Job summary
Symphony Healthcare Services are recruiting for Healthcare Assistants to be based at our Symphony North practices (Burnham on Sea, Berrow & Highbridge Medical Centres) and Burnham on Sea War Memorial Hospital (MIU) in Somerset, with the flexibility to work full or part time covering the hours of 08:00 to 20:00.
You'll be a confident, able to independently support and manage patients, while working collaboratively with our multidisciplinary teams to deliver safe, patient focused care.
We have various hours vacant - so full-time, part time and job share opportunities will be considered - please state your preference in your application.
This vacancy will close at midnight on 11th October 2026.
Main duties of the job
Working under the supervision of the Nurse Manager and strictly in accordance with specific practice guidelines and protocols, the Healthcare Assistant will assist the practice clinical team in the provision and delivery of prescribed programmes of patient care.
The post-holder will also work within the Minor Injuries Unit, supporting the Emergency Care Practitioner and Primary Care Practitioner in the assessment and management of patients presenting with minor injuries and urgent primary care needs. The Healthcare Assistant will assist with clinical observations, wound care, patient preparation and general support to ensure safe, effective and timely patient care in accordance with practice protocols and scope of practice.

**PLEASE NOTE: This is a fixed-term or secondment opportunity until December 2027 (to cover Maternity Leave).**
This static post provides an exciting opportunity to join a forward thinking and highly valued therapy team within Elderly Care.
The role involves providing specialist therapy for older people and involvement in service development projects to provide continuous improvement within the area.
The post will be operating across the six inpatient elderly care wards including the Hip Fracture Unit; working with individuals with a variety of complex needs. Additionally, there are clinical responsibilities for the weekly Movement Disorders and Falls MDT Clinics.
The role also requires a willingness to develop leadership skills, an excellent opportunity for a Band 5 or 6 looking to further their career.

We are looking for an experienced patient access coordinator to join our busy Emergency Department Admin team. This role will include being the first point of contact for patients which will including registration of patients that attend by foot and my ambulance. - Registration of patients into the department both on foot and by ambulance

We are recruiting for a Clinical Pharmacist to join North Sedgemoor PCN. This is a permanent role for an independent prescriber, who will work across our practices and as part of a multi-disciplinary team based in our Clinical Hub.
You will take responsibility for areas of long term condition management, undertake structured medication reviews, and lead on medicines optimisation and safety. Job summary
This exciting appointment is for a newly developed Consultant Psychiatrist (10 PAs) post working within the Herefordshire and Worcestershire CAMHS Learning Disability Service. The post-holder will provide a consistent and child centred provision into the CAMHS Learning Disability Service.
The post is substantive with 10PAs per week being offered. The service operates Monday to Friday, with working hours primarily between 9am and 5pm. There is an on call rota associated with this post.
The CAMHS LD service is a friendly, multi-disciplinary team that includes specialist LD/CAMHS clinicians, psychologists and medical colleagues.
The base for this position will be discussed and agreed at interview and could be anywhere across the two Counties including Hereford, Worcester, or Kidderminster.

At Band 7, the post holder will work as a developing specialist pain management psychologist, providing high-quality psychological assessment and intervention for individuals with persistent pain as part of a multidisciplinary team. Job summary
An exciting opportunity has arisen for a full-time band 6 rotational post across stroke and neurology. The post will rotate over the Acute Stroke Unit (Derriford Hospital), Stroke Rehabilitation Unit (Mount Gould Hospital),Neurosurgery/ICU (Derriford Hospital),Community Neuro Rehabilitation Team (Livewell South West) and Plym Neuro Rehabilitation Unit (Livewell South West). As a therapy service we are committed to providing regular clinical and non-clinical support andsupervision; conduct yearly appraisal; allocate time for Continued Professional Development (CPD) and involve all grades of staff in the In-Service Training programmes.

Main duties of the job
The post-holder is responsible for the provision of a quality OT service, under the direction of the Team Leader, ensuring high standards of clinical practice are provided.

This is a hybrid role, with two days per week based in the office and three days working from home.
Purpose of this role:
To analyse and evaluate the accuracy of accounting systems and procedures within the Operations Internal Audit team, following the yearly audit plan to ensure the required percentage of facilities are tested throughout the year and any risks are highlighted in a timely manner.

Main duties of the job
As the successful candidate, you will be responsible for providing effective care, with an approach based on recovery and rehabilitation, to Service Personnel (SP) and entitled civilians, with timely access to skilled and effective assessment, treatment and occupational recommendations that meets the needs of the individual and the Chain of Command (CoC).
The team expects to receive on average 10 new referrals a week and will see urgent cases by the next working day and routine cases within 15 working days. The consultant psychiatrist is expected to carry a compact caseload of the most complex and unstable cases but will also be available at short notice to provide consultation and advice to other team members. It must be stressed that the caseload contains far lower levels of morbidity and risk than a National Health Service (NHS) general adult population and there is not a high emergency burden.

We are looking for a part time practice nurse help us care for our 18,000 patients in the centre of the city.
Main duties of the job
The Practice Nurse is to support other healthcare professionals within the organisation and across the wider network to enhance integration with community, secondary, and social care services.
This role is responsible for the delivery of evidence-based nursing services, working as part of the practice multi-disciplinary team, delivering care within their scope of practice to the entitled patient population.
Working autonomously, the Practice Nurse will be responsible for a number of clinical areas such as public health and screening programmes, including infection prevention and control, chronic disease management, immunisations, health promotion and prevention, well man/woman clinics and cervical screening, Furthermore, they embed population health management approaches within general practice, targeting health inequalities and improving access.
The post-holder will be an integral part of the general practice team, as well as supporting the wider practice nurse network and multi-disciplinary team. They are to participate in formal training events and to promote best practice in their area of expertise as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

Job summary
You will support the delivery of engagement teams work in line with the 10 year plan and other key relevant policies, to support the current and future needs of commissioners, employers, trade unions. Through smart and efficient ways of working (systems and technology), enable and assist the engagement manager to develop, design and deliver network activities.
