Job summary
The Clinical Lead for Community Surgery Incision/Excision is responsible for providing clinical leadership and oversight of a community-based minor surgery service for eligible adult patients registered with GP practices in Barking and Dagenham. The role involves assessing referrals for suitability, undertaking incision and excision procedures where required, maintaining high standards of clinical governance, and ensuring appropriate documentation, audit and follow-up.
The postholder will also support service quality and performance through self-audit and peer audit, monitoring patient safety and outcomes, contributing to monthly reporting, and ensuring that all interventions comply with the relevant Minor Surgery DES requirements.

Main duties of the job

The General Practice Assistant (GPA) provides a unique opportunity to work in the NHS primary care team and implement a fulfilling role providing care for the local population alongside a team of GPs and other clinical and non-clinical members.

Primary responsibilities
The GP Assistant will:
Sort clinical post and prioritise for the GP in terms of actions. Signposting documents to others such as clinical pharmacist etc.
Extract all information from clinical letters that needs coding and adding to clinical notes.
Arranging appointments, referrals, tests and follow up appointments for patients
Preparing patients to see the GP, taking a brief clinical history and performing basic assessments in readiness for the GP appointment.
Testing urine, taking blood pressure, ECGs & phlebotomy.
Completing basic administration for the GP to approve and sign documents such as insurance forms, mortgage, benefits agency forms etc.
Explaining treatment procedures to patients.

Main duties of the job

Job summary
We are seeking a practitioner psychologist to join our dynamic and busy psychology and psychological therapies provision that sits within the multi-professional mental health and well-being team at HMP Forest Bank.
Men at HMP Forest Bankare provided with the equivalence of care received within the community and as such you will be joining a large team of professionals who work across the prison establishment providing mental health and well-being support. Psychological and formulation driven intervention is offered across all areas of a stepped care model, including self-help, short and medium term intervention and therapies, MDT care planning and preventing unnecessary hospitalisations. This is a well-established role, the post holder will work alongside all staff but most closely with our other Practitioner psychologists, CBT therapist and psychological wellbeing practitioners providing intervention and therapy at Step 2 and 3, and our clinical nurse specialists, mental health nurses and psychiatrists offering Step 4 provision. You will also supervise and line manage Assistant psychologists and assist in client formulation and multi-professional planning to support transition and referrals to other services or back into the community. The post holder will also be expected to contribute to the reflective, compassionate and trauma informed culture within the mental health and neurodevelopmental conditions teams.

To work within the integrated mental health team and contribute to the psychological care provided to service users at HMP Forest Bank.

To carefully consider service-user's interpersonal interactions, management of emotional regulation, past traumatic experiences and develop detailed assessments and formulations to consider how this is linked to their mental health and behavioural presentation within custody.

A Theatre Healthcare Support Worker (HCSW) assists the surgical team by preparing theatres, supporting Patients, managing equipment and supplies, and ensuring cleanliness and waste disposal, all while maintaining confidentiality and following strict policies for safe Patient care before, during, and after surgery.
They work as part of a multidisciplinary team, requiring good communication. Main duties of the job Patient Care: Escorting Patients to theatres, transferring/positioning them, and supporting them.
Theatre Preparation & Maintenance: Setting up instruments and equipment, checking stock levels, restocking supplies, and cleaning/tidying areas post-surgery. Surgical Support: Managing specimens, assisting with blood product collection/administration, and ensuring all safety procedures (like WHO checklists) are followed.
Team Collaboration: Working closely with Surgeons, Nurses, Anaesthetists, and other staff; communicating effectively with families and Patients. Compliance: Adhering to Trust policies, mandatory training, and procedures for infection control, health & safety, and confidentiality
Main duties of the job
The post holder will work as part of the multidisciplinary team delivering a range of designated tasks/duties of a clinical and non- clinical nature including personal compassionate care to Patients, working within agreed policy and procedures.
Responsible for the moving and handling of Patients between the department, wards and other relevant departments. Responsible for the transport of medical equipment, specimens, linen, beds, blood products, hazardous substances, drugs and fluids between Theatres and relevant service areas. Cleaning of clinical areas as necessary within the department.
The post holder will assist with the preparation of equipment. The post holder will work under the supervision of the NMC Registrant or the Health and Care Professions Council Registrant at all times, who holds delegated accountability for the work designated.

Job summary
The role will be to deliver a high quality reception / customer service to the clients, visitors, staff and tenants of Sheffield Futures.
You will be enrolled on the Level 2 Customer Service Practitioner apprenticeship, where youll learn and apply your new skills whilst working for the charity in this role. Youll do your training during your working hours and we will provide you with the support you need.
Main duties of the job
The core responsibility is to deliver a high quality reception / customer service to the clients, visitors, staff and tenants of Sheffield Futures.
This will include:
- Welcoming visitors and those accessing services, for both Sheffield Futures and tenants, as part of a professional Reception Service at Star House.
- Communicating and liaising with members of the public and representatives from different agencies and organisations.
- Responding and redirecting contact via telephone and email
- Accurate data inputting and recording activity.
- Supporting the preparation of meeting rooms and refreshments.
- Other general reception and housekeeping duties.

Main duties of the job
The post-holder will provide general medical services, managing a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered patients.
You will work with the practice team to support the achievement of outcomes for Quality Outcomes Framework (QOF); Directed Enhanced Services (DES); Local Incentive Schemes (LIS) and Prescribing Incentive Schemes (PIS).
We are a paper-light practice and looking to further streamline processes.

Job summary
Join our dedicated Diabetes and Endocrinology team and make a meaningful difference to the lives of people living with diabetes.
We are seeking an enthusiastic and compassionateDiabetes Specialist Nurse who is passionate about delivering high-quality, patient-centred care and empowering people to manage their condition effectively.
Applications will be considered from NMC registered Nurses currently working within Diabetes who have relevant experience and are in the process of completing the Non medical prescribing course. For candidates who have not yet completed this, the post will be offered as a developmental post at the appropriate Annex 21 salary in accordance with Agenda for Change terms and conditions.
Main duties of the job
Working as an autonomous practitioner within a supportive multidisciplinary team, you will collaborate with consultants, specialist nurses, primary care colleagues and other healthcare professionals to deliver high-quality, evidence-based diabetes care. You will play a key role in shaping and developing services through quality improvement, audit, research and education, helping to improve outcomes for people living with diabetes across our local community.
This role is ideal for an enthusiastic and experienced nurse with specialist diabetes knowledge, strong leadership and communication skills, and a commitment to delivering outstanding patient-centred care.
Key Responsibilities- Provide specialist assessment, treatment and ongoing management for patients with complex diabetes.
- Lead nurse-led clinics and support the delivery of inpatient diabetes services.
- Triage referrals and develop appropriate management plans.
- Deliver education and training to patients, carers and healthcare professionals.
- Act as a diabetes expert, offering specialist advice and support across the Trust.
- Contribute to service development, audit, research and the implementation of best practice.
- Support the learning and development of nursing colleagues and students.

Job summary
An exciting opportunity has arisen for aTeam Leader to join the Cardiology team at King's Mill Hospital, providing day-to-day leadership and support to our Cardiorespiratory administrative services.
The successful candidate will lead and support our Waiting List Coordinators, Receptionists and administrative teams, helping to ensure patients move smoothly and efficiently through their Cardiology diagnostic pathways. The role has a key focus on waiting list management, patient bookings, pathway tracking and supporting the delivery of local and national performance standards.
We are looking for an organised, motivated and approachable individual with strong communication and leadership skills who can manage competing priorities in a busy environment. You will work closely with the Associate Operations Manager, Physiologists, Consultants and wider multidisciplinary team to provide a high-quality, patient-focused administrative service and support continuous service improvement.
Main duties of the job
The post holder will provide day-to-day leadership and supervision to the Cardiology administrative teams, including Waiting List Coordinators and Receptionists, ensuring a responsive, efficient and patient-focused service. Job summary
The South Kerrier Integrated Neighbourhood Team (INT) are seeking a forward thinking GP !
This role would suit a GP who is interested in supporting people with frailty to live and die well in the South Kerrier community and those who live in care homes. If you are a GP who is passionate about personalised care and has expertise in tolerating uncertainty to support people to live and die well in line with their wishes, then this role will be perfect for you!
The post holder will work alongside the INT Clinical Lead and INT Frailty Lead to provide GP input into the frailty model that is being collaborated designed in the South Kerrier area. The role will sit across the South Kerrier area and will therefore be required to work collaboratively with the 4 GP practices in the area, as well as NHS partners, Cornwall Council, voluntary sector providers and independent sector colleagues.
We will consider flexible working arrangements including sessions ranging from 4 sessions to 6 sessions per week.
Main duties of the job
The Frailty General Practitioner will be responsible for the following (there may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels):
- Attend and contribute to frailty multi-disciplinary (MDT) meetings
- Assist the frailty lead to co-design and develop a comprehensive frailty service for the South Kerrier population
- Act as the clinical conduit between primary, community and secondary care
- Assist practices in the management of complex frail patients, linking with the named accountable GP for the practice
- Commence and lead work on the hospital pull list reviewing previous day admission data to consider a prompt and effective discharge wherever possible
- provide a clinical lead role to facilitate collaborative working with hospital geriatricians and colleagues developing key relationships and understanding of patients need to support an integrated approach to care planning
