Main duties of the job
The Community Staff Nurse in the School Nurse Team is a registered general nurse working as part of the public health nursing team in providing preventative and supportive health care to children, young people and their families. The post holder works with a public health focus under the supervision of a Specialist Community Public Health Nurse (SCPHN) with a defined population. As part of the public health nursing team, the post holder collaborates with a wider multi-disciplinary and inter-agency team in offering public health services to the community. The work takes place in a variety of settings, including home, clinic, and community venues. The main responsibilities include delivery of the healthy child programme, health assessments in schools, and participation in case conferences and inter-agency meetings. The nurse will also assist the SCPHN in providing targeted support to children identified as at risk or with complex needs.

Main Responsibilities
- Delivery of the healthy child programme at four levels as delegated by the Specialist Community Public Health School Nurse.
- Ability to recognise health needs for individual clients and situations in relation to the health of children, young people, individuals and families, sometimes in complex situations, identifying situations where referral for support from Public Health Specialist is necessary.
- Undertake health assessments in schools.
- Prepare for, attend and participate in relevant case conferences and inter-agency meetings as delegated by the Specialist Community Public Health nurse (SCPHN). Undertaking health needs assessments and care planning as appropriate.
- Support the SCPHN in providing specialist targeted support to meet the needs of children identified as at risk or with complex needs and ensure Partnership plus plan in place.
- Support the SCPHN to identify, prioritise and implement programmes of support in response to the public health needs of the school age population
- Undertake full assessment of individual public health needs and provide support or onward referral as appropriate. Specifically; sexual health, smoking and weight management
- Offer advice and support to children, young people and families in accordance with evidence based practice.

This role is 30 hours per week and will be based in the Woking & Surrey Heath area of Surrey.

- Excellent IT skills and able to use electronic client record such as SystmOne or Emis

Applications are invited to the post of a Breast Trust Grade Registrar (6 months) based at Ashford and St Peter's Hospitals NHS Foundation Trust.
This post is intended for someone who has completed basic surgical training with attainment of MRCS.
This post is an ideal opportunity for the successful candidate to gain more experience within the speciality of Breast to improve their competitiveness at National Selection for higher training.
The post will involve supporting one-stop (triple assessment) urgent suspected cancer clinics, assisting in theatre lists, and attending MDT meetings and presenting, as necessary.
The department of Surgery at Ashford and St Peter's is a friendly and dynamic one. Working as a member of the team the successful candidate will provide a high-quality service that aims to deliver excellent patient care.
There is also a requirement to work the General Surgery on-call rota at St Peters Hospital.
Main duties of the job
- To conduct daily ward rounds with the medical team, review patients and formulate appropriate clinical plans.
- To be part of the seven day working model adopted by the Trust.
- To be part of the emergency on call rota
- To ensure continuity of a high standard of care, assessing the health needs of the patients and taking appropriate actions including:
- Ordering of diagnostic tests.
- Making and receiving referrals.
- Discharging patients within agreed protocols.
- To provide advice and support to patients, their families and other healthcare professionals following diagnosis through to treatment.
- To accept responsibility for own caseload, ensuring all patients have an accurate plan of care recorded, which reflects the assessments undertaken and incorporates the issues and recommendations made, ensuring clear documentation in the patient's records.
- To be actively involved in in-patient care, liaising with ward staff and other members of the clinical team as appropriate.

Are you a compassionate Registered Nurse looking to make a meaningful difference to patients and families at one of the most important times in their lives? We are looking for a Community Staff Nurse to join our Hospice at Home team, supporting the delivery of specialist palliative and end-of-life care across North West Surrey.
Working as part of our Community Services Team, you will help patients receive high-quality, personalised care in their own homes and communities, enabling them to achieve their preferred place of care wherever possible. This is a rewarding opportunity to use your clinical skills, develop your palliative care experience and work alongside a supportive multidisciplinary team.
Main duties of the job
In this role, you will provide holistic assessment, care planning, symptom management, emotional support and practical care for patients with specialist palliative and end-of-life care needs. You will work closely with Clinical Nurse Specialists, GPs, District Nursing Teams and other professionals to coordinate safe, responsive and person-centred care.
You will also maintain accurate clinical records, support families and carers, contribute to bereavement support where appropriate, and act as a positive clinical role model for junior colleagues, students and new team members.
About you
We are looking for a registered nurse with post-registration experience, excellent communication skills and a commitment to delivering compassionate, dignified and patient-centred care. You will be confident working both independently and as part of a multidisciplinary team, with the ability to manage a varied caseload and respond sensitively to the needs of patients, families and carers.
Experience in palliative care, oncology, community nursing or multi-agency working would be an advantage, although we are equally interested in hearing from nurses who can demonstrate transferable skills, a willingness to learn and a passion for hospice care.

Duties and responsibilities for the role include but are not limited to:
- To maintain the integrity and stock of the Dispensary and Pharmaceutical stores in accordance with current regulations and requirements.

To undertake intravenous injection of contrast media in compliance with the Imaging Service protocol, following training and if required by the role.
To supervise, advise and mentor staff.
To work closely with the Superintendent Radiographers to provide optimum patient care and constantly seek ways of improving standards and performance.
Organise, plan and prioritise workload according to clinical need. Take responsibility for and organise sections of the department as required.
To oversee and assist in the training of new staff and continuous development of departmental staff within the area to enable adequate cover and to meet ongoing demands of the service.
To assist in the development, documentation and monitoring of protocols for working practice within the area.

To monitor the performance of equipment and report and act upon all potential and actual faults and hazards. Ensure that the appropriate documentation has been completed and filed. Ensure that the appropriate staff, including the Superintendent, are kept informed.
To ensure that a quality assurance programme for all equipment is implemented and maintained, ensuring quality assurance has been performed following servicing, repair and installation. Ensure that corrective actions are taken as required. Records must be kept and filed appropriately.

Management of Staff, Leadership and Development
To deputise for the Superintendent Radiographer as appropriate which may include managing radiography staff ensuring appropriate delegation and supervision of tasks.
To demonstrate leadership and to motivate staff.
To manage rotas ensuring adequate cover for service requirements in and out of hours, maximising the cost effective, efficient use of staffing resources.
To assist the Superintendent Radiographers to identify and meet staff training and development needs.
To provide training, support, guidance and supervision to Radiographers.
To identify to the Superintendent Radiographers where staffs performance or behaviour is below an acceptable standard and to take initial informal counselling action where appropriate.

Responsibilities for Department Administration and Operation
To ensure that all clinical and non-clinical information records are fully and accurately completed and maintained.
To ensure all data input onto the Radiology computer system by all staff is accurate and updated as necessary. Where required, to facilitate image transfers to other Trust upon request.
To be proactive in assisting the Superintendent Radiographer to meet financial and contractual targets, and cost improvement plans (CIP).
Where required: To oversee the consumable stock ordering, receipt and recording for the area according to departmental procedures and within agreed financial limits.
To maintain levels of cleanliness and infection prevention to meet the Trusts Infection Prevention Policies. This includes compliance with audit requirements for the area.
Maintain clean and tidy work areas and waiting rooms participating in regular cleaning rotas for work areas in line with department and Trust policy. This may require contact with bodily fluids and on occasions unpleasant smells.

Main duties of the job
The post holder will be responsible for delivering health promotion, outreach services, Surrey Chlamydia and Gonorrhoea screening programme and Condom Distribution Service for under 25s. This post is permanent, based in Woking and providing outreach services across Surrey.
The service operates in partnership with other commissioned community sexual health support in Pharmacies, General Practice and the Third Sector and complements clinic based work. The post holder would provide training sessions to groups of staff and other professionals and awareness raising sessions to young people.
Candidates should have experience in Health Promotion and or Sexual Health Services. The post holder would work regular hours with flexibility to work very occasional evenings and weekends (to cover specific events).

Job Introduction
We are looking for an enthusiasticChildrens Occupational Therapist to join our multidisciplinary Childrens Community Health Team in Surrey. In this role, you will deliver highquality Occupational Therapy assessment, diagnosis and treatment to children with a range of developmental, physical and sensory needs.
You will work closely with families and carers, supporting them in developing effective selfmanagement strategies to promote independence and wellbeing.
You will be based at one of our Surrey office hubs, with the flexibility to work in a hybrid pattern as part of HCRG Care Groups flexiblefirst approach. Our teams are agile and collaborative, ensuring you are supported while making a meaningful difference to the lives of children and their families across the county.
Main Responsibilities
As an Occupational Therapist, youll play an important part in supporting children and their families across our service. Your role will include:
Planning & Organising
- Providing daytoday support and guidance to therapy colleagues.
- Managing and prioritising your own caseload with independence and confidence.
- Assessing childrens needs and offering clear, practical advice to help them and their families manage challenges at home and in school.
- Recommending and arranging specialist equipment when needed.
- Helping the Team Lead and Therapies Manager meet service goals and quality standards.
- Taking part in team supervision, training and regular meetings.
Patient Care
- Carrying out assessments, making diagnoses, planning treatment and delivering therapy tailored to each childs needs.
- Communicating sensitively with children and families, especially when discussing complex or emotional information.
- Working closely with parents, carers, schools and professionals from other services to ensure joinedup support.
- Training families and school staff in the safe use of equipment and therapy strategies.
- Adapting your approach to suit each childs cultural, linguistic and personal needs.
- Contributing to reports for Education, Health and Care Plans and attending relevant meetings when required.
- Ensuring safe practice, including incident reporting, mandatory training and safeguarding responsibilities.
- Supporting the wider team by working flexibly across the area when needed.

- Qualified Occupational Therapist with HCPC registration.
