As an Advanced Nurse / Clinical Practitioner, youll be part of a dynamic, multidisciplinary team working within a telephone and digital triage environment. Youll help ensure patients receive the right care, in the right place, at the right timereducing unnecessary hospital visits and improving outcomes across the system.

Our OOH service is open from 18.30-08.00 Monday to Friday and 24 hours over the weekend. We can offer a range of sessions to allow you to flexibly choose the right shift pattern for you.

Main duties of the job
Delivering remote clinical assessments via telephone and digital platforms for patients with urgent and unscheduled care needs.
Making safe, evidence-based decisions to direct patients to appropriate services, including self-care, primary care, urgent treatment centres, or emergency services.
Applying advanced clinical knowledge to assess undifferentiated presentations and manage risk effectively.
Supporting clinical governance, contributing to service development, and mentoring colleagues.
Collaborating with GPs, paramedics, mental health professionals, and other clinicians to ensure integrated care across the region.
About us
Registered Nurse or Paramedic with MSc in Advanced Clinical Practice or equivalent
Experience within Out of Hours, Primary Care GP Surgery, or Urgent Treatment Centre
Holder of V300 / Non-Medical Prescribing qualification and minimum 6 months experience post qualification.
Ability to autonomously see and treat patients of all ages
Confident clinical decision-maker with strong assessment skills.
Excellent communicator with a patient-centred approach.
Flexible, self-motivated, and committed to ongoing professional development.
Live within a 1-hour commute to site in Ipswich

Main duties of the job
Providing comprehensive clinical assessments and management for patients requiring urgent care outside of normal surgery hours across base working and home visitations
Managing a varied caseload including acute minor illnesses, injuries, and chronic condition exacerbations.
Utilizing the Adastra clinical system to accurately record consultations and clinical notes.
Working collaboratively with healthcare colleagues, including nurses, paramedics, and administrative staff, to ensure continuity and quality of patient care.
Making appropriate referrals, liaising with secondary care services, and ensuring patient safety through effective clinical decision-making.
Participating in clinical audits, governance activities, and continuous professional development.
About us
Fully qualified General Practitioner with GMC registration and relevant UK licence to practice.
Previous experience working in an Out of Hours setting is essential.
Proven experience using the Adastra clinical system for patient record management.
Strong clinical assessment skills and the ability to manage a diverse and often complex caseload.
Excellent communication and interpersonal skills with the ability to work both independently and as part of a team.
Commitment to providing compassionate, patient-centred care aligned with Practice Plus Groups values.

Main duties of the job

Main duties of the job
As part of the Community Rheumatology Service, you'll be a key part of the multi-disciplinary team including Physio's, OT's, APP's, Nurses, GPwSI's and Consultant's; as well as working alongside our wider MSK service and Community Pain Team. The role is focussed on expert assessment and management of potential Rheumatological patients. You'll be highly skilled in the assessment and management of this patient group. This could include but is not limited to; evidence of specific training in this area, injection therapy, bloods ordering and interpretation skills, radiological imaging skills, independent prescribing and ultrasonography. Some of these skills we would support once in the role. You will help develop and influence the design of the service, ensuring you maintain strong relations with the existing service provision within the hospital that is to focus more on the management of high-risk complex patients. Whilst undertaking comprehensive assessment and diagnosis of patients with a wide variety of physical and emotional needs, you must formulate, deliver, review and progress individual treatment programmes and ensure effective treatment, specialist advice and timely discharge is achieved.

An exciting opportunity has arisen to join the Transplant Specialist Nurse team. The post holder should have extensive knowledge of renal nursing, excellent communication skills, a passion for delivering high quality care and be able to work collaboratively within the team. Job summary
East and North Herts Diabetes Service is a nurse led service. We are looking for a highly motivated expert Diabetes Specialist Clinical Lead Nurse with a strong desire to improve outcomes for people living with diabetes to work 30 hours per week - 08:30 to 16:30 across 4 days.
The successful candidate will have extensive experience in diabetes specialist nursing and have the skills and experience to provide strong professional leadership and inspire a team of specialist nurses and dietitians and to provide expertise to the wider clinical staff within East and North Hertfordshire.
Ideally you will be an independent nurse prescriber with a strong understanding of the legal and professional issues surrounding this role. You will be a champion of patient structured education and working knowledge of the DAFNE programme will be an advantage.
The postholder will need excellent communication and organisational skills and be able to facilitate clinical staff development both within the team and for appropriate staff in the wider Trust.
You will also be a key stakeholder for the diabetes provision across the Central East Integrated Care System working in conjunction with representatives from the local acute Trusts, primary care and other providers of diabetes care to shape the strategic direction of provision.
Main duties of the job
To provide strong professional leadership, advice, support, and direction to the E&N Diabetes Service in the planning, coordination and evaluation of the service, liaising with Diabetes Dietetic Lead, Operational Lead and Assistant Operational Lead where appropriate
To be professionally accountable for the Diabetic Specialist Nurses (DSN's) within the E&N Diabetes Service including clinical supervision, training needs and professional accountability.
To deliver a minimum of one clinical session per week (pro rata) within E&N Hertfordshire.
To actively contribute to the ICB Diabetes work stream ensuring that appropriate links are established and maintained with the Herts Valleys Integrated Diabetes Service (HIDS) and ENHT Diabetes Service to ensure all national initiatives are delivered locally.
To provide strategic diabetes clinical leadership for HCT within East and North Hertfordshire.

We are looking for an experienced Head of Quality Improvement to operationally lead the Trust's Quality Improvement portfolio and help embed a culture of continuous improvement across South London and Maudsley NHS Foundation Trust.

Job summary
Are you an experienced registered mental health or general nurse with a passion to improve the physical health of individuals with a mental illness?

We are looking to recruit a senior nurse with a genuine interest in physical health working with service users within the mental health service. The successful candidate will be able to work in a fast paced environment and be able to work autonomously and as part of a well-established team within the health and well-being clinics.
Previous experience of working with service users prescribed Clozapine is desirable and an ability to act as a senior clinician within the Clozapine clinics.
Main duties of the job
The role will require the oversight and management of a very busy clinic, providing monitoring for the physical health of patients being prescribed Clozapine, there will also be times where you are required to carry out home visits so access to transport to complete duties of the role essential.
Extensive knowledge of running a clinic and managing clozapine medication is required, alongside physical health monitoring such as ECG's, Venepuncuture, Pochi Machine operation and escalation of physical health concerns to the appropriate teams.
This is a very busy area and part of a growing team.

Job summary
An exciting opportunity has arisen for a competent, confident, and enthusiastic full time Band 6 Occupational Therapist to join our team at Oak Ward West Park Hospital. We are a 12 bedded functional ward, caring for patients who are predominately over 65 years old.

Main duties of the job

Main duties of the job
Mowbray Square PCN, together with Tees Esk and Wear Valleys NHS Foundation Trust, are working together to transform local community mental health services for people with mental health problems. New funding has enabled the PCN and local GP surgeries, to create a multi-disciplinary team that will work in close collaboration with a wide range of organisations.
The postholder joins the existing First Contact Mental Health Practitioner that has been extremely successful with good outcomes for patients and the PCN. They will work in partnership with practice colleagues to continue to build the service within Mowbray Square PCN. The PCN's ambition is to improve access to emotional and mental health support for everyone.
As a valued team member, you will develop close working relationships with all our friendly Clinical staff, including our existing Mental Health Practitioner. Your role will involve offering specialist mental health advice and providing clinical care forpeople with a range of mental health needs, ensuring that those with mental illness do not fall between gaps in services. You will improve access by being a 'first contact mental health worker', undertaking triage, assessments, brief interventions, and reviews, with clinical supervision provided by a B6/B7 Mental Health Practitioner. Main duties of the job
The post holder will provide leadership for Oxleas safeguarding adult and Prevent policies and procedures including assurance that the trust is compliant with all statutory requirements under legislation.

The post holder will monitor safeguarding adult activity, developing robust consistent safeguarding practice in line with best practice and statutory requirements.

The post holder will actively contribute towards the quality and assurance agenda including training, audit, collection and analysis of relevant data.
