Through the delivery of evidence-based smoking cessation support, the team plays a vital role in reducing health inequalities, preventing smoking-related illness, and helping individuals make positive, lasting changes to their lives.
Working as part of a supportive and forward-thinking service, the Tobacco Control Team provides personalised support to a diverse range of clients, helping them to overcome nicotine dependence and achieve a smoke-free future. The team works closely with healthcare professionals, community organisations, and local partners to ensure smoking cessation services are accessible, inclusive, and responsive to the needs of local communities.

Main duties of the job
To manage caseloads efficiently and in line with service priorities.
To engage with individuals by telephone, using a holistic triage approach to assess readiness for change and identify the support needed to improve their lifestyle and wellbeing.
To support the SPA in onboarding and training new smoking specialist services
To deliver training to SPA Care Navigators on the new smoking specialist service introduced into the SPA.
The role involves supporting all individuals, with a strong focus on including vulnerable adults, MH, seldom heard communities. LTCs through the Single Point of Access and within clinic settings on an unsupervised basis. This may include instructing, assessing, providing advice and/or guidance on well-being, and requires full adherence to safeguarding and regulatory requirements.
Ensure all interactions comply with relevant policies, procedures, and professional standards.
When required, assist in delivering and promoting community health events and awareness campaigns across Essex to raise awareness about smoking cessation services
Accurately record and manage client information using systems such as SystmOne and Priority Digital Health.

Job summary
The Care Co-ordination Centre (CCC) provides a single access point into the organisation for all customers. The CCC team responds to customers' needs and deals with enquiries, prioritising urgent need within a seamless integrated system. The CCC team also manages the needs of customers by maintaining standardised processes across all services. This includes:
- Managing the booking, cancellation and re-scheduling of appointments for Provide services
- Managing referrals into Provide services.
- Ensuring that the 'Referral to Treatment' procedure is adhered to ensuring a seamless flow of information between Provide services and customers.
- Providing advice and signposting customers to other appropriate services that may help them.
Main duties of the job
- Duties will include dealing with ad hoc patient queries, basic triaging of patient needs (non-clinical), accessing and checking patient and service records. This is not an exhaustive list
- Dealing effectively with first point of contact and non-clinical queries and providing routine information orally, in writing and/or electronically including via the telephone (internal and external) to and from patients, other employees, managers and possibly members of the public.

UCRT is a 7-day service providing rapid assessment and treatment for patients experiencing a health crisis in their own homes, including nursing and residential care homes. Working closely with partners across Mid and South Essex--including ambulance services, acute hospitals, GPs, social care, community services and the voluntary sector--we provide holistic assessments within two hours of referral and deliver timely, coordinated care.
As a clinical lead you will be expected to provide senior clinical leadership within the URCT team and undertake a range of clinical duties and nursing procedures along with supporting the team to manage more complex patients within their own home.
You will need to demonstrate expert knowledge surrounding community care and a commitment to provide high quality, safe and effective care for patients requiring rapid nursing intervention within their own home to prevent hospital admission.

To improve outcomes for people with frailty and complex co-morbidities and reduce the need for these people to attend or be admitted into hospital.

Job summary
UHNM is looking to recruit two full time Consultant Spinal Surgeon's. In conjunction with consultant colleagues, the post holder will provide a comprehensive complex and non-complex spine surgical service to UHNM. The desired candidate will be comfortable performing decompressive and complex spinal surgery and spine emergency surgery in a major trauma setting, with an appropriate elective sub-specialty interest which will be in line with departmental requirements. The post holder will not be required to undertake any spine deformity surgery.
The successful applicant should demonstrate that they have had dedicated fellowship training in spine surgery and are able to perform at consultant level in both complex and non-complex spine surgery including evidence of being able to fulfil a major trauma centre spine surgical on call commitment.
The successful applicant must have current or recent Major Trauma experience at consultant or senior fellow level
The successful applicant's will be expected to work across both our sites at Royal Stoke University Hospital and County Hospital. There will also be sessions in other hospitals in the West Midlands region as partnership working in a hub and spoke model (with UHNM being the hub).

About us
The Directorate includes Major Trauma, Orthopaedics, Spines and Neurosurgery. Facilities include a dedicated trauma theatre for spine surgery, dedicated elective spine surgery theatre as well as trauma and orthopaedic theatres, neurosurgery and elective orthopaedic theatres.
Our screened elective orthopaedic ward at County site is exclusively used for orthopaedic and spine surgery elective patients therefore there is no risk of cancellations for emergency demand. Spines and Neurosurgery have dedicated theatre staff for each specialty at the Royal Stoke site. There is a dedicated spine trauma list Tuesday to Friday, and at weekends theatres are shared with Trauma and Neurosurgery. Trauma theatres run 7 days per week.
UHNM is the Major Trauma Centre for the North West Midlands and North Wales and has an excellent reputation. The MTC serves the population of Staffordshire, South Cheshire, Shropshire and North Wales and is one of the busiest for spine and orthopaedic trauma.

Job summary
This post is designed to offer experience, and supervised training to doctors who have obtained the MRCP qualification, or are due to soon, and plan to pursue further training in specialty medicine in the UK. This is not a Deanery post; however, the posts are structured to provide a portfolio-based Postgraduate Training Programme similar to the Deanery model.
Essentially the structure, content and responsibilities of the posts are similar to Specialty Registrar training posts. Training will be supervised by a named consultant. At the end of the training period a Certificate of Postgraduate Training will be awarded. The duties of the posts are normal registrar-level responsibilities that include care of inpatients, specialty outpatient clinics, and relevant diagnostic or invasive procedures under consultant supervision. Training will be supervised by a named consultant.
The Programme will include Audit projects, Clinical Governance, and where possible, research. The typical candidate should have full MRCP, or at least Part I MRCP with a plan to take Part II/PACES in the next 6 months. Completion of the post will help prepare the candidate for further specialist training in the UK or abroad. Fellows will be expected to join the weekly teaching sessions and Grand Rounds. In-house training is provided for PACES/MRCP part II, and study leave will be available to help candidates attend courses for MRCP preparation, or other areas of training.
Main duties of the job
Inpatients consultant and registrar ward rounds, ward referrals. General Medicine on-call. We are very lucky to have a fantastic nursing team.
We have 2 Advanced Nurse Practitioners with a wealth of experience, Linda (our clinical nurse lead, who works 4 days per week) and Karen, who works 3 days per week. They predominantly have same-day, face-to-face appointments. Additionally, we have Elaine, our Asthma and COPD nurse.
Our Treatment Room nurses include Marie, working 4 days per week, maintaining a very high standard of care for our 766 patients with diabetics. We are very proud that she was awarded Doctor Magazine Practice Nurse of the year award in 2012 and has continued to be amazing ever since! Bea works 5 days providing our main treatment room services, performs learning disability checks and home visits for long term condition assessments and frailty checks.
Alice works as our Healthcare Assistant and is passionate about promoting health and wellbeing. She completes multiple long-term condition reviews, including CHD, AF, stroke plus MMSEs, ECGs and dressings.
We have additional support for our COPD patients with remote clinics run by 2 specialist respiratory nurses.
Administrative Team
Our Business Manager, Chamila Perera joined us in January 2023, bringing a wealth of experience with 20 years managing in General Practice. Our Operations Manager, Andrew Poole joined in August 2022 and has modernised the way we work, bringing new innovative ideas. We have a fantastic team comprising 3 medical secretaries and 12 part-time administrative staff (including team-leaders, care navigators, coders and a care-coordinator). The usual front line pressure withstanding, the team is cohesive and effective. We embrace a very open and supportive atmosphere. We host social occasions through the year, with an annual Christmas party funded by the Partners.
Out of Hours & Home Visits
Our patients have access to Out of Hours through 12 Point Care, at the nearby Sollershot GP surgery. There is also an Urgent Care Centre at Lister Hospital, Stevenage, which includes Mental Health Urgent Care.

Job summary
An excellent opportunity has arisen for a Long-term condition nurse or ACP.
This role will play a key role in delivering anticipatory, personalised care for patients living with long-term conditions and frailty across community and care home settings. With a clinical background in managing conditions such as diabetes, COPD, heart failure, and multimorbidity, the postholder will reduce avoidable deterioration, support independent living, and improve quality of life through holistic assessment, care coordination, and early intervention. Working within an integrated multidisciplinary team, the nurse will champion continuity, equity, and proactive planning particularly for vulnerable and housebound cohorts.
One of the main aims of the proactive care team is to help reduce unplanned hospital admissions and A&E attendances.

- Identify and stratify patients with moderate to severe frailty using validated tools (e.g. eFI, Rockwood)

The Essex Integrated Care Board (ICB) is a new organisation formed through the consolidation of three ICB areas in response to the NHSs ambition to deliver a more streamlined, strategic and cost-effective model of health commissioning. The Senior Advanced Analyst will lead advanced analytical and evaluative work to support strategic commissioning, population health intelligence, prevention, health inequalities, transformation and resource allocation across the ICB. The postholder will apply advanced statistical, economic and population health methods to complex system questions, helping the ICB understand drivers of demand, outcomes, variation, inequality and cost.
The role will focus on evidence generation, hypothesis testing, impact evaluation and translating complex analysis into clear intelligence for strategic decision making. The postholder will work closely with BI, data engineering, information governance and commissioning colleagues, but will not be primarily responsible for routine reporting, dashboards or data engineering.
The Senior Advanced Analyst will be required to:
- Work in partnership with other analytical functions across Essex, e.g. in NHS providers and local authorities, to ensure a coordinated picture of health and care demand and capacity to inform decision making, market shaping, procurements and contract management
- Support the development and management of the analytical team, including day to day staff management, regular appraisals, staff development and performance management as required
- Support compliance with data sharing agreements, information governance, and NHS Federated Data Platform (FDP) standards to support secure and effective data integration

The Senior Advanced Analyst will lead advanced analytical and evaluative work to support strategic commissioning, population health intelligence, prevention, health inequalities, transformation and resource allocation across the ICB. The postholder will apply advanced statistical, economic and population health methods to complex system questions, helping the ICB understand drivers of demand, outcomes, variation, inequality and cost.
The role will focus on evidence generation, hypothesis testing, impact evaluation and translating complex analysis into clear intelligence for strategic decision making. The postholder will work closely with BI, data engineering, information governance and commissioning colleagues, but will not be primarily responsible for routine reporting, dashboards or data engineering.
The Senior Advanced Analyst will be required to:
- Work in partnership with other analytical functions across Essex, e.g. in NHS providers and local authorities, to ensure a coordinated picture of health and care demand and capacity to inform decision making, market shaping, procurements and contract management
- Support the development and management of the analytical team, including day to day staff management, regular appraisals, staff development and performance management as required

We are seeking to recruit an experienced senior wellbeing practitioners to join the Tower Hamlets Education Wellbeing Service (THEWS).
THEWS is a Mental Health Support Team (MHST) established following the government Green Paper: Transforming Children & Young People's Mental Health Provision. The service has undergone recent expansion.
The three core functions of a Mental Health Support Team (MHST) are:
