Job summary
An exciting opportunity has arisen for a keen and enthusiastic band 6 Practitioner to join the Forensic Community Mental Health Team (FCMHT). FCMHT is part of the Forensic Community Service within the Specialist Services Network. FCMHT is based at The Parlick Centre at Guild Loge in Whittingham, Preston. However, the role will require travelling within the areas of Lancashire and South Cumbria.
FCMHT is a specialist mental health team, providing care designed to support and supervise service users who have a diagnosis of severe mental disorder with a risk to others who have previously received inpatient treatment at low, medium, or high security settings. The FCMHT provides an accessible, responsive and adaptable service, prioritising the needs of service users with complex needs, to maintain the safety of service users, families, carers and the public.
The post of Practitioner sits within a multi-disciplinary team made up of nurses, social workers, doctors, occupational therapists, clinical and forensic psychologists and support workers.
The post holder will support all aspects of the service delivery, in particular supporting the delivery of the clinical model, case holding, engaging in supervision, assessing the needs of those referred, developing collaborative formulations, undertaking specialist risk assessments with the wider MDT. Being part of the 24hour on call provision of the service.
Main duties of the job

The successful applicant will be working as part of a dynamic and enthusiastic team providing person-centred care to adults over 18 who present with forensic risk and mental health conditions.
You will support people referred to the service to access primary and secondary health care as appropriate.
You will be responsible for the maintenance of high levels of evidence-based clinical practice, contributing to and facilitating Care Processes and Pathways.
Providing consultation as part of Forensic Outreach Service.
You will manage and supervise the work of others, and act as a mentor and preceptor.
The ability to work independently as well as within a team will also be required.
Candidates will need to be IT literate and have experience using digital clinical records, as well as the desire to explore new ways of engaging with stakeholders using technology.
The candidate will also be required to work with multiple agencies and service providers across Lancashire & South Cumbria which will require excellent communication skills.

Job summary
Exmoor Surgery is looking for a Medical Receptionist to undertake a wide range of administrative duties while providing effective support to the multidisciplinary team. Responsibilities include, but are not limited to, scanning and processing documentation, managing and filtering medical reports and correspondence via DOCMAN, and supporting reception tasks such as welcoming and directing patients, efficiently using the appointment system, booking appointments, handling patient information, and assisting patients as required.

Cumbria Medical Services is looking to recruit a Patient Booking Coordinator. Primarily the Booking Coordinator ensures the effective management of all incoming NHS referrals using the E-Referral system and ensures that adequate clinic slots are available at all times.

Job summary
An exciting opportunity has arisen to play a key role in supporting the delivery of high-quality amyloidosis care across the newly established Amyloidosis National Network, led by the Royal Free London NHS Foundation Trust.
NHS England has introduced a new model of care for systemic amyloidosis services in England. The transition from a single national centre to a hub-and-spoke network enables patients to receive high-quality specialist care closer to home, while maintaining the expertise and clinical oversight of the UK National Amyloidosis Centre (NAC) at the Royal Free London NHS Foundation Trust.
As MDT Coordinator, you will support the coordination of patient pathways and multidisciplinary team activity across the network, helping to ensure seamless communication, continuity of care, and an excellent experience for patients and clinicians alike.
Main duties of the job
Coordinate and administer the Amyloidosis Multidisciplinary Team (MDT) meetings across the UK Amyloidosis Network (UKAN), ensuring all eligible patients are identified, reviewed and discussed in a timely manner.
Work closely with Amyloidosis clinicians, specialist nurses and MDT members to establish and maintain processes for identifying patients requiring MDT review.
Prepare and distribute MDT agendas, patient lists and supporting clinical information within agreed timescales.
Ensure all relevant clinical information, including medical records, pathology, genetics, imaging and investigation results, is available to support MDT decision-making.
Accurately record MDT discussions, treatment recommendations and agreed actions, ensuring completion of all required MDT documentation.
Act as a key contact for MDT-related queries across the network and facilitate effective communication between clinicians, administrative teams and partner organisations.
Support the Amyloidosis clinical teams and UKAN Programme Manager in achieving service objectives and maintaining compliance with national and local standards.
Develop and maintain effective working relationships with internal and external stakeholders across the network.

The Leeds Paediatric Renal Service provides regional specialist kidney care for children across East Yorkshire, West Yorkshire, North Yorkshire and the Humber. This service is one of 13 designated services for paediatric Nephrology in the UK and one of the 10 providing a fully comprehensive service including transplantation. The services at Leeds Children's Hospital are based in the renal unit, inpatient bed base on L50 and adjacent haemodialysis unit. This is a shared ward with paediatric hepatology and includes a shared 13 bed base including 8 cubicles (2 with the capacity for haemodialysis). Patients are also routinely cared for on the High Dependency Unit and in Paediatric Intensive care post renal transplant. Children will need to come to Leeds for specialised treatments or procedures which include renal transplantation, acute kidney injury requiring dialysis, insertion of long-term dialysis access and problems relating to long-term dialysis, renal biopsy, specialist imaging, renovascular angiography and treatment and children with complex nephro-urological conditions. Haemodialysis (haemodialysis and haemodiafiltration) is delivered in centre with routine haemodialysis delivered Monday to Saturday (6-day service) with a 7 day/week daytime urgent dialysis cover. The number of children receiving dialysis ranges from 8-12. Leeds is one of the KQuIP (Kidney Quality Improvement project) establishing home haemodialysis and as part of this there is a haemodialysis nursing staff expansion programme including rotational nursing staff from the ward and an nurse educator role.
Peritoneal dialysis
There is an established peritoneal dialysis programme with patient numbers ranging from 4-10. There is urology surgical support for catheter insertion for acute and chronic peritoneal dialysis. There is a training package for carers led by the specialist nursing team with a combination of in hospital and at home training. There is ongoing support from the specialist nursing team at home with electronic access to each dialysis session. There is monthly outpatient review of all patients receiving peritoneal dialysis in our MDT clinic.
Transplantation
There is an active transplantation programme in Leeds with an average of 10 transplants per year, with a current cohort of 65 transplant patients under follow up. There is specialist nursing support for transplant work up and a comprehensive MDT transplant listing programme involving nephrologist, transplant surgeons, immunological laboratory, specialist nurses, psychology and social workers. The transplant programme is supported by the transplant surgical team Mr Masood, Ms Martinez-Lopez and Mr Falconer.
Nephro-Urology
There are close links with the paediatric urology MDT. There is a monthly nephro-urology MDT to discuss complex cases and a monthly joint nephro-urology clinic. There is access to non-invasive and invasive urodynamics and an evolving medical bladders service.
There are a number of specialised clinics including antenatal counselling, regional stone service, Tuberosclerosis clinics and joint rheumatology clinics. Job summary
We are looking for an organised, patient, flexible and efficient person with excellent customer service skills to support us with day to day admin within the Residences Team. The Residences Team looks after doctors on-call rooms, longer term accommodation via tenancy agreements and the Bexley Patient Hotel.
Main duties of the job
The role requires liaising with a wide range of people across LTHT, both in person and via email/phone and beyond, and therefore excellent communication and interpersonal skills are essential. This is a very busy and demanding service, and the successful candidate will be required to use their initiative to prioritise work and meet deadlines. A major part of the role is to prepare long term accommodation between tenants. This involves raising maintenance issues and ordering any new items required. All stock for the properties then needs to be unpacked and the property staged for the next incoming tenant. There is also a requirement to move between sites in addition to responding to daily requests from tenants in our accommodation, therefore you will be required to be mobile and on your feet for some of your day. The post is full time. Working Monday Friday, 8am 4pm with a 30 min lunch break. This will be a challenging role, but for the right person offers an excellent development opportunity. Leeds Teaching Hospitals is one of the largest employers in Leeds, employing over 20,000 staff and providing care to 1.6 million patients every year. The Estates & Facilities Division keep the Trust sites running and manage one of the largest portfolios across the NHS.
Our Residences Dept provides accommodation for clinicians who are on call and for those who require longer term accommodation due to being newly recruited to the Trust. We also manage the Bexley Patient Hotel where Out of Area patients stay whilst receiving out patient treatment.
We are a small team of 3, consisting of one Residences Manager and two Administration Officers.
What you will bring to the role- Dealing with patients and doctors who require accommodation. This could be via telephone, email or in person.
- Answering email queries regarding all aspects of accommodation.
- To restock houses following departure of a tenant, and preparation for the next clinician who requires accommodation.
- To raise tenancy agreements and eviction documents when required.
- To take payment, and keep records of payments for accommodation.
- To attend meetings with service providers eg Facilities Staff
- To contribute to the production of information for distribution across the Trust including web based information.

Part time care navigator required for our busy multi-sited GP surgery. Experience preferred but full training will be given for the right candidate. Main duties of the job
We are looking for theatre practitioners with scrub and anaesthetic skills
You will need to be a registered nurse or ODP with a minimum of 3 years theatre experience at band 5.
Successful candidates will need to demonstrate that they are motivated, able to work on their own initiative, are a team player with the ability to prioritise workload and support the team leader, theatre Coordinator and team to provide a safe and effective service for our patients.
Following our most recent CQC inspection the division of surgery was rated as Good across the 5 key standards so you would be joining a high performing team and would be expected to support the Team Leader to influence and drive improvement within Nucleus 7&8.
