North East Derbyshire Primary Care Network Limited View all jobs

Job summary
This is an exciting opportunity for an experienced leader to join North East Derbyshire Primary Care Network Ltd (NED PCN), as our new PCN Manager.
We are looking for a motivated, enthusiastic, innovative leader to join our focused, professional, friendly and supportive senior management team. The ideal candidate will have proven leadership experience, a proactive mindset, and will be dedicated to making NED PCN the best it can be, for its member practices, their patients, and its staff.
We are proud of all our PCN has to offer, and everything we have achieved to date, much of which is as a result of our ongoing investment in collaborative working between practices and a shared commitment to sustaining the core principles of General Practice.
Main duties of the job
As PCN Manager, you will be integral to the ongoing success of the PCN. Playing a lead role within the complex environment of Primary Care, you will be responsible for identifying and delivering a wide range of operational and business improvement initiatives, providing strategic leadership and transformational support.
This is an outward-facing role and the ability to cultivate strong, effective relationships with a wide range of stakeholders is critical to develop collaborative and innovative approaches to service delivery, which will sustain and further strengthen the resilience of the PCN, as well as aligning with the wider NHS Neighbourhood strategy.
You will be responsible for maximising the impact of all PCN resources (including financial), and you will be accountable for the development, integration and wellbeing of the PCN employed workforce. You will ensure every decision is made with the best interests of the PCN member practices, and the patients they serve, at its heart.
About us
NED PCN is an organisational group of four proactive GP practices in North East Derbyshire. The Practices are Barlborough Medical Practice, Killamarsh Medical Practice, The Springs Medical Partnership and The Valleys Medical Partnership.
NED PCN is a relatively small PCN with just over 40,000 patients. We feel that this makes us big enough to develop a successful team but small enough to be an approachable, friendly place to be.
The NHS Long Term Plan describes the prominent role Primary Care Networks play in delivering proactive, personalised and more integrated health and social care for their local populations. This will require collaborative working between organisations including GP practices, acute and community health, social care organisations and the voluntary and community sector.
Core practices opening hours of 8:00am 6:30pm Monday to Friday each week.
If you would be interested in having a confidential informal discussion about this opportunity or for further details, please contact:

Job summary
Are you passionate about understanding neurodevelopmental differences and helping children, young people and families access high-quality assessment and support?
We are looking for a motivated and enthusiastic registered professional to join our Neurodevelopmental Pathway in Selby CAMHS. This is an exciting opportunity to become part of a skilled multidisciplinary team delivering specialist Autism diagnostic assessments for children and young people.
The successful candidate will play a key role within our assessment pathway, undertaking specialist assessments, contributing to multidisciplinary formulation and supporting timely, child-centred diagnostic decision making. We welcome applications from Nursing professionals with current NMC registration.
Candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework at the point of application.
This role offers the opportunity to develop specialist skills within neurodevelopmental assessment, working alongside experienced clinicians and contributing to a service committed to continual improvement and compassionate care. Experience of Autism Diagnostic Observation Schedule (ADOS-2) and Autism Diagnostic Interview-Revised (ADI-R) assessments is desirable, although training and development opportunities may be available for the right candidate.
Main duties of the job
As part of the Neurodevelopmental Pathway team, you will predominantly contribute to the assessment and diagnostic process for children and young people presenting with suspected Autism.
Your responsibilities will include undertaking developmental and parental interviews, completing neurodevelopmental screening assessments, conducting ADOS-2 assessments and gathering information from educational settings and partner agencies. You will work closely with families, schools and multidisciplinary colleagues to develop a comprehensive understanding of each child's strengths, needs and presentation.
The role may also involve undertaking school observations where appropriate, contributing to multidisciplinary team discussions and supporting formulation and diagnostic decision making. A key aspect of the post is producing high-quality assessment reports that clearly communicate findings and recommendations to families, education professionals and partner agencies.
You will be supported by an experienced clinical team and will contribute to a culture of learning, collaboration and service improvement.
