<p>Are you a Physiotherapist looking for an opportunity to develop your rehabilitation skills while helping people regain their independence?</p><p>Were looking for a motivated Band 5 Physiotherapist to join our Intermediate Care Rehabilitation Service in Luton.</p><p>Youll work as part of a multidisciplinary team, assessing and treating patients in their own homes as well as within rehabilitation beds. Youll manage your own caseload, working closely with patients and their families to understand what matters to them and develop personalised rehabilitation goals.</p><p>Your role will involve completing holistic physiotherapy assessments, planning and delivering appropriate interventions, reviewing progress and adapting treatment plans to help patients achieve the best possible outcomes.</p><p>Youll also have the opportunity to respond to urgent referrals and develop your experience within Intermediate Care, while receiving support and supervision to continue building your clinical confidence and professional skills.</p><p>If youre passionate about rehabilitation, enjoy working as part of a collaborative team and want a role where you can see the difference your interventions make to patients everyday lives, wed love to hear from you.</p><p>A UK driving licence and access to a car are required, as the role includes visiting patients within their own homes.</p><h4>Pack

We are seeking a trained and experienced Phlebotomist to join our friendly Practice. You will support the delivery of high-quality primary care by providing safe, accurate and compassionate venepuncture services to our diverse, semi-rural patient population.
The position offers a minimum of 15 hours and up to 37.5 hours per week, working between the hours of 8:30 am and 6:30 pm, Monday to Friday. The specific days of work will be agreed at interview.
There will also be a requirement to cover some Saturday mornings on a team rota basis.

Main duties of the job
Working under the direct supervision of the Lead practice nurses and strictly in accordance with specific practice guidelines and protocols.
Blood Sample Collection :Perform venepuncture to collect blood samples from patients following clinical guidelines.
- Patient Care & Comfort: Ensure patients feel at ease, addressing any concerns and managing anxious individuals professionally.
- Sample Processing: Label, store, and prepare blood samples for transportation to the laboratory, ensuring accuracy and compliance with protocols.

- Record Keeping: Accurately document patient details, tests requested, and sample collection in the clinical system.

Main duties of the job
The successful candidate will:

The aim of the PCN Heart Health Checks in Surrey Heath Primary Care Network is to provide a quality service that will help people live longer, healthier lives. The longer term aim is a reduction in incidence or early detection of heart attacks and strokes, type II diabetes, chronic kidney disease and vascular dementia.
As a Health Care Assistant with us, you will play a key role working under supervision and delivering our heart health checks which identifies the persons risk of developing cardiovascular disease and diabetes. This health check focus on preventative care, health promotion and education - all within an environment dedicated to providing respectful, high-quality service to every patient. To undertake NHS health checks
Phlebotomy every patient will have a blood test as part of the heart health check
Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, recording findings accurately

Ensure timely and accurate recording of clinical information and patient interventions on relevant electronic database systems, e.g. EMIS, and following business continuity plans if the electronic system is unavailable

We are seeking a motivated and enthusiastic Band 6 Dietitian to join our well-established and highly respected renal dietetic team at Barts Health NHS Trust. Main duties of the job
To provide a specialist clinical dietetic service at Band 6 level to patients cared for by the Bart's Health Renal Services. This will include renal wards at the Royal London Hospital; renal outpatient and dialysis patients at Newham University Hospital, Hornchurch Dialysis Unit, Whipps Cross Hospital, King George's Hospital or other Satellite units which may be opened in the future. The main base will be at The Royal London Hospital but there is a requirement to visit your designated satellite unit to provide dietetic services.

Job summary
In line with policy to increase the access and availability of mental health and wellbeing support, we are seeking to recruit two graduate level (or equivalent) practitioners to provide CBT based guided self-help and parenting interventions to children and young people. They will be taught to deliver evidence based 1:1 and group interventions for children and young people experiencing mild to moderate mental health difficulties. The post holder will use their newly developing skills to support children and young people who are referred into the service and trainees will receive supervision to support their professional development.

In collaboration with the patient, to plan and implement single or a programme of interventions using treatment techniques to meet the patient's needs re; timely discharge. These will be achieved using professional and theoretical knowledge, and specialist clinical reasoning skills.
To monitor, evaluate and modify treatment interventions to measure progress and ensure effectiveness, involving patient, carer and MDT as appropriate.
To ensure Therapy goals are integral to the multidisciplinary care plan re: safe and timely discharge.
To apply an enhanced understanding of effects and impact of illness, frailty and disability on patient's abilities and provide advice and education regarding lifestyle changes / adaptations to their physical and social environment.
