Join us as a Community Link Worker in Renfrewshire.
We are recruiting for a part time Community Link Worker to be based in GP practices across Renfrewshire. This position is permanent and part time working 30 hours per week across Tuesday to Friday (7.5 hours per day). This post requires the ability to travel between locations, as such a driving license and access to a vehicle is required.
We provide a mix of in person appointments from GP practices, home visits, meetings at community based venues and remote support via telephone or an online platform. Main duties of the job
Our Community Link Workers are uniquely placed third sector workers operating within GP Practices. They bring a unique skill set and third sector perspective that complements the work of the multidisciplinary primary care team. Through 1:1 appointments, using evidence based interventions and small group work we help people change their lives for the better. It is all about getting the right support to people at the right time.
Common reasons for referral to our service include low level mental health difficulties, problems with housing, poverty, relationships and bereavement.
You can expect a comprehensive induction and training, alongside the support of an experienced team to support you in understanding your role. Our team covers the whole of Renfrewshire, you will have monthly 1-1 support, 6 weekly reflective practice sessions and team meetings online and in person. We have a supportive team culture and plenty of opportunities to connect with colleagues.
As a Community Link Worker, you will need an independent approach to your work and the ability to network and connect with services, locally and nationally. You will need to understand the impact of deprivation on health inequalities and have demonstrable skills in working with people to achieve their health and wellbeing goals. You will be confident in working with communities to understand local needs.

JOB DESCRIPTION - Duties and responsibilities
The following is not an exhaustive list of duties, but is indicative of the work that will be required. Duties are allocated within the team by the Office Manager and some functions may be carried out at Woodbank Surgery. Allocation of duties between sites and individuals may change over time. The practice works in the ever-changing environment of the NHS and the tasks required will change from time to time.
Following Practice policies, protocols and guidelines, duties may include:
Reception and Administration duties
Ensure that all patients, visitors and staff receive a friendly service and their enquiries are dealt with efficiently.
Provide a welcoming and efficient telephone service, managing own time to ensure maximum waiting times are kept low.
Provide a welcoming and efficient counter service.
Explore patient queries and requests fully, taking account of information in the medical record, to ensure that enquiries are handled by the most appropriate person, without the need for a clinician appointment when appropriate, and that any appointments booked are the most appropriate for the request.
Refer to procedures, guidance, colleagues or senior staff when unsure.
Book, cancel, rearrange and receive patients for appointments.
Deal with patient requests for home visits.
Process repeat prescriptions, ensuring appropriate checks are made.
Explain systems to patients, e.g. appointments system, repeat prescriptions system.
Respond appropriately to any potential emergency situation, ensuring access to care is not delayed.
Arrange ambulances and interpreters.
Extract and file medical records and correspondence accurately.
Collect information from patients and enter accurately into the computer system, as requested.
Contact patients as appropriate.
Keep track of all tasks and ensure they are completed in a timely fashion.
Take payment from patients for chargeable services, ensuring they are correctly recorded, that a practice receipt is issued on every occasion and that cash is placed in the designated secure place with the appropriate copy of the receipt.
Open and lock up the building as required.
Ensure the reception area and waiting room are kept clean and tidy.
Stock leaflets and organise displays.
Register new patients and process changes of details, ensuring all checks are made.
Operate the registration links system, ensuring transactions are processed promptly and queries addressed.
Maintain paper patient records, including receiving, sorting, filing and sending.
Operate laboratory results system (paper and electronic) including giving authorised results to patients.
Chase up outstanding results, as requested.
Operate call and recall systems, e.g. for cervical cytology and chronic disease, including keeping records, sending letters to patients and failsafe checks.

Job summary
We are looking for an experienced Physiotherapy Assistant to join our specialist 7-bed bariatric rehabilitation service at Ribble Court Care Home.
Working alongside our registered Physiotherapist, you will support the delivery of individual rehabilitation programmes designed to improve residents mobility, strength, function and independence, with the aim of enabling them to return home following their period of rehabilitation.
This is a hands-on role working with a small, defined caseload of seven bariatric residents with complex rehabilitation needs.

The Pre-Operative Department plays a vital role in preparing patients for elective gynaecological surgery, ensuring they are clinically optimised and fully informed prior to admission. Working within a specialist women's health environment, the successful candidate will provide high-quality nursing assessments, coordinate investigations, identify and escalate potential risks, and contribute to the safe and effective planning of patient care.

We are looking for a Clinical Coder / Document Administrator to join our Administration and Coding Team on a bank contract basis.
The postholder will accurately review, extract and record clinical information within EMIS Web, supporting patient safety, data quality and healthcare performance targets.

Main duties of the job
The post holder will be part of the Administration and Coding Team Hub supporting the Cumbria Health practices. The post holder will be responsible for a variety of administration duties including data input, work flowing and coding of clinical correspondence. The post holder will code all incoming discharge letters and any patient related incoming post into the patients medical record and workflow to the appropriate clinician.
The post holder will be based at Cumbria Health headquarters in Carlisle. Remote working is optional.

An exciting opportunity has arisen to recruit a motivated and enthusiastic Clinical Support Worker Higher Level - Community - Band 3to work within Keswick Community Hospital.
ICCs aim to address the challenges of an aging population and increasing demand on health and care services by providing more out of hospital care and empowering people to take control of their health and wellbeing. This will require new ways of working to focus on preventing ill health and ensure services are sustainable for the future. These new ways of working will follow the same approach across all eight ICCs with flexibility to tailor services to the needs of the local people.
This role requires you to demonstrate the skills and knowledge to support the provision of high quality care, to support patients with complex needs within the community setting.
This is an excellent opportunity for someone who is looking to develop their clinical skills and expertise in caring for patients, working closely with health and social care colleagues to prevent hospital admission, facilitate hospital discharge and promote independence through reablement and management of long term conditions.
Daily working hours are 12 hour shifts, 07:00-19:30 and 19:00-07:30.

As a Clinical Support Worker Higher Level - Community - Band 3 in a multi-disciplinary team, work with other health and social care professionals to develop the ethos of multi-disciplinary, integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.As a Clinical Support Worker Higher Level - Community - Band 3, working under the direction of a registered health care professional, undertake a range of delegated patient nursing and care duties including clinical observations and recording patient information. This will include supporting patients with personal hygiene and activities of daily living.
Working largely alone and unsupervised, assess patients, plan and implement care in the community, provide advice and education to patients, carers and colleagues, maintain associated records. Carry out nursing procedures.
Carry out delegated management tasks including line management of other team members.
Flexible working is required to meet the needs of the patients group.
The Care Group is commissioned to provide 24/7 nursing services.

Meet basic care needs of patients, washing dressing, assistance with feeding etc
When trained and deemed competent undertake clinical activities, using appropriate equipment (these will be activities relevant to speciality) e.g. venepuncture, ECGs, urinalysis.
Provide personal care to patients by supporting / providing the patient assistance with all aspects of daily living providing individualised care and promoting independence, using appropriately identified aids e.g. hoists, special mattresses.
Ensure the nutritional needs of the patient are met by assisting them to eat and drink. Also by helping them complete their daily menu card. Be aware of increased nutritional needs. Record accurately nutritional and fluid intake, reporting concerns to Registered Nurse.
Ensure privacy and dignity at all times.
Where relevant to the clinical area, understand why observations are taken and be able to take / record patients, pulse, blood pressure, respirations, temperature and any other observations including accurately recording them. Calculation of the early warning scores, e.g. MEWS and reacting to that score, reporting any problems to the nurse in charge.

The core purpose of the Pharmacy Department is to ensure the safe, effective, and efficient use of medicines to improve patient outcomes and support high quality clinical care.As a key member of the pharmacy leadership team, the Deputy Lead Pharmacist contributes to this purpose by providing expert professional and operational leadership across all aspects of pharmaceutical service delivery. Main duties of the job
The Deputy Lead Pharmacist plays a key leadership and operational role within the Pharmacy Department, supporting the Lead Pharmacist in the strategic and day-to-day management of pharmacy services. Main duties of the job
The purpose of this post is to provide an efficient and comprehensive administrative service within the Community Resource Team (CRT). Acts as the first point of contact for telephone communication for the CRT.
The post holder will work within the Conwy CRT (Abergele, Colwyn Bay, Llandudno, Llandudno Junction & Llanwrst) alongside District Nurses, Therapy Services, Local Authority colleagues and Voluntary Sector to provide a seamless administrative service to the Multidisciplinary Teams.
The Clerical Officer is required to work accurately and independently, within guidelines after training. Using their own initiative and judgement, rather than supervised, deciding when necessary to consult their manager. All duties must be carried out in accordance with Departmental and Health Board Policy.

Avocet ward is a 16 bedded male only inpatient ward and this post will require assessment and management of a wide range of psychiatric presentations.
RC/AC status and section 12 required for this role.
We are offering a £40,000 welcome bonus to the successful candidate, paid over the course of your first two years in post.
Main duties of the job
The ward takes direct new admissions of a range of presentations. You will be supported in this role by range of junior doctors ( FY2 and GPVTS) and it is expected you will have regular dialogue with the two consultant psychiatrists on other two adult inpatient wards onsite. The Welland centre is a modern stand alone 4 ward psychiatric inpatient unit and is staffed by a MDT including nursing staff, occupational therapists and psychologists. Cross cover will be provided by both inpatient and outpatient consultant colleagues on a rotational basis as is the system currently.

As part of Betsi Cadwaladr University Health Community's Journey to Excellence, the Acute Medical Unit within the Emergency Quadrant here at YGC is undergoing some major quality improvement and service developments.

This post is Fixed Term/Secondment for 12 months due to the needs of the service .

Main duties of the job
Assist the registered nurse/practitioner to deliver a high standard of holistic patient care, ensuring that the patients` dignity and equality is maintained at all times.
Ensure the principles set out in the fundamentals of care are incorporated into daily practice to ensure high quality service for patients and clients.
Assist the patients to maintain their own hygiene and personal appearance.
To perform duties within the plan of care delegated by a qualified practitioner, only for which the post holder has the relevant training and assessment.
Prepare ward/clinical area for receiving patients by ensuring area is generally clean and tidy - assist in maintaining the standards in the clinical area and report any untidiness/clutter to the appropriate person.
Assist in the admission of new patients into the department by preparing bed/trolley. area, welcoming patient, directing to bed/other area, showing patient around the ward.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
