Job summary
East of England Community Health and Care is recruiting an Operations Manager for the Operations Centre based at Norwich Community Hospital.
The Operations Centre is the point for clinical services support and escalation across the Trust, particularly inpatient and community services.
You will be responsible for the day to day running of the service which includes completing operational reports and providing senior representation of the Trust and Norfolk County Council on operational calls with Integrated Care System partners. During weekend service hours the role includes leading the EEC first level On-Call response.
There is requirement to answer email and telephone queries in a timely and efficient manner. The hours are full time working shifts across 7 days including Bank Holidays, with expected 1 in 3 weekend working. Any variation to this please contact the recruiting manager to discuss.
You will provide support to the existing Operations Managers and be allocated line management responsibilities.
We require an enthusiastic, autonomous and motivated individual with experience leading a team and operations management in a clinical setting. This is a great opportunity for someone who thrives in a fast paced, dynamic environment and is looking to develop their experience of operations management.
Please note that the interviews will be taking place in person at Norwich Community Hospital.
Main duties of the job
To facilitate the Operations Centre daily meetings, co-ordinate the flow of patients through key Trust serviced including Intermediate Care beds, Community Nursing and therapy, ensuring appropriate placement of patients according to their clinical need.
To develop strong, transparent working relationships with key stakeholders in the Integrated Care System including acute hospitals, social services, and the voluntary sector.
To be responsible for responding to services requiring senior out of hours support 5pm -7pm Monday to Friday and 8am-7pm weekends/Bank Holiday (out of hours). To communicate with the affected service/individual to understand and resolve issues, escalating as required to the Executive Director Manager On-Call.
To be responsible for the leadership of the discharge co-ordinators, flow co-ordinators and/or administrators alongside peer Operations Managers

A fantastic opportunity has arisen to be a Family Nurse in Tameside. You will deliver the Family Nurse Partnership programme, which has evidence based outcomes, to a caseload of young vulnerable parents and their babies.
We are seeking a flexible, warm and innovative Family Nurse for our established Family Nurse team. You will need to be available to complete the FNP Family Nurse training programme if not already completed and be highly motivated with a good understanding of infant psychology, building therapeutic and empowering relationships, safeguarding children, child development, and you will enjoy working with this age group of parent.
Family Nurse Partnership is well embedded within the local early help offer and is in its 10th year of delivering the programme to the young parents in the borough. Our multi-agency approach to parent-infant mental health and our early attachment service are already highly regarded, and as a Family Nurse you will work closely with these and other services.
This post requires the ability to travel independently and efficiently across multiple community sites to meet the operational requirements of the role.
Main duties of the job
The Family Nurse role is responsible for delivering the FNP intensive and preventive home visiting service to vulnerable and hard to reach young woman who are expecting their first baby. The post holder will be required to develop high level generalist and specialist skills to work within the clinical methods of the service delivery. We are looking for an experienced, dynamic, creative, positive and motivated Mental Health Practitioner to join our Community Mental Health Service on the Isle of Wight.

Job summary
As a Reablement Specialist Practitioner, you'll be working across all disciplines within the Reablement service including occupational therapy and the community response team (CRT). You'll be part of a progressive team, using an enabling, adaptive approach, supporting people to live in their own homes where possible, and in the least restrictive way.
Working Patterns
Our Service runs 07:00-21:00 every day of the year, therefore you'll be required to work evenings and/or public holidays and/or weekends as part of your Normal Working Hours for which enhancements are paid and days off in lieu can be accommodated.
The current working hours for this position are a mix of day shifts 08:30 - 17:00 and evening shifts 13:00 - 21:00 on a 7-week rolling rota. Requirements to work weekends will be 1 in 7 pattern - hours 08:30 - 17:00. Working patterns are subject to change based on service requirements; any changes will be formally discussed and agreed with you.
Main duties of the job
- Alongside a multi-disciplinary team of Health and Social Care professionals, you'll triage referrals, complete holistic assessments of need and plan supportive interventions.
- Use a strengths-based approach when providing advice and information throughout someone's reablement journey, enabling them to problem solve and make informed decisions.
- Build positive working relationships with service users and their wider support network, healthcare professionals and local community networks.
- Support professional practitioners with safeguarding cases.

The primary purpose of this post is to set up a workable program of works which will beadded to the PPM schedule to clean all the window, Gullies and plant rooms across thesite.

Job summary
An exciting opportunity has arisen for an enthusiastic and motivated dietitian to join our friendly, supportive and forward-thinking dietetic team. We are seeking an experienced paediatric dietitian to work within our specialist cardiac service.
The successful candidate will provide expert nutritional assessment and management for infants, children and young people across the inpatient and outpatient cardiology pathway. We are looking to appoint a TB Clinical Nurse Specialist to join our team.
Along with your NMC registration and post registration experience within the speciality of TB or respiratory, the successful applicant will form part of the Tuberculosis Prevention and Control Nursing Service. Serving SaTH and the communities of Shropshire, Telford & Wrekin and Powys.
