Job summary
This is an exciting opportunity to join the Musculoskeletal Physiotherapy Outpatient Service within Somerset NHS Foundation Trust, with a permanent Band 5 role at Bridgwater Hospital. You will work within a supportive multidisciplinary team including Exercise Instructors, Physiotherapists, Osteopaths, Enhanced and Advanced Practice clinicians, alongside First Contact Practitioners.
As a Band 5 MSK Physiotherapist, you will deliver high-quality, patient-centred care to a varied outpatient caseload, including orthopaedic, musculoskeletal and sports-related conditions. The role involves assessing patients, using clinical reasoning to diagnose and develop evidence-based treatment plans, and supporting shared decision-making to achieve positive outcomes.
You will manage your own caseload with increasing autonomy while benefiting from structured supervision, peer review and senior clinical support. Development is actively encouraged through CPD, training and service improvement work.
You will also support the development of students, assistants and Exercise Instructors, and work collaboratively with GPs, consultants and wider healthcare professionals to ensure coordinated, high-quality care.
This role offers an excellent opportunity to develop MSK skills within a dynamic, supportive and forward-thinking service.
Main duties of the job
Deliver a high-quality, patient-centred musculoskeletal physiotherapy service to an outpatient caseload, including assessment, diagnosis and evidence-based treatment of a wide range of MSK conditions.
Independently manage and prioritise a clinical caseload, using advanced clinical reasoning skills to develop and implement individualised treatment plans.
Work collaboratively within a multidisciplinary team including Physiotherapists, Osteopaths, Exercise Instructors, Enhanced and Advanced Practice clinicians, and First Contact Practitioners to ensure integrated patient care.
Communicate effectively with patients, carers and healthcare professionals, supporting shared decision-making and promoting self-management.
Provide clinical support, guidance and supervision to Band 4 Exercise Instructors, students and junior staff, contributing to training and development within the team.
Maintain accurate, timely and comprehensive clinical records and produce reports in line with professional and Trust standards.
Contribute to service development, clinical audit, and quality improvement initiatives to enhance patient outcomes and service delivery.
Participate in ongoing professional development, including appraisal, training and peer review, to maintain and develop clinical competence.
Work flexibly across community hospital settings and participate in service requirements such as weekend rotas where required.

We operate extended opening hours (9am - 8:30pm weekdays) and the successful applicant will be required to participate in a shift system to cover evenings, weekends and Bank Holidays. The successful applicant will also be expected to work across all trust sites.
Main duties of the job
The main duties of the role will be accuracy checking of dispensed medicines and day to day supervision of a team in our automated dispensaries, aseptic supervision and ward based medicine optimisation duties.

The post holder will provide mentoring for other Pharmacy Technicians, PTPT's and Pharmacy assistants

The Discharge to Assess team are looking for a Registered Nurse, to fulfil an exciting opportunity within the community in Stockport NHS Foundation Trust.
We need people who share our excitement, embrace different ways of working and are able to work flexibly across organisational boundaries. An opportunity has arisen for a Registered Nurse - Band 6, to work flexibly within the Discharge To Assess/Intermediate Care at Home Team.
The Discharge To Assess element is a multi-professional service who assess patients upon discharge from hospital in the community. We provide individualised care to meet the person's needs and facilitate transfer for onward care into the appropriate service: Health, Adult Social care or partner agencies. The Intermediate Care at Home Team provide nursing and therapy rehabilitation to patients within the Stockport community.
Main duties of the job
The Discharge to Assess element: to support people who are clinically optimised and do not require an acute hospital bed, but may still require care services with short term, funded support to be discharged to their own home or another community setting.
Assessment for longer-term care and support needs is then undertaken in the most appropriate setting and at the right time for the person. We provide holistic multidisciplinary interventions and support in order to stabilise patients in their own home or usual residence. This comprises a multi-disciplinary integrated team.
The Rehabilitation Element incorporates a free, short-term rehabilitation service for adults to help prevent unnecessary hospital admission, enable early discharge from hospital and prevent unnecessary admission to long-term residential care. This role will require the post holder to be flexible to cover all areas within the Discharge to Assess service, covering 7 days 08.00-21.00.

If you are passionate about podiatry and want to provide a quality service for the high risk patients of Bolton, we are looking to welcome you into our integrated specialist podiatry team.
The successful candidate will be responsible for treatment planning and management of foot complications associated with e.g. diabetes, rheumatoid arthritis and vascular / ischaemic long-term conditions. Main duties of the job
You will contribute to the day-to-day management of imaging quality, quality control, radiation protection and administrative functions as well as covering mainly Electrophysiology and Ablation procedures, some Coronary Angiography and PCI work is undertaken as well as TAVI once suitable competence has been attained. The Cardiac Radiographers are responsible for adjunctive imaging to include IVUS and OCT in supporting PCI procedures - full training will be provided. You will act, at all times in accordance with the Society of Radiographers code of professional conduct. You will be taking ownership and fulfilling the requirements of any required statutory and mandatory training; ensuring all training courses are completed within the required timeframes. You will adhere to the local radiation rules, whilst contributing to the reviewing and reinforcing of all local rules regarding radiation protection with the broader team. An exciting opportunity has arisen for suitably qualified and experienced candidates who are registered with the NMC within our new Specialist Outreach Psychosis Service within the Borough of Ealing. Ealing is mobilising these new services bringing together the existing Community Rehabilitation teams alongside new investment to develop an assertive outreach pathway for service users with complex psychosis who need an assertive approach to engagement as part of their clinical presentation to provide care, treatment and support.
Successful candidates will show knowledge and experience of the clinical processes and treatments that support the care of people experiencing psychosis with complex needs. They will also demonstrate experience of working within a multi-disciplinary team setting; taking responsibility for assessing need and setting up robust care plans to support service user recovery. They will be able to take a flexible, robust and persistent approach to working alongside service users and their support networks to deliver care and get the greatest benefit from treatment.
Experience of working with complex mental health needs and high risk clients is essential as well as strong skills in risk assessment, care planning and crisis management. Experience of community based and outreach working is also essential.
Main duties of the job
The post holder will be working in a Specialist Outreach Psychosis Service within the Assertive Outreach Pathway, taking responsibility to assessing, planning and coordinating the care plan and treatment of a defined caseload of service users with complex needs related to psychotic illnesses who are difficult to engage.
The Specialist Outreach Psychosis Service is a multidisciplinary, community-based service responsible for providing on-going holistic assessments, treatment and support to clients with severe and enduring mental illness living in the community. The service will enable patients with severe mental health problems to be supported and cared for in the community in the least restrictive setting through an assertive, personalised and integrated approach to care and treatment.
The service will provide an intensive community-based mental health service delivered in service users' home environments, in the community and in supported living accommodation or other locations agreed with the service user utilising "A whole system approach" to recovery from mental ill health. The approach aims to maximise an individual's quality of life and address social inclusion by supporting engagement, encouraging their skills, promoting independence and autonomy in order to give them hope for the future which leads to successful community living through appropriate support.

Maintenance Technician (Multi-skilled (Mechanical & Plumbing Bias) Band 5 - 37.5 hours per week
This is an exciting opportunity to join the Estates team at Stepping Hill Hospital. We are seeking a highly motivated, appropriately skilled, reliable and enthusiastic Plumber/Mechanical Engineer that is able to work well under pressure to carry out plumbing/mechanical maintenance within a hospital environment.
The successful applicant will have a recognized apprenticeship and hold a City & Guilds Certificate or equivalent NVQ qualifications. The post holder will be required to undertake rostered overtime.
Main duties of the job
The post is at Stepping Hill Hospital but you may be asked to work at other sites within the Trust. Experience working in an Acute Hospital maintenance environment would be an advantage, as would familiarity with large commercial/industrial estates. You will be part of a multi trade team delivering a quality service to support patient services within the Trust.
There will be a requirement to work overtime; both rostered and ad-hoc; at short notice as required to meet the needs of the Trust and its healthcare environment. There will also be a need to form part of the on-call response team to assist our out of hours staff where required.
