Job summary
Walker Medical Group is looking for a Salaried GP to join our friendly and experienced team for 4 - 6 sessions per week.
Main duties of the job
- Deliver safe, effective and patient-centred General Practice services.
- Assess, diagnose and manage acute and long-term conditions.
- Undertake clinical administration including results, correspondence, referrals and prescriptions.
- Work collaboratively with the multidisciplinary team to ensure patients are seen by the most appropriate clinician.
- Participate in the practice on-call rota, proportionate to sessions worked.
- Triage SystmConnect requests; arranging face to face/telephone consultations or signposting patients to appropriate services.
- Contribute to clinical governance, quality improvement and practice development.

- Complete medicines reconciliation following hospital discharge, outpatient appointments and other transitions of care.
- Ensure patients' medication records are accurate and up to date.
- Identify discrepancies and communicate with relevant healthcare professionals to resolve them.
Medication Reviews and Patient Support
- Support clinical pharmacists and GP/Nurse Practitioners with structured medication reviews.
- Contact patients regarding medication-related queries and provide appropriate information within the scope of the role.
