Job summary
As part of the NHS England Additional Roles Reimbursement Scheme (ARRS), we are seeking a skilled and motivated Advanced Nurse Practitioner (ANP) to join our Primary Care Network (PCN) team. The Advanced Nurse Practitioner (ANP) will be able to assess and treat varied clinical cases, having advanced skills to assess the patient with a differentiated or undifferentiated clinical diagnosis. Advanced communication, problem-solving and decision-making skills will be used to initially assess and provide on-going patient care. Working within local and nationally agreed protocols and guidelines, the ANP exercises independent judgement to assess, investigate, diagnose, plan, implement and evaluate the clinical care and management of patients.
The role is for Monday, Thursday and Friday
Main duties of the job

- Provide expert clinical assessment, diagnosis, and treatment for a wide range of acute and chronic conditions, within the scope of practice.
- Conduct comprehensive health assessments for patients with complex care needs and those with multiple long-term conditions.
- Prescribe medications and interventions in accordance with clinical guidelines and scope of practice.
- Undertake home visits as needed to assess and support patients in their own environment.
Patient Management
- Develop, implement, and evaluate patient management plans in collaboration with patients, their families, and the multidisciplinary team.

The successful applicant will be based at The Limes Medical Centre and will also support patients registered at Chapel Street Surgery, offering the opportunity to work across practices and gain experience with a diverse patient population.
You will be part of a supportive multidisciplinary team, working closely with both PCN and practice colleagues. Our wider PCN team includes Clinical Pharmacists, First Contact Practitioners, Care Coordinators, Social Prescribers, Mental Health Practitioners and Health and Wellbeing Coaches, alongside operational and administrative staff. At practice level, you will work alongside GPs, Practice Nurses, Healthcare Assistants, Reception and Care Navigation teams, and Practice Management staff. Multidisciplinary working is central to our approach, helping us deliver joined-up, high-quality care while supporting learning, development and career progression.

Job summary
The Medical Centre, Driffield, is looking for a salaried GP to work between 6-8 sessions per week. Driffield is a market town in the East Riding of Yorkshire surrounded by the picturesque wolds, with easy links to nearby York and Beverley and is approximately 10 miles from the beautiful Yorkshire coastline.
The main surgery is located in Driffield town centre with a branch surgery in the village of Wetwang 6 miles west of Driffield. It is a semi-rural practice with 12500 patients.
We dispense to approximately 50% of our patients from Driffield and Wetwang.
Main duties of the job
The practice is a young, enthusiastic team which comprises of 2 partners, 5 salaried GPs, and 4 Advanced Nurse Practitioners. We are a training practice with GP trainees from ST1-ST3.
About us
1. A good rating from CQC
2. High QOF achieving (>99%)
3. Above CCG and national average ratings on the GP patient survey
4. 3 GP registrars have all remained in the practice and we have a very low staff turnover rate
5. Paper light practice using Emis Web.
We do participate in extended access which is shared between all GPs working out to approximately 3 weekend sessions of 4 hours totalling 12 hours per year.

JOB TITLE: SALARIED GENERAL PRACTITIONER
REPORTS TO: THE PARTNERS (Clinically)
THE PRACTICE MANAGER (Administratively)
HOURS: 6-8 SESSIONS PER WEEK
Job Summary:
The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
Clinical Responsibilities:
- In accordance with the Practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers within the organisation
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and early signs of illness
- In consultation with patients and in line with current Practice disease management protocols, developing care plans for health
- Providing counselling and health education
- Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
- Recording clear and contemporaneous consultation notes to agreed standards
- Collecting data for audit purposes
- Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
- Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate

We are looking for a Social Prescriber Link Worker to join our team within Bosworth Primary Care Network.
This role is for 18.75 hours per week over four of our member practices Ratby Medical Centre, Heath Lane Surgery, Desford Medical Centre, and Newbold Verdon Medical Practice. The team undertake a wide variety of admin tasks including, checking patients in for appointments, booking appointments, answering telephone queries and processing incoming post. The role would suit some one who is good at multitasking, problem solving with an empathetic nature, who has a passion for helping patients and providing excellent customer service.
The reception team is currently made up of 12 staff members who cover the Outpatients activity predominately based at Dorking & Redhill with travel to our other sites in Epsom and Warlingham. The staff work on a rota basis, which includes some evenings and weekends, Monday to Saturday, the shift pattern varies from week to week with no set days, only set hours.
The ideal candidate will have previous patient or customer facing experience and excellent communication skills. Within this fast-paced environment it is necessary to remain calm and maintain a high level of attention to detail. You will be able to help manage the workload efficiently and effectively on a daily basis to facilitate the patients care being as smooth and efficient as possible.
We will provide the training necessary to use our inhouse IT systems, however a good working knowledge of Microsoft Office would be beneficial. You will be required to maintain patient confidentiality at all times.
Main duties of the job
The role will be shift based at all sites, predominately based at Dorking & Redhill with travel to our other sites. To undertake tasks essential to the smooth, effective provision of quality care and efficient level of service for patients and their families. This will include covering the reception at DHC for some evenings and weekends on a rotational basis.
It is essential that the post holder exercises initiative and judgement whilst maintaining a high level of confidentiality and diplomacy at all times without immediate access to line supervision.

An opportunity has arisen for a part time medical receptionist to join our friendly and hardworking GP practice to cover maternity leave. About us
Firdale Medical Centre is a member of Northwich Primary Care Network. We currently look after over 11500 patients as well as mentor and support one of the local nursing homes.
The practice is led by four GP partners, and we employ five salaried doctors. To complete the clinical team we have two nurses, health care assistant, a pharmacist, pharmacy technician and two Musculo-skeletal (MSK) specialists.
We are also teaching practice supporting foundation and specialty doctors.

Hoveton and Wroxham Medical Centre is looking for a well organised and motivated practice administrator to join us for either on a full or part time basis workingMonday to Friday 8am 4pm or 8.30am 4.30pm. Main duties of the job
- Scanning, summarising and coding of clinical correspondence. Following the practice protocol and using own initiative to highlight urgent actions to GPs where relevant.
- Summarising and management of medical records, including the use of the National Data Repository.
- Processing OOH reports, NHS 111 reports and Admission & Discharge information.
- Recording cervical screening and bowel screening results within SystmOne.
- Supporting the Nursing Team with some of the Admin functions in relation to the Cervical Screening Management Service.

Main duties of the job
As a Salaried General Practitioner within our group, you will play a key role in delivering high-quality, patient-centred care while contributing to the continued development of modern general practice.
The role offers variety and flexibility, combining traditional face-to-face clinical sessions with digital-first care through our Digital Triage Hub. You will assess and manage a wide range of patient presentations, provide clinical decision-making and support timely access to care across our practices.
You will work collaboratively with GPs, ARRS colleagues and the wider multidisciplinary team, providing clinical leadership, supervision and support where appropriate, including contributing to the development of our wider workforce.
The role includes responsibility for providing safe and effective patient care through consultations, telephone reviews, digital triage, home visits and appropriate follow-up. You will contribute to quality improvement, service development and the ongoing evolution of care pathways across our organisation.
