We are looking for an enthusiastic Ward Clerk to join our busy and friendly team at Westhaven Community Hospital.
You will have strong organisational skills and the ability to work on your own initiative as well as working well as part of a wider team. You will be the primary point of contact for patients and staff for any administrative tasks relating to the ward.

Main duties of the job
To provide administrative and clerical support, while contributing to ensure an effective and efficient service.

Are you an Employment Specialist looking to make a difference in peoples lives? Do you wish to help people who are under our Secondary Mental Health Services get back into paid employment? If so you're in luck! Midlands Partnership University NHS Foundation Trust have an opportunity to join our Step On team for a 12-month fixed term contract,
You will champion of the Trust's Individual Placement and Support (IPS) Service via the Step On Team and effectively manage a caseload of 20-25 people in receipt of secondary mental health services, in order to facilitate individuals to gain and retain paid employment and/or engage in Higher Education.
A key to success will be enabling these clients to successfully make the transition into the labour market and reducing the number of service users who are not engaged in any form of vocational activity. You will work directly with mainstream employers to secure employment opportunities for service users whilst providing on-going support according to needs therefore to enable people who have experienced mental health problems to gain and retain education/employment.
You will be trained in Individual Placement and Support (IPS) or be willing to train to diploma level.
Main duties of the job
As an Employment Specialist you will effectively manage a caseload of people in receipt of secondary mental health services.
As parts of this role, you will be expected to:
- Receive complex and sensitive information in the form of referrals from Care Coordinators, to meet regularly with Case Managers to co-ordinate and integrate vocational services into mental health treatment.
- Communicate effectively with clients, giving empathy when disclosing sensitive or contentious information, providing reassurance, training and the motivation to move forward and to overcome any barriers to understanding.
- Understand a range of work procedures and practises following the Centre for Mental Health's intensive IPS Training course, Motivational Interviewing training and benefits advice programme.

We are offering an exciting and unique new role as a Band 4 Health Care Support Worker within our Integrated Mental Health Teams (IMHT) across South Staffordshire, supporting our new First Transitions Pathway.
The First Transitions Pathway is a new service to help support individuals with complex mental health as they move from the structured support of forensic services to the community-focused care offered by our Integrated Mental Health Teams. You will be instrumental in providing practical and emotional support directly to service users, both during the immediate transition phase and in the crucial weeks and months that follow. Job summary
The post holder will be a specialist in implantable cardiac rhythm management (CRM) devices, including CRT, conduction system pacing and leadless pacemakers, working primarily in CRM follow-up clinics and implant laboratories. They will work at an advanced Healthcare Scientist level, supporting the Principal Cardiac Physiologist for CRM in complex device implantation, follow-up and remote monitoring, and providing cross-cover for the CRM service.
The post holder will support development of the CRM MRI service, including referral triage and staff competency; lead the training and development of junior staff, including PTP/STP students; and contribute to advanced clinical practice such as Healthcare Scientist/Physiologist-led ILR implantation. They will participate in CRM/EP multidisciplinary meetings, support service management, quality improvement and patient support initiatives, and undertake delegated managerial responsibilities.
The role also requires flexibility to work across Cardiac Physiology, including non-invasive diagnostics and interventional catheter laboratories for urgent coronary and structural heart procedures. Main duties of the job
- Provide cross-cover and support to the Lead Physiologist(s) for CRM in the day-to-day management and delivery of the CRM service.
- Contribute to the smooth, efficient and effective running of a high-quality clinical service and undertake additional managerial responsibilities as delegated by the Principal Physiologists.
- Work collaboratively within the multidisciplinary team, including Electrophysiology/CRM Consultants, Arrhythmia Nurse Specialists and the Heart Failure team, to support the safe and effective management of patients with cardiac implantable electronic devices.
- Demonstrate and maintain clinical proficiency and competency across a wide range of cardiac physiology techniques, working across interventional catheter laboratories and non-invasive diagnostic services as required.
- Work with the Principal Cardiac Physiologists to develop and implement advanced clinical practices within the department, including Healthcare Scientist/Physiologist-led ILR implantation services.
- Participate in the on-call service for primary angioplasty and the emergency arrhythmia pathway, providing specialist support for urgent and potentially life-threatening cardiac conditions

We are looking for a caring, organised and proactive Frailty Care Coordinator to join our team at Shap Medical Practice on a fixed-term basis until 31 March 2028, working 16 hours per week. Salary will be dependent on experience.
We are ideally looking for someone with a background in healthcare, social care or the third sector, although we are also interested in people who can demonstrate the right skills, experience and approach. Most importantly, we are looking for someone who genuinely cares about supporting our frail and older patients and who is able to build positive, trusting relationships with patients, their families and the wider team.
The postholder will work closely with GPs, nurses and the wider multidisciplinary team to manage a caseload of patients, helping to identify needs, coordinate appropriate support and ensure that patients are connected with the services and organisations that can help them.

An exciting opportunity has arisen for a Patient Pathway Co-ordinator to join our team. This key role supports the effective delivery of elective access and theatre services, ensuring patients progress smoothly through their treatment pathways and receive timely care.
The post holder will manage complex Referral to Treatment (RTT) pathways, coordinate theatre capacity, monitor service performance, and work closely with clinical and operational teams across multiple departments and sites. Job summary
An excellent opportunity has arisen for the appointment of a Band 6 Lead Practitioner within Mexborough Theatres covering both podiatry and orthopaedics.
Location: Mexborough Theatres (Rotation to other sites may be required)
Main duties of the job
The applicant must have qualifications of an Operating Department Practitioner/Registered Nurse with at least 2 years' experience at band 5 with current registration and be able to demonstrate recent expert clinical practice and be able to work dual role. It is desirable to hold the advanced life support qualification or be willing to complete.
The applicant must be able to demonstrate a positive attitude, the ability to lead a team and be open and receptive to change and have ambition to help drive the service forward. In return for your expert clinical skills and commitment to service development, we will support you to develop your leadership through a personalised development plan and leadership responsibilities.
The position is full-time 37.5 hours a week, however part-time 30 hours will be considered.
The applicant must be able to demonstrate an awareness and understanding of current professional issues and practice evidenced by a current portfolio.
Evidence of any training, mentorship or preceptorship of student ODPs and Nurses.
Evidence of any management experience will be desirable as the role involves managing the department in the absence of the Band 7.
The post will involve the successful candidate to undertake advanced life support.

Job summary
Access to Wellbeing Single Point of Access Mental Health service have an exciting opportunity for a Band 6 Community Mental Health Practitioner to join the newly formed team.
To work as a qualified practitioner in the Access Wellbeing Single Point of Access which provides a referral and access point for professionals into Access Wellbeing.
To provide specialist screening and triage via a telephone service, as part of the wider multi- disciplinary care team.
To complete face to face urgent mental health assessments as required.
To provide specialist advice to others in relation to referrals and potential treatment pathways or interventions.
Provide supervision, mentorship, training and clinical support to other professionals and the VCSE Sector as required.
To provide a high-quality person-centred approach to care delivery which always considers people's safety, privacy and dignity.
Main duties of the job
The role involves screening, assessing, and providing specialist mental health care through the single point of access, including telephone-based and occasional face-to-face assessments, risk management, signposting, and autonomous clinical decision-making. It requires maintaining advanced, evidence-based clinical expertise, effective communication of complex and sensitive information, and active involvement in quality improvement through audit, evaluation, and complaint handling.
There are leadership expectations: modelling Trust values, supporting change, and contributing to service development. The post provides guidance, supervision, and training to junior staff, promotes effective multidisciplinary teamwork, and maintains high standards of documentation and data reporting.
Responsibilities also include efficient use of resources, participation in audits and research activity, contributing to policy development, and upholding professional codes of practice. The role demands strong IT competence, continuous professional development, and the ability to challenge poor practice.
Environmental demands include prolonged desk-based work, sustained concentration, exposure to distressing information, and the need for expert judgement when advising on acute situations.

Main duties of the job

Working as part of a dedicated multidisciplinary team, you will provide comprehensive assessment, deliver person-centred rehabilitation programmes, and lead on complex discharge planning to ensure safe transitions from hospital.
