Job summary

Main duties of the job
As a HCA on the evening team you will be allocated via our electronic system onto a handheld tablet, patients to visit in their own homes, primarily across the South of the county. These would normally be within a geographic area, but some flexibility is expected to cover patients countywide. We are looking for a caring, motivated Healthcare Support Worker to join our friendly multidisciplinary team, providing care to patients in their own homes and residential care settings.

Main duties of the job
As a Band 3 Healthcare Support Worker within the team, you will provide high-quality, person-centred care to patients in their own homes, supporting them to maintain their independence and wellbeing. You will identify and report changes in patients' conditions, maintain accurate clinical records, and contribute to care planning. Main Responsibilities
In this role, you will support the delivery of high quality care to patients on Kestrel Ward, working under the guidance of registered nurses. You will assist with both clinical and personal care, ensuring patients are treated with dignity and respect at all times. You will be involved in supporting patients with a range of complex needs, including those at end of life, those requiring rehabilitation and those in crisis. This is a varied role where you will have the opportunity to make a real difference to patient wellbeing and experience. Where you will be working

You will be working at Stanley House, a service which offers a multidisciplinary approach to its long term care provision and provides the specialised care and expert therapeutic interventions. Working as part of the multidisciplinary team, you will support and enable residents to undergo reviews of their specific and/or changing needs. Stanley House offers therapies and activities which include Physiotherapy and Art therapy and receives input from a Professor in Neuropsychiatry and monthly specialist GP clinics.
Residents at Stanley House have varying needs, from being able to manage some of their own needs, to being fully reliant. This includes personal care involving washing, dressing, shaving and helping with continence needs. Helping residents to eat is also a big part of the daily routine.
End-of-life care is also provided to enable residents to stay in the comfort and familiar surroundings at Stanley House, where their family can also be supported.
There’s plenty of fun to be had too, through supporting residents to take part in regular communal activities, from providing wheelchair and walking assistance, to afternoons of laugher with bouncy castles, family fun days and trips to the local café, shops and bowling. Its these precious moments that are most rewarding and make it all worthwhile.

Main duties of the job
As a Healthcare Assistant, your responsibilities will vary. You will

Qualifications required

Job summary
Older Persons Mental Health Team in Portsmouth are looking for a motivated and enthusiastic Health Care Support Worker to join our friendly and supportive team.
This is an excellent opportunity for someone who is passionate about making a difference to the lives of older people and is keen to develop a career in mental health services. Previous experience in older adult mental health is welcomed but not essential, as full support, training and development will be provided to the right candidate.
We are looking for someone who is caring, compassionate and eager to learn. An interest in working with older adults and supporting patients and their families is essential. You will gain experience in caring for people living with dementia and other mental health conditions, while developing your understanding of physical health, frailty and holistic patient care.
You will work both independently and as part of a multidisciplinary team, visiting patients in their own homes, monitoring progress and treatments, and helping to ensure they receive high quality care and support. Strong communication skills, a positive attitude and a willingness to learn are more important than previous experience.

You don't need previous experience in mental health care to apply. We're looking for someone who is caring, reliable and eager to learn.
In this role, you will:
- Visit patients in their own homes as part of a supportive team.
- Build positive relationships with patients, their families and carers.
- Support older people experiencing mental health difficulties, including dementia.
- Monitor patients' wellbeing and report any concerns to qualified staff.
- Carry out basic health checks following training, such as blood pressure, pulse and temperature.

Band 3 Healthcare Assistant (Windsor Dialysis Unit)
PLEASE NOTE: This post is based at Windsor Dialysis Unit, Old Ambulance Station, Maidenhead Road, Windsor, SL4 5EY- and attracts a Fringe payment of 5% of gross basic salary.
We are seeking a Healthcare Assistant to work full-time 37.5 hours per week at our Windsor Dialysis Unit - you must be able to work all shifts AM, PM and Twilight Shifts.
Windsor dialysis unit is 24 bedded dialysis unit located in the middle of Windsor Town near Windsor Castle and the racecourse.
We work as a team to provide high quality of care to our patients.
Windsor Dialysis Unit is proud to be part ofBerkshire Kidney Unit: University Department of Renal Medicine. We are recognized by the University of Reading for our excellence in clinical care, education and research that the whole department delivers.
A visit to our unit is highly recommended.
Main duties of the job
The post holder will provide high quality fundamental care to patients under the guidance and supervision of a Registered Practitioner in a caring, compassionate and sensitive manner.
Acting always in the patient's best interests.
Protecting and maintaining the patient's privacy and dignity at all times.
Providing individualized care for a diverse group of patients respecting personal beliefs and preferences.
Alert the Registered Practitioner and/or Ward/ Department Manager to any change or concern relating to the welfare of the patient in a timely manner.
Assisting nurses in the Haemodialysis Unit to provide a holistic care to patients. Training will be provided for enhanced practice.
As part of the induction process, all new Healthcare Assistants will be supported to complete the nationally recognised Care Certificate (attached document refers) developed by Health Education England, Skills for Health and Skills for Care (2015).
This fantastic opportunity will allow you to gain formal recognition for the skills and knowledge you develop within the first 12 weeks of employment within the Royal Berkshire NHS Foundation Trust.

Job summary
High Lane Medical Centre is seeking to recruit an experienced Healthcare Assistant for16 hours weekly over 2 days Mondays and Wednesdays 8am to 4.30pm. High Lane Medical Centre is situated in a semi-rural area, 10 mins from Stockport and 30 mins from Manchester. We are a forward-thinking training practice, supportive of continued education, personal development and focused on providing high quality patient care to approx. c.7,500 patients. You will be joining an enthusiastic, well led and innovative practice team comprising: a principal GP, eight assistant part-time GPs, practice manager, six receptionists, 2 administrators, two secretaries, two practice nurses and a healthcare assistant. We have 2 small residential care homes and a large number above average of elderly patients.
Our local federation is Viaduct Care who provide us with a pharmacy hub team, seasonal respiratory hub clinics, First Contact Practitioner (physio), Well-being Coordinator, Mental Health practitioner, and additional care coordinators. We host other services, in house; minor surgery, Midwifery services, Ultrasound, Audiology, joint injections, coil insertions and implants.

Job summary
Langdon Hills Medical Centre has a fantastic opportunity for a healthcare assistant to join the practice, and support the nursing team, on a full or part-time basis, for at least 25 hours per week.
