Job summary
An exciting opportunity has arisen for a full-time forward thinking and motivated experienced and IP qualified Clinical Pharmacist to join our North Connect PCN in Northwood on a 18 month fixed term contract.
The role will be based at Mountwood Surgery.
The successful candidate will work at Mountwood Surgery developing this role closely supported by the GPs. They will be part of a group of Clinical Pharmacists under the guidance and supervision of a delegated clinical supervisor.
The role will suit a forward-thinking, innovative and enthusiastic clinical pharmacist who is able to work collaboratively across the team. If you are adaptable, self-motivated and organised, possessing the necessary qualities and skills for this post, we look forward to hearing from you.
Main duties of the job
The post holder will will work as part of a multidisciplinary team to develop and run processes for repeat prescription authorisation, management of medicines on transfer of care and systems for safer prescribing. They will work directly with patients to assess and treat conditions, manage long-term conditions as well as promote self-care.
Skills required:
- Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
- Minimum of 2 years experience as a pharmacist
- Have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice
- Working towards or holding an independent prescribing qualification
- Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct
- Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines.

- Clinics are done face to face, via telephone, home visits and in the future, possibly video consultations.
- The post holder will undertake clinics for patients or develop one to one relationships in order to de-prescribe where necessary or offer clinical leadership on medicines optimisation and quality improvement.
- Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicine adherence ensuring they get the best use of their medicines (i.e. medicines optimisation). Make appropriate recommendations to GPs for medicine improvement.
Patient facing Care Home and Domiciliary Clinical Medication Reviews
- Undertake clinical medication reviews with patients and produce recommendations for clinicians on prescribing and monitoring. Work with care home staff to improve safety of medicines ordering and administration.
Management of common/minor/self-limiting ailments
- Managing caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence.
- Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate
Medicines support
- Provide patient facing clinics and a telephone helpline for those with questions, queries and concerns about their medicines in the practice
Medicine information to practice staff and patients
- Answers relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams (e.g. community pharmacy) and patients with queries about medicines.
- Suggesting and recommending solutions.
- Providing follow up for patients to monitor the effect of any changes
- Telephone medicines support
- Provide a telephone help line for patients with questions, queries and concerns about their medicines.
Helping tackle the issue of unplanned hospital admissions
- Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.
- Put in place changes to reduce the prescribing of these medicines to high-risk patient groups.
Management of medicines at discharge from hospital
- To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
- Set up and manage systems to ensure continuity of medicines supply to high risk groups of patients (e.g. those with medicine compliance aids or those in Care Homes).
Signposting
- Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period e.g. pathology results, common/minor ailments, acute conditions, long term condition reviews etc.
Repeat prescribing
- Produce and implement a repeat prescribing policy within each PCN practice. Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review.
- Ensure patients have appropriate monitoring tests in place when required
Risk stratification
- Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches. This might include risks that are patient related, medicine related, or both.
Service development
- Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets)
Medicines quality improvement
- Undertake clinical audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the relevant practice team.
Medicines safety
- Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.
- Auditing practices compliance against NICE technology assessment guidance.
Education and Training
- Provide education and training to primary healthcare team on therapeutics and medicines optimisation.
Care Quality Commission
- Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.
Collaborative Working Relationships:
- Participates in the Primary Care Network Multidisciplinary Team
- Liaises with CCG colleagues including CCG pharmacists on prescribing related matters to ensure consistency of patient care and benefit in surgeries
- Foster and maintain strong links with all services across the PCN and neighbouring networks.
- Explores the potential for collaborative working and takes opportunities to initiate and sustain such relationships
- Recognises the roles of other colleagues within the organisation and their role to patient care
- Demonstrates use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e.g. CCGs)
- Can recognise personal limitations and refer to more appropriate colleague(s) when necessary

Job summary
This is a replacement Consultant post. The remit of the post holder will be to work within the multi-disciplinary team providing and further developing care for stroke and TIA patients. In addition, the individual will be required to play an active role in the general workload of the Care Group while having a significant commitment to teaching and training of junior staff
Main duties of the job
The post will be based at Princess Royal University Hospital and the proposed Job Plan is attached. This is subject to review within 3 months of the post holder commencing in post.
There is a regular weekday daytime and evening stroke on-call commitment, 1:6 weekend daytime on call rota. We are offering an opportunity for an enthusiastic registered Dosimetrist/Treatment Planning Radiographer to join our Radiotherapy Physics team, at the Cancer Centre in Cottingham, East Yorkshire.
The successful applicant will join the Radiotherapy Treatment Planning team, a section of Radiotherapy Physics that underpins the Radiotherapy service, which is located in an award-winning facility comprising 6 Linacs, 2 CT scanners, Xstrahl SXR, Papillon-50+ SXR, Flexitron HDR and state-of-the-art Varian (Eclipse) treatment planning system.
You will be joining an established team of Healthcare Science professionals, comprising Clinical Scientists and Technologists, and support staff. The team prides itself on high staff retention and has a pragmatic approach to achieving safe and effective service delivery. We are committed to providing a broad scope for the development of all members of staff.
The department was the first Radiotherapy Centre in the North of England to clinically implement the Varian Halcyon linac, to develop and benefit the patient service, and now has 2 of these units. A Varian TrueBeam has recently been installed, to bring the complement to 4. In addition the department has recently installed SGRT across the fleet.
Main duties of the job
Building effective relationships with the multidisciplinary Radiotherapy department, and supporting the Lead Dosimetrist and wider planning team in the provision of the Radiotherapy Treatment Planning and Mould Room service, will be fundamental to achieving the high standards expected from this role.
The department has long-standing evidence of research and collaboration, developments in equipment and clinical service, and high commitment to quality and governance; these include implementation of a new and novel image guided treatment for the direct irradiation of rectal cancers using electronic brachytherapy, innovative use of VMAT, 4DCT and image guided treatment techniques.
Our commitment to the utilisation of technology includes the advanced use of electronic quality management systems and extensive use of the Varian ARIA system, which have allowed us to implement innovative process management and paperless working. The department was the first Radiotherapy Physics service to gain UKAS accreditation to the emerging BS70000 MPACE quality standard, evidencing that commitment to the delivery of high-quality clinical service is a core value of the team, and we continue to maintain ISO9001 certification for the entire Radiotherapy service.

Applications are invited for a Locum Consultant in High Risk Obstetrics. This post is for a fixed term for the period of 6 months where the successful applicant would join a team of 15 consultant obstetricians to provide antenatal care, high risk intrapartum care and postnatal care.

Main duties of the job
The post is suited to a highly motivated and enthusiastic individual who has completed their training in Obstetrics and Gynaecology. The successful applicant would join a team of 15 consultant Obstetricians (13.6 WTE) delivering high risk antenatal, intrapartum and postnatal care. This post covers general antenatal, intrapartum and postnatal obstetric care, and consultant labour ward presence will be covered by 14 consultants with resident cover for approximately 100 hours per week. There is separate consultant on call cover for gynaecology.

This is an exciting opportunity to join our team in the Pre-analytics section in Laboratory Medicine who are looking for a team of dedicated, enthusiastic people to join our team of Pre-analytics (Reception) supervisors. 7. Assisting with quality management systems tasks such as SOP writing, audit, EQA/IQC, validation/verification under direction of registered Healthcare Scientists.

Are you an outstanding Clinical Psychologist Specialising in Neuropsychology or aspiring Consultant Neuropsychologist with a passion for delivering high quality care for men who have sustained a brain injury?
Cygnet have been providing a national network of specialist mental health services for more than 30 years. They provide neuropsychiatric services to people with neurological conditions, mental health issues and behaviour that challenges in 7 different sites across the United Kingdom.
Main duties of the job

At Cygnet Paddocks, you will be overseeing the psychology service and providing supervision to the on-site psychology team at Cygnet Paddocks, comprising of a qualified psychologist and two assistant psychologists.
The unit uses a positive behaviour support model, with ABC charts and incident management systems in place to support the functional assessment of behaviour. There will be opportunities to carry out cognitive assessment, neuro-rehabilitation, capacity assessment, risk assessment and psychological therapy.
Cygnet has 8 neuropsychiatry services across the country, each of which has at least one qualified psychologist/neuropsychologist on site. Psychologists at these sites form the national neuropsychology team. The national team has monthly governance meetings to discuss best practice guidelines, support standardised practice and share clinical cases. There is a consultant neuropsychologist already in post within the national neuropsychology team.
The national neuropsychology team have a variety of training in skills in areas such as forensic psychology, trauma therapy and EMDR, and Acceptance and Commitment Therapy. Some members of the team are being supported to complete Qualification in Clinical Neuropsychology and are currently undertaking the knowledge component.

Cygnet Paddocks: Cygnet Paddocks is our specialist Neuropsychiatric rehabilitation service for men. The service provides a dedicated care pathway for men affected by acquired brain injury or neurological conditions, where behaviours that challenge are the overarching impairment to recovery and rehabilitation.
The service opened in June 2024. The individually designed wards replicate the well-established rehabilitation and treatment pathways already successfully offered by Cygnet in other Neuropsychiatric Rehabilitation services; facilitating a clear and distinctive care and treatment pathway led by an all-encompassing in-house clinical and medical team.

Provide effective and customer-focused technical, fault resolution to the staff based at Walsall Healthcare Trust, CCG and WHT community staff.
The Support Technician is the first point of contact of the IT Department with a day-to-day responsibility to provide first-class support to users of information technology.
The role will ensure that an increased number of calls can be resolved over the telephone without an engineer visit being required.
To have ownership of calls through to resolution ensuring individual and departmental targets/KPIs are met.
Provide effective levels of customer service and communicate effectively with all staff at all levels, including Doctors, Practice Managers, Clinical Staff, and administration staff.
Respond and deal with all IT Faults, incidents, and requests that are raised to the team by telephone, email or via the DTS portal (Self Service Portal). Take ownership of calls taken or assigned by a team leader/manager.
To ensure that the systems that the clinical staff are reliant upon, for the provision of good patient care, are operational all of the time, and in the event of any failure resume operation as quickly as possible. This ensures that the care of the patients never suffers as a result of IT equipment or Systems failure.

Main duties of the job
To work as part of the Community MSK Physiotherapy team which operates from two sites; Haslemere Community Hospital and Cranleigh Village Community Hospital. Job summary
We are seeking an experienced and innovative Band 7 Occupational Therapist to join our existing community Frailty Clinical Specialist Occupational Therapists working in our East PCN.
The post-holder will work as part of our multi-disciplinary PCN Wellbeing Team, which includes two other Frailty Occupational Therapists, Social Prescribers, a Health and Wellbeing Coach and Safeguarding Care Coordinators.
This role is an extended scope, advanced practice role working in the community with people in their own homes. The purpose of the role is to provide advanced Occupational Therapy clinical assessments, interventions and planning. This includes advanced care planning discussions. Liaising closely with the MDT including the GPs at the surgeries to facilitate and support these assessments. To proactively manage frailty and ageing well in the community, reducing un-necessary hospital admissions and supporting individuals to achieve the best possible quality of life.

Main duties of the job
To undertake comprehensive frailty assessments from an advanced clinical Occupational Therapy perspective. Delivering specialist Occupational Therapy interventions to support independence, function, safety and quality of life.
Deliver person centred, specialist Occupational Therapy intervention and goal planning, working collaboratively with the person, their support network, GPs and the wider MDT to improve patient outcomes.
Identifying risks, proactive planning and strategies to reduce unnecessary hospital admissions in collaboration with the wider MDT.
Facilitation and proactive engagement in Occupational Therapy practice placement education.

We are looking for an Administrator to join our Integrated Network Team in South Gloucestershire!

Are you a Nurse, Therapist or Paramedic passionate about supporting people to return to their own homes following acute illness or injury?
Bristol NHS Foundation Trust have a Transfer of Care Hub at Weston General Hospital where the aim is to support patients with discharge planning from admission through leaving hospital and into the community. The Transfer of Care Hub brings together the Acute Trust, Community Partners (Sirona), Local Authorities and Voluntary Sector colleagues across BNSSG to support complex discharge planning.
Sirona Community Transfer of Care Hub (CToCH) are the community experts in discharge planning working together with the Transfer of Care Hub partners to navigate patients to the most appropriate community discharge pathway.
As a case manager you will need to have experience working in a community setting and an understanding of the levels of clinical acuity that can be supported in peoples own homes. Alongside this you will have an understanding of rehabilitation pathways, discharge planning and mental capacity issues, excellent interpersonal and negotiation skills.
In return, we will provide you with regular training and supervision and enable you to become involved in future service developments.
This role is based mainly in Cadbury Garden Centre Offices with inreach days into Weston General Hospital.
Main duties of the job
- Be a key member of the multi-disciplinary team, responsible for providing person-centred, holistic assessment to facilitate discharge from the acute hospitals to the right discharge pathway.
- Responsible for triaging referrals received by CToCH. This may include liaising with the patient, family/carers and ward staff to identify the most appropriate discharge pathway.
- Support the CToCH to discuss complex cases with all partners in the Transfer of Care Hub to agree a joint action and discharge plan.
- To utilise multi-disciplinary assessment skills, supported by sound clinical reasoning to facilitate timely hospital discharge to the most appropriate environment
- To be able to effectively triage and prioritise a large caseload and be able to adjust daily work plan in response to system-wide pressures and escalation. Pro-actively optimising the usage of community capacity on a daily basis.
- High level of problem-solving required, gathering information from a range of sources, using professional experience and clinical knowledge to inform decision making. This includes being able to complete mental capacity assessments and make best interest decisions where appropriate.
