Job summary

Rock Health Care are looking for a professional Patient Advisor to join our friendly Practice team.
Working on our main reception desk, you will step straight into a vital role as our first point of contact for everyone who walks through the door. Daily life in this role is all about patient care, meaning you will guide visitors through their enquiries and ensure a smooth, helpful experience whether they call us or visit in person.
Shifts will be between 08:00 and 18:30, Monday to Friday.
Main duties of the job
- Act as the main point of contact for the practice, welcoming patients, medical students, pharmaceutical reps, and external visitors from the CQC and ICB
- Handle a high volume of face-to-face and telephone enquiries from patients and other healthcare professionals, helping them navigate services efficiently
- Manage the clinic diaries by booking patients in with GPs and other healthcare staff quickly and accurately to minimise waiting times
- Generate and process repeat and routine prescription requests accurately and efficiently in line with practice guidelines
- Update and maintain electronic patient records, handling clinical coding and document processing using Docman
- Stay on top of daily workflow tasks, patient messages, and incoming correspondence in the generic practice email inbox
- Support the team by preparing meeting agendas, distributing paperwork, and taking accurate minutes during staff meetings
- Monitor stock levels and handle ordering to ensure the reception area and front desk have all necessary supplies
- Take responsibility for the reception environment, keeping patient noticeboards and information leaflets organised, accessible, and up to date

We're looking for a compassionate, proactive registered nurse to join the Phyllis Tuckwell Hospice at Home Team. Youll deliver the highest standards of palliative and end of life nursing care to patients, their families and carers in the communityproviding holistic assessment, identifying needs, planning care and delivering timely interventions in partnership with Phyllis Tuckwell multidisciplinary team and colleagues across health and social care. - A successful Registered Nurse will have:
- RGN Degree Level Qualification
- Current NMC registration in adult or general nursing
- Palliative / end of life care or long-term condition experience.

- Experience of multidisciplinary team working and with external partners
- Able to make autonomous decisions demonstrating decision-making, critical thinking and proactive caseload management

Job Purpose and Summary:
To work as a proactive member of the Phyllis Tuckwell (PT) multidisciplinary community team. The postholder will maintain the highest standards of nursing care with patients, their families and carers in the community setting. Bordesley East PCN is seeking an enthusiastic and motivated Salaried GP to join our friendly and supportive Primary Care Network team.
This is an exciting opportunity to work across two well-established practices within the PCN, delivering high-quality patient-centred care to a diverse population in East Birmingham. Main duties of the job
o The successful candidate will join the existing orthopaedic trauma 'on-call' rota at the Royal Victoria Infirmary. This will involve making up a team of four Consultants, each doing one 'on-call' weekday (during which the team fracture clinics take place) every four weeks, followed by an all-day trauma list on the following day. In addition, the successful candidate will be 'on-call' for orthopaedic trauma one weekend in 13.
o The successful candidate will be encouraged to develop a special interest in upper limb surgery, to complement the existing surgeons within the team.
o The successful candidate will be required to:
o provide an elective surgery service.

Job summary
To support the Facilities (Hotel Services) team in delivering catering services to patients, staff and visitors in accordance with the Health Board Food Safety policy and all related and relevant policies, procedures and legislative requirements.
To assist in developing, maintaining and promoting the highest standards of service delivery, supporting, team working practices and the Facilities Unit commitment to EFQMS; and helping to maintain open and effective communications.
The post holder will work either as part of the ward based catering team or as part of the Main Kitchen catering team and will be responsible for food preparation or regeneration, meal service and cleaning of all catering areas.
Main duties of the job
The Principal Duties of the job are as follows:
- To prepare food and beverages in accordance with Health Board policy, procedures and legislative requirements;
- To assist nursing staff with meal service at ward level;
- To serve beverages at ward level in accordance with patient requests;
- To serve staff and customers in the dining room providing a professional and polite service;
- To operate the cash register accurately;
- To ensure all catering areas are cleaned in accordance with daily and weekly cleaning schedules.
- In the event of a major incident or to support adverse weather or other Facilities services contingency, the post holder may be required to be called in outside of normal working hours to support the Facilities Services in the contingency actions when and where required.
The duties of this post are not inflexible or exhaustive and will be reviewed periodically and may be changed to be consistent with the grading of the post as the organisation or department develops and following consultation with the post holder.

Job summary
As a Reablement Service Practitioner, you'll be working across all disciplines within the Reablement service including occupational therapy and the community response team (CRT). You'll be part of a progressive team, using an enabling, adaptive approach, supporting people to live in their own homes where possible, and in the least restrictive way.
Working Patterns
Our Service runs 07:00-21:00 every day of the year, therefore you'll be required to work evenings and/or public holidays and/or weekends as part of your Normal Working Hours for which enhancements are paid and days off in lieu can be accommodated.
The current working hours for this position are a mix of day shifts 08:30 - 17:00 and evening shifts 13:00 - 21:00 on a 7-week rolling rota. Requirements to work weekends will be 1 in 7 pattern - hours 08:30 - 17:00. Working patterns are subject to change based on service requirements; any changes will be formally discussed and agreed with you.
Main duties of the job
- Alongside a multi-disciplinary team of Health and Social Care professionals, you'll triage referrals, complete holistic assessments of need and plan supportive interventions.
- Use a strengths-based approach when providing advice and information throughout someone's reablement journey, enabling them to problem solve and make informed decisions.
- Build positive working relationships with service users and their wider support network, healthcare professionals and local community networks.
- Support professional practitioners with safeguarding cases.

- Experience in a Reablement, Rehabilitation, Health or Social Care setting is preferred but not essential.

We offer flexible working hours and are seeking someone who can work up to 3 days a week. Experiences in diabetes management is desirable and would be considered an advantage.
You will be working alongside our fantastic nursing team which consists currently of two practice nurses and two HCAs.

The team consists of 1 X WTE PEG Specialist Nurse with Gastroenterology Consultant support.

Job summary
This is a replacement post. The post is located at Roseberry Park Hospital, Middlesbrough, where inpatient forensic beds and clinical and managerial offices are all co-located.
There is a need for a Consultant on Ivy/Clover/Kestrel ward a low secure female learning disability ward.
Main duties of the job
There is a system of weekly MDT meetings for the ward which the post holder is expected to attend, in addition to meetings for CPA (6 monthly), formulation and risk management meetings. The length of stay currently on the ward is around five years.
The consultant will be supported by a ward manager, clinical lead working on each ward. In addition, they will receive support from occupational therapist, psychologist and social worker. Pharmacist will attend when support is required. A SAS/trust doctor, which is shared (50% time) will be supporting the post holder. The post holder can be the identified supervisor for the junior doctor. There will be opportunity to become a trainer for trainees in the future.
All patients will be detained under the Mental Health Act. As Responsible Clinician, the post holder will be expected to write statutory reports and attend First Tier Tribunals, Hospital Manager Hearings, S117 and MAPPA meetings as appropriate.
Referral rates for the ward are anticipated to be 4 per year from both internal and external sources. They can be seen jointly with the other staff members (psychologist/ nursing staff). The average length of stay on the ward is approx. 5 years.
Within the Secure Inpatient Services Directorate, the post-holder will be working alongside 14 consultant colleagues, 4 specialty grade doctors, and up to 3 core trainees.

Job summary
An exciting opportunity to join the senior nursing team on ward 32/CCU has become available.
Candidates will be expected to have at least two years band 5 experience with recent Cardiology/CCU and cardiac day ward experience to enable you to lead the team day to day and during an emergency.
Candidates will need to have completed ILS training or be booked in to complete it.

Main duties of the job

Main duties of the job
We are a 19 bedded unit which includes one High Dependency bed. We can accommodate up to 6 children for day surgery and work alongside the Paediatric Assessment Unit. Outpatient clinics also run Monday to Friday from the ground floor of the unit.
As part of this role you will be expected to rotate to Peterborough City Hospital Children Wards when there is a clinical need and to offer opportunities to expand on skills and knowledge.

Job summary
This role will involve the practitioner working within the team in a busy Cardiology department providing a range of complex cardiology investigations and producing detailed and evidence based clinical reports.
The post holder will perform a range of invasive and non-invasive cardiac investigations according to grade and within scope of training. The post would suit a newly or recently qualified Cardiac Physiologist/PTP looking for a springboard to further their career. Ensuring a flexible and responsible service, which is based on an individualised approach to patients, respecting their privacy and dignity at all times. Act in accordance with Trust health and safety policy ensuring a safe environment for patients, staff and visitors. Report any untoward incidents as set out in the Trust policies and procedures. Participate in out of hours and weekend working as required to ensure safe and equitable service delivery to our patients.
Able to liaise closely with patients, including those with whom it may be difficult to communicate, and with their carers, explaining procedures and results as appropriate.
The post holder will be required to deal with some patients who are acutely unwell and /or distressed. Ensure that all patients are dealt with courteously and promptly whilst maintaining their comfort and dignity as much as is possible

Job summary
We are recruiting to two full time permanent roles.
Public Health Wales provides health protection services to the people of Wales. The service is integrated and is primarily delivered through health protection teams, Communicable Disease Surveillance Centre (CDSC); health protection programmes (e.g. healthcare associated infections, antimicrobial resistance & prescribing (HARP) immunisation and vaccine preventable diseases (VPDP), Blood borne viruses (BBV), sexual health, TB and Inequalities, and environmental health protection) and the microbiology laboratories and reference laboratories / specialist services. The post holder will provide information management and analysis support for the Health Protection Division, focusing on support for the Health Protection Division. This will involve liaison with a range of stakeholders, including close working with colleagues in Health Boards and Public Health Wales Informatics.
Main duties of the job
We are looking for an experienced, innovative, collaborative and dynamic individuals to join a team of analytical specialists in the Communicable Disease Surveillance Centre (CDSC) to provide a proactive and responsive service.
The post holder will provide analytical support to the development and maintenance of public health surveillance schemes in the CDSC. This will include developing analysis plans and advising on efficient analysis of large datasets, as required. Support may be across any of the CDSC areas of expertise as required, including:
- Healthcare Associated Infection, Antimicrobial Resistance and Prescribing Programme (HARP)
- Blood Borne Viruses, Sexual Health, Tuberculosis and Inequalities
- Gastrointestinal Infections and Zoonoses
- Vaccine Preventable Disease and Respiratory Infection
Cross-cutting groups include field epidemiology; settings-based surveillance; and environmental surveillance.
The post-holder will be based within the BBV, STI, TB and Inequalities team, but will also be expected to undertake surveillance tasks as part of the CDSC rota and to cover other areas as required as part of a team of information analysts.
