Job summary
Health Care Assistant: Permanent - Part time - (2 positions available - 18 hours and 27 hours)
The post holder will work as a member of the Overnight Nursing Service providing direct and indirect patient care as directed by a trained member of the team in order to support patients and their families within their own home environment. The post holder will undertake clerical duties as directed including electronic documentation and working as an effective member of the Multidisciplinary team.
Due to the lone working policy the post holder will never work alone and will always be with a band 6 specialist nurse.
Main duties of the job
- To provide basic nursing, therapy and social care to people and carers, carrying out delegated clinical, therapeutic and personal care tasks
- To promote the physical, mental and emotional health and wellbeing to individuals and client groups.
- To be an effective member of the multi-disciplinary team.
- Communicate effectively with patients and carers, recognising their needs for alternative methods of communication.
- To carry out care/other duties under direct supervision of a registered professional.
- To support the band 6 nurse with tasks in patients homes.

A newly formed Humber & North Yorkshire Procurement Collaborative, hosted by Hull University Teaching Hospitals NHS Trust, will provide procurement and supply chain services to its Partner Trusts, and may extend over time to service other bodies within the Integrated Care System.

We are looking for a proactive and highly organised Procurement Business Manager to support the senior leadership team within the Humber & North Yorkshire Procurement Collaborative (HNYPC). Main duties of the job
This is a varied role at the heart of procurement governance, assurance and business management. Working across multiple NHS organisations, you will help coordinate governance and assurance activity, support the development and management of policies and procedures, monitor compliance requirements, and work closely with colleagues on contractual, supplier and procurement-related matters.
We are particularly interested in candidates who bring strong analytical, legal, commercial or governance awareness. You may come from a legal, paralegal, contract management, procurement, compliance, audit or governance background. A legal qualification is not essential, but you should be comfortable interpreting guidance, legislation, policies and contractual documentation, identifying risks and issues, asking the right questions, and supporting proportionate and well-evidenced decision-making.
You will work guided by precedent and clearly defined procedures. Works without supervision however can seek advice from Line Manager when required.

An exciting opportunity has arisen for a motivated and adaptable Band 2 Healthcare Therapy Assistant to join the team at the Stamford Unit. The Stamford Unit Intermediate Centre comprises of 96 beds which are located on the Tameside Hospital site in Ashton-under-Lyne.
You will work unsupervised or distantly supervised by experienced Nurses and Therapists to provide care and support with activities of daily living whilst incorporating rehabilitation to optimise the independence of the patients.
The unit offers a multidisciplinary approach to the rehabilitation of patients who have a wide range of medical conditions and different levels of need. The focus is to offer excellent care and rehabilitationin preparation for returning home safely.If you are committed to excellent patient centered care in a busy environment and are enthusiastic about working as part of a multidisciplinary team building new skills and competencies then this could be the role for you
Main duties of the job
To aid the care and rehabilitation of patients by undertaking assigned tasks according to set protocols and patient centred care and therapy plans as identified by nursing and/or therapy staff.
To work with direct and indirect supervision, reporting back patient progress and informing of any problems.
The successful candidate will be required to work flexibly as thepost includes shift patterns and 7 day working including bank holidays.

The Tameside & Glossop District Nursing Service are currently looking to recruit Band 5 Community Staff Nurses.Our service works across the 5 neighbourhoods of Tameside. Main duties of the job

Clinical Fellow - Acute Frailty Hub: Fixed term (12 months)
We are seeking a motivated Clinical Fellow to join our Frailty team, supporting the delivery of high-quality, patient-centred care for older adults.
The post holder will work closely with a team of Specialty Doctors and Consultants within a skilled multidisciplinary team. You will play a key role in the assessment, diagnosis, and management of frail and older patients presenting to the Emergency Department and subsequently admitted to the Frailty Same Day Emergency Care (SDEC) service and Frailty Decisions Unit.
This role offers an excellent opportunity to develop expertise in frailty medicine, with a strong focus on comprehensive geriatric assessment, early decision-making, and multidisciplinary working.
The primary aim of the service is to provide timely, safe, and effective care that supports patients to return to their usual place of residence as soon as possible, avoiding unnecessary hospital admission wherever appropriate.
Main duties of the job
Participate in Consultant-led ward rounds within the Frailty Decisions Unit.
Identify patients within the Emergency Department who are suitable for assessment and management within the Frailty Same Day Emergency Care (SDEC) service.
Undertake Comprehensive Geriatric Assessments (CGA) as part of a multidisciplinary team, under the supervision of the Consultant.
Deliver high-quality medical care in line with agreed clinical management plans.
Facilitate safe and timely discharge of patients, including completion of accurate and prompt clinical documentation and communication with General Practitioners to ensure continuity of care.
Ensure discharge plans are effectively coordinated with community health and social care services to support ongoing patient needs.
Support the training and supervision of Foundation Year (F1/F2) doctors under the guidance of Specialty Doctors and Consultants, and participate in teaching activities including Grand Rounds.
Participate in clinical audit, quality improvement, and governance activities as agreed with the Educational Supervisor.
Maintain compliance with data protection requirements and information governance standards.
Adhere to all relevant Trust policies, procedures, and clinical guidelines.
Undertake additional duties in response to clinical emergencies or unforeseen circumstances as required by Consultants or Specialty Doctors. Any such duties will be temporary and not of a sustained or regular nature.

We are recruiting ACPs on a substantive basis to join our established and developing Primary Care service within the Rotherham Urgent and Emergency Care Centre (UECC). This is an exciting opportunity to work as part of a well-supported MDT.
ACPs within our service work across an extended-hours rota, including evenings and weekends, with shifts running up to midnight.
This is a varied and rewarding clinical role. ACPs contribute across key areas of the service including Urgent Primary Care, GP Out of Hours and front-door streaming within the Emergency Department/UECC.
The role requires confident clinical assessment, decision-making and risk management, alongside appropriate escalation to GP or Emergency Department colleagues where required. You will work as an autonomous practitioner making appropriate referrals, initiating treatment and facilitating safe discharge, supported by critical thinking and an evidence-based approach.
We are looking for ACPs who are confident working with undifferentiated patients, demonstrate sound clinical judgement and are able to work effectively within a MDT. Applicants should hold an MSc in Advanced Clinical Practice, or equivalent and registered with either the NMC or HCPC.

To work as part of the practice nursing and wider multidisciplinary team to provide safe, effective and patient-centred nursing care to the practice population.

We are looking for an enthusiastic, self-motivated, and caring individual to join our team, working between Thorpe Health Centre and Norwich Community Hospital as our Admin Support - Receptionist.
This is a part-time, Band 2, substantive post (pro-rata). This role will initially work 27.5 hours per week, 08:30-14:00 Mon-Fri, increasing to 30 hours per week, 08:30 - 14:30 from Monday - Friday. The base for this post is Thorpe Health Centre, however you will be required to travel to Norwich Community Hospital one day per week. You will need to be versatile and have access to a vehicle or be able to make alternative travel arrangements.
Main duties of the job
The postholder will contribute to the smooth running of the reception at Thorpe Health Centre and Norwich Community Hospital, providing daily support for direction.
The role will include administrative support duties such as answering telephone calls, greeting patients upon their arrival, assisting patients with queries and use of NHS computer systems, including SystmOne, to check in patients and additional admin support as required. This role also involves duties to maintain the stock levels for our Community Nursing Team based at Thorpe Health Centre, which includes counting and ordering the stock on a weekly basis. This role is required to maintain strict confidentiality and will be expected to communicate with patients and relatives with courtesy and discretion at all times.
Being able to demonstrate excellent organisational and effective communication skills is essential, along with being able to work collaboratively as part of a team, using your own initiative, and working to deadlines. You will have experience of undertaking a range of administrative duties and have excellent customer care skills. You will need to be able to evidence a good solid background working in a busy office environment and will be required to work proactively. Experience of working in the NHS is preferential. Knowledge of SystmOne would be advantageous, but training will be provided for the successful candidate.

Job summary
We are recruiting passionate and proactive Neighbourhood Case Coordinators to join the Hertsmere Neighbourhood Proactive Care Service. This is an exciting opportunity to play a key role in a new team delivering community-based, person-centred care for residents. The focus will be on holistic assessments, care coordination, addressing health inequalities and delivering on the strategic ambition of a 'neighbourhood health service'. This role has both an administrative and patient facing element.Working as part of a multidisciplinary team supporting a cohort of residents with moderate to severe frailty who have had 2 or more unplanned admissions within the last year, linking them to health, social care and voluntary sector services.
This service will see the shift from reactive care to proactive identification using risk stratification searches within Primary Care records. Searches will identify a cohort of patients who would benefit from targeted intervention to improve outcomes, overall experience and reduce the likelihood of further unplanned admissions. Operating alongside a number of acute and community services, with the ability to step patients up into appropriate services - including the highly successful virtual ward models in South and West Hertfordshire.
As this is a community-based role, there is a requirement to hold a full, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job
This Neighbourhood Case Coordinator role is a hybrid clinical/administrative role which would suit a person with experience working as a health care or therapy assistant in community settings who also has good administration skills to support with producing clinic letters, MDT meeting minutes, appointment bookings, etc.
Clinically you will be visiting patients at home or seeing them in clinic based settings to carry out assessment or monitoring of their long term conditions, providing advice and support to enable them to live well and avoid unnecessary hospital admissions, and liaising with health and social care professionals to ensure that care is well coordinated.
Within this role, the post holder will be responsible for:
o Working under delegated authority from a registered nurse or allied health professional (Neighbourhood Case Manager) to support patients on the Proactive Anticipatory Care Service, with the aim of reducing unnecessary admission to hospital by the completion of preventative comprehensive care plans and care coordination across a range of services.o This support may take the form of virtual consultation and review, face to face assessment or care delivery, professional liaison, and arranging and attending meetings with colleagues from across health and social care.o Work may be delivered in patient's own homes, in clinic, or other community settings.

Job summary
We are excited to advertise a part-time Band 8a maternity cover in the ELFT Community Eating Disorder Service (https://www.elft.nhs.uk/services/east-london-eating-disorder-service-adults). We are seeking a Senior Psychologist or CBT Therapist to join our expanding team of psychologists, therapists, clinical associates in psychology, and dietitians.
The post will support people meeting diagnostic criteria for an eating disorder whose relationship with food, eating and body image significantly affects their health and wellbeing. The role will focus on those with moderate impact on physical health.
The successful candidate will take a senior role in daily service operations: screening referrals, completing assessments, developing treatment plans, holding a caseload, and supporting the team. They will contribute to the development and management of a FREED-informed model of care across East London, working into IAPT services, supporting primary care networks, and liaising with St Ann's Specialist Eating Disorders Service.
This is a leadership role within a vibrant, progressive team. Applicants should have experience supporting people with eating difficulties, but we are equally interested in your unique skills and qualities. Main duties of the job
Financial Reporting
Produce accurate, timely and appropriate monthly financial and performance management information for the Group(s), including the monthly management accounts within the timescales determined by the Trust. This will involve the collection, collation and processing of complex data from a variety of sources into appropriate information to allow input into the computerised ledger.
Calculate accruals, prepayments and adjustments to be reflected in the monthly management accounts and maintain a clear audit trail by means of adherence to trust financial procedures and International Financial Reporting Standards (IFRS).
Quality check month end reports for accuracy.
Produce year- end financial projections to assist in the management of budgets.

Job responsibilities
- Undertake comprehensive pre-admission assessments for babies, children and young people with life-limiting, life-threatening or complex health conditions.
- Gather and review relevant clinical information, including diagnoses, current treatment, medication, symptom management, ventilation or respiratory support, nutrition, moving and handling, epilepsy plans, emergency care plans and advance care planning where appropriate.
- Assess whether the hospice can safely meet the child or young persons needs, escalating concerns or gaps in provision to the appropriate clinical lead.
- Work with families to understand routines, preferences, communication needs, cultural and spiritual considerations, play interests and what matters most to the child or young person.
- Prepare clear admission summaries and contribute to individualised care plans, risk assessments and handover information for hospice teams.
- Maintain awareness of Deprivation of Liberty Safeguards (DoLS), ensuring concerns are identified, escalated and recorded appropriately in line with organisational policy.
