The Buckinghamshire Social Care Team is looking for an experienced Social Worker to join our social work team as an inpatient Social Worker across Buckinghamshire. Within Oxford Health we deliver mental health social care support within a partnership agreement with Buckinghamshire Council.
Your role will include working within a small, fast paced, supportive team, assessing inpatients under the Care Act to support timely, safe discharges into the community.
You will work with a small team of experienced social workers based at the Whiteleaf centre also assessing our out of area patients
We have a robust induction whereupon you will attend corporate induction days, you will be given time to undertake initial relevant training and have shadowing opportunities to help you settle into your role from the first day on the job.
Main duties of the job
Your duties will include:
- Work with adults, and their families, who are ready to be discharged from hospital, to form an understanding of their needs and wishes and develop strength-based plans.
- Understand and apply the relevant statutory frameworks to promote independence and autonomy while balancing risks and the needs of others including children and vulnerable adults.
- You will use relationship based Social Work skills to understand the needs of this adult service user group and work collaboratively to create care and support plans that reflect person-centred outcomes.
- To provide a high quality and effective professional approach, taking responsibility for working and managing a caseload within a framework of appropriate supervision, dependent upon the complexities of these cases and the social worker's experience.
- To collate holistic information to ensure appropriate, person-centred and supportive safeguarding decisions are made.
- Demonstrate and model good practice that reflects a commitment to relationship and strengths-based practice, working collaboratively with adults and their families.

To work as a member of the ward and community team, assisting the Midwifery staff with supporting the care needs of women and their babies. To undertake assigned tasks and appropriate interventions for women and their babies with minimal supervision but under the direction of the midwife.
Be able to work in a flexible manner regarding hospital/community placements and shift patterns/on-calls depending on the needs of the service. To provide leadership and support to junior colleagues and ensure high standards are maintained in a safe environment which promotes equitable and compassionate care.

- Works collaboratively with midwives, service users and the multidisciplinary team to provide quality care.

We are looking to expand our team with inspirational Associate Lecturers (ALs) who have a passion for delivering high-quality learning experiences along with substantial industry experience for an inspirational individual to join our medical ultrasound team as an Associate Lecturer (AL).
You must have relevant clinical experience in medical ultrasound, practicing as a Sonographer and be someone who can inspire, enthuse, and connect with our students and may be interested in combining this with your enthusiasm and ability to pass on your knowledge and experience by creating and delivering high-quality learning, teaching, and assessment.
You must have well developed and current knowledge and understanding of the critical, practical, and theoretical concepts of medical ultrasound practice and have had experience in clinical practice working as a Sonographer.
You must have good honours degree (i.e. UK 2.1 or 1st Class) in radiography or a relevant allied health profession or equivalent professional qualification, and a Post graduation Diploma (PgD) or MSc qualification in medical ultrasound.
Becoming an AL can be an excellent introduction to teaching in higher education. Some teaching experience would be an advantage; however, you may have well developed and current knowledge and understanding from an academic and/or professional perspectives.

We are seeking an experienced and compassionate Social Worker to join our hospital-based palliative care team. You will provide specialist psychosocial support to patients and their families living with serious or life-limiting illness, helping them navigate the emotional, social and practical challenges of their diagnosis.

Main duties of the job

Job summary
In this role the post holder will be taking a lead for the Occupational Therapy service in a further education college for people with autistic spectrum conditions and/or profound and multiple learning disabilities.

We have a recent and exciting opportunity to appoint Assistant Practitioners in the Islington Crisis Resolution Team and are looking for enthusiastic, committed individuals who enjoy working with acutely unwell service users with a broad range of mental health and social issues.
Covering the central London Borough of Camden & Islington, the team offers an intensive crisis assessment, resolution and home treatment service providing mental health crisis care to our local residents age 18 and upwards. Crisis home treatment promotes social inclusion and also supports the social network in caring for the individual in the least restrictive environment. The Crisis Team acts as an alternative to hospital admission. In order to fulfill this role, the team provides a fully operational 24 hour service, 365 days per year.
Main duties of the job
Successful applicants will be required to have proven and post qualification experience with desirable acute setting experience . You must be willing and committed to working as part of a 24-hour rota to include weekends. As most of our service user contact takes place within their own home, you must also be able to either drive (we utilise Trust vehicles) or to be able to travel independently via public transport. Although we do work as a team, the capacity and confidence to undertake lone working is essential.
In addition to face to face interventions (brief supportive counselling, crisis de-escalation, assessment), you will also possess strong telephone negotiation and support skills and be part of a rotating staff roster which operates Camden and Islington's crisis single point of access line
Here, our staff are able to successfully support the community by offering a clinically led mental health telephone line which triages crisis calls, helping to problem solve at this initial point of contact whilst allowing colleagues on the ground to focus on delivering quality face to face contacts.
Excellent verbal and written communication skills are an expected pre-requisite as is the capacity to utilise all forms of electronic communication. The Trust uses electronic record-keeping throughout all its services and we are working towards becoming a paper-free organisation.

This is an exciting time to join NLFT with the opportunity as a Senior Systemic and Family Psychotherapist to work with service users, their families and networks across Haringey's Psychological Therapies Pathways, Community Core Teams and Complex Emotional Needs Teams.
You will join a community of Systemic Family Psychotherapists working across Barnet, Enfield, Haringey, Camden, Islington and the Tavistock. We take pride in developing our CPD and training opportunities available locally and more widely, and alongside, regular supervision and support is provided.
With psychologists, psychological therapists and multi-disciplinary staff within the Core Community Teams, you will provide psychological/systemic input to the care of those requiring the highest level of support from mental health services. You will collaborate across different service, pathway and team contexts and develop a system of engagement that promotes a trauma informed, strength based focussed offer and treatment that can form the foundation for the sustainable recovery of service users, their family and carer networks.
Psychological/systemic assessments, consultation and direct work with the team and pathways regarding clients with complex presentations are provided, many of whom present with high risk and may be difficult to engage. You will be expected to provide training, supervision and consultation to other professional and non-professional disciplines, including trainees.
Main duties of the job
- Provision of psychological and systemic therapies covered by NICE guidelines to individuals, families, networks.

The Autism Advisory Team Speech and Language Therapist will be part of a newly established, specialist multi-disciplinary team created to strengthen autism-informed practice across inpatient and community mental health services in North Central London. Main duties of the job

Job summary
To lead on generic health advising within sexual health services, offering specialised advice and support in providing a comprehensive service for investigation, treatment and control of sexually transmitted infections (STIs) including HIV.
To provide advice, information, education counselling and provide partner notification (PN) to patients diagnosed with sexually transmitted infections, e.g. chlamydia, gonorrhoea, herpes, syphilis and hepatitis B and C. This service aims to impact positively on the experience of patients by providing high standards of care, advice and clinical practice, working across boundaries and in conjunction with primary and secondary care and other agencies.

Main duties of the job
Sexual Health in Bournemouth and Poole is part of a Pan-Dorset service. We are an integrated team, providing people in Dorset support for any sexual health need, including; but not restricted to; contraception, Genito-Urinary issues, termination advice, psycho-sexual counselling and HIV provision.
The Sexual Health Adviser, is an integral part of the team. They work closely with local labs and the results coordinator, to ensure all patients receive their correct results in a timely and efficient way. They have oversight of all positive results in the service. They a point of contact for those who require treatment, including support with their diagnosis, or support with telling partners about their infection by providing a safe and confidential space.
Liaison with all members of the team is important for best practice and evidence-based care, so excellent communication skills are required.
We are a teaching Trust so have many students from many disciplines, including medics, nursing, nurse associates and psychologists. Sexual Health is a popular choice for placements and spoke days, so skills with mentoring and supervision will be helpful.

Working on our acute inpatient wards at Highgate Mental Health Centre West, the post-holder will be supervised by the ward OT and will work with the ward team to co-ordinate a programme of meaningful ward activities based on service user choice and interests.
Training and a robust induction will be provided to support the post holder in this sometimes challenging environment and support role and professional development. Main duties of the job

The working pattern will comprise two shift patterns (Early 9-5, Late 12-8), as well as every third Saturday/Sunday (9-5).

Job responsibilities
The postholder will provide comprehensive clinical advice, assessment and case management to NHS Continuing Health Care and NHS Funded Nursing Care functions with particular responsibilities for people who have complex health care needs, this includes all categories of needs e.g. physical, end of life, Mental Health and Learning Disability needs.
The postholder will follow the process set out by the NHS National Framework for CHC & FNC to provide comprehensive assessment, case management, review and follow up on all CHC & FNC functions for the CHC Team. In addition, they will provide support and advice to other workers, MDTs, carers and patients as required in implementing the National Framework for CHC &FNC, contributing to the ongoing training of other colleagues as necessary.
The post holder will procure safe and appropriate care packages based on individual needs and support the Local Authority with jointly funded packages of care.
The postholder will be required to undertake complex assessments to identify eligibility for Continuing Health Care and Funded Nursing Care by completing the National Decision Support Tool. Gather evidence to support complex clinical judgements, which takes into account the knowledge and advice of other professionals and individuals / relatives views.
In addition, the postholder will assist with complaints, IRPs and retrospective cases from a clinical viewpoint.
The postholder will ensure risks are observed and actioned and are aware of the local governance arrangements and work within this when carrying out duties on behalf of the CHC team.
The postholder will be expected to keep accurate, high-quality documentation, ensuring information flow to the business and admin team is up to date in order that they can manage data and process payments.
The postholder will provide and receive highly complex clinical and social information relating to patients and explain this to MDT meetings, team meetings, other agencies patients and carers where there may be barriers to understanding and resistance to messages being accepted-i.e. if care cannot be recommended or funded.
The postholder will manage difficult relationships from other parties which arise out of case management, reporting difficulties and seeking support to manage as required. They will work flexibly within a team and communicate well with other colleagues, ensuring all records are comprehensive and that colleagues are briefed on potential issues, which may arise during any planned absence.

Job summary
The Respiratory Department at Sherwood Forest Hospitals currently has three base wards for in-patients at King's Mill Hospital with an eight-bedded respiratory support unit to allow acute and chronic non-invasive ventilation. Respiratory outpatient clinics are held at King's Mill and Newark Hospitals as well in the Ashfield and Mansfield Community Hospitals.
As well as general Respiratory out-patient clinics, we have also sub-specialist clinics for Lung Cancer, Interstitial Lung Disease, Sleep/Ventilators, COPD, TB and Bronchiectasis. The Respiratory Department sees approximately 2600 new outpatients per year and over 6400 follow up patients. There are bronchoscopy sessions at both King's Mill Hospital and in Newark. We also perform Endobronchial ultrasound, Thoracoscopy and indwelling pleural catheters at King's Mill. We support pulmonary rehabilitation and oxygen services at three sites across mid-Nottinghamshire.
The Respiratory department has excellent clinical support from 10 respiratory physiologists with access to full pulmonary function tests, exhaled nitric oxide, exercise tests and muscle strength tests. There are also facilities for in-patient and out-patient sleep studies, with 4000 patients under follow-up for sleep disorders. The Respiratory department also organises the community oxygen assessment programme.

Job summary
We are a very busy and friendly Radiology Department looking for a new member of the team to meet and greet patients in a professional, compassionate and efficient manner and to ensure that patients and their families are welcomed and kept updated during their stay with us in Radiology.
Main duties of the job
To meet and greet patients into the Radiology Department in a caring and compassionate manner remaining professional at all times.
To maintain records regarding admission and discharge on Radiology systems.
General clerical duties including daily printing and scanning of documentation.
To liaise with the wider team, other departments, GP's and colleagues in a professional manner.
Follow departmental or standard operational procedures.
Assist with induction of new staff.
Ensure the confidentiality of patients' records in line with Trust and Caldicott guidelines at all times.
Undertake any training that is required for the safe effective execution of the duties of the post.

- To be an ambassador for the Radiology Department by providing an efficient and professional reception service.
- To be responsible for maintaining the administration and storage of data, in accordance with local policies and procedures.
- To deal with enquiries from staff, GPs, various external agencies, members of the public and visitors to the service in person and over the telephone, referring appropriate queries to the relevant member of the Radiology Team when necessary.
- To communicate effectively with all members of staff and external agencies ensuring that a professional manner is always maintained.
- Providing information to the management team when requested and use initiative to escalate any issues as appropriate.
