We are looking for an enthusiastic and motivated Clinical Pharmacist to join our multidisciplinary team for 30 hours per week as part of a 12month maternity cover period.
This is an exciting opportunity to work in a supportive environment where pharmacists are valued as integral clinical decision makers. You will contribute to medication optimisation, long term condition management, medicines safety, and patient facing clinics within your scope of practice. If you hold an Independent Prescribing qualification - or are working towards it - you will have the chance to use and develop your skills further.
You will be joining a friendly, collaborative team including GPs, ANPs, Nurses, HCAs, CP's, PT's, Paramedics, PCN colleagues, and administrative staff, all focused on delivering excellent patient outcomes. We offer flexibility in working hours across both Core and Extended Hours, making this an ideal role for someone seeking a balanced and rewarding position within a modern, supportive practice.
Main duties of the job
As a Clinical Pharmacist at Priory Medical Centre, you will play a key role in delivering safe, effective, person centred care. Working as part of a supportive multidisciplinary team, you will clinically assess and treat patients, provide expert medicines guidance, and contribute to longterm condition management. You will undertake structured medication reviews, optimise complex polypharmacy, support frail and vulnerable patients, and help reduce health inequalities. Independent Prescribers, or those working towards qualification, will deliver patient facing clinics and prescribe within scope.
You will lead on medicines optimisation, antimicrobial stewardship, shared care protocols, and high risk drug monitoring. The role includes managing medicines queries, supporting repeat prescribing systems, improving prescribing quality through audit and education, and contributing to QOF and enhanced services. You will work closely with community, hospital, and specialist pharmacy teams to improve patient outcomes and access to care.
Clinical supervision, professional development, and integration within a wider pharmacy network are central to the role. You will support public health campaigns, uphold safeguarding responsibilities, maintain accurate clinical records, and contribute to continuous quality improvement across the organisation.

Priory Medical Centre is a well-established Warwick GP Partnership with a list size of approximately 19,000 patients and one branch surgery. We are seeking to recruit two enthusiastic, energetic and clinically confident clinician into a Team Leader's role for Lambeth teams. They will have overall management responsibility for the delivery of care within an integrated community perinatal service, in accordance with Trust policies and procedures. There is an exciting opportunity for two Assistant Education Mental Health Practitioners to join our Seisdon Mental Health Support Team.

The Mental Health Support Teams sit at the 'getting help' aspect of the Thrive model and work closely with the different pathways as part of the Children's Mental Health Function.
In the role you will be expected to contribute to assessments and undertake planned interventions with children, young people and their families as directed by qualified practitioners within MHST and/or Supervising Practitioners.
You will be responsible for working with colleagues in enabling schools to develop and embed their mental health and wellbeing strategies. This includes liaising with teaching staff and associated roles within the education setting, as well as with partner services and agencies. They will be involved in early identification and intervention of mental health need as well as delivering emotional health and wellbeingpromotion activities.

Main duties of the job
- To prepare, plan and carry out individual or group therapeutic interventions with children, young people and parents as indicated within the child / young person's care plan, with supervision and guidance from qualified practitioners.

- To be responsible for documentation and ensure that standards of record keeping are adhered to.
- To participate in and facilitate parent support and parenting skills groups.

We have an exciting opportunity for a Band 5 Occupational Therapist, to join the Shropshire Mental Health Rotation, starting the first rotation within in Children and Young People Mental health services (CAMHS) community team.
The Shropshire mental health rotation currently includes 5 clinical areas including both in-patient, and community settings:

Each rotation is for a 12-month period, changing in November each year. You will be expected to have had experience of working within a multidisciplinary team pre-registration, excellent communication skills and be able to operate your own initiative and participate in supervision from a senior practitioner. You will provide a range of standardized Occupational Therapy assessments, have effective clinical reasoning and demonstrate a passion for working with people with a range of mental health difficulties. There will be an expectation for some community based roles to travel between sites and community locations within the geographical area covered by individual teams.
Where required, an OT mentor will be allocated to support you through the AHP preceptorship process.
Main duties of the job
- Take responsibility for the management of a complex caseload, which includes specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans (Care Programme Approach and Single Assessment Process).

Job summary
Extended Hope is a support service for young people in mental health crisis in Surrey. It is based in a small children's home in Redhill but covers the entire County. This home provides a short-break service for up to two children who experience social, emotional and mental health difficulties. It is operated by Surrey County Council and the local NHS trust. Other interventions provided include after hours contact by arrangement for young people and families and some in-reach to acute hospitals when there are admissions for mental health concerns.
Main duties of the job
Main duties include the assessment of, and support for, children in mental health crisis and assisting local care networks to effectively plan and execute intervention plans. Other focus areas are concentrated on being an impactful member of the team (along with Residential Support Workers) providing direct care for young people who are staying in the house during respite breaks.
It is important that the post holder has access to a vehicle with business insurance.

To work closely with Mindwork services during office hours, responding to emergency referrals of young people presenting in crisis.
Work as an autonomous practitioner, undertaking clinical assessment of young people who may have complex co-morbid mental health and/or social care needs and present in escalating crisis. You will be working alongside medical, nursing, social care and other professional colleagues to ensure that the holistic approach to meeting the needs of the young person and family/carer is considered. As a practitioner at the Extended Hope Service you will consider how your service can complement the short-term plan that can allow the least restrictive options for the young person at that time.
To co-ordinate and liaise with ED, Paediatric wards, PLNs, or HBPoS when young people with mental health and/or social care needs attend Acute General Hospitals or are admitted to the Paediatric wards or HBPoS. As this role in out of hours you will have access to senior colleagues should support regarding decision making be required.
Work closely with the Emergency Departments, CYPS Crisis Line, Paediatric Triage Line, the Adult and CYPS Psychiatric Liaison Services, Mental Health Crisis Teams, On Call Duty Psychiatrist, On Call Managers, ensuring young people are seen in a timely manner and risk is assessed and a management plan is developed.
To work alongside the Hope Community service to support young people who are required to stay (for emotional or psychological reasons) in acute care settings through in-reach visits and engagement insitu. This will be an adjunct to the work of that service and constitute an additional responsibility in alignment with that services Care Coordinators.
To support formal Mental Health Act assessments of young people when required, in order to guide care and treatment planning by AMHPs and s12 Drs.
To manage own caseload from referral to discharge/transfer, holding medium to high risk/needs cases.
Ensure that outcome measures for the service are implemented and audited. Contribute to relevant research and ensure that interventions on the pathway are evidence based.
Be responsible for the initial assessment and subsequent facilitation of admission to the Extended Hope House Respite facility and onward referral to Hope or other services and discharge to appropriate services.

Job summary
Are you looking for anew opportunity where you can make a difference in the lives of vulnerable young people aged 10-18 who are at risk of offending behaviour.
We have an exciting opportunity for Mental Health Nurses to join Surrey & Borders NHS Foundation Trust (SABP) in the Children and Young People Services (CYPS) Mindworks Community Hubs.
We are looking for clinicians who are creative, highly motivated, shows initiative and passionate and committed for ensuring high quality services for people with mental health needs.
There are 3 full-time posts with SABP bases across the county.
Main duties of the job
To manage a caseload of young people referred to the service, working collaboratively with other services both internal and external, to provide consultations and mental health assessments to assist young people overcome the obstacles they face and to achieve their potential. These young people will already be working with the Youth Justice Service.
To build a trusting relationship to develop a deeper understanding of the needs of young people and be able to signpost or refer to more appropriate services. This role will involve outreach into the community to meet young people in their own or choice of environment.
To work in partnership with young people, families/ carers and other services regarding decision making and planning to help them to make informed decisions, to enable them to clarify and express their needs and to co-produce solutions.

This really is an exciting time to join the Psychosis team, we have recently reviewed the community directorate and have undergone a community remodel. The Psychosis Team includes 4 pathways: Assertive Outreach, Psychosis, Early Intervention in Psychosis, and At Risk Mental State. This role will primarily be within the psychosis pathway. Main duties of the job
This is an exciting opportunity for an experienced Registered Mental Health Nurse (RMN) to join our team as a Band 6 Community Mental Health Nurse. The right candidate will be enthusiastic and a highly organised mental health professional who is able to provide high quality nursing care, assessment and treatment.
The successful candidate will provide a comprehensive Community Mental Health Nursing Service across a whole range of clients experiencing mental ill health within the psychosis pathway. The service will be delivered in collaboration with representatives from other agencies and therefore the ability to work independently and with partner agencies is essential.
It is essential that applicants can demonstrate post registration experience as an RMN, which may include inpatient and community settings.

Job summary
Exmoor Surgery is looking for an enthusiastic and experienced Clinical Pharmacist to join our friendly and supportive multidisciplinary primary care team for 2 to 3 days per week. Job summary
We are seeking an Occupational Therapist to provide fixed-term maternity leave cover in our Epsom Community Mental Heath Recovery Service (CMHRS).
We offerhigh quality and evidence based occupational therapy assessments, interventions and consultations to enable people who access the CMHRS to reach their full potential.
We use Model of Human Occupation to guide all our occupational therapy assessments and interventions.
The occupational therapy leadership team is committed to offering extensive, co-ordinated CPD programmes including a robust supervision structure, care group meetings, Trust wide and core competence training and an annual conference.
Main duties of the job
- To provide high quality and evidence based occupational therapy assessment and interventions, both group and individual, to people who access services and their carers/supporters/family members, in line with the occupational therapy pathways.
- To communicate effectively with people who access services, carers, the multi-disciplinary team and when appropriate outside agencies.
- To contribute to the generic functioning of the team.
- To provide supervision and mentoring to more junior occupational therapists, occupational therapy apprenticeships, support workers and activities coordinators (as appropriate)
- To provide occupational therapy student placements.
- To contribute to service development and audits.
- To take on the role of lead professional as appropriate.
