Job summary
The post holder will ensure the effective delivery of the Patient Safety Incident Response Framework (PSIRF), whilst maintaining robust governance, assurance and oversight arrangements for all patient safety incidents reported across the Trust.
Working closely with clinical and corporate leaders, the post holder will lead systems that support incident reporting, learning responses, risk identification, assurance and organisational learning. They will use data and intelligence to identify emerging risks, monitor performance and ensure that learning is translated into measurable improvements in patient care and safety.
The role requires an experienced leader with expertise in patient safety, governance and risk management, supported by strong analytical and digital skills, capable of using data systems and business intelligence tools to provide assurance and drive improvement.

Main duties of the job

Additional Responsibilities
- Act as deputy to the Associate Director of Quality for Patient Safety and Risk, providing visible leadership, strategic support and operational continuity in their absence.
- Lead and manage the day-to-day delivery of the Trust's patient safety and risk management functions, ensuring effective oversight of incident reporting, triage, investigation, learning responses, risk management and action tracking.
- Support the Associate Director in the recruitment, induction, appraisal, training and professional development of staff within the Patient Safety and Risk team, fostering a culture of continuous improvement, learning and high performance.
- Provide expert leadership and direction to the Patient Safety and Risk team, ensuring workloads, priorities and resources are effectively managed to meet organisational and statutory requirements.
- Quality assure the outputs and deliverables produced by the team, ensuring investigations, reports, learning responses, risk assessments and committee papers are accurate, evidence-based, timely and of a consistently high standard.
- Oversee systems for monitoring incident trends, investigation quality, action completion, thematic learning, emerging risks and organisational compliance, ensuring timely escalation where concerns are identified.
- Support the ongoing optimisation of patient safety information systems and reporting processes, ensuring information is translated into meaningful intelligence that supports organisational assurance and continuous improvement.
- Lead the design, delivery and evaluation of education, training and development programmes relating to patient safety, PSIRF, incident management and risk management, developing resources and presentations tailored to the needs of clinical and non-clinical staff.
- Prepare and present high-quality reports, assurance papers, dashboards and briefings for Trust committees, executive leadership meetings and Board-level forums, providing clear assurance regarding patient safety performance, risk, investigations, organisational learning and compliance with statutory and regulatory requirements.
- Provide expert advice and guidance to senior leaders, managers and clinicians on patient safety incidents, investigations, governance requirements, organisational learning and risk management.
- Work collaboratively with operational, clinical and corporate teams to ensure learning arising from incidents, complaints, claims, inquests and risk reviews is embedded into practice and translated into measurable improvements in patient care.
- Represent the Trust at external meetings, networks and forums, acting as an ambassador for UHCW's approach to patient safety, organisational learning and risk management, while maintaining awareness of national developments and best practice.
- Ensure high standards of data quality, validation and governance across all patient safety and risk datasets.
- Use advanced analysis of incident, harm, mortality and risk data to identify trends, emerging risks and opportunities for improvement.
- Promote the effective use of digital tools, including incident management systems, Excel-based analysis, business intelligence platforms and reporting solutions, to strengthen organisational oversight and assurance.

This is an exciting opportunity to work within our translational research depression clinic, supporting the delivery of research studies and helping to develop new approaches to the assessment and treatment of people experiencing depression.
Youll be part of a supportive, multidisciplinary research environment, working alongside experienced clinicians, researchers and academic partners. The role offers opportunities to develop your research skills, contribute to publications and presentations, and gain valuable experience in clinical research and Good Clinical Practice.
With strong clinical and academic support, this one-year opportunity is ideal for a motivated doctor looking to build experience in research alongside their clinical career.
Main duties of the job
You will support the delivery of clinical research across Devon Partnership NHS Trust, with a particular focus on mood disorders and our translational depression clinic.
Key responsibilities include supporting participant recruitment and follow-up, contributing to clinical assessments, data collection and analysis, and helping to deliver research projects to Good Clinical Practice standards.
You will work closely with clinical teams, the Research & Development team and academic partners, with opportunities to contribute to audit, quality improvement, conference presentations and academic publications.
The role is supported by experienced research and clinical colleagues, with administrative support from the R&D team. You will also have opportunities to become involved in wider mood disorder research and developing research activity across the Trust.

An exciting opportunity has arisen on Ward 36 for a Health Care Assistant to join the renal dialysis nursing team at Arrowe Park Hospital.
We are a busy unit providing outpatient and inpatient haemodialysis for around 90 patients, over 3 shifts, 6 days a week.
We are looking to recruit highly motivated individuals who play an integral role at all stages of a patient's care. As a Health Care Assistant, you will be a valuable part of the nursing team. You will find yourself assisting patients with hygiene and comfort needs, taking blood pressure and pulse rates, communicating with colleagues and patients and record keeping.

Ward 27 is a 29 bedded Acute Medical Ward for Care of the Elderly patients.
A Clinical Support Worker (CSW) is responsible for providing healthcare to patients and supporting with clinical duties under the supervision of Registered Nurses, doctors and other registered healthcare professionals.

Wirral Women and Children's Hospital provides care for approximately 3,300 low and high-risk women per year, encompassed within the unit is a Level 3 Unit for Neonatal Services, Children's Services and Women's Unit.

Main duties of the job
The Delivery Suite Co-ordinator will become part of an established team of Band 7 midwives.
Their role includes shift leading on the main Delivery Suite area, the alongside Midwifery Led Unit (Eden Suite), the Induction of Labour bay (Harmony Suite) and Maternity Triage or working in the midwifery numbers on the delivery suite on a shift by shift basis.
The successful candidate will work leading teams of rotational midwives and support staff that work across the maternity unit each shift, working closely with the multidisciplinary teams across maternity to provide safe, high quality care.
You will work closely with the Delivery Suite Manager to provide management support for the effective running of the Delivery Suite and providing leadership and clinical care in Maternity Triage.
Out of hours the Co-ordinator will hold the maternity bleep and manage unit acuity and staffing across maternity.

An exciting opportunity has arisen for a motivated Band 6 Clinical Nurse Specialist with an interest in respiratory care to join our expanding respiratory service within Frimley Health, based at King Edward VII Hospital, covering East Berkshire.
You will deliver specialist, patient-centred respiratory care as part of a multidisciplinary team including Respiratory Consultants, Physiotherapists, Advanced Clinical Practitioners, Clinical Nurse Specialists, HCAs and Therapy Support Workers. This demanding yet rewarding role offers varied experience to develop your clinical skills and professional portfolio.
The post holder will assess and manage patients with respiratory conditions including COPD, Bronchiectasis, Interstitial Lung Disease, Asthma and those requiring oxygen therapy. You will support holistic assessment, care planning and delivery of individualised treatment across hospital and community settings.
You will work with GPs, Urgent Care Teams, Community Nurses and Palliative Care Services to ensure integrated care pathways.

An exciting opportunity has arisen for a Registered Nurse to join our busy Endoscopy Team. Experience of working in Endoscopy is desirable but not essential.
The Unit is a modern, well equipped, 7 room, JAG accredited Endoscopy Unit. A capital build to improve on existing facilities was completed in September 2014. Our Endoscopy Unit performs in excess of 17,000 procedures per annum across a full range of gastrointestinal and respiratory endoscopy.
The successful candidate will support the Consultant Gastroenterologists, Consultant Surgeons and our team of nurse endoscopists to provide safe care and treatment for patients during their time on the Unit.
This will involve assisting the endoscopist during endoscopic procedures and caring for patients before, during and after their procedures.
The Unit operates across 6 days a week including Bank Holidays (Excluding Christmas Day) Shifts are based between the hours of 8am-7pm. Saturdays/Bank holidays are rostered with a day off in lieu.

We are looking for a dedicated Band 3 Health Care Support Worker to join our team on the Childrens Ward at Worcestershire Royal Hospital. An exciting opportunity has arisen on Ward 19 to join the senior nursing team, as we will be extending to 28 medical beds as we head into the winter period.
Within ward 19, we work closely as a multidisciplinary team to provide care and support to our patients who experience a diverse range of medical conditions. Main duties of the job
Provide high-quality administrative support to the Urology team. Coordinate appointments, investigations, results, and patient pathways. Monitor cancer and RTT targets, escalating delays or capacity concerns. Act as a key contact for patients, carers, clinicians, GPs, and other services. Maintain accurate patient records and clinical correspondence. Support efficient clinic operations and assist with training and service improvement. Please refer to JD for further details.

The Financial Services team at Imperial are a major component of the overall Finance function. We cover financial and capital accounting, financial governance, transactional services, finance systems and payroll.We also play important roles in the relationships with key outsourced functions including internal audit, counter fraud and procurement. Main duties of the job
- The Financial Controller is a key leadership role in the Finance Directorate, part of the Finance Senior Leadership Team and a professional expert on finance, with responsibility for overseeing and managing the financial transactions, services and core finance systems for the Trust.
- The Financial Controller provides highly complex, sensitive and contentious financial information to staff at all levels from financial and non-financial backgrounds, and provide expertise in complex technical areas including cash-flow forecasting, capital accounting, VAT, and the application of financial reporting standards.
- The Financial Controller responsibilities are distributed across the two posts as outlined below. However, each postholder is expected to perform all functions within the JD. Cove ward is a 17 bedded male only inpatient ward and this post will require assessment and management of a wide range of psychiatric presentations.

Main duties of the job
The Breast Unit is a busy department working within both the National Breast Screening service and a Symptomatic Breast service. The ability to work both in a team and independently, and to communicate in a wide variety of settings, is essential.
The Newcastle Breast Screening Service delivers a screening service to a large rural area therefore a current driving licence and access to a car is essential.
Applicants for the above post must hold a degree in Diagnostic or Therapeutic Radiography, be HCPC registered and have working knowledge of NHS policies and procedures. A postgraduate qualification in Mammographic studies or equivalent as approved by NHS Breast Screening Programme is also essential.

Job summary
The role of Specialty Training Programme Director for or Pre-Hospital Emergency Medicine (PHEM) will work with and support the Head of School for Medicine in leading the delivery of a wide range of functions, aligned to the NHS England mandate and long-term workforce plan. A PHEM TPD will be responsible for coordinating the PHEM training programme within each region. The programme will usually be hosted by the School of Emergency Medicine or the School of Anaesthesia. We would welcome applications from candidates across the South West region.
The rolewill focus upon improvement of learner supervision, assessment and experience, engagement of faculty and ensuring effective educational outcomes, both now and in the future.
The appointment will be made on the basis of a part-time secondment to NHS England, alongside existing employment with an NHS Trust in the South West geographical footprint.
This is a senior educator position appointed on a secondment basis until 31 October 2027.

The ideal candidate will be an experienced clinician with a thorough understanding of education and training across the region. They will also have strong leadership and communication skills and be able to work effectively as a member of a multi-professional team.

An exciting opportunity has arisen for a dynamic individual to join the Virtual Frailty Ward as an Advanced Complex Care Practitioner, providing care across Scarborough, Whitby and Ryedale. We are looking for a skilled, organised, and dynamic Nurse - AHP- with community experience to recruit as an Advanced Complex Care Practitioner (ACCP). You will need to be hard-working, patient focused, passionate, driven, with a problem-solving approach to patient and team management.
You will work in an integrated multidisciplinary team, including community nursing, therapies, also working closely with acute care, social care, GPs and voluntary sector colleagues to deliver individualised, safe and effective, high-quality care at home.
The post holder will proactively identify service users' needs, be able to quickly identify a deteriorating patient, have knowledge and experience of caring for those with complex long-term conditions within their home environment understanding the experiences of patients and their families. With the overall aim of supporting patients to remain at home or in their normal place of residence, avoiding unnecessary hospital admissions. Also facilitating early discharge where patients have been hospitalised.
You will work as an autonomous practitioner and will be confident working collaboratively with colleagues across a wide range of professions and organisations.
Main duties of the job

There will be an excellent induction program and clinical supervision support. You will be encouraged to develop expert skills in the area in order to take a lead role in clinical practice issues, raising the profile of the service and offering specialist advice to planning and service development teams in order to assist in the implementation of service improvements.
ACCPs manage a complex caseload of service users providing:
- Robust clinical triage identifying appropriate referrals from across multiple partner organisations and through agreed clinical diversionary pathways.
- Advanced holistic clinical assessments, person-centred care plans, treatment escalation plans, and complex treatment in the community setting.
- Coordination of the integrated multidisciplinary team and liaison with partners in wider health, social care and voluntary sector to support admission avoidance where clinically safe
- Assist the service user to navigate health and care systems to achieve successful outcomes
- Support technology focused patient care, where appropriate for patient care to be safely and effectively at home
