Salaried GP (4 Sessions per Week)
We are seeking a motivated and compassionate Salaried GP to join our friendly and established team at Concept House and Sefton Road Surgery.
The position is for 4 sessions per week, worked across Thursday and Friday, with Thursday incorporating on-call duties and Friday comprising regular clinical sessions. This arrangement supports continuity of patient care and ensures appropriate clinical cover across our two practice sites.
There is also an opportunity for applicants interested in working additional sessions to undertake work with Extended Services through our GP Federation. This could provide an excellent opportunity for a GP seeking a broader portfolio role or additional clinical experience.
Please note: Thursday and Friday are fixed working days for this position. Any additional sessions through Extended Services would be separate from the four sessions required by the practice.

The post holder will be responsible for providing high-quality, patient-centred clinical care to the practice population. Core responsibilities will include:
-
Providing clinical assessment, diagnosis, treatment, and referral in line with evidence-based practice and clinical guidelines.
-
Delivering the full range of General Medical Services (GMS), including management of long-term conditions, medication reviews, preventative care, and health promotion.

-
Supporting education and training within the practice, including the supervision and development of trainees and students where appropriate.
-
Attending and contributing to practice meetings, ensuring effective communication and engagement in practice priorities and planning.
About us
We are a forward-thinking training practice with approximately 6,200 patients across Concept House and Sefton Road Surgery.
Our clinical team includes three part-time and two full-time Salaried GPs, supported by experienced ANPs, Practice Nurses, HCAs and a dedicated administrative team. We train F2 doctors and medical students, fostering a collaborative environment that encourages learning and professional development.
We pride ourselves on being a friendly, welcoming and supportive team, committed to delivering high-quality patient care. With developing interests in commissioning, minor surgery and cancer services, we encourage clinical leadership, innovation and specialist interests.
Our GP Partners work remotely but remain closely involved in clinical leadership and practice management, with weekly meetings and open communication.
Both Partners are Board Members of our GP Federation, giving us strong links with South Sefton PCN and Extended Services. These connections offer opportunities for additional sessions, portfolio working and involvement in wider primary care initiatives.

Job summary
We are looking for an enthusiastic, experienced and proactive Clinical Pharmacist to join The Castle Practice on a full-time, permanent basis. The successful candidate must have demonstrable primary care experience in a GP practice or Primary Care Network (PCN) setting.
This is a hands-on clinical role designed to add meaningful capacity to the practice. You will work closely with our GPs and wider multidisciplinary team, with a particular focus on providing daily clinical pharmacy support to the Duty Doctor, managing medicines-related workload autonomously and helping patients receive safe, timely and effective care.
A key part of the role will be taking ownership of acute prescription requests and medication-related queries from patients, community pharmacies, reception and clinical colleagues. We are looking for a proactive problem-solver who will investigate and resolve issues within their scope of competence rather than routinely passing work back to the GP or reception team. Appropriate escalation to a GP will always be supported where clinically necessary.
The balance of work will vary according to practice demand. The successful candidate will therefore need to be flexible and comfortable moving between acute workload, long-term condition reviews, structured medication reviews, medicines optimisation and other clinical priorities.
Main duties of the job
Provide daily clinical pharmacy support to the Duty Doctor and wider practice team.Manage acute prescription requests and medication queries, including urgent supply issues, monitoring requirements and queries arising from hospital correspondence or transfer of care. Undertake long-term condition reviews and associated medication reviews, including appropriate monitoring and medication optimisation. Undertake structured clinical medication reviews, particularly for patients with complex polypharmacy, frailty, high-risk medicines or multiple long-term conditions, with a proactive focus on appropriate deprescribing and reduction of dependence-forming medicines, including controlled drugs and benzodiazepines, in partnership with patients and in line with clinical guidance. Support safe and efficient repeat prescribing, medicines reconciliation and discharge medication processes. Prescribe, initiate, stop or titrate medicines within scope of competence and in accordance with local and national guidance where appropriately qualified. Provide expert medicines advice to patients and members of the practice team. Identify recurring medicines-related problems or inefficient processes and proactively develop practical solutions with the practice team. Support medicines optimisation, prescribing safety, quality improvement, QOF and relevant enhanced service work. Work flexibly across clinical priorities according to day-to-day practice workload.
About us
The post holder will be supported by experienced GPs and will work as an integral member of the practice multidisciplinary team. We are looking for someone who wants to make a visible difference to patient care, prescribing safety and the day-to-day workload of general practice.

This is a newly created post as a result of additional funding received to expand our healthcare services within our prison portfolio. This post will require working across two category C prison's, HMP Erlestoke & HMP Guy's Marsh. Both sites house sentenced prisoners and due to the stability of their populations, there is scope for some transformational work in service users lives within a team and prison setting structured to deliver this. The prison population is complex from a Mental Health perspective with SMI, neurodevelopmental disorders, substance misuse, trauma and personality disorder all significantly overrepresented in relation to the general population. The post will be divided as follows;

- 2 days HMP Guys Marsh
Main duties of the job
The post holder will work across HMP Erlestoke and HMP Guy's Marsh and provide medical leadership to the mental health in-reach teams at each site. There is significant input from psychology to work with trauma and personality disorders prevalent in the prison population and the teams work to a model of trauma informed care. The mental health services work alongside a dedicated substance misuse service provided by Change Grow Live. The role of the Consultant Psychiatrist is thus in particular to assessment and management of those conditions requiring pharmacological input, most commonly chronic psychotic illness, mood disorders and ADHD. The work takes place in the wider context of rehabilitative work delivered by the prison who deliver a variety of Offending Behaviour Programmes and interventions at these sites.
Much of the clinical work involves seeing patients within a healthcare department of the prison on an Outpatient basis. Patients are case worked by the In-reach team, analogous to a CMHT. Occasionally a prisoner may require transfer from prison to hospital under the Mental Health Act and if a prisoner becomes extremely unwell it may be necessary to assess them on the prison wing.
