Job summary

A comprehensive professional training programme will be provided to you, including on-the-job training, e-learning and access to our in-house education programme d Academy.
Main duties of the job
Under the supervision of a registered nurse, you will be expected to support the nursing team by undertaking a range of duties. Duties can include but are not limited to maintaining clean equipment following appropriate training, preparing supplies and packs for treatment, undertaking health checks with patients pre and post dialysis, escorting patients to the clinical and waiting areas and serve refreshments during treatments.

Your Responsibilities:

Escorting patients to clinical and waiting areas.
Serving refreshments during treatments.
Our Requirements:
Must be educated to GCSE level or equivalent.
Minimum NVQ level 2 in Health; Social Care or equivalent (preferably working towards level 3).
Ideally, previous experience in a hospital and healthcare setting.
Ability to work effectively both independently and as part of a team.
A positive and dynamic approach to delivering safe and effective care.
Excellent communication skills (oral + written) with fluency in English.

Monday to Saturday shifts (please note there are no night shifts and currently no Sunday working).

Job responsibilities
The following are the core responsibilities of the practice nurse. There may be on occasion a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels:
a. Develop, implement and embed health promotion and well-being programmes
b. Implement and evaluate individual treatment plans for chronic disease patients
c. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients health

Main duties of the job
- Maintain effective databases for Antenatal and Newborn Screening ensuring that working procedures relating to Data protection are adhered too.
- Ensure accurate records are maintained and provide statistical information to the Antenatal & Newborn Screening Coordinator on a regular and ad hoc basis.
- Use own initiative to identify deviations from expected pathways, taking actions to prevent screening incidents where possible and escalating issues to Antenatal & Newborn Screening Specialist Midwives / Matrons / Consultants as necessary.
- Use own initiative to identify missing antenatal appointments (specifically 20 week fetal anomaly scan appointments) and rectify in conjunction with the administrative team.
- Ensure that all patients eligible for antenatal and newborn screening are invited during the required screening timeframe.
- Ensure all patients who do not attend are managed and referred to the correct pathway for onward management.
- Monitor the Newborn screening process to ensure all babies are screened within required timeframe and escalate non-compliance.
- Input and maintain audit trails to demonstrate compliance with required Failsafe measures.
- Responsible for the quality of the data and will participate in regular data cleansing exercises and liaising with staff from other organisations involved in the programme.
- Responsible for the designing, improvement, maintenance and monitoring of failsafe administration procedures.

This is a very busy service and the post-holder is part of an admin team which supports the multi-disciplinary clinical team. An exciting opportunity has arisen for an enthusiastic and motivated Band 5 Dietitian to join our Integrated Nutrition and Dietetic service at Stockport NHS Foundation Trust. The Inpatient Dietitian department has 16 WTE Dietitians. The team comprises of an Integrated Nutrition and Dietetic Service Lead, Team leader, band 7, band 6 and band 5 Dietitians, Dietetic Support Workers and Secretary. We are integrated service with community dietetic and patient education colleagues, as well as with the wider inpatient Therapy directorate.
The role will be focused on providing a high standard of dietetic care to inpatients requiring nutritional support. There will be opportunities to cover a variety of ward allocations and rotations, working alongside senior dietitians in their specialist areas. You will have involvement with general outpatient clinics.
Newly qualified staff are required to complete a Preceptorship programme. Professional development is encouraged through supervision, appraisal, mandatory training and CPD activities. Supervision is provided by senior clinical staff.

The post could be filled on a full or part time basis, as a trust we promote a healthy work life balance. Main duties of the job
This post attracts 10.00 programmed activities per week as part of the Middle Grade rota
SPA is provided as part of the job plan
Regular job planning meetings will be undertaken with the post holder to ensure that the appointee is remunerated for any additional activities undertaken and that the job plan accurately reflects the overall workload undertaken by the post holder.
The supporting professional activities (SPAs) allocation within the provisional job plan is an illustrative guide and will be finalised following individual agreement with successful candidates. Further discussion on supporting professional activities will depend on the Trust's requirements and the individual's particular expertise.

Whilst this role is a B6 position, B5 therapists may apply and the role may be offered as a B5-B6 progression post if appropriate.
Main duties of the job
The successful candidates will hold a Degree/Masters level qualification or equivalent and be a member of the Royal College of Speech and Language Therapists. They will also be registered as a Speech and Language Therapist with the HCPC.
The ideal candidate will have experience of working with children as a Speech and Language Therapist. This experience could have been gained in a range of settings such as community clinics, child development teams, mainstream or special schools. They will have a good grasp of the SEND Code of Practice, universal, targeted and specialist approaches and experience of working with partners to achieve the best possible outcomes. Excellent communication skills are essential, as is the ability to work both as part of a team and autonomously.

Main duties of the job
Complete medicines reconciliations post hospital discharge, investigating queries with hospital departments and community pharmacies.
