Job summary
Social Worker - Moving into Adulthood Team Join Oxfordshire County Council's Moving into Adulthood Team and play a key role in supporting young people as they transition into adult social care services. Working within a supportive and collaborative team, you will use a strengths-based approach to empower individuals to identify their goals, maximise their independence and achieve positive outcomes. You will undertake assessments, develop support plans, work closely with families and carers, and collaborate with a wide range of professionals and partner agencies to ensure young adults receive the right support at the right time. The team is committed to delivering high-quality, person-centred practice and offers regular supervision, manageable caseloads and excellent opportunities for continued professional development. You will be encouraged to contribute to service improvement, participate in group learning and develop evidence-based practice while maintaining the highest professional standards. We are looking for a qualified Social Worker registered with Social Work England who is passionate about supporting young people and committed to promoting independence, equality and social justice. Strong communication skills, a collaborative approach and a commitment to continuous learning are essential. Main duties of the job
As a Social Worker in the Moving into Adulthood Team, you will support young people transitioning into adult social care services through strengths-based assessments, support planning and partnership working. You will work closely with individuals, families and partner agencies to promote independence, wellbeing and positive long-term outcomes.

An opportunity has arisen for a dynamic Health Visitor to join the Oxford City South locality on a part time basis. The post is to work in the integrated 0-19 Childrens and Young People Public Health Service here in the Iffley/ Cowley/ Leys area. The new service brings together Health Visiting, School Health Nursing, and the Family Nurse Partnership, delivering the Healthy Child Programme (HCP) to provide an innovative and high-quality service that helps children, young people and families to thrive.

There has also been an introduction of a new four-year health and development review to support school transition and address any school readiness concerns.

We have an amazing opportunity available to join our well-established and forward-thinking organisation of NHS Blood and Transplant. If you're experienced in facilities or maintenance management or working as an assistant facilities manager, you would experience valuable growth and support in working with our state-of-the-art systems in a well-structured and progressive organisation.
You will be supporting the Estates and Facilities Manager in managing the maintenance of our Oxford site and other sites in the surrounding area. The Enhanced Intensive Intervention and Risk Management Service (EIIRMS) is a specialist partnership between Oxford Health NHS Foundation Trust and HM Prison and Probation Service (HMPPS). The service aims to improve psychological wellbeing, address barriers to engagement, reduce risk linked to offending, and minimise the risk of serious harm.
The role involves supporting the assessment and delivery of practical psychosocial interventions under the supervision of qualified clinicians, engaging service users, and contributing to recovery-focused, multidisciplinary care plans.
Main duties of the job

Work collaboratively with members of the Thames Valley Integrated OPD Pathway Service multidisciplinary team to develop and deliver appropriate psychosocial interventions for service users.

Support the planning, development, and delivery of a psychosocial and psychologically informed service for individuals with offending histories and associated interpersonal difficulties.

Deliver interventions under the supervision of the Clinical Lead for the Thames Valley Integrated OPD Pathway Service.

Main duties of the job
You will play a crucial role in improving mental health outcomes for our service users. Providing statutory social care assessments under the Care Act and s.117 after-care Mental Health Act is a key feature of the role and you will provide a strengths based, rights focused assessment to identify and meet need, ensuring timely review of packages of support.
Working with complexity and risk is a key feature of the role and you will use your safeguarding adults knowledge to identify abuse and neglect and work with the person and other agencies to seek ways in which to protect and safeguard the person. At times, you will need to use legislative frameworks such as the Mental Capacity Act and Mental Health Act to support people who are unable to make decisions for themselves, balancing and protecting the rights of people and others.

Main duties of the job

As a AACC Practitioner, the role includes delivery of a variety of functions across the AACC service,including: - Clinical Duty including NHS Continuing Healthcare Fast Track Pathway - Assessments - Reviews - Care Planning/Clinical Commissioning - Case management - Appeals -Retrospective Reviews (Previously Unassessed Periods of Care)
The Practitioner will work across other areas of AACC, including audit, appeals, retrospective assessments, complaints, assessment verification, and many more.
You will be supported to develop the necessary skills and experience to work across the whole pathway, safely, confidently andeffectively. The role will require travel on occasions, to carry out assessments and reviews across Surrey and Sussex.
Access to transport is therefore necessary, be it personal or public. All AACC staff are expectedto work flexibly, when required, to meet the needs of the service.

Main duties of the job
o The provision of technical and operational advice on all matters of medicines efficiencies
o To assist the Principal Pharmacist Medicines Efficiency to lead and coordinate the Medicines Efficiency Programme across the Trust, ensuring timely implementation of agreed switches and cost-saving initiatives
o To act in collaboration with the Principal Pharmacist Medicines Efficiency as key liaison between pharmacy, finance, procurement, and clinical stakeholders for medicines efficiency projects.
o To ensure compliance with governance, audit, and KPI reporting requirements.
o To support the overall medicines efficiency work-plan, providing regular status reportsand assisting teams in achieving deadlines.
