Registered Mental Health Nurse / Occupational Therapist, Dartford
Invicta Health CIC View all jobs
Posted 1 month(s) ago
Reference: B0082-26-0060
Job summary
Unfortunately we are not able to provide sponsorship for this role. Please do not apply if you do not have a right to work in the UK.
Invicta Health in partnership with Kent and Medway NHS and Social Care Partnership Trust (KMPT) are seeking to recruit Community Mental Health Practitioners to be part of Mental Health Together.
What is Mental Health Together?
In Kent and Medway, the community mental health transformation model has created a new service called Mental Health Together.
It is a multi-agency offer, which means bringing all the services and agencies that support people with varying mental health needs much closer together. This ensures that people get the right care, without having to navigate service boundaries and without repeatedly telling their story.
Mental Health Together will add new roles, as part of a new joined-up model of care which bridges the gap that currently exists between primary (i.e. GPs) and community mental health (i.e. KMPT). It will treat people with complex mental health needs by:
- Providing someone with support in a person-centred, holistic way
- Removing barriers to access, and thresholds across services working together
- Involving service users and carers as equal partners in their care
- Building in involvement and co-production at all levels of service design delivery
- Delivering needs-led and personalised trauma-informed care.
Main duties of the job
The Registered Mental Health Nurse / Occupational Therapist will be expected to deliver stepped care by delivering a range of evidence-based interventions which will promote recovery, social inclusion and normalization for service users who suffer from mental health problems.
They will work in partnership with the MDT, CRHT, Acute Inpatients Services, other community teams, Primary Care and the Voluntary Sector to provide support and treatment to service users and facilitate the safe management of their needs within a range of community settings.
The principle purpose is to promote mental and physical health and prevent ill health by treating mental health problems effectively through evidence based interventions that build on strengths and supports choice and is underpinned by a single care plan within a model of care based on inclusivity, particularly for those people with co-existing needs, who experience marginalisation
They will be a Registered Mental Health Nurse or Occupational Therapist responsible for patients allocated to them and will be involved in assessment, management planning and delivering care.
The post will support their peers and team leaders and be an effective team member whilst working on their own initiative. They will supervise Band 5 Nurses/OTs and pre-registered students and hold a mentoring qualification. They will have a commitment to collaborative working and actively supporting and liaising with other health and social professionals.
About us
Invicta Health is a not for profit company, with a passion for primary care. We have a wide range of services such as GP surgeries, services in Urgent Treatment Centres , a mental health service, GP staff training service and the list is growing all the time.

We are looking to recruit a Care Coordinator-Navigator to work 37.5 hours per week, joining our supportive and friendly practice family.
You MUST be able to work until 6.30pm on Tuesday, Wednesday, Thursday and Friday.
We are looking for someone who is flexible, reliable, and committed, someone who sees this as a long term career opportunity. This role suits a person who enjoys learning, thrives in a busy environment, and takes pride in helping patients access the right care at the right time.
Experience in a busy customer facing role is essential, and experience within a healthcare setting is desirable. This is a varied and fast paced role where no two days are the same. You will present a positive, friendly, and professional image to patients and visitors, whether in person or over the phone. You will help patients access the right clinician or service in a courteous, efficient, and effective manner, while supporting communication between patients, the primary care team, secondary care, and other healthcare agencies.
Key responsibilities include (but are not limited to):
Handling incoming calls from patients and healthcare agencies
Supporting patients to access the right service, clinician, or pathway
Managing patient administration, including registrations, deductions, and demographic updates
Processing clinical and administrative information from secondary care and other providers
Completing administrative tasks as directed by clinicians and the leadership team
Welcoming and checking in patients and visitors in a safe, polite, and efficient manner
Helping coordinate care plans, follow up actions, and referrals
Supporting proactive care initiatives and population health management activities
About us
The Homewell Practice is committed to delivering high quality patient care and was rated GOOD by the CQC in 2022. You'll be joining a warm, collaborative, and supportive GP practice where every team member plays a vital role in patient experience.
We offer:
A friendly and welcoming working environment
Membership of the NHS Pension Scheme
Good local transport links
A paid day off on your birthday, because we believe in recognising and celebrating our staff
A supportive team culture where your wellbeing and development truly matter

Reference: 234-25-B1630
Job summary
**A 14.5% recruitment & retention premium will be included with this post.**
An opportunity has arisen for an experienced, enthusiastic optometrist with an interest in hospital optometry to provide clinical services within the ophthalmology directorate of this NHS foundation trust. The post is 37.5 hours per week, but we will consider applications for part-time work.
The post will be based within the Ophthalmology department at the Norfolk & Norwich University Hospital. The friendly, vibrant city of Norwich has excellent social and cultural facilities for all ages, with the Riverside development and large shopping malls. The surrounding countryside and Norfolk Broads are within easy reach and is less than 2 hours by train to London (or less than two hours by car to London's North Circular Road). The City of Norwich was voted 'Best place to live in the UK' by the Sunday Times in 2026'
Main duties of the job
The successful candidate will possess excellent clinical and communication skills, will be able to function effectively within a multidisciplinary team, and will be able to demonstrate initiative in delivering a high standard of patient care.
In addition to the 'core' hospital optometrist roles within specialist contact lens and low vision clinics, optometrists at this trust also work within several ophthalmic clinics in an 'extended role' capacity.
Optometrists staffing these clinics work with the support of formal clinical protocols which can allow extended role optometrists to manage patients with a significant degree of autonomy. Current extended roles include Anterior Segment Laser Clinic (LPI, SLT & YAG capsulotomy), Cataract, Cornea, Glaucoma, Medical Retina, OERS (Ophthalmic Emergency Referral Service or 'Eye Casualty'), Paediatric and Research.
Previous experience of specialist contact lens and low vision practice would be an advantage. Previous experience of work in the extended roles referred to above and/or relevant post-graduate qualifications would be an advantage. Appropriate training and supervision will be provided in-house. Funding is available for independent prescriber qualifications.
The Ophthalmology Directorate is based at the Colney site; a £229 million PFI hospital facility which opened in 2001. The unit has 15 Ophthalmic Consultants covering all the main clinical sub-specialties.
Informal enquiries for more information are welcome.
