We are looking for a motivated and experienced Independent Prescribing, Clinical Pharmacist to join our team. You will be part of a team of five Clinical Pharmacists (FRMG & PCN SPINE) providing multi-disciplinary support to the practice and its patients through primary care. You would be based at Farnham Practice but would be expected to travel to our other locations and our PCN SPINE partner, Kumar Medical Centre, within Slough, to be involved in the wider running of the practice, such as participating in clinical meetings and contributing to the development of new services. You will be part of the Clinical Pharmacist team ensuring prescribing medication is safe, effective, and is optimised for and with our patients. Main duties of the job
To work closely with SPINE PCN GPs and act as point of contact in the practice for all medicines related queries for health care professionals and patients.
To manage the repeat prescribing reauthorisation process and undertake medication reviews of patients, some of which may need to be undertaken in the patients home.
To facilitate good working relationships between the practice and Community Pharmacies and Hospital Pharmacies.
To support the practice in delivering on Prescribing Indicators, Prescribing Budget, QOF and Locally Commissioned Services.
Manage practice formularies to improve the quality, safety, and cost effectiveness of prescribing.
Where appropriate, review pathology results for patients on known medicines.
To help plan, develop and support the introduction of new working processes within the practice to optimise the quality and safety of prescribing and undertake medication audits.

We are seeking a Highly Specialist Occupational Therapist (Band 7) to join the team on a fixed term 9-month contract, at 0.80 WTE. The current post holder is taking maternity leave.
The successful post holder will demonstrate a passionate for delivering rapid, holistic, and patient-centred care within the community. Previous experience in admission avoidance / A & E / acute medicine or similar is highly desirable. You will possess advanced assessment and clinical reasoning skills, enabling you to make timely decisions and deliver effective interventions that promote independence and reduce unnecessary hospital admissions.
This is a collaborative role, building strong working relationships across health and social care services to ensure coordinated, seamless care for patients. You will be adaptable, proactive, and enthusiastic about contributing to safe, effective, and efficient service delivery, while contributing to shared learning, professional development, and continuous service improvement.
Main duties of the job
Provide expert Occupational Therapy and extended scope assessment, intervention, and care planning for patients within the community.
Work autonomously, and collaboratively with colleagues and partner services across a various settings to support patient care.
Demonstrate strong clinical judgement in the triage of new referrals, prioritising and assigning resources to ensure the timely allocation of care for patients with urgent needs.
Provide clinical oversight of the therapy caseload within the Therapist in Charge role, ensuring the safe, efficient, and coordinated delivery of care across the therapy team.

About us
GSTT @home is a hospital at home service and a joint venture between health and social care. @home provides high quality, safe, intensive multidisciplinary treatment and monitoring in the patient's usual place of residence that would otherwise be carried out in hospital.
The service operates seven days a week and requires therapists to work flexibility across a four-week rota inclusive of weekends. Full-time staff (1.0 WTE) work between 08:00 and 20:00. This post is a 0.8 WTE fixed-term position, with anticipated shift hours of 08:00-18:30. The Department of Anaesthesia at Leicester General Hospital provides comprehensive anaesthetic services to a wide range of specialties, including obstetrics, elective orthopaedics, elective gynaecology, urology, general surgery, day-case surgery, and the High Dependency Unit (HDU).
Leicester General Hospital is a tertiary referral centre for urology and delivers major urological and gynaecological robotic surgery. The Pain Management and Sleep Medicine services are also based at this site.
The recently opened East Midlands Planned Care Centre (EMPCC) is now fully operational, providing day-case and 23-hour care across a broad range of surgical specialties, including general surgery, urology, gynaecology, ENT, and ophthalmology. The hospital has a total of 18 operating theatres, including two located within the EMPCC.

To provide Consultant Anaesthetic cover for anaesthesia, elective and emergency surgery at UHL including preoperative assessment and postoperative care.
