We are seeking a skilled, motivated and compassionate Community Practitioner to join the Newcastle & Gateshead Step Up Hub. This is an exciting opportunity to work within a dynamic multidisciplinary team, supporting individuals to remain safely within their community and avoid unnecessary hospital admission. Applicants must be registered nurses with a current professional registration.
This post offers a rewarding mix of community and hospital in-reach responsibilities, with a focus on delivering high-quality, rehabilitation-oriented care.
In this role, you will provide care coordination and nursing support to service users with complex mental health needs across both community and inpatient settings. You'll play a crucial part in ensuring smooth transitions between hospital and community care, attending Mental Health Act tribunals and working closely with multidisciplinary teams.
You will need to be clinicallyexperienced, solution focussed and competent in working with individuals, their families, and carers. You should have a collaborative and enthusiastic approach as well as the passion and commitment to develop and promote recovery-based interventions for adults with mental health related needs.

Main duties of the job
- Deliver high-quality, person-centred mental health care in both community services
- Undertake comprehensive risk assessments and care planning
- Attend Mental Health Act tribunals and professionals' meetings as required
- Prepare formal clinical reports for tribunals
- Work collaboratively with inpatient teams to facilitate discharge planning and continuity of care
- Work collaboratively with your service users and relevant individuals/professionals they have involved in their care.
- Provide support and input during mental health crises and implement robust risk management plans
- Mentor and support junior staff, students, and colleagues
- Liaise effectively with GPs, social workers, carers, and other services

An interesting and varied vacancy has become available for a qualified Band 4 Registered Nursing Associate. The role will be in the Community Mental Health Team function within the wider Older Adult integrated Mental Health services.
The core responsibilities of the post will be to oversee and monitor your own caseload and support senior staff in delivering quality care and treatment programmes to patients with mental health challenges with a focus on supporting with monitoring physical health needs of patients. Supervision will be provided to support you in the role
The successful candidate will be enthusiastic about the role and, its development, and be driven to provide excellent mental health services to the local older adult population.
In addition to core professional skills, the post holder will also be expected to undertake a range of other generic clinical duties to support the team functions and be responsive to the needs of patients, their family and carers
Main duties of the job
- You will be a Registered Nursing Associate working within a multidisciplinary team and carry out a diverse range of duties.
- As a Band 4 Registered Nursing Associate within the team you will be regularly supported and supervised by a senior nurse and you will be encouraged to take personal responsibility for clinical supervision and your continued professional development plan.
- You will be required to undertake the administration of depot medications; however, these will not include medications deemed as high risk. You will also monitor for side effects and efficacy of other medications, including cognitive enhancers, and provide titration advice.
- You will actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and Code of Conduct. You are required to maintain links with your professional body and provide evidence for the revalidation process.

An opportunity has arisen to join a forward-thinking and expanding community adult speech and language therapy service. This role would suit and new or experienced band 6 with an interest acquired neurological disorders and service development.
Postgraduate training and experience in dysphagia and independent dysphagia management is a requirement of this post. You will have relevant experience in managing adults with acquired swallowing and communication difficulties.
You will have opportunities to contribute to audit, QI and service development and will receive regular clinical supervision, support through team meetings, monthly clinical forums and access to further CPD opportunities. Main duties of the job

Band 4 transitioning to Band 5 once all course modules are completed, full clinical hours are gained as part of the training year and notification has been received from the university of a successful pass. Main duties of the job
The Mental Health Support Team for schools and colleges in the Black Country will be supporting approximately 365 settings in 2027 and is designed to deliver three core functions:
Evidence-based interventions for mild-to-moderate mental health issues;
To support thesenior mental health leadin each school or college to introduce or developwhole school or college approach
Provide timely advice to school and college staff and liaise with external specialist service to help children and young people to get the right support and stay in education.
During your training year you will develop skills and competencies to deliver short term interventions to young people in schools and colleges throughout the localities where they demonstrate a need for support with their emotional health and wellbeing. Some community work will also be required dependent on the needs of the young person and their family and how the service expands and develops.

An opportunity has arisen to join our Adult Community Speech and Language Therapy (ASLT) team as a Bank Band 4 Adult Speech and Language Therapy Assistant in Bromley.
We are a supportive and friendly team who will work with adults with a range of communication and swallowing disorders including voice and dysfluency. We also have specialist SLT services such as an outpatient Fibreoptic Endoscopic Evaluation of swallowing (FEEs) service. The successful candidate will have the opportunity to support provision of SLT services to adults within the Bromley Healthcare services with a range of conditions such as Dementia, Parkinson's Disease and Motor Neurone disease.
This post would suit a motivated person with prior experience in an SLT team or assistant role, or someone looking to develop skills working as an Adult SLT assistant. Driving is an essential requirement of this job. You will be provided with training appropriate for the role and we have a highly developed internal supervision structure.

We offer a range of clinical fellowships, supervised by experts in their field. These include:

Main duties of the job
The post is for a clinical fellowship and will involve a majority of clinical sessions (~50%) in the chosen subspecialty. The post provides half a day per week (averaged) for research, education and management. Fellows will be encouraged and expected to contribute to the department through engagement in quality improvement, audits, teaching or research, and will be provided support. Within ELHT Maternity services, an exciting opportunity has arisen for a Fixed Term 12 month post as Midwifery Team Leader for the Burnley Birth Centre.This is a temporary Band 7 post, involving managerial duties, and clinical duties as required. The Community Hospice provides community, hospital and inpatient specialist palliative and supportive care to people with life limiting illnesses and their families within the London boroughs of Greenwich and Bexley.

The post holder will be responsible for participating in the aseptic manufacture of chemotherapy doses, checking volumes for other Aseptic staff, generating worksheets and labels using Epic - the trust's chosen electronic prescribing system, advising the Chief Pharmacy Technician and Associate Chief Pharmacist on amending work practices and documentation to improve the running of the service, maintaining equipment according to procedures and actively encouraging adherence to all procedures relating to the aseptic department and Good Manufacturing Practice.
Main duties of the job
The post holder will work alongside pharmacists and technicians in the pharmacy section 10 production unit at the Princess Royal University Hospital site based in Orpington, Kent. The post holder will assist in the preparation and dispensing of aseptically produced products in isolators, as well as assisting with the daily monitoring and cleaning of isolators, and the monitoring of the clean room. The post holder will be expected to work to the principles of "Good Manufacturing Practice".
The post holder must be flexible to working at other King's College Hospital trust sites when required to fulfil the service needs of cross-site working.
All work will be carried out according to procedures and standards of professional practice in the Department; however, staff are expected to develop their own safe systems of work to ensure a high professional standard of work conducive to patient safety.
