We have an opportunity for a Band 6 Mental Health Nurse (RMN) or Occupational Therapists (OT) to join the Crisis Resolution & Home Treatment Team (CRHT) at Woodlands Centre for Acute Care, St Leonards-on-Sea, East Sussex.
The CRHT team is a part of acute care and works closely with the acute hospital and community teams to support clients in a mental health crisis. It offers clients care and support in their own homes as an alternative to hospital admission.
CRHT are able to offer all the benefits of a hospital admission while remaining at home including home visits from our experienced team, with support from doctors and psychology. We also facilitate supportive discharge from hospital, managing the transition back to community-based service.
Main duties of the job
As an Urgent Care Practitioner in our team you will work in partnership with service users with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives.
You will be responsible for the assessment, planning and implementation of top-quality care to service users. This will include working alongside service users, carers, families and partner agencies such as voluntary sector agencies, schools and colleges.
As well as providing specialist nursing advice and carry out nursing procedures, and monitoring the effectiveness of treatments in order to progress and ensure effectiveness of intervention.
You may also be involved in providing clinical supervision to staff and students. This may include acting as shift co-ordinator as needed to lead the team on shift, as well as allocating and delegating tasks to colleagues. You will be expected to liaise with other services and external agencies to triage referrals to the service and gatekeep inpatient admissions.

We are seeking a proactive and detail-oriented RTT Validator to support our Ear, Nose and Throat (ENT) and Sleep Surgical Services. This role is key to ensuring the accuracy and integrity of Referral to Treatment (RTT) pathways, helping the service deliver timely, high-quality care for patients.
Working as part of a collaborative administrative and clinical team, the postholder will be responsible for validating RTT data, booking patients and pathways, identifying pathway discrepancies, and supporting the effective tracking and management of patient journeys.
You will play an important role in ensuring compliance with national RTT standards and contributing to continuous service improvement.
The successful candidate will have strong organisational skills, excellent attention to detail, and the ability to work collaboratively across teams.
You will be supported within a positive, team-focused environment where accuracy, shared learning, and continuous improvement are encouraged.

Main duties of the job

An exciting opportunity has arisen for an experienced and highly skilled clinician to join the Community Referral Hub (CRH) team as a Band 7 Senior Triage Clinician. This senior clinical role combines expert assessment, triage, decision-making and clinical leadership within a fast-paced service.
The CRH provides a centralised route into community services, supporting timely access to care and helping prevent avoidable deterioration, hospital attendance and admission. The post holder will undertake predominantly telephone-based clinical assessment and triage to identify risk, determine urgency and direct patients to the most appropriate care pathway.
Working autonomously, the Senior Triage Clinician will manage referrals for patients with a wide range of needs, including increasing acuity, complexity, frailty and multiple health and social care requirements. The role requires the ability to recognise deterioration and red flags, prioritise referrals effectively and escalate concerns where urgent or emergency intervention is required.
The post holder will contribute to the development of clinical pathways, triage processes and service standards, supporting clinical governance, quality improvement and evidence-based practice.

This is a new and exciting opportunity as the supporting role for both Reception and Administration and is for someone who would like a varied role across the 2 teams. Its fast paced and no 2 days are the same.
Main duties of the job

Answering the phones and dealing with emails, patient enquires, meeting and greeting, dealing with numerous administration requirements and tasks from GPs, hospital, out of hours, blood results, coroner, patients requests, sending and receiving referrals and work flowing letters via scanning and eventually read coding. Your working hours will vary from week to week depending on business need. 30 hours post will allow one day per week off not fixed and 37.5 hours working 5 days per week
You will be given appropriate training (a lot of which will be on the job training as we have found this works best) and you will get formal training such as Medical Terminology and Read Coding when it is available.
There are no current anti social hours or weekend working which is subject to change if the General Medical Services contract changes currently not mentioned or requested buy the governing bodies.

The Respiratory Healthcare Support Worker will support the delivery of a high-quality, patient-centred Community Respiratory Service across Powys, contributing to the assessment, diagnosis, monitoring and management of patients with respiratory conditions. The post holder will play a key role in supporting the provision of respiratory services across community, clinic and domiciliary settings, helping to improve patient access to care closer to home. This will include assisting with respiratory assessments, spirometry and other respiratory diagnostic testing, monitoring patients with long-term respiratory conditions such as COPD, asthma, bronchiectasis and interstitial lung disease (ILD), and supporting the delivery of evidence-based care and treatment plans.
The role will involve preparing patients for assessment and diagnostic procedures, accurately recording clinical observations and test results, maintaining equipment and consumables, supporting virtual and face-to-face clinics, and assisting with patient education and self-management programmes. The post holder will contribute to the delivery of pulmonary rehabilitation, oxygen assessment and review services, smoking cessation support, exacerbation monitoring, and remote monitoring pathways where appropriate.

The national maternity and neonatal landscape is changing quickly. Key drivers include the Ockenden reports, the National Maternity Review, the Thirlwall Inquiry, the Maternity and Neonatal 10 Point Plan, and the NHS Resolution Maternity Incentive Scheme. The postholder will be central to making sure our service responds to these. Main duties of the job

Job summary
This post is highly suitable for candidates with subspecialty training in Maternal-Fetal Medicine who wish to contribute to both maternal and fetal medicine services, including the diagnosis and management of fetal abnormalities and disorders of fetal growth. The appointed individual will join the obstetric consultant on-call team and contribute to the delivery of high-quality care for women with complex pregnancies. We are seeking a compassionate and motivated Hospice Doctor to join our dedicated, experienced and welcoming multidisciplinary team. This is an exciting opportunity to develop your experience in palliative medicine while making a meaningful difference to patients and families living with life-limiting illness. The successful candidate will work regular clinical sessions within our Hospice Outreach Team and participate in the non-resident evening and weekend on-call rota.
Weston Hospicecare provides expert care, guidance and support for people living with life-limiting illnesses and those important to them. Our aim is to help people live as well as possible, through high-quality specialist palliative care delivered by a skilled multidisciplinary team.
The post holder will support the Hospice Outreach Team, providing specialist palliative care advice and support to patients, relatives and healthcare professionals, and responding to urgent clinical scenarios and crises in the community. They will work closely with the hospice community clinical nurse specialists with support from the medical consultants and collaborating with the wider multidisciplinary team.
We welcome applications from doctors who demonstrate a strong interest in palliative medicine and a commitment to delivering compassionate, person-centred care. Previous experience in palliative care is desirable but not essential. The post is suitable for doctors wishing to gain experience in palliative medicine alongside another clinical role, in particular (but not limited to) GPs or senior GP trainees seeking to develop or continue a specialist interest.

About the Role
As a Registered Nurse - Outpatients, you will play an important role in delivering safe, high-quality care across a varied outpatient caseload.
You'll work closely with Consultants and multidisciplinary colleagues, carrying out patient assessments, supporting clinics and procedures, performing ECGs and undertaking wound care, including minor operations dressings. You'll help ensure every patient receives a professional, efficient and positive experience whilst maintaining the high clinical standards expected within our hospital.
This role offers variety, autonomy and regular patient interaction, making it ideal for a nurse who enjoys a fast-paced outpatient environment and takes pride in delivering exceptional patient care.

We are seeking to appoint a proactive Band 5 Occupational Therapist to join our current dynamic and motivated group of therapists on the rotational scheme.
You will be based within a specialist team at one of our locations within the Trust providing Occupational Therapy assessment and intervention for patients within the defined specialist clinical area, gaining experience of working in a variety of settings and working as an integral member of the multidisciplinary team.
The variety of specialities included on the Band 5 rotation include Medicine (Including Older People's Medicine) Surgery (Vascular, Plastics and General Surgery), Orthopaedics (Trauma and Elective), Major Trauma Rehabilitation, , Paediatrics, Neurosciences, Hyper acute Stroke and Stroke rehabilitation, Adult Oncology .
Band 5 rotational posts are designed to consolidate and build on graduate skills and academic knowledge, giving the newly qualified occupational therapist a broad practical experience of a range of patients and conditions and areas of work.
