the end of training due to be ratified, and wants to develop their skills and experience within our team. The post holder will work with people with different cultural backgrounds and ages, using interpreters when necessary and should be committed to equal opportunities.
The successful candidate should:
- Have successfully completed an academic course of study in line with the IAPT Low Intensity National Programme and be qualified as a PWP (e.g. PG Cert Low Intensity Psychological Therapies)
- We are also interested in receiving applications from current Trainee Psychological Wellbeing Practitioners who are enrolled on a PG Cert in Low Intensity Psychological Therapies and are due to qualify. If you are a current Trainee PWP you would initially be employed at Band 4 and once marks have been ratified by your university examination board, and you have gained PWP registration this would be uplifted to a Band 5.
- Have a current registration with the BABCP or BPS, or be able to immediately apply for this once in post (PWPs will remain at band 4 until registration has been submitted and confirmed).
Main duties of the job
In this role you will:
- Undertake triage screening assessments and make onward referrals
- Provide a range of low intensity interventions e.g. guided self-help (both telephone, online consultation and face-to-face), psycho-educational courses and computerised CBT
- Have the opportunity to contribute to a number of innovative service improvement projects, champion roles and specialist interest groups
- Liaise with external agencies and services to promote Sunderland Talking Therapies
- Have the opportunity to act as a mentor to trainee PWPs

Main duties of the job
We are looking for a hard-working person to join our deep clean team. You will be based at Kendray Hospital in Barnsley but work trust wide, deep cleaning our sites. The role involves hand and machinery cleaning of hospital premises and wards. Experience would be great, but training will be provided for the right candidate. We use trust vehicles to get around our many sites, so a full clean driving licence is essential. Shifts are normally Monday to Friday but will also include some weekends and Bank Holidays. Shifts are normally 6AM 2PM although we do work as late as 3PM 11PM in certain areas. Shift times will occasionally change at short notice. Flexibility is key in this role.

1.1 KEY RESULT AREAS:

JOB SUMMARY:
The post holder will:
Be part of a comprehensive domestic cleaning service at South West Yorkshire Partnership NHS Foundation Trust.
Perform domestic duties including deep clean in line with National Cleaning Standards and Operational Procedures.
Assist in the provision and maintenance of a high quality, customer responsive service by providing general linen service duties.
1.1 KEY RESULT AREAS:
To be responsible to a high standard of cleanliness in clinical areas, offices, public areas and external areas using the appropriate recognised cleaning methods.
Responsible for advanced and deep cleaning projects including internal and external windows, carpets and high level dusting (dependant on work area)
Responsible for identifying areas to be cleaned, plan and organise a cleaning programme with relevant ward or departmental staff, ensuring patient, staff and visitor safety is not compromised.
Operating cleaning equipment, machinery in accordance with manufacturers instruction ensuring that it is in a good clean working order, reporting faults immediately to supervisor.
Use the correct use of PPE appropriate to the area and task undertaken in line with Trust policies. Ensuring the colour coded equipment used is appropriate to the area and task undertaken.
Change curtains on rotation or in the event of infection.
Follow policy and run all water taps/showers in designated areas on a daily basis and record on checklist.
Check out spot cleaning to ensure spillages and incidents are dealt with swiftly and efficiently, supported by clinical staff as appropriate.
To adhere to all relevant COSHH assessments and safety data sheets when preparing, using and disposing of cleaning chemicals.

Job summary
Due to organisational growth we currently have vacancies for Advanced Nurse Practitioners to work in our Extended Access service.
The primary focus of the role is work as an autonomous advanced practitioner providing high quality and timely patient centred care.

Main duties of the job
- Act as an advanced practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.

Triage patients
- Undertake consultations, both face to face and via the telephone, with patients of all ages (neonate to elderly) as an autonomous practitioner and using own clinical judgement to diagnose, treat, refer and/or discharge patients
- Make direct referrals to primary, secondary and social services within our agreed pathways, guidance and protocols.

Job summary
Applications are invited for a qualified Physicians Associate on a fixed term period of 10 months. Working within the Community Services Department (Diabetes and Endocrinology) based in Ysbyty Gwynedd Hospital the post holder will be part of a highly motivated and dynamic Multidisciplinary Team. The post holder will be welcomed by those working in the department, with every encouragement provided to develop and consolidate their Medical knowledge.
This post is a fixed term contract/ secondment for 10 months till 27th of June 2027 to cover maternity leave. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.
Main duties of the job
As a Physicians Associate you will provide care for patients including clinical assessments, diagnosis and treatment under the mentorship of a consultant.
The successful candidate will be autonomous, a team player and will be expected to work with the wider practice team to deliver services to meet the needs of the patients. The ability to multi-task and to work under pressure is essential and you must also be adaptable to a varied and high volume workload.
Working knowledge of using Microsoft Office applications, an understanding of GDPR and maintaining strict confidentiality in the healthcare setting are all essential together with excellent organisational and communication skills, flexibility approach to work and good time management.
Education and supervision is available through accredited and in-house training and the successful applicant will be supported by experienced trainers and clinical mentors. The level and type of supervision will be dependent on the post-holders skills and knowledge. A comprehensive induction plan will be tailored for successful candidate.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Due to the continued growth of The Communication Hub, Hywel Dda University Health Board is seeking to recruit an enthusiastic individual to join our patient-facing Single Point of Contact service. This is an exciting opportunity to contribute to a team dedicated to improving access and communication for patients across the Health Board.
The position is based on a Monday to Friday, 9:00am to 5:00pm working pattern, with a hybrid arrangement of three days per week at Hafan Derwen and two days working from home.
The post holder will be Service Coordinator for Waiting List and Cancellations. A key function within the Communication Hub providing patients with accurate and safe information.
The team is seeking an expert in health service administration, patient communication and team leadership. Main duties of the job

Job summary
We require a fully qualified Plumber. You should be qualified to City and Guilds Advanced standard or equivalent NVQ level 3 and have served a recognised apprenticeship or undergone modern training to an equivalent standard.
The post holder will hold the necessary maintenance skills to ensure the safe and continuous operation of mechanical and electrical engineering services and building plant. As an integral member of the Maintenance team should be able to demonstrate leadership skills. Community Team Manager - Specialist Childrens Mental Health Services (SCAMHS)
East Suffolk
This is an exciting opportunity to join East Suffolk Specialist Childrens mental health team, supporting young adults aged 4-18yr during a crucial stage of their development. Our service sits within Children, Families and Young People (CFYP) services and benefits from a well-established clinical treatment pathways designed to deliver responsive, high-quality care.
As Community Team Manager , you will provide operational oversight leadership for service delivery, working closely with the SCAMHS Triumvirate with Principle Psychologist and Consultant Psychiatrist. You will be supported in your post by a Community Matron and an Operational Manager to ensure excellent outcomes for service users. You will also be part of a supportive wider network, linking closely with East Suffolk YAMHS teams, benefiting from strong peer support and clinical collaboration.
This role offers a range of leadership, development and training opportunities to support your professional growth and career aspirations.
We are looking for an enthusiastic and motivated leader with experience of working in community mental health services with the under 18 age group. You will bring experience of working across primary and secondary care, as well as with VCSE partners, and share our commitment to delivering safe, effective and compassionate care.
Main duties of the job
As Community Team Manager, you will provide professional leadership and governance to ensure the delivery of a safe, responsive and high-quality service for service users, their families and carers.
Key responsibilities include:
- Providing visible, values-based leadership and acting as a positive role model within the service
- Managing and supporting the team to ensure staff are appropriately trained, informed and competent in their roles
- Ensuring robust systems are in place for supervision, appraisal, objective setting and personal development
- Allocating and overseeing caseloads to ensure safe practice and the effective use of resources
- Maintaining safe staffing levels within agreed budgets
- Monitoring team performance and contribution to Trust objectives
- Promoting a person-centred approach that actively involves service users, families and carers in care planning and delivery
