About Evolve Psychology
At Evolve Psychology, we do things differently. We are looking for a part time Social Prescriber (30 hours) to join our growing team. The applicant needs to be friendly, engaging, and empathetic individual to work within General Practice to support and empower people to take control of their health and wellbeing. Social Prescribers work 1:1 with people, focusing on what matters to me and building a trusting relationship with the person referred to them. We are building strong partnerships with Adult Social Care, Specialist Mental Health Services and both local and national charities. Together, our aim is to transform how we care and support our patients. They create a shared personalised support plan and connect people to local community groups, VCSE organisations and services. The role helps people to work on their health and wellbeing, improving health outcomes and addressing wider determinants of health, such as debt, poor housing and physical inactivity, as well as other lifestyle issues and low-level mental health concerns. This approach particularly helps people with long term conditions (including support for mental health), people who are lonely or isolated, or who have complex social needs which affect their wellbeing.
Main duties of the job
This role is perfect for you if you enjoy working with people, are a good listener, can think creatively and can make beneficial connections between people and organisations. You should be proactive in getting to know and support local community assets and should be motivated by helping people to become active and independent members of their local community. You should have experience of working positively with people facing complex social and emotional challenges, and some knowledge of solution-focused coaching approaches. You will be working with people from diverse cultural and social backgrounds and will need to work confidently and effectively in a diverse, and sometimes challenging environment. A problem-solving attitude and a natural curiosity about people and communities are an asset. You will be a team-player, and will learn quickly and on your own initiative.

Job summary
We are delighted to be seeking a new colleague to join our expanding team of substantive consultants based at Wrexham Maelor Hospital. This newly created post adds to our existing team of nine consultants working across the East area of the Health Board, reflecting our ongoing commitment to service development and improvement.
This opportunity arises from our plans to reconfigure and strengthen current services, with a particular focus on enhancing our well-established provision in general geriatrics and rehabilitation.
We are committed to embracing innovation and modern ways of working. Within the team, we already support a variety of flexible working arrangements, including less than full-time and annualised consultant contracts. In addition, we have introduced three consultant therapist roles, further supporting our multidisciplinary approach to patient care.
While we have recently expanded our workforce, we are also progressing a business case to secure additional funded consultant posts. This forms part of our strategic aim to future-proof our services and ensure we can meet the increasing demands associated with an ageing population.
Main duties of the job
- Deliver a consultant-led Geriatric Medicine service at Wrexham Maelor Hospital for the diagnosis and management of both outpatients and inpatients.
- Work within a multidisciplinary team of healthcare professionals and managers to further develop Care of the Elderly services. There are many areas of future growth for our department and a successful candidate will be welcome to play an integral part in shaping future services.
- Participate in the general medical on-call rota and post-take ward rounds, providing leadership and support to the on-call medical team.
- Take consultant responsibility for allocated inpatients, with support from a designated team of junior doctors.
- Contribute to the teaching and training of junior doctors and medical students. Many consultants are Educational and Named Clinical Supervisors to junior doctors, and most consultants are offered honorary undergraduate clinical teacher status with Cardiff University.
