The Care Navigator is primarily responsible for undertaking a wide range of reception duties and the provision of general support to the wider multi-professional team. A sound professional working relationship will be required with the GPs and staff working within the Practice. We are expanding the reception team and the role requires high levels of organisation and excellent interpersonal skills.
The role includes:
- Greeting and signposting patients
- Effective use of the appointments system, booking appointments and processing information
- Acting as the central point of contact for patients
- Liaising with multi-professional team members and external agencies such as secondary care
- Familiarity with Docman and EMIS Web clinical software systems
Main duties of the job
The post requires excellent communication skills, high levels of organisation and attention to detail as well as the ability to exercise tact, discretion and initiative. The ability to work under pressure and as part of a team are equally important.
The Practice will support continuing education and professional development in the form of attendance at study days, courses and with in-house training, as deemed appropriate for the benefit of both the individual and the Practice.

An opportunity has arisen within the Rocket demand team to work within the COPD and Oxygen nurse specialist services.
Working across primary and secondary care the nurse specialist will act as the clinical expert and professional resource to the multi professional team, patients and carers within the specialist service.
The post holder will work alongside the rocket, oxygen and virtual ward teams to ensure the development and delivery of a high quality service for patients a requiring a respiratory nurse assessment. Providing education, assessment, monitoring and follow up for patients with COPD and those requiring home oxygen. This will include the management of a case load of patients and the provision of specialist advise within the clinical area. The role will include the provision of ward based assessments, advice and management of patients being discharged under the COPD/O2 team and home visits. To provide support and education for existing service users.
The post holder will act a clinical leader and role model for the ongoing development of clinical practice, research and standards of care. Effective liaison and good communication skills are essential to the post, a good knowledge of respiratory conditions and a degree module at diploma or degree level would be an advantage.
Having a valid manual driving licence is essential.
Main duties of the job
The main aspect of this role relates to providing an excellent level of care in the assessment and management of individuals referred to the COPD/Oxygen Nurse services.

To have knowledge of COPD and oxygen patients to be able to assist with virtual wards.
As part of the multidisciplinary team, the post-holder will be alert to national guidelines and clinical pathways for both new and chronic presentations of respiratory conditions
As such, the post holder will be reviewing patients referred to the Oxygen/COPD team within the acute setting. In addition, the post-holder will manage their own caseload of patients supported by a senior respiratory nurse or consultant.
The post holder will be confident in the management of individuals with respiratory conditions. This will include patient reviews in their own homes, delivering individualised care, facilitating lifestyle changes through education and support to patients and carers.
A Full driving licence is required for this post as community work is involved.

Francis Willis Unit is the only low secure unit within Lincolnshire Partnership Foundation Trust. It is a 15-bedded male-only low secure unit supporting patients who have a forensic history. The ward is for adults between the ages of 18-65 who are experiencing severe and enduring mental health difficulties.

Job summary
Our Neonatal Unit is looking for a highly motivated, enthusiastic individual to join our friendly and well established Neonatal team as a QIS Band 6 Deputy Sister/Deputy Charge Nurse.
The post holder must be 1 year post Neonatal QIS qualification.
The post holder will be responsible for the assessment of care needs and the development, implementation and evaluation of care required for the sick neonate and their family. In addition the post holder will participate in the day to day management of the unit, providing support and leadership in the absence of a Clinical Sister.

Main duties of the job
Our Neonatal Unit is looking for a highly motivated, enthusiastic individual to join our friendly and well established Neonatal team as a QIS Band 6 Deputy Sister/Deputy Charge Nurse.
We are a Level 2 Local Neonatal Unit, with 16 cots providing care for up to 2 Intensive Care, 6 High Dependency, 8 Special Care neonates and 4 TC.
This role requires a passionate, highly motivated and experienced neonatal trained nurse with excellent communication skills who wishes to continue to develop their clinical and leadership skills.
Within this role you will provide clinical, professional leadership for the unit with responsibility for the delivery of high standards of care for all babies and their families who access our service.
As an experienced Neonatal Nurse you will be a role model providing support, sharing your knowledge and facilitating teaching and supervision to more junior members of staff and students.

Job summary
An exciting opportunity has arisen for an apprenticeship post as a Clinical Associate in Psychology (CAP) in one of our Neurodivergence Services. Clinical Associates in Psychology fill an identified skills gap between assistant psychologists and qualified clinical psychologists. They are able to practice semi-autonomously with appropriate support, working within their scope of practice, under the supervision of a registered psychologist.
Applications are invited for a Band 5 Clinical Associate in Psychology apprenticeship post.The expected start for the apprenticeship is November 2026. If there is a gap between starting in post and beginning the CAP training course, then successful candidates would initially start in a Band 5 Assistant Psychology role and will move onto the CAP apprentice role and job description at the start of the apprenticeship programme.The CAP Apprenticeship is an eighteen-month full-time training divided between work-based experience (up to 4 days per week) and academic teaching and study, which leads to a Masters level qualification.

Main duties of the job
CAPs are trained to work with a particular service user group and in this case with adults with suspected/ confirmed neurodivergence therefore providing a more proscribed range of assessments and interventions than Clinical Psychologists. We are seeking to recruit a trainee CAP to be based in the North Essex Autism service (Holmer Court, Colchester base as well as some remote working). Our Autism and ADHD services will though be combining across the Trust to form an Essex wide Neurodivergence Service so it is an exciting time to be joining the teams.
Those appointed will work alongside qualified Psychologists, and together with MDT colleagues including nursing staff, Occupational Therapists, Consultant Psychiatrists, other CAPs and Assistant Psychologists. The role will include screening referrals for diagnostic assessments for autism and/or ADHD, carrying out initial assessments of autism and/ or ADHD as well as developing skills and learning to support with formal diagnostic assessments and facilitating interventions including primarily groups and some individual work. If successful in their training, on qualification, CAPs can apply for a Band 6 CAP role.

We are looking for passionate, dynamic and inspired nurses to join us. Our Community Nursing teams are based in Osprey House, Exeter and we provide care across the locality of the city.
It is therefore vital you are an adaptive and flexible worker throughout the day. You are expected to work occasional weekends and bank holidays as we provide a 7-day service but this during the core business hours of the day.
How a Day in community Nursing looks;
You will start the day with the support of the daily virtual hub meeting (including all the nursing teams in your cluster) to plan an effective and safe list of patient visits for the day.
Then you meet later with your nursing team at patient safety handover to support each other through any concerns or advice for patient treatments or conditions as a team. Both of which are integral to our day to ensure staff work safely and within their own skill set and expertise.
The rest of your working day you could be lone working or supervising Unregistered or Student Nurses through their clinical skills, patient risk assessments and documentation.
Newly appointed staff will be provided with a comprehensive induction into Community Nursing and will work alongside other nursing staff to ensure they are supported into the post robustly.
Staff are supported with supervision both in team meetings or a one to one basis. This is also integral to our work and considered a priority to support staff at work both to develop professionally but also to support your health and wellbeing and provide a safe environment to discuss professional matters.

The work Community Nurses do is hugely varied so your portfolio of clinical skills can grow in complex and advanced nursing competence. Our core work includes Palliative care, supporting people at home with complex and long-term conditions, administering medications, advanced tissue viability and vascular access.
We work closely with the wider community MDT to promote admission avoidance and empowering patients to stay at home to receive their nursing care. We work very closely with our GP, Hospice, Physio and Occupational Therapy, Social Work, Voluntary and Urgent Care Services.

This Position is for 24 hours per week
We are a fun, friendly team of secretaries, and looking for someone to join us who is also a team player. This position will be based at Sandwell Hospital. We offer flexible working with the option for some working from home days each week.
The key minimum requirements for this position are the AMSPAR qualification, or a minimum of 2 years previous medical secretary experience.
Candidates should also have experience of managing 18 week pathways and waiting list management, plus knowledge and evidence of patient information systems and other relevant IT systems. Personal organisation, great communication skills and the ability to priorities workloads to juggle the ever changing priorities of the NHS are also essential, as is the experience of working in a fast paced environment.
Main duties of the job
Actively managing the administrative processes associated with patient pathways and contributing to the effectiveness of clinical teams (including patient experience, patient flow and waiting list management) for specialty teams.
Co-ordinating, navigating and tracking a caseload of patients efficiently and effectively through their care pathway i.e. outpatients, day case, diagnostics and inpatients from referral to treatment and discharge for Paediatrics.

Applications are invited for the post of Band 3 Administrator within Urgent Emergency Care which includes Care Coordination, Urgent Community Response, Virtual Wards in Solihull Community Services.
Division 7 focuses on the system working across Birmingham and Solihull between our health and social care partners. The Division brings together Solihull Community Services, our Discharge Hubs across all our acute hospital sites and UHBs intermediate care beds into a single management structure. Division 7 also works closely with Division 3 on the strategy and delivery of Older Persons Assessment and Liaison Team (OPAL), the multidisciplinary interface service based at our hospital's front doors.
The Solihull Urgent Community Response and Virtual Ward Team work as part of the Division to deliver Urgent Community Response, supporting patients within their own homes who are at risk of hospital admission to facilitate them being seen, assessed and treated at home. Virtual Wards aim to deliver intervention within the patient's usual place of residence to provide rehabilitation at home, supporting hospital avoidance and early supported discharge. The team work collaboratively with GPs, OPAL plus, West Midlands Ambulance Service, Solihull Community Nursing, Solihull Community therapy etc, with the aim to work as a multidisciplinary team to facilitate care delivered within the community.
Main duties of the job
We are looking to recruit an enthusiastic and committed administrator to join our friendly Admin team. This is a full time post which can be flexible within our shift times. The Admin Team support the Community Services teams based on Ward 10 at Solihull Hospital. We provide Admin cover between 8am and 6pm Monday to Friday and 9am -- 5pm Saturdays and Sundays. We also cover bank holidays throughout the year.
The role involves taking referrals over the telephone from GPs, Care Homes, other agencies and the general public. As part of the team, you will also undertake stock control and ordering, as well as supporting in meetings by taking minutes. You will work with the Admin Supervisor to support the Admin Team as well as the clinicians. You will need to be computer literate with excellent communication skills, passionate about working in the NHS and helping to support the local community.
This is a fantastic opportunity to join a friendly and supportive service. This is a very busy and dynamic work environment and as a Band 3 Clinical Administrator you will also have multiple responsibilities. Previous Reception / Clerical experience and customer service is essential.

Job summary
The Outpatient Appointments Administrator supports the Outpatient Appointments team by coordinating and managing patient appointments within a call centre and administrative environment. The role involves booking and monitoring outpatient appointments, managing patient referrals in line with key performance targets, and ensuring accurate data entry across hospital systems.
The post holder provides high-quality customer service to patients, clinicians, and colleagues, handling enquiries professionally and efficiently. They are responsible for prioritising workloads, maintaining data quality and confidentiality, and supporting the smooth running of administrative processes to facilitate timely patient care. The role also requires adherence to Trust policies, values, and standards, while contributing to a collaborative team environment.
This post holder may be required to work flexibility across both County Hall and Musgrove Park Hospital.
Main duties of the job
Book, amend and monitor outpatient appointments in line with speciality booking rules
Manage and track patient referrals using e-Referral systems and worklists
Provide a high standard of customer service when dealing with patients and staff via phone, email and face-to-face
Handle enquiries, messages and queries efficiently, escalating where appropriate
Maintain accurate data entry and ensure data quality across hospital systems
Support the rebooking of cancelled clinics under supervision
Ensure patient information is processed and available in a timely manner to support care
Follow Trust policies, particularly around confidentiality and data protection
Maintain organised administrative systems, including scanning, filing and handling confidential waste
Work collaboratively within a team, providing cross-cover and supporting colleagues as needed
Assist with training and induction of new staff where appropriate
