Role: Mental Health Practitioner, Development role
Team: Ealing Acton MINT
Working Hours: 22.5 (3 days a week). The team works Monday to Friday 9-5p.m. closed Bank Holidays.
An exciting part time opportunity has become available in Ealing Acton MINT. This role is perfect for nurses who may want to develop their skills and experience working in a community setting. You will be joining at an exciting time as we transform our services following the Community Mental Health Framework. This framework represents a new approach to situating and providing place-based integrated support, care and treatment in the community for people with mental health problems whatever their level of need.
You will get to work in a multidisciplinary team and also get to work alongside General Practitioners and our community partners.
Community mental health nurses play a key role in dissolving the barriers that can exist between mental health and physical health, social care, voluntary and community social enterprise (VCSE) organisations and local communities, and primary and secondary care, to deliver integrated, personalised, place-based and well-coordinated care for adults and older adults. We are moving towards a 'whole system' of mental health care, which will provide timely access to support, care and treatment.
Main duties of the job
We need Community Mental Health Nurses to support people with mental health needs to access care where and when they need it and be able to move through the system easily.

Job summary
This is a new consultant post created to provide medical leadership to Hammersmith & Fulham's new Complex Psychosis Team, which brings together the existing Community Rehabilitation Service (CRS) and a new Assertive Outreach (AO) function under a single clinical and operational structure, each delivered as a distinct "arm" of one team with a single Service Manager. The post holder will hold overall medical responsibility for both arms, working closely with a 0.5 WTE Specialty (SAS) Doctor whose recruitment is running in parallel with this post.
Because the AO function is a new, NHS England-mandated service still in its mobilisation phase, this post carries a genuine and time-limited service development component: helping to finalise the tri-borough clinical model, establishing pathways and standard operating procedures, and shaping caseload and workforce arrangements as the service beds in. This is reflected explicitly in the job plan (Section 9)
Main duties of the job
Direct clinical care across the CRS and AO arms: new assessments, ongoing reviews, medication management, CTO reviews, and Mental Health Act assessments as required.
Assertive, community-based contact with the AO cohort, including home visits and joint visits with care co-ordinators, reflecting the AO model's emphasis on engagement as a therapeutic intervention.
Weekly multidisciplinary team meeting for the Complex Psychosis Team (both arms), plus daily flagging/triage of urgent concerns.
Support to the team's duty function for urgent risk and safeguarding concerns.
The post holder will be an Approved Clinician under the Mental Health Act and to hold Section 12 approval, consistent with the clinical responsibilities of this role.
No dedicated outpatient clinic ratio applies in the traditional sense; clinical contact is organised around MDT review clinics, home/community visits and duty cover, as set out in the job plan (Section 9). There are no inpatient beds or day-patient services attached to this post.
Be professionally accountable to the Clinical Director - H&F and the H&F Clinical Lead for Community Mental Health Services, and operationally accountable to the Clinical Director - H&F and the Service Manager for the Complex Psychosis Team.
Be a member of the H&F Medical Advisory Committee (MAC), contributing to operational and strategic matters and to discussion of major service developments.

We have 1.5 WTE Band 6 opportunities available and welcome applications for full or part-time working. This could be appointed as one full-time and one part-time post, or two part-time posts. Working hours can be discussed with successful candidates, subject to service requirements.
This is an exciting opportunity for nurses with an interest in Infection Prevention and Control (IPC) who want to develop their specialist knowledge and make a positive contribution to patient safety and the prevention of healthcare-associated infection.
Working within our multidisciplinary IPC Team, you will provide specialist advice and support, contribute to infection surveillance and outbreak management, and support assurance, education and improvement activity across the Trust.
We are looking for proactive, approachable nurses committed to high standards of care, patient safety and continuous improvement. You will be supported by experienced IPC colleagues to develop your specialist practice and progressively manage your own workload.
The role has a Trust-wide remit across acute and community services and includes weekend working as part of the IPC service provision.
Main duties of the job
The post holder will work as part of the multidisciplinary Infection Prevention and Control Team (IPCT), supporting the prevention, identification and management of healthcare-associated infections (HCAIs) and promoting high standards of IPC practice across Sheffield Teaching Hospitals NHS Foundation Trust.
Working across acute and community services, the post holder will provide specialist IPC advice and support to clinical and non-clinical teams; review patients with known or suspected infections; and support the investigation and management of outbreaks, incidents and emerging infection risks.
The role will contribute to HCAI surveillance, audit and assurance activity, identifying trends and supporting improvements in practice. The post holder will contribute to the development and implementation of evidence-based IPC policies and guidance, provide education and training, and support decontamination, environmental cleanliness and wider IPC assurance.
Working collaboratively with multidisciplinary teams, patients, families and wider stakeholders, the post holder will promote patient and staff safety and contribute to quality improvement and service development.
The post holder will manage an allocated workload with support from senior IPC colleagues and work flexibly across acute and community services, including participation in weekend IPC service provision.
About us
At Sheffield Teaching Hospitals NHS Foundation Trust, you will be joining one of the UK's largest and busiest NHS teaching organisations, providing care to the people of Sheffield and beyond across our hospital and community services.
Our people are at the heart of everything we do. Our PROUD Values and Behaviours - Patients First, Respect, Ownership, Unity and Delivery - guide how we work together, how we support our colleagues and, most importantly, how we provide safe, high-quality and compassionate care for our patients.
We are committed to creating an inclusive and supportive working environment where colleagues feel valued, respected and able to contribute to improving the services we provide. As a major teaching organisation, we also recognise the importance of learning, research, innovation and continuous improvement and are committed to supporting the professional development of our staff throughout their careers.
Joining Sheffield Teaching Hospitals means becoming part of a diverse multidisciplinary workforce with opportunities to learn, develop and make a difference across a wide range of specialist, acute and community services. We also recognise the importance of supporting colleagues to balance their professional and personal lives and provide a range of staff benefits, wellbeing support and flexible working opportunities in line with service requirements.
