Job summary
Lincolnshire Partnership Foundation NHS Trust (LPFT) is a Talking Therapies service. The opportunity has arisen for Psychological Wellbeing Practitioners based across county. The Service provides evidence-based interventions within a stepped care model, for the treatment of people with mild to moderate common mental health problems.
This is an exciting opportunity to be part of a major national initiative, which has fundamentally changed the way in which psychological treatments are provided. Post holders will be expected to work at least one evening session a week and possibly Saturdays in the future. The Psychological Wellbeing Practitioner will work as part of a multi-disciplinary team to provide, assessment and treatment of common mental health problems based on the NICE guidance. These include computerised cognitive behavioural therapy, psycho-educational groups and guided self-help; the majority of interventions will be based on a cognitive behavioural therapy (CBT) approach. Interventions may be delivered as group work or individual work through telephone contact, computer messaging, email or face-to-face. Regular case management and clinical skills supervision will be provided by qualified supervisors.
The service has a strong commitment towards Continuing Professional Development (CPD), staff wellbeing and providing quality services.

The site currently offers a 24-hour service and shift patterns
Monday to Sunday, 7.30am to 7.30pm and 7.30pm to 7.30am

This position is full time, youll receive an annual salary of £34,428 - £41,659 depending on experience.

Main duties of the job
As a Primary Care Nurse your responsibilities will vary. You will
- Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
- Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
- Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
- Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
- Deliver non-judgemental care that makes a real difference
- Inspire excellence by forging strong links with our partners and stakeholders
- Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.

We are looking for a Physiotherapist with excellent communication skills to join the Physiotherapy team at Dartford & Gravesham NHS Trust. You will be based at Darent Valley Hospital. St George's Healthcare NHS Trust is one of the largest healthcare facilities in the United Kingdom with over 1100 beds. Based in Tooting, it treats 80,000 inpatients/day cases and over 300,000 outpatients annually. The Trust employs more than 5000 people, including 700 medical and dental staff. In 2003, the neurosciences services based at Atkinson Morley's Hospital in Wimbledon relocated to the Tooting site. Main duties of the job
To provide a clinical service in respiratory medicine as part of a team of respiratory specialists.

Applications are invited for a part-time substantive Consultant appointment in the Lipid Service (1PA) at Cambridge University Hospitals NHS Foundation Trust (Addenbrooke's Hospital) on the Cambridge Biomedical Campus.

Main duties of the job
The successful candidate will have the opportunity to participate in the development and expansion of our nationally recognised Lipid Service. The appointee will be expected to contribute to out-patient duties in the Institute of Metabolic Science of the University of Cambridge. There is no on-call rota.
The successful applicant will be able to offer an expert opinion on a range of dyslipidaemias and the management of atherosclerotic cardiovascular risk more generally and be expected to monitor patients receiving any of the lipid modifying agents including the injectable treatments. You will be expected to contribute to the weekly Lipid Clinic meeting and to participate in the teaching and training of junior medical staff, medical students and other healthcare professionals.

We have an exciting opportunity for someone to join our Waiting List Team, based at the Queen Elizabeth Hospital, Gateshead. As a member of the team, you will assist with managing waiting lists for elective surgery and working in accordance with Government Access targets to ensure patients are treated in line with Trust Access policies.

Job summary
We are looking for a compassionate and motivated Senior Support Workerto join ourChildren and Young People's Eating Disorders Team. This is a unique opportunity to support young people and their families through recovery while working as part of a supportive multi-disciplinary team.
In this role, you will work directly with children and young people experiencing eating disorders, helping them and their families build healthier routines, improve wellbeing and remain supported within their home and community.
What You'll Do
- Support children and young people and their families with eating disorder recovery plans
- Help stabilise eating patterns and develop positive routines
- Work alongside clinicians to deliver agreed care plans and interventions
- Monitor and record physical health observations such as blood pressure, pulse, temperature and weight
- Complete blood tests & ECG's
- Support psychoeducation groups and family interventions
- Identify changes in wellbeing and escalate concerns appropriately
- Encourage young people to engage in education, activities and community life
What We're Looking For
You'll be someone who is compassionate, organised and confident working with young people and families in a community setting.
Experience in eating disorders and physical health monitoring is helpful including Bloods & ECG, butfull training can be provided for the right candidate.
The role will involve travel across South Staffordshire, access to a vehicle will be required.
Main duties of the job
- Support young people with daily routines, mealtimes, and structured therapeutic activities.
- Provide practical and emotional support to young people and families in line with individual care plans.
- Assist in delivery of therapeutic interventions and psychoeducational sessions under the supervision of qualified staff.
- Promote recovery, independence, and positive coping strategies.
- Observe, monitor, and report any changes in presentation, behaviour, or physical health to senior staff.

An exciting opportunity has arisen for a Team Administrator, to work in the Staffordshire Planned Care Teams, supporting the East Staffordshire Heart Failure team.
We are looking for a highly motivated, enthusiastic, and proactive individual to support our Heart failure service covering the Burton and Uttoxeter areas.

The Children's Community Learning Disability team in South Staffordshire are expanding and havean exciting opportunity for a creative and innovative Specialist Nurse who has experience of working with children who have a learning disability and their families. In the role you will have the opportunity to work alongside an experienced group of Learning Disability Nurses, Psychologists, Occupational Therapist, SLT and Psychiatrist.
In MPFT Children and Families Mental Health Function, we work with young people who have a wide range of needs using an inclusive, person-centred approach. Our model places our young people's needs at the heart of what we offer to ensure that they are getting the best care possible. The Children's Learning Disability team offers: support and intervention for young people with emotional and physical healthcare needs who may be at risk of placement breakdown or hospital admission.
You will be responsible for the management of a complex caseload, which
includes, specialist assessment (within service specification), care planning in
conjunction with service user and carer, evaluation of individual treatment
plans for CYP with learning disabilities.
Main duties of the job
- Be responsible for the assessment of mental health and/or medical risk factors relating to children and young people, including co-morbidity (e.g. suicidality, self-harming behaviours and medical risk associated with mental health issues)
- To undertake physical health monitoring as appropriate i.e height, weight, blood pressure etc and be responsible for identifying appropriate action to be taken
- Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance.
- Complete and contribute robust and dynamic risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve and offer optimal quality of life and health outcomes
- Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
- Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process. Assist in ensuring that the aims and objectives of the service are fulfilled and to identify factors which may inhibit these from being achieved.

Then we would love for you to join our dynamic and supportive team of multi-disciplinary Sonographers here at Somerset NHS Foundation Trust. We are currently looking to recruit a highly motivated and professional sonographer to join our team. You can be either a general medical or obstetric sonographer or both. You will get the opportunity to perform all obstetric including Nuchal scans, general abdominal, gynaecological ultrasound examinations at Somerset NHS Foundation Trust and peripheral sites, working in line with Band 7 expectations in all areas of practice.
Our purpose built Diagnostic Ultrasound department has a fleet of flag ship GE equipment, a successful, more than 10 years, rolling Sonographer training program, 7 day affable Radiologist support and dedicated Interventional Ultrasound facilities.
Our refurbished Antenatal clinic is based adjacent to the antenatal clinic, providing Antenatal care to approximately 3500 women a year and has state of the art Canon and GE equipment in a 4 bed unit, providing an interdisciplinary fusion of Radiographer, Nurse and Midwife Sonographer led services.
Main duties of the job
As an Obstetric Sonographer, you will be an integral part of the maternity service and work closely with the Obstetric teams to provide exceptional care to all patients. This includes working alongside the ANC Screening team and FMC to deliver the FASP & achieve all KPI's within your practice.
As a General Medical Sonographer, you will be a strong team player, working as part of team undertaking a wide range of examinations. The Radiologists at MPH are approachable with an eagerness to facilitate learning and development to providing exceptional service to all patients.
You'll perform and report ultrasound examinations across both disciplines to a high standard in a professional manner, working within departmental guidelines whilst being accountable for your own practice. Reports are expected to be of high quality providing advice on future patient management to other clinicians.
Excellent communication skills are essential within both spheres, you will be expected to adapt your style of communication to meet the needs of a variety of situations, some of which can be distressing.

Main duties of the job
As Harm Reduction Team Leader, you will provide operational leadership, supervision and support to a team of Harm Reduction Workers delivering services across Hampshire.
You will oversee outreach, in-reach, pharmacy liaison and community-based harm reduction activity, ensuring services are safe, effective, responsive and delivered in line with contractual, organisational and quality standards.

Job summary
Salaried GP required to cover maternity leave. Fixed term 12 months.
Start date: Early November.
** 6 sessions per week over Wednesday, Thursday and Friday. **
**Requirement to be part of the oncall rota on alternate Friday mornings. **
** Requirement to carry out a Nursing Home ward round on alternate Thursdays. **
To include a maximum of x2 home visits per day when not on call.
Main duties of the job
In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations and emergencies, telephone consultations and queries, triaging, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, patient test results, patient medical reports and examinations (e.g. Insurance Companies) on behalf of the Practice, referral letters NHS/private, paperwork and correspondence in a timely manner;
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation;
Recording clear and contemporaneous I.T. based consultation notes to agreed standards;
Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible);
Prescribing in accordance with locally agreed or national guidelines.
In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
