Job summary
This is an exciting opportunity for an experienced Registered Mental Health Nurse (RMN) to join the psychosis team, in the assertive outreach pathway as a Band 6 Community Mental Health Nurse. The right candidate will be enthusiastic and a highly organised mental health professional who is able to provide high quality nursing care, assessment and treatment.
The Psychosis Team is inclusive of The Psychosis Pathway, Assertive Outreach, At Risk Mental State, and Early Intervention in Psychosis. This role will be primarily based in the assertive outreach pathway, however depending on service need, work across the psychosis team may be required.
Main duties of the job
The successful candidate will provide a comprehensive Community Mental Health Nursing Service across a whole range of clients experiencing mental ill health within the assertive outreach pathway. The service will be delivered in collaboration with representatives from other agencies and therefore the ability to work independently and with partner agencies is essential.
It is essential that applicants can demonstrate post registration experience as an RMN, which may include inpatient and community settings.

Working as part of a supportive multidisciplinary team, you will undertake comprehensive mental health assessments, develop and deliver person-centred care plans, coordinate care under the Care Programme Approach (CPA), and provide evidence-based interventions that promote recovery, independence and social inclusion. You will work closely with service users, carers, primary care colleagues and partner agencies to ensure holistic and effective care is provided.

We are looking to recruit existing or new graduate Band 5 Occupational Therapist onto our physical specialities rotation at South Tyneside and Sunderland Foundation Trust. This is a great opportunity to develop clinical skills and develop knowledge of a wide range of conditions within several different teams between Sunderland Royal Hospital, South Tyneside Hospital and Recovery at Home. Our teams are large, friendly and supportive and supervision will be provided by a senior therapist. We have an established Band 5 support group who plan their own CPD activities and provide peer support, and a 12-month preceptorship programme to support development of essential skills and knowledge. Main duties of the job
Our physical specialities rotation provides the opportunity to experience therapy intervention with patients throughout the whole acute patient journey. Our well-established Interface Team work into ED and EAU to help prevent hospital admission and also to facilitate safe and timely discharges from our short stay areas; our inpatient OT teams assess patients for discharge from base wards and our Recovery at Home Therapy Teams assess and work with patients at home on discharge from hospital, as well as responding to situations to help prevent admission to hospital.
With each rotational area, you will hold your own caseload of patients and will be responsible for assessing the functional needs of your patients to either prevent admission to hospital or facilitate discharge from hospital. You will also be assessing for and prescribing aids, equipment and wheelchairs, and liaising with partner agencies whilst adapting your core skills to the patient group or speciality you are working in. You will also have the opportunity to develop your manual handling skills and your knowledge of specialist equipment.
Due to the need to visit patients homes as part of assessment and treatment interventions, you will need to have a valid UK driving licence and be able to drive our department vehicles in the acute teams at Sunderland Royal, and have access to a vehicle for work purposes for the Recovery at Home rotations.

We are looking for a highly motivated Registered Nurse to join our Specialist Nurse Service. The team provides a 24/7 Specialist Stroke Nurse service based at the Calderdale Royal Hospital. Hello, I'm Lisa and I am looking for Urgent Care Practitioners to join the Mental Health Liaison team who can bring knowledge and expertise to this dynamic area of mental health service provision.
You will be working at the interface between physical and psychological health and bring your knowledge and expertise to your general hospital colleagues. You will undertake specialist and timely mental health assessments and treatment for patients attending the General Hospital. We support patients with a wide range of problems, including self-harm, adjustment to illness and physical and psychological co-morbidities. As there is no entrance requirement for A&E and people's perceptions of what constitutes a mental health emergency or crisis can vary, our team are referred people with all sorts of problems, including those people presenting to mental health services for the very first time.
This post offers the Trust's £2,000 Golden Hello or Relocation Incentive for Band 6 Nurses & Practitioners, (pro rata and subject to terms and conditions; please see attached Protocol Document for full details).
Main duties of the job
I am looking for practitioners who are confident in the management of common mental health problems and who can bring their knowledge to their general hospital colleagues both informally and by undertaking teaching sessions. We have recently merged with our Older Persons Liaison Team so are now providing assessments and support to anyone over the age of 18.
You will need excellent communication skills and be able to foster excellent relationships with patients, carers, general hospital colleagues and other parts of the health and social care system.
This is a challenging but rewarding role, offering an exciting opportunity to nurses, occupational therapists or social workers who have strong assessment and therapeutic skills. You will need the ability to work autonomously whilst still contributing to the team as a whole. If you enjoy the challenge and stimulus of working under pressure and are able to remain calm in quickly changing circumstances, you will be well suited to this role.
About us
As our Coastal Urgent Care Pathway moves towards CORE 24 standards, the post-holder will work flexible hours to support delivery of 24/7 services. It is essential that you are able to work on a shift pattern to include nights.
This role will support the work of and across Urgent Care Teams (Crisis Resolution Home Treatment Team and The Haven) and as such the post holder will be expected to work across these teams as required. Our three services are integral to our Urgent Care Pathway and ensure people presenting to acute mental health services receive a compassionate, person- centred assessment and timely care plan whether this occurs in an acute setting such as The Haven; the General Hospital or in their home.

Kimberlow Hill Surgery is open 8am-8pm Monday Thursday, 8am-6pm Friday and 9am-1pm Saturday meaning there is a lot of flexibility with hours. Wenlock Terrace surgery is open 8am-6pm Monday Friday. We work on a rolling shift pattern, at interview your hours will be discussed, we will then propose some hours to you, once accepted these will be your hours going forward. Overtime is available.
The whole practice is one big team here at Unity Health and we often arrange social events outside of work to get together and enjoy each other's company away from work.

Main duties of the job
As a Band 6 Specialist Nurse, you will provide specialist assessment, care coordination and therapeutic interventions for adults with complex mental health needs within the community. The post holder will manage a caseload of people with complex mental health presentations and will need to be able to prioritise their time effectively and maintain a degree of flexibility around their time management.
