Our Integrated Neighbourhood Teams are forward-thinking and focused on personalised care. You'll work alongside OTs, Physiotherapists, Nurses, Specialist Practitioners and Support Staff to deliver high-quality care in patients' homes.

Main duties of the job
- Deliver holistic assessments, planning and evaluating care for patients with acute and complex needs
- Use clinical reasoning to provide evidence-based, person-centred interventions that promote safe, high-quality care
- Manage your own caseload and support effective team coordination and service delivery
- Work proactively with MDT colleagues to prevent avoidable admissions and support timely discharges
- Collaborate with patients, families and carers to set meaningful goals that maximise independence and quality of life
- Facilitate referrals and coordinate care based on individual needs

- Support, supervise and contribute to the development of junior staff, peers and students

ABOUT THE ROLE
As a Bank Residential Senior Carer at a Barchester care home, you'll look after the different needs of our residents to enable us to deliver quality, person-centred care and support. You will be responsible for leading a team of care staff, which will include training, supervisions and delegating duties on shift. You will also support with the administering and ordering of medication, reviewing and updating resident care plans and risk assessment, and liaising with GP's, district nursing teams and resident family members. Like everyone here, as a Residential Senior Care Assistant, you'll always respect the dignity and preferences of our residents as well playing your own part in creating an environment that's warm, welcoming and vibrant.
ABOUT YOU
The successful candidate will have NVQ Level 3 or Advanced Diploma in Health & Social Care to join us as a Residential Senior Care Assistant. We are looking for an enthusiastic Senior Pharmacy Technician to work as a key part of the pharmacy team within our modern automated, dispensary area. This is an ideal opportunity for an experienced registered pharmacy technician to work as an ACPT and gain skills in staff management and service development in a busy hospital pharmacy department.
