The post holder will support the delivery a high quality, comprehensive mental health service for children and young people with emotional health/mental health difficulties. The role involves managing a defined caseload with a focus on working with children and young people and their families who experience a complex range of needs and difficulties. Acting as a key member of the team, the post holder will provide assessment, psychosocial, evidence-based interventions using a collaborative approach and will be skilled at risk assessment and management. They will provide supervision for other team members in an area of interest such as family work, safeguarding, SEND as agreed with the Operational Manager.
They will lead on supporting the team with the whole school approach developments and evaluation and offer liaison, advice and consultation with schools and a wide range of external agencies.
Main duties of the job
Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), and care planning in conjunction with service user and carer, evaluation of individual treatment plans for CYP with mental health needs.
Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
Assess patient's individual care needs, develop, implement and evaluate programmes of care.
Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance.
Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
To encourage and support service user and carer involvement and participation.
To attend and contribute to MHST business and development forums.
Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process. Assist in ensuring that the aims and objectives of the service are fulfilled and to identify factors which may inhibit these from being achieved.

Experienced nurse needed to join our mutually supportive practice. Experience working in a GP team isn't essential but desirable. Three years post graduate nursing experience is essential.
This requires a working knowledge of:-
- Undertake appropriate management of long term conditions, screening programmes, immunisation programmes within their level of competency and with adequate and recognised training.

- Ensure accurate completion of all necessary documentation associated with a patients healthcare.
- Perform home visits where appropriate.
- Responsibility for reviewing and acting on own sample results or those referred onto you from another nurse or associated healthcare professional, or referring to a more senior clinician when appropriate.
- Providing nursing treatments to patients in participation with general practitioners or independently, meeting agreed protocols.
- Support and promote the development of team working to ensure a high standard of nursing care within the treatment area.
- Adhere and work to defined polices and protocols, including health and safety, equality and diversity, and confidentiality within the practice

The Bordon and Petersfield Community Older Persons Mental Health (OPMH) Team operates from the main team base at Chase Hospital and covers Petersfield, Bordon and some of Waterlooville working 9-5 Monday -- Friday. This is a lively, dynamic and busy multi-disciplinary team which provides care coordination and recovery focused care for individuals with both organic/functional mental health needs. Main duties of the job
The Community Mental Health Support Worker will work as a core member of the Memory Assessment Services as well as supporting functional patients within the community. The primary aim is to support those patients who have been given a specialist assessment including dementia or a functional illness offering post post-diagnostic support. This will include treatment initiation, titration of medication post diagnostic care and support, care planning, risk management and signposting to other professional and voluntary services.
The support worker will work in accordance with an agreed plan of care and under the direction and supervision of professionally qualified staff to promote the maintenance and achievement of recovery focussed goals for the service user including post diagnostic support.

We have an exciting opportunity for a motivated and compassionate Hospital Engagement Worker to join Inclusion Recovery Hampshire, supporting individuals affected by alcohol use who are admitted to hospitals across South Hampshire.

Job responsibilities
Working with clinical, operational, digital, technical and information governance colleagues, the post holder will lead a complex portfolio of interoperability developments and translate system priorities into clear, deliverable plans. They will oversee programme governance, performance, risks, dependencies, finances, resources, benefits realisation and reporting, ensuring that agreed outcomes are achieved.
The role will manage the Shared Patient Record development pipeline, assessing and prioritising new requirements according to patient benefit, clinical risk, strategic alignment, technical feasibility, affordability and available capacity. The post holder will lead discovery and scoping activity, develop business cases and options appraisals, and present recommendations to senior governance and funding groups.
They will manage key supplier and contractual relationships, including the Oracle Health Cerner interoperability platform, ensuring effective service performance, contractual compliance and value for money.The post holder will build strong relationships with provider organisations, Integrated Care Boards, suppliers and regional and national NHS bodies. They will represent the service at relevant forums and act as a senior subject matter expert for shared records and interoperability.
They will also support Central East partners to develop a coherent roadmap across the three Shared Care Record environments, identify opportunities for alignment and convergence, and ensure local plans remain aligned with emerging NHS Single Patient Record priorities

Job summary
We are seeking a mental health nurse, social worker, psychologist or any Allied Health Professional registered with a professional body to join our CAMHS team. The post is located in our well established partnership team. and you will receive regular supervision and support to develop your clinical skills. You will provide a range of high quality interventions appropriate to the child or young person's needs which will include; mental state assessment, risk assessment and management, family and social assessment, consultation, formulation and evidence based interventions. The post holder will have a range of skills and knowledge base to deliver clinical aspects of the role in a multi-disciplinary team setting and will be competent to do this work as an autonomous practitioner within professional practice guidelines. The post holder is also expected to promote and develop partnerships and interface with other professionals and agencies that work within the Child and Adolescent Mental Health Services framework.
Main duties of the job
To assume responsibility for management of a defined caseload and exercise the highest standards of professional responsibility for the assessment, treatment and discharge of children, young people and their families/carers using specialist knowledge and evaluation of care programmes in collaboration with children, young people and their families/carers, liaising with and referring to relevant agencies.
To undertake assessment of children, young people including risk assessment, gathering appropriate information their family/ carers and other relevant agencies
To develop a formulation and plan for intervention with the child, young person and parents/carers. Communicate this to the referrer/other relevant professionals
To offer therapeutic, evidence-based practiced, interventions in line with NICE guidelines.
To implement, co-ordinate and regularly review further assessment and a range of therapeutic interventions, including risk management,
To routinely monitor effectiveness of clinical work by use of Routine Outcome Measures (ROM)
Ensure that the perspective of children, young people and families is incorporated into all aspects of assessment and treatment
The role involves travel to other locations within MFT and within the local community.

We are looking for a permanent part-time Medical Receptionist to join our team based at The Whitswood Practice part of Better Health MCR. The post is 16 hours per week: Wednesday: 8:30-13:45, Thursday 08:30-14:00, Friday 13:00 - 18:15.

Job summary
A golden opportunity has arisen for a full-time Consultant in Oral and Maxillofacial Surgery to join the University Hospitals Coventry and Warwickshire NHS Trust (UHCW NHS Trust). This is a replacement post with opportunity to develop a subspecialty interest, although an interest in Trauma and TMJ Surgery would be an advantage.
This large and dynamic Trust moved to a new PFI site in July 2006 and is now one of the leading teaching hospitals in the UK, as well as an Adult Major Trauma Centre, equipped with state-of-the-art technology and facilities. It is the clinical home of Warwick Medical School, providing wide-ranging opportunities to contribute to undergraduate and postgraduate teaching and research. UHCW is situated in the heart of the West Midlands with easy access to the rural Warwickshire countryside, Birmingham and London.
We intend to appoint a dynamic team player who will take part in the general adult and paediatric Oral & Maxillofacial services, and who will be encouraged to develop a subspecialty interest that complements the department and meets the needs of the local population.
Main duties of the job
The successful candidate will form part of a team of consultants providing a comprehensive Oral & maxillofacial Surgery service to the population of Coventry and Warwickshire. In-patient operating and some clinical activity will be at the University Hospital, but the successful candidate's remaining clinical sessions are likely to include a peripheral site which may be at Warwick Hospital, Rugby St Cross Hospital or the George Eliot Hospital, Nuneaton.
The successful applicant will be expected to manage a mixed workload of adult and paediatric patients. On call duties are currently 1 week in 8 based at UHCW NHS Trust.

We are looking for an enthusiastic colleague with an interest in Acute Medicine to join our existing team of consultants. We currently have 19 consultants, many of whom benefit from a shared job plan within their chosen speciality field.
With the implementation of Direct Access Pathways, the UHCW Acute Medicine department is undergoing an exciting period of change and expansion. We are looking for passionate physicians to play an integral role in developing our existing medical admissions and short stay services, whilst also helping shape our new Same Day Emergency Care (SDEC) and Acute Frailty units. We would welcome applicants with an interest in Acute Frailty and could support a Job Plan with this as the main area of clinical practice.
For consultants looking to undertake CESR application, we have a wealth of experience within our existing consultant workforce. We have successfully supported several of our consultants through CESR and offer peer support for staff are exploring CCT by this route.
The appointee will be expected to provide, and further develop, acute services for the Trust. This will be done in collaboration with colleagues in all disciplines, including primary and tertiary care, to provide an integrated and high-quality service for patients. This includes delivering services in an effective and efficient manner, ensuring the principles of risk management and clinical governance are always maintained.
Main duties of the job
To take on going responsibility for patients under his/her care.
Develop and maintain collaborative relationships with medical colleagues in other specialties and participate in regular clinical meetings and other postgraduate activities.

Main duties of the job
As a Pharmacy Technician, you will be providing a medicines management service to patients within the prison with the professional support of a prison service pharmacist. You will administer medicines to patients against a valid prescription, ensuring this is taken correctly, provide advice to patients about the appropriate use of medicines and promote healthy lifestyles to aid their rehabilitation.
You will dispense prescriptions and stock items, whilst supervising assistants when in the dispensary. There will also be elements of stock control involved in your role, such as undertaking stock top-up services and reviewing expiry dates and storage facilities for all medicines, reporting any deficiencies to the prison service pharmacist.

Are you a Registered Adult Nurse or Midwife who is passionate about reproductive healthcare?
We are looking for a Nurse or Midwife Practitioner to join the clinical teams at our Milton Keynes and Northampton units. You will work 23 hours per week over Tuesday to Friday. More details can be found in the further information section below.
About the role
As a Nurse or Midwife Practitioner you will provide high quality clinical care to our clients across a range of BPAS reproductive healthcare services including termination of pregnancy, STI testing & treatment, contraception & vasectomy. You'll be involved in all stages of the client pathway from consultation through to treatment & will work in all areas of the unit including theatre & recovery. This role will involve providing treatment & clinical information to clients both in person & over the phone.
About You
You will be a Registered Adult Nurse Level 1 or Midwife (practising) with active NMC registration and PIN. You'll have clinical practice experience of at least 12 months & have a keen interest in sexual health & reproductive healthcare. Clinical knowledge & experience of safeguarding and Fraser/Gillick competencies are essential for this role.
You will have excellent interpersonal, communication and listening skills and you will be adept at caring for clients in a compassionate & empathetic way.
Main duties of the job
- You will work in accordance with NMC guidelines for Professional Practice & the wider regulatory frameworks of the Abortion Act 1967 & Health & Social Care Act as well as BPAS clinical policies and procedures.
- Ensure that the client understands her options in relation to the pregnancy &, if choosing termination, meets the ground of the Abortion Act.
- Provide information on STI Testing, undertake tests and provide results and also discuss, inform and confirm the clients future choice of contraception.
- Carefully check for and implement any procedure indicated on client case notes, including administering medication for Medical Abortion procedures or cervical preparation.
- Carry out post treatment check-ups by assessing the physical condition of the client and discussing any concerns
- Complete relevant paperwork and in the event of post treatment problems, make arrangements with the clinic or refer to an NHS agency if necessary.
- To care for and support clients undergoing surgical treatments. This could include general anaesthetic, conscious sedation or local anaesthetic.
We are also looking for:
- Sound & accurate administrative competence with attention to detail
- Gynaecological nursing experience or relevant specialty i.e., sexual health, contraception
- Venepuncture / cannulation skills
- Comfortable using IT and Microsoft Packages including Outlook, Word and Microsoft Teams.
