Job summary
Interested in an exciting clinical prescribing role working to delivery the new Integrated Neighbourhood model of care within Lewisham Borough as part of the Lewisham Integrated Medicines Optimisation Service (LIMOS)?
We are looking for one enthusiastic senior pharmacist with strong established clinical skills to join the already established Lewisham Integrated Neighbourhood Team Programme and improve outcomes for patients with multiple long-term conditions in Lewisham.
INTs are a model of care designed to meet the holistic needs of the local population, working with professionals and as a local community to ensure people get the right care at the right time, from the right people and to tackle health inequalities. This post will be key in ensuring delivery of this new model. As the programme will focus on patients with three of more long term conditions around CVD, diabetes and AF in the first instance, experience of management of patients in these areas would be beneficial.
Main duties of the job
The Senior Clinical Pharmacist Prescriber will be responsible for providing clinical care to conduct patient facing multi-morbidity reviews and optimise medicines management for patients across one or two Neighbourhood footprints. A key part will be close working with and supervision of non-clinical members of the core INT team including caseworkers, Health Coaches and link workers. Close working and communication with these members of the core INT and wider teams across both health and social care and primary and secondary care settings to ensure effective patient management and delivery of the INT programme will be key.

Job responsibilities
Key Result Areas & Performance:

4. Participate in clinical audit to support evaluation of service delivery and inform improvement.
5. To work closely with other members of the core INT team including link workers, key workers, health equity team, GPs and community groups, linking with clinicians and staff within other community and secondary teams within both health and social care to ensure a holistic and multidisciplinary approach to care maintained.
6. Provide medicines management expertise and support to patients, other clinical and non-clinical staff as part of the INT model of care.
7. To practice and promote appropriate evidence-based and cost-effective prescribing and .de-prescribing where appropriate.
8. Provide highly specialist medicines management support and training for GPs, nurses and other members of the multi-disciplinary care team for patients with multi-morbidity in all settings.
9. To support the Principal Pharmacist with development of protocols in collaboration with key stakeholders as necessary to support service delivery.
Service Development1. To work with the Principal Pharmacist for INT and other stakeholders to develop and refine service delivery models within the INT programme to support effective delivery of activity across all PCNs/ Neighbourhoods.
2.To work closely with colleagues and other staff involved in the INT programme to promote and embed the role of the INT into local pathways of care.
3.To provide operational and clinical support to developments around multi-morbidity and delivery of the INT model of care.
4. To support the planning and actions required to continuously improve the quality of service provided and ensure a quality improvement approach is taken to medication optimisation and any service development or re-design.
5. To contribute to and support the evaluation of outcome measures associated with the INT service
Clinical Services & Patient Care
1. To act as a clinical role model to other members of the wider MDT and provide direct supervision to non-clinical members within the core team in allocated Neighbourhood(s) to ensure improved outcomes achieved through the provision of holistic, team approach to care, which includes the safe and clinically cost-effective use of medicines.
2. To provide clinical leadership on medicines optimisation in the context of multi-morbidity across the core INT and wider setting.
3.To conduct patient facing and telephone consultations to independently review patients with complex multi-morbidity focusing on optimisation of cardiovascular disease management and improving long-term outcomes.
4. To collaborate with multidisciplinary teams (MDTs) to develop and implement individualised care plans.
5. To provide clinical advice and support to other members of the INT including complex scenarios to ensure a high standard of patient care delivered within the INT framework.
6. To manage complex and non-routine medicines management problems in patients under the INT service, making onward referrals when needed.
7.To demonstrate an intuitive grasp of situations based on knowledge, expertise and understanding and to make decisions about drug therapy with limited information.
8.To work with colleagues to facilitate robust communication and onward referral pathways to support seamless care for patients managed by the INT service where complex medicines and/or other needs are identified.
9. To ensure efficient transfer of information related to complex medicines management needs across care interfaces and reconcile information received to ensure the safe transfer of care for vulnerable patients with complex medicines management needs.
10. To work as an independent prescriber and review medication to ensure that patients medicines are optimised for safety, effectiveness and cost-efficiency in line with competency and agreed scope of practice as approved by the relevant Trust governance process.
Information & Data Management
1. To contribute to the development of quantitative and qualitative performance measures, co-ordinate their use and be involved in disseminating, acting on and sharing learning from information gathered.
2. To participate in the design and collection of clinical audit data, leading implementation in designated areas and participate in research projects to improve the quality of care and inform best practices.
3. To utilise available information from audits, national guidelines, and benchmarking data to influence and implement improvement in service delivery.
4. To work closely with PCNs ensuring seamless integration of patient data for improved care co-ordination.
Staff Management and Education and Training
1. To line manage one to two caseworkers and link workers/health coach within the INT providing supervision, guidance and support to ensure effective teamwork and delivery of goals. This will include matrix working with PCN clinical leads and business managers and leads to effectively manage and co-ordinate team members. This will include supporting appraisals, objective setting and sickness management.
2. To work with the Principal Pharmacist and other leads to identify and support the design and delivery of education and training around the INT service (including medicines optimisation in multi-morbidity and holistic, cost-effective prescribing), to colleagues, and more widely across the system when needed. Training delivery will range from small communications to formal presentations.
3. To participate and contribute to clinical training programmes for trainee professionals across Lewisham where exposure to the INT model would aid learning and practice.
4. To evaluate and ensure continuous development of own professional knowledge and competencies by attendance at in house/external training programmes, and appropriate self- education.
Financial
1. To support and advise relevant leads and GP/primary care prescribers on delivering relevant QIPP, QoF and local targets in relation to medicines optimisation.
2. Toprovide medicines advice regarding the use of cost-effective drugs across the primary and secondary care interface to the relevant stakeholders.
3. To provide a value for money service to contribute to reduction of medicines related readmission of patients with complex medicines management needs by optimising medicines management and communication about medicines across care interfaces.
4. To support the delivery of targets in the general practice setting in relation to medicines optimisation within the quality and outcomes framework and enhanced services in defined areas of multi-morbidity.
Research & Development
1. To participate in clinical audit and research and development within the INT service and support publication of service results.
2. To develop initiatives in the delivery of medicines optimisation within the INT model of care and contribute to development quality standards for clinical effectiveness.
3. To develop working relationship and research links with academic units to promote collaboration in practice and research around medicines optimisation in multi-morbidity.
Communications and Relationships
1.To communicate clearly and sensitively about patient medicines management issues both internal and external to the Trust and in Primary Care Network setting.
2. To communicate and collaborate with the multidisciplinary healthcare team including doctors, nurses, pharmacists and social care professionals on a daily basis to address the complex needs of patients with multi-morbidity.
3. Tocommunicate complex and sensitive medication/ health care related issues to patients and relatives, including medication history taking, medication review and counselling and overcoming barriers to understanding and communication such as language, hearing, visual or cognitive difficulties. Tact and persuasive skills are required when negotiating with patients to improve adherence with medication advice, with empathy and re-assurance necessary to address patient concerns. Communication may be either verbal in the form of face-to-face contact, over the telephone or in written form.
4. To work closely with other services within primary care to ensure patient referrals that support improved outcomes for patients are made with appropriate escalation where required.
5. To act as a link between primary and secondary care, facilitating smoother transitions for patients and promoting integrated care pathways.

An exciting opportunity has arisen for a Band 4 Senior Health Care Assistant within the new Health Protection Service (HPS).
The newly created HPS will work in partnership, to respond to health protection measures and threats as required. Main duties of the job

Main duties of the job
In line with the ELFT Trust Mission, Values and Strategy, the post holder will be the Clinical Lead and contribute to the highest standards of multidisciplinary health care in the BLMK Adult Autism Diagnostic Service. The post holder will also deputise as required by the Consultant Psychological Professions Lead, Bedfordshire who has professional responsibility for The Specialist Healthcare Team, Intensive Support Team and the AAD Service.
The post holder will ensure the systematic provision of a high-quality comprehensive specialist psychological service to the service users of the Adult Autism Diagnostic Service. They will provide senior clinical and strategic input into these services.
The post holder will directly provide highly specialist psychological assessments, formulations, specialist risk assessments and advise on risk management, as well as the provision of psychological advice and consultation to others involved in service user care. The post holder will also supervise and support other psychologists, psychological professionals and members of the MDT as appropriate, with devolved governance responsibility for agreeing case and workload levels, and systems of monitoring quality of practice. This is a fantastic opportunity for an HCPC registered Clinical Scientist (or someone close to registration through STP/Route 2) to join a friendly and supportive team delivering a wide range of nuclear medicine services. Main duties of the job
The role of the post-holder is to provide clinical scientist expertise in the field of Nuclear Medicine. The role involves autonomy and personal responsibility.
The post-holder is required to participate in the routine clinical service (both diagnostic imaging, non-imaging and radionuclide therapies) as well as clinicaltrials.
The post holder is expected to take a leading role in the teaching and training of staff and students.
The role involves using highly complex imaging and measurement equipment, developing and applying information technology, dealing with hazards suchas radiation and biological samples.
Interpreting and complying with extensive legislation is an essential part of the post-holder's duties.
The post-holder will be required to contribute to staff and service development in Velindre Cancer Centre, both within and outside the department.
Liaison with professional colleagues, including senior medical staff, the MPE and RPA, is an essential requirement of the post-holder's duties.

We are seeking an enthusiastic, values-driven Consultant Psychiatrist to join Birmingham and Solihull Mental Health NHS Foundation Trust in a unique combined post within the Early Intervention Service and the Mental Health Mission Midlands Translational Centre, in partnership with the University of Birmingham. This full-time 10 PA role brings together 6 PAs of permanent clinical consultant activity and 4 PAs of academic activity on a 24-month fixed-term basis. The post offers an exciting opportunity to combine senior clinical leadership in early psychosis with protected academic time as a Senior Clinical Research Fellow, helping to shape innovative, evidence-based mental health care across Birmingham and Solihull.
The clinical component will be based within the Early Intervention Service, providing expert psychiatric assessment, formulation, treatment planning, senior decision-making and clinical leadership for people experiencing early psychosis or at-risk mental states. The academic component will support the delivery and development of high-quality translational mental health research aligned with the Mental Health Mission, Trust priorities and the development of research-active clinical services.
The role is ideal for a consultant psychiatrist who wants to deliver excellent patient-centred care while building or extending a clinical academic portfolio in psychosis, early intervention, personalised care, translational research and innovative service delivery.
Main duties of the job
The post holder will provide comprehensive psychiatric assessment, formulation and management plans for people under the care of the Early Intervention Service, providing senior clinical decision-making, risk management and shared clinical leadership within a recovery-focused multidisciplinary team. They will work collaboratively with service users, families, carers, primary care and partner organisations to deliver safe, inclusive and person-centred care, and will support effective transitions into, within and out of services, including liaison with wider Trust and community pathways.
The post holder will also lead and support research activity within the Mental Health Mission Midlands Translational Centre, including study delivery, clinical trials, research governance, recruitment to studies, dissemination, publications, service evaluation, audit and future grant development. They may act as Principal Investigator or support senior research leadership for studies relevant to psychosis, early intervention, personalised prognosis, treatment innovation and reducing inequalities in access and outcomes.
The role will contribute to grant applications, academic outputs, national research networks and the development of competitive research fellowship applications, while engaging with industry partners to expand experimental medicine and innovation capacity.

The post holder will provide consultant psychiatric input to the Early Intervention Service, supporting people experiencing early psychosis or at-risk mental states. Responsibilities will include assessment, diagnosis, formulation, treatment planning, prescribing, risk assessment and management, multidisciplinary team working, care planning and liaison with families, carers and partner agencies. They will provide senior clinical advice and leadership to support safe, recovery-focused and person-centred care, including effective transitions between services and pathways.
The academic component will involve supporting translational research through the Mental Health Mission Midlands Translational Centre, including study delivery, recruitment, research governance, dissemination, publications, service evaluation, audit and future grant development. An exciting opportunity has arisen for a highly experienced and motivated Transformation Programme Lead to join the Lancashire and South Cumbria Provider Collaborative, hosted by Lancashire Teaching Hospitals NHS Foundation Trust.
The Provider Collaborative is central to the delivery of system-wide transformation across clinical, operational and corporate services. The programme is supporting the development and implementation of new models of care across Lancashire and South Cumbria, helping to deliver service recovery, clinical transformation, improved outcomes, reduced health inequalities and more equitable access to care and treatment.
The postholder will provide senior leadership across a number of complex, high-profile transformation and reconfiguration programmes. Working closely with senior clinicians, executives, providers, commissioners, NHSE, regulators, patients and wider system partners, the successful candidate will lead the planning, implementation and delivery of agreed transformation priorities.
This is a senior, autonomous role requiring strong political awareness, excellent programme management skills, and the ability to influence across organisational boundaries. The postholder will be a key member of the Provider Collaborative team and will deputise for the Portfolio Director as required.
Main duties of the job
The Transformation Programme Lead will be responsible for leading and coordinating major clinical transformation programmes across Lancashire and South Cumbria.
Key duties will include:
- Providing project and programme leadership for one or more large-scale transformation and service reconfiguration programmes.
- Supporting the development, delivery and implementation of agreed transformation plans across the Provider Collaborative.
- Working with SROs, clinical leads, trust executives, providers, commissioners and wider system partners to deliver programme objectives.
- Establishing and maintaining robust programme governance, including programme boards, steering groups, assurance arrangements, risk management, reporting and key programme documentation.
- Leading the production of strategic documents, including business cases, strategic outline cases, funding bids and implementation plans.
- Managing complex interdependencies across organisations, programmes and workstreams.

We always say that our clerical officers are the back bone of our departments and this role is no different!
Our booking services Clerical officers are the first friendly faces our patients will see on reception when attending the imaging department and your friendly and approachable manner will be key to providing a professional and courteous service to all visitors and staff. Don't worry, no two days are going to be the same!
We aim to ensure that our patients have a positive experience when coming into the department; this will include meeting and greeting them as they come into the department, supporting with the booking of their appointments, answering any queries and concerns that they may have and ensuring that an appointment letter has been sent to them.
Whilst experience working in an admin environment would be beneficial, it is not essential as full training will be provided. If you're a welcoming person with an approachable manner this will be key to providing a professional and courteous service to all visitors and staff and will help you succeed in this role!
Main duties of the job
So what does a day in the life of a clerical officer look like?
You will be performing an ambassadorial role, representing the department and the Trust as a 'front of house' reception service. You will be meeting and greeting different patients and colleagues alike! You'll have a variety of different admin tasks to complete, such as organising incoming and outgoing post, typing letters and other correspondence, filing reports and results and entering data onto our Trust systems. You'll also have the opportunity to put your excellent communication skills to the test, as you will be answering incoming telephone calls to the department and liaising with other departments across the Trust as necessary.

This dynamic job role will consist of working across 3 different sites, Royal Preston Hospital (RPH), Chorley District Hosptial (CDH) and a Community Diagnostic Centre mobile scanner. The MRI department at RPH is a bright and spacious building which provides a suite of 4 scanners including a 3T Siemens Skyra scanner, 2 x 1.5T Philips Ingenia scanners and a soon a Esaote Magnifico 0.7T open scanner. The CDH department houses a new Ingenia Ambition helium free 1.5T scanner.
You will join a welcoming MRI team who have continuously worked hard to receive 7 consecutive GOLD star ratings in recent STAR (Safety triangulation accreditation review) visits.
In this post, you will perform a diverse range of MR examinations for both adult and paediatric patients in outpatient, inpatient and emergency department settings. You will interact with a number of multi-disciplinary teams, such as the theatre team for patients undergoing MRI under anaesthetic and the pacing team, for patients with a cardiac implantable electronic device undergoing MRI.
As an MRI radiographer, you will be a key part of the imaging team, ensuring that our service user's needs are met through the delivery of high-quality care.

To assist in the provision of an efficient seven-day MRI service.
To work within the MRI team to deliver the best possible care to patients whilst producing optimum quality diagnostic images.

Job summary
An exciting opportunity has arisen to appoint a new IT & Operational Coordinator to join our friendly team of 4 Partners and 2 salaried doctors in a training practice located in Hayes, which has its own premises and gated car park. We are looking for a conscientious and experienced candidate with previous General Practice leadership experience who can support the Practice Manager with HR management, and with any other duties as appropriate.
The successful candidate will have experience of leading and managing IT within a General Practice and should be confident and highly competent in the use of EMIS Web, including advanced EMIS functions. Main duties of the job
