The post is based with the Hambleton and Richmondshire Memory Service a service for the assessment and management of cognitive disorders (working age adults and Older Adults)
You will work within a Community Mental Health Team for older people assessing and treating Functional and Organic Mental Disorders.
Work will involve attendance at Out Patient Clinics and home visits as directed by the supervising consultant.
Main duties of the job
Assess and manage common psychiatric emergencies and conditions - including competent dementia assessment and perform suicide risk assessment.
Be aware of the procedures for assessment and implementation of detaining /admitting patients under the Mental Health Act.
Make appropriate referral to other professionals.
Work with colleagues within the multi-disciplinary setting
Participate in the resident doctor on-call rota

As a Senior Substance Misuse Nurse, your responsibilities will vary. You will
- Work as part of a multi-disciplinary team, prioritising patient care to develop individualised packages of care through comprehensive assessment and recovery planning
- Deliver a range of treatment options including clinical and psychosocial interventions in both a group and 1-1 setting with recovery at the centre of our healthcare wellbeing model
- Be able to carry out comprehensive assessments, initiate and review individual recovery plans and confidently communicate the full range of treatment options available to service users.

We are pleased to invite applicants for this rare opportunity to apply for a post as a Specialty Doctor within Autonomics. The successful applicant will support the nationally and internationally recognized Autonomic Unit based at the National Hospital for Neurology and Neurosurgery.
Main duties of the job
The Autonomic Unit has pioneered the translation of autonomic research into clinical practice in the UK and worldwide, and is closely involved in national, European and international autonomic and neurological societies, and in International Consensus Groups and Task Forces providing guidelines and policy. Clinical Autonomic Scientists work along with the clinicians, to ensure that the clinical service is underpinned by a strong investigation and evidence base. They have devised and implemented new diagnostic tests for central and peripheral autonomic function, and novel treatments. They have pioneered novel, mainly non-invasive techniques for the investigation of pathophysiological mechanisms in various autonomic disorders.
The team has in the last 5 years established additional patients' pathway including the delivery of autonomic assessment, rehabilitation and immunotherapy one of the most severe but treatable autonomic failure condition, such as autoimmune autonomic ganglionopathy. We have also established the first Autonomic adolescent service and a new diagnostic assessment for postganglionic sudomotor dysfunction. The expanded spectrum of patients seen at the Autonomic unit require additional senior specialist in autonomic neurology leading on the delivery of assessment and treatment of patients with autonomic failure.

The main purpose of this post is to provide comprehensive training and developexpertise in all areas of complex inflammatory bowel disease. This post is bestsuited to a candidate aspiring to a sub-speciality career in IBD.
The postholder will be joining one of the largest IBD units in the UK alongside a diverse team of specialists, including dietitians, clinical nurse specialists, GIsurgeons, and mental health professionals, to provide cutting edge, evidencebased, patient-centred care to over 6500 patients.
This unique opportunity will provide training in complex IBD including perianal
Crohn's, treatment refractory disease, pregnancy, adolescent (13+), peri-operativemanagement and IBD endoscopy including surveillance, pouch assessment andileoscopy.
The postholder will contribute to high-quality clinical research and the developmentof guidelines that change care across London.
Main duties of the job
The post holder is expected to have broad experience of general gastroenterology disorders including independent gastroscopy and ideally diagnostic colonic. The applicant should demonstrate an clear interest in IBD. The post holder will deliver both specialist IBD and general gastroenterology clinics, relevant diagnostic and therapeutic endoscopy procedures The postholder will be expected to attend the weekly multidisciplinary team meetings, departmental clinical governance meetings, journal club and teaching. As a Clinical Pharmacist in Surrey Heath you will join the growing clinical pharmacy and multi-disciplinary PCN team. Working across a nominated general practice to support the responsibilities and deliverable of the Surrey Heath Primary Care Network.
This role is based across two practices and the Surrey Heath footprint working four days per week.
You will be responsible for supporting the practice(s) to implement effective medicines management, identifying areas for improvement, and initiating and managing change. In addition the role will be responsible for supporting the Primary Care Network to implement the medicines optimisation requirements of the new Network Contract, Direct Enhanced Service for enhanced health in care homes and other PCN medicines management responsibilities.
Main duties of the job
This is a training role which requires the successful applicant to complete a CPPE course, therefore please ensure you understand the training requirements. These roles have been designed to be interesting and fulfilling roles, providing a comprehensive range of appropriate clinical pharmacy services to general practice in Surrey Heath. The post holders will undertake resident/patient-facing activities, and associated medicines management support activities, to ensure effective and efficient use of medicines within the practices and across their registered populations.

We are advertising for a Band 3 Health Care Assistant.

Aston Ward is a 22 bedded mixed treatment offering acute mental health care to adults. The ward provides single sex accommodation and has a multi-disciplinary team including psychologists, occupational therapy, recreational workers as well as the nursing and medical team.

Main duties of the job
The Health Care Assistant will have an appreciation and knowledge of unit procedures, competency based clinical observations, and will be able to organise day to day tasks and activities based on an agreed work plan.
This role will work with more experienced Registered and Unregistered practitioners providing support to both health and social care professionals. The aim is to assess the service users within a short period of time and identify the most appropriate treatment pathway.
The successful candidates will:
- display a considerable degree of initiative andpossess mature judgment
- maintain a calm, friendly and efficient manner when dealing with service users, their relatives, the police, colleagues and visitors
- have experience in mental health
- have experience of caring for vulnerable adults

The Salaried GP will provide high-quality primary care services to patients registered with the practice, including temporary patients. The role involves delivering clinical care, participating in practice meetings, and contributing to the overall functioning and development of the practice.The Salaried GP will work collaboratively with the practice team to manage a caseload and deal with a wide range of health needs in a primary care setting ensuring the highest standards of safe, effective, patient-centred care for all registered and temporary patients.

Main duties of the job
- In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion Including reviewing and actioning lad reports, managing Docman and other clinical correspondence.

Joining our Clinical Pharmacy Team across General Practices and Care Homes in Surrey Heath and playing an important role complementing the clinical pharmacists and other members of the multi-disciplinary team (MDT). A key focus will be supporting the GPs and clinical pharmacists in implementing effective medicine management in care homes and across the practices population, and identifying areas for improvement and initiating and managing change.
Main duties of the job
- Function as part of the general practice and PCN team and working across the practices and care homes in Surrey Heath, developing professional supportive relationships.
- Carry out medicines optimisation tasks, to improve the quality and cost effectiveness of prescribing, and patient adherence and administration.
- Undertake tasks to support structured medication reviews/medication reviews.
- Build and run EMIS reports to identify/stratify patients/residents for review.
- Undertake medicines reconciliation, with support of other clinical staff review medication changes and communicate change appropriately, including for new care home residents / patients and post hospital discharge / out-patients.
- Synchronising medicines for patient transfer between care settings, linking with local community pharmacists, and referring to the clinical pharmacist for structured medication reviews (SMRs).
- Use consultation skills, where required, to work in partnership with patients/residents to ensure they use their medicines effectively (e.g. checking inhaler technique).
- Where competent, provide specialist expertise to address both the public health needs and social care needs of residents/patients, including providing lifestyle advice, service information, and helping tacking health inequalities.

We have an exciting opportunity for a dynamic and motivated Band 6 Senior Radiographer to join The Imaging Department at Harefield Hospital, part of Guy's and St Thomas' Foundation Trust.

About us
Harefield Hospital, part of Guy's & St Thomas' NHS trust is a world-renowned specialist centre for heart and lung care. Famous for pioneering organ transplants and housing one of Europe's largest dedicated heart attack centre. The hospital has 168 beds, more than 1,300 staff, five operating theatres and four catheter laboratories.
The Four catheter laboratories have dedicated angiography equipment, a Toshiba Infinix, Siemens Q.zen, Philips Allura ( about to be replaced with a Philips Azurion) and a Philips Allura FD10/10 Bi-plane. Work in the department mainly relates to cardiothoracic imaging.
Plain Film general work is also undertaken using a Philips DR unit and a Philips fluoroscopy room. Mobile x-ray units for use in the intensive care, recovery units and other wards. There is a mobile image intensifier mainly used for thoracic theatre cases.
There are 2 GE CT scanners, and 2 Siemens MRI scanners, an ultrasound and nuclear medicine service and a Dexa Scanner.
The Imaging Department at Harefield Hospital is comprised of a team of Radiographers, Imaging Support Staff, Admin Staff and Radiologists.
As well as working within the Imaging Department, Radiographers rotate through the Catheter Laboratories working in the larger multidisciplinary workforce. At our dedicated heart attack centre, we deal with heart attack emergencies from outer north-west London and further afield, providing primary angioplasty in our specialist catheter laboratories which operate 24/7.
