Job summary
- The post holder will work within their clinical competencies to provide specialist pharmaceutical support to the practice as part of a multi-disciplinary team to improve care and support safe and effective prescribing.
- The role will have a patient facing element either face to face or telephone - including chronic disease management, repeat prescription management and structured medication reviews. This will also include the management and review of patients with polypharmacy
- The post holder will need to be proactively involved in quality improvement and clinical audits as well as managing some aspects of the Quality and Outcomes Framework to deliver an excellent service within general practice
Main duties of the job
- Clinical medication reviews for patients both in person and via telephone, with single or multiple long term conditions where medicines optimisation is required
- To reconcile medicines following discharge from hospitals or intermediate care and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge and to reduce risk of readmission

Job summary
Little London Surgery is a busy and friendly training practice serving approximately 9,700 patients. Our clinical team includes three GP partners alongside salaried GPs and a wider multidisciplinary team.
We are looking to recruit an experienced and enthusiastic Medical Secretary to join our supportive team.
This is an important role within the practice, providing high quality secretarial and referral support to our GPs and wider clinical team. The successful candidate will be responsible for processing and monitoring patient referrals, managing clinical correspondence and liaising with patients, clinicians, hospitals and other healthcare services.
Previous experience of working with General Practice or primary care, together with a good working knowledge of GP referrals and NHS referral pathways is essential for this position.
We are looking for someone who is organised, accurate and able to prioritise a busy workload while maintaining excellent attention to detail. You will need to be confident working independently as well as being an effective member of the wider practice team.
Experience of e-RS and Advice and Guidance is essential, with EMIS Web and Accurx experiences desirable.
Main duties of the job
Process referrals generated by GPs and other clinicians accurately and promptly.
Use the NHS eReferral Service eRS and other appropriate referral systems.
Process and manage Advice & Guidance requests. Ensure referrals are directed to the appropriate service and pathway.
Check referral documentation for completeness and identify missing information. Liaise with clinicians where clarification or additional clinical information is required.
Monitor and action rejected, returned or incomplete referrals. Follow up referrals where necessary to support continuity of patient care.
Maintain accurate records of referrals and actions taken within the patient's clinical record.
Liaise with hospitals, community services and other healthcare providers regarding referrals.
Respond appropriately to patient enquiries relating to the progress or administration of their referral.
Provide an efficient secretarial service to GPs and other members of the clinical team.
Prepare, type and process clinical correspondence, referral letters and other documentation.

An exciting opportunity has arisen for a dedicated, experienced and motivated administrator to join The Dudley Group of Hospitals NHS Foundation Trust as our Dudley Integrated Liver Service (DILS) Coordinator. Dudley Integrated Liver Service, DILS, was launched in late 2021 strengthening our well established Gastroenterology Team, working alongside our dedicated Gastroenterology Consultants, Specialist Nurses and administrative team.
Main duties of the job
The DILS Coordinator will be responsible for supporting our Alcohol Care and Hepatology Nursing teams with the day to day running of the service. This will encompass providing administrative support within the teams, monitoring helpline, liaising with internal and external stakeholders, supporting with audits internal and external, outpatient clinic management, data collection and maintenance and other tasks that are required as per the role.

Main duties of the job
Your primary role being to undertake medicines reconciliation and take drug histories in order to ensure the patients receive the correct medications right from the start of their hospital stay to ensure that there is appropriate usage of Patient's Own medication and to promote one stop dispensing.
You will be involved throughout the patient journey helping the wards to ensure TTOs are dispensed prior to discharge, communicating effectively with our community colleagues when appropriate.
You will be an accredited checking technician (ACPT) and will have regular checking slots in the dispensary. In liaison with the Deputy Chief Pharmacist, Clinical Services and other members of the pharmacy team you will also be involved in the management of medicines on the ward, audit and financial savings projects.
You will participate in late duty, Bank Holiday and weekend rotas and be part of a seven day service.

The Approvals Service Directorate comprises distinct teams involved in the operational delivery of HRA Approval, including the Research Ethics Service in England and the Confidentiality Advice Team. The Approvals Directorate teams work to deliver the UK-wide Research Ethics Service, aligned Four Nations operational policy and process and HRA involvement in government driven initiatives.

This supports the government's pledge to cut the time it takes to get a clinical trial set up to 150 days or less. The post holder will be working with the wider team to support the successful roll out, implementation and adoption of an enhanced Technical Assurances (TA) service. The post holder's priority will be the administration of the process and engagement with the Technical Assurance reviewers to ensure a smooth and effective increase in registration of Technical Assurance reviewers whilst ensuring business continuity.
Main duties of the job
The Implementation Officer will:o Work with the Service Improvement Manager and Leads to register Pharmacy and Radiation professionals as reviewers across relevant NHS/HSC organisations, in line with existing requirements.o Manage the end-to-end registration of new reviewers, following the defined process and escalating risks and issues. o Support service improvement and redesign across Technical Assurance, Improvement, Operations and Coordination and Standardisation, implementing protocols to improve TA reviewer recruitment.o Work with the Service Improvement Manager and Professional Experts ensuring they are connected to relevant meetings and national groups where potential reviewers will be identified to sign up to the reviewer pool. Job summary
An exciting opportunity has arisen for a part-time Band 4 Senior Administrator to join the Merton Children's Administration Team, working 22.5 hours per week, Wednesday to Friday, based in Morden.
The postholder will join the Single Point of Access (SPA) team, supporting children's clinical services by managing the daily flow of referrals and ensuring they are processed, prioritised and directed to the appropriate teams safely and efficiently.

Main duties of the job
This is a full-time position comprising of 10 programmed activities (PA's) based at University Hospitals Plymouth NHS Trust NHS Trust. As per trust guidance a minimum of 1.5 PAs are dedicated to SPAs. You will be expected to participate in a minimum of 1 in 12 acute oncology service cover including weekend rounds and off site cover at night. On call duties are supported by an Oncology SpR. Appropriate dedicated specialist nursing and administrative and operational support will also be provided.
All members of the consultant team would be expected to undertake the following weekly routines:
Two Inpatient ward rounds for your named patients
Cancer site specific MDTs
New Patient and Follow Up clinics.
Supporting resident junior medical staff with training.
AOS rounds (when undertaking on call weeks)
Chemotherapy prescribing

Brent Ward provides 28-specialist Oncology inpatient beds and is supported by resident junior doctors and a team of specialist nurses. There is also an assessment area located on Brent Ward to facilitate direct admissions as well as elective procedures.
The Chemotherapy Day Case Unit currently has 24 treatment chairs and 3 side rooms and is in Fal Ward. Plans are in place to move this unit to a new ward, closer to our outpatient and radiotherapy suite.
The Oncology Research Department and a Macmillan Cancer Support Centre are also located within the Oncology Centre. The latter offering a complete package of supportive care including complementary therapies, counselling, and financial advice.
Our offices and outpatient clinics are collocated with our Radiotherapy and Radiation Physics Team in a dedicated part of the trust main building.
Staff facilities
There is a dedicated Staff Support Hub, which pulls together a range of resources and materials to help colleagues. This is designed to help find information quickly to support staff health and wellbeing. There are four areas within the Support Hub which each contain more detail and further links to advice and guidance on the different Psychological, Financial,Physical and Professional support available to all.
The successful candidate will be provided with dedicated office space, their own desk and allocated a medical secretary to support patient administration.

The successful candidate will work collaboratively within the SCBUs in CDDFT and be based and primarily responsible for the unit at Darlington Memorial Hospital.
They will manage the nursing workforce within the SCBU and supervise and develop the team. They will be responsible for coordinating the provision of nursing services and ensure high clinical standards are maintained.
They will role model compassionate and inclusive leadership and drive a culture of development and engagement to enable the team to deliver quality care whilst maximising the professional development of every member of the workforce.

- This is an additional post which has been created mainly to backfill existing clinical work and with a view to developing Sleep and Ventilation services. The successful candidate will have ample opportunity to lead the service development within Sleep and Ventilation.
- We are looking for an enthusiastic candidate ideally with extensive experience within general respiratory medicine including pleural diseases. They will also have the opportunity to develop and update clinical pathways already in place for pulmonary embolism and pneumonia.
- The trust provides Ebus services and is developing thoracoscopy services, working towards being an independent service provider.
- A dedicated SDEC (Same day emergency care unit) and PACU (planned ambulatory care unit) have been operational since 2020. The Respiratory Physicians do three clinics a week (2 pleural and 1 Respiratory Hot clinic). These deal with patients referred urgently from the Emergency Department and particularly those with pleural effusions. There is also a pathway for ambulatory management of suspected pulmonary embolism (PE). Both pathways require an enthusiastic physician to lead and develop these further.
- The Respiratory Physicians provide an in-reach service for the 42 bedded Acute Medicine Unit. Early review of patients with respiratory failure, respiratory tract infections and pleural effusions is particularly important, but cannot always be provided due to leave. With increasing numbers of Respiratory Physicians this service will be provided more reliably.
- Respiratory Services at the trust are currently delivered by 4 consultant physicians and 1 Specialist doctor. In addition to 1 Speciality Trainee and 1 Trust Grade Registrar there are 2 lung cancer nurses, 3 COPD nurses who run an Early Discharge Service and 2 asthma nurses (1 WTE). A physiotherapist supports the bronchiectasis clinics. Each Consultant has a dedicated 0.49 WTE Band 4 secretary with 1 WTE typist supporting the service.
- The respiratory ward has 27 beds. Nursing staff are trained to set up NIV for patients with an acute exacerbation of COPD and in the after care of patients with chest drains. The ward is the step-down facility for ITU, with the ability to provide high flow nasal canula support within the ward. The respiratory physicians provide ad hoc input for patients on ITU.
- The lung function unit has 3 WTE physiologists who do full lung function testing, mannitol bronchial challenge test, skin prick testing, home sleep studies and CPAP set up for OSA, cardiopulmonary exercise testing and run a HOS-AR service with the COPD nurses. New equipment was installed in September 2019.
- The Department runs a TB clinic every 4 weeks. This is supported by a TB clinical nurse specialist. A monthly meeting at University Hospital Coventry & Warwickshire provides expert advice for patients with interstitial lung disease. A weekly radiology meeting allows review of imaging with the radiologists.

As part of the Hospice in Your Home Team (HIYH) the post holder will work as a Paramedic to support palliative and end of life patients within their own homes.
Working as part of a multidisciplinary team alongside Registered Nurses and Palliative Care Assistants, the Paramedic will work in a pair to provide a responsive, mobile service undertaking short visits to several patients across the shift. Job summary
An exciting opportunity has arisen for a Band 7 Advanced Speech and Language Therapy post to provide a service for children and young people within Special Schools in Hull. Full-time and part-time applications will be considered.
The successful applicant will have the opportunity to play a pivotal role in developing clinical pathways for delivering services within Hull Special Schools and for AAC users across Hull and the East Riding of Yorkshire.
The therapist will work within a small team of Speech and Language Therapists and Specialist Speech and Language Therapy Assistants in the Special Schools, while also being part of a larger service. They will work closely with other health professionals, educators, external agencies, and parents/carers to ensure communication and feeding needs are understood, supported, and embedded throughout the school day and beyond.

An exciting opportunity has arisen for an Assistant Practitioner within the Breast Research Team at The Royal Marsden NHS Foundation Trust. The post holder will be based in Chelsea.
The Royal Marsden is a world leader in cancer research, treatment, and education. Together with our academic partner, The Institute of Cancer Research (ICR), we are the largest comprehensive cancer centre in Europe and the only National Institute of Health Research (NIHR) Biomedical Research Centre (BRC) specialising in cancer in the UK.
We are looking for enthusiastic and self-motivated team members to work alongside the Research Nursing team on our new and existing studies contributing to the ground-breaking research run out of this unit.
Main duties of the job
The Assistant Practitioner is an essential valued member of the unit team appointed to support Registered Nurses (RN) in the delivery of patient care. The role involves contact on a physical and psychological level with patients and their families. The post holder will undertake a range of activities under the supervision of a registered nurse, having been assessed and deemed competent in the activities required.
As part of a multidisciplinary team the post holder will contribute to the efficient service provision by providing clerical, clinical and sample processing support and will take a lead on the handling and storage of tissue samples and survival follow up of long-term clinical trials.
The post holder will carry out assigned duties under direct and indirect supervision of senior nursing staff, assisting in the provision of routine clinical and research care for research participants. They will also provide, a safe environment for the treatment of patients and protection of staff.
Clinical tasks undertaken by the AP will be within their sphere of competence and in line with trust policies. The research nurse team leader will be responsible for ensuring competency is achieved and maintained and as such clinical tasks to be undertaken will therefore be specific to the clinical area.

An exciting opportunity has arisen to join the Breast Research team at The Royal Marsden as a Clinical Trial Coordinator.

Our Breast Research team manages a diverse and high-profile portfolio of clinical trials, offering an excellent opportunity to develop your clinical research career within a busy, supportive and experienced team.
As a Clinical Trial Coordinator, you will take day-to-day responsibility for a portfolio of clinical trials, supporting studies from set-up through to delivery and close-out. You will work closely with investigators, research nurses, sponsors, pharmaceutical companies, the Clinical R&D team and other departments across the Trust to ensure studies are delivered safely, efficiently and in accordance with Good Clinical Practice and regulatory requirements.
Main duties of the job
We are looking for a highly organised and motivated individual with experience of working within a clinical trial setting. You will have excellent attention to detail, strong written and verbal communication skills and the ability to manage competing priorities across a varied workload.
Strong IT skills, including confidence using Microsoft Excel, and experience of data entry and data management are essential. Recent GCP training and either a Life Sciences degree or equivalent relevant experience are also required.
Experience within the NHS and/or oncology research would be advantageous, as would a good understanding of the regulatory framework governing clinical research.
This role offers an excellent opportunity for someone looking to build on their existing clinical trials experience, take greater ownership of study delivery and develop within a world-leading cancer research environment.
The post is based across The Royal Marsden's Chelsea and Sutton sites, with travel between sites and home working as required and agreed with the line manager.

The Oncology service is delighted to open the opportunity for a Clinical Oncologist ( Lung and Urology) and a Medical Oncologist (Breast and GI) to join our expanding team. Facilities include an on-site Chemotherapy Day unit. Our Radiotherapy Suite is also nearing completion of a major upgrade for completion in 2026.
University Hospitals Plymouth is proud to offer our patients a broad portfolio of clinical trials and supports all staff to engage in research, all Oncology staff are supported with job-planned time, and a team of Research staff.
Each Consultant manages their own site-specific workload, working collaboratively as a site team to ensure that new patients are seen within 7 days of referral as much as possible. Main duties of the job
These are full-time positions comprising of 10 programmed activities (PA's) based at University Hospitals Plymouth NHS Trust NHS Trust. As per trust guidance a minimum of 1.5 PAs are dedicated to SPAs. You will be expected to participate in a minimum of 1 in 14 acute oncology service cover including weekend rounds and off site cover at night. On call duties are supported by an Oncology SpR. Appropriate dedicated specialist nursing and administrative and operational support will also be provided.
All members of the consultant team would be expected to undertake the following weekly routines:
Two Inpatient ward rounds for your named patients
Cancer site specific MDTs
New Patient and Follow Up clinics.
Supporting resident junior medical staff with training.
AOS rounds (when undertaking on call weeks)
Radiotherapy planning
Chemotherapy prescribing

Brent Ward provides 28-specialist Oncology inpatient beds and is supported by resident junior doctors and a team of specialist nurses. There is also an assessment area located on Brent Ward to facilitate direct admissions as well as elective procedures.
The Chemotherapy Day Case Unit currently has 24 treatment chairs and 3 side rooms and is in Fal Ward. We hope to move this unit to a new ward, closer to our outpatient and radiotherapy suite.
The Oncology Research Department and a Macmillan Cancer Support Centre are also located within the Oncology Centre. The latter offering a complete package of supportive care including complementary therapies, counselling, and financial advice.
Our offices and outpatient clinics are collocated with our Radiotherapy and Radiation Physics Team in a dedicated part of the trust main building.
Staff facilities
There is a dedicated Staff Support Hub, which pulls together a range of resources and materials to help colleagues. This is designed to help find information quickly to support staff health and wellbeing. There are four areas within the Support Hub which each contain more detail and further links to advice and guidance on the different Psychological, Financial,Physical and Professional support available to all.
The successful candidate will be provided with dedicated office space, and allocated a medical secretary to support patient administration.
