We are looking for a motivated Advanced Nurse Practitioner or Advanced Clinical Practitioner to join our friendly and supportive team at Millwood Surgery. This is an excellent opportunity to work in a large, forward-thinking practice with a diverse patient population and strong GP support.
You will work alongside a team of six ANPs providing high-quality care to patients within our large practice population. Our diverse patient demographic offers a varied and rewarding scope of practice. The duty GP is available throughout the day for clinical support when required.

Main duties of the job
You will provide advanced assessment, diagnosis, and management of patients, working autonomously with GP support. Applicants must be a registered healthcare professional (nurse and paramedic considered) with postgraduate-level training in advanced practice and ideally experience in a general practice setting. The role involves working across the four pillars of advanced practice clinical practice, leadership, education, and research contributing to patient care, service development, and professional growth.
About the Practice
Millwood Surgery is a well-established GP practice serving approximately 19,800 registered patients. Our multidisciplinary team includes 8 salaried GPs, Advanced Nurse Practitioners, a dedicated pharmacy team, and an in-house home visiting team, supported by experienced nurses, healthcare assistants, and administrative staff. We also work closely with colleagues within our Primary Care Network (PCN), providing additional support from a wider multidisciplinary team to deliver high-quality, patient-centred care. Appointments are typically 15 minutes for ACP team, with protected administration time built into the schedule. Double appointments are available for patients presenting with more than one condition.

ANPs/ACPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and interpretation of test results. They can independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.
ANPs/ACPs are registered nurses with advanced education and clinical training including non- medical prescribing. They must meet the NMC requirements for advanced practice, which includes the 4 pillars clinical practice, leadership, education and research.
Key Responsibilities:
The following are the core responsibilities of the Advanced Nurse Practitioner/Advanced Clinical Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
- Develop, implement and embed health promotion and wellbeing programmes
- Provide high-quality and advanced care for patients of all ages with a range of conditions, including acute and chronic illnesses. They perform physical exams, take medical histories, and order diagnostic tests. They also develop and implement care plans, prescribe medications, and manage ongoing care via face to face, video or telephone consultations.
- Make clinical decisions based on diagnostic results and adjust treatment plans as needed. They must demonstrate safe clinical decision-making and expert care. And work within own scope of practice.
- Work with physicians, specialists, and other healthcare professionals to provide patient-centered care. Provide clinical triage of patients to appropriate services and healthcare professionals.
- Educate patients and their families about healthy lifestyle choices, disease prevention and self-management techniques.
- Stay up to date with the latest evidence and standards of care through ongoing education and research.
- Mentor less experienced nurses and staff.
- Support the Practices future development and share the Practices values.
- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
- Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
- Provide wound care where appropriate
- Provide specialist clinics such as contraception where appropriate
- Undertake the collection of pathological specimens
- Request pathology services as necessary
- Process and interpret pathology and other test results as required
- Maintain accurate clinical records in conjunction with extant legislation
- Ensure SNOMED CT codes are used effectively
- Be responsible for the implementation of an effective immunisation programme
- Chaperone patients where necessary
- Prioritise health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Recognise, assess and refer patients presenting with mental health needs
- Be an extended and supplementary prescriber, adhering to extant guidance
- Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
- Contribute to practice targets (QOF, etc.), complying with local and regional guidance
- Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
- Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
- Support the clinical team with all safeguarding matters in accordance with local and national policies
- Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriatelyProvide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

Secondary Key Responsibilities:
In addition to the primary responsibilities, the Advanced Nurse Practitioner/Advanced Clinical Practitioner may be requested to:
- Act as the audit lead, effectively utilising the audit cycle
- Act as the IPC lead for the practice, ensuring compliance, undertaking audit as necessary
- Monitor and ensure the safe storage, rotation and disposal of medicaments
- Participate in local initiatives to enhance service delivery and patient care
- Support and participate in shared learning within the organisation
- Continually review clinical practices, responding to national policies and initiatives where appropriate
- Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)
- Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation

This is an exciting opportunity for a Radiographer experienced in Interventional Radiology to become an integral member of our Radiology team at The Shrewsbury and Telford Hospital NHS Trust.
Our Philips Azurion interventional suite is currently based at The Royal Shrewsbury Hospital. We are looking for an enthusiastic, motivated person to work in our busy Pharmacy Production Unit. You will be working as part of a team of pharmacists, technicians and assistants aseptically preparing medicines, chemotherapy and antibiotic injections.
Our busy aseptic service compounds PN for adults (8000 pa ), and neonates , dispenses chemo drugs (50,000 pa ) and IV additives ( 12,000 pa ).
Main duties of the job
You must be reliable, hardworking and able to work to high standards of accuracy and precision. You will be expected to assist with stock control and be confident using a computer. Good communication skills, manual dexterity, and a pass (grade A-C/9-4 in GCSE or equivalent) in English, Mathematics, and a Science subject, are essential.
There will be a commitment to regular Sunday, bank holiday, early morning and late duty roster working on a rota basis.
Previous experience is preferred, although not essential as full training will be provided.

Main duties of the job

An exciting opportunity has arisen for a motivated and adaptable Band 2 Healthcare Therapy Assistant to join the team at the Stamford Unit. The Stamford Unit Intermediate Centre comprises of 96 beds which are located on the Tameside Hospital site in Ashton-under-Lyne.
You will work unsupervised or distantly supervised by experienced Nurses and Therapists to provide care and support with activities of daily living whilst incorporating rehabilitation to optimise the independence of the patients.
The unit offers a multidisciplinary approach to the rehabilitation of patients who have a wide range of medical conditions and different levels of need. The focus is to offer excellent care and rehabilitationin preparation for returning home safely.If you are committed to excellent patient centered care in a busy environment and are enthusiastic about working as part of a multidisciplinary team building new skills and competencies then this could be the role for you
Main duties of the job
To aid the care and rehabilitation of patients by undertaking assigned tasks according to set protocols and patient centred care and therapy plans as identified by nursing and/or therapy staff.
To work with direct and indirect supervision, reporting back patient progress and informing of any problems.
The successful candidate will be required to work flexibly as thepost includes shift patterns and 7 day working including bank holidays.

District Nursing Specialist Practitioner 28.5 hours

Main duties of the job
The post holder will have responsibility for management of the routine activities of the District Nursing Team, organising workload and staff rotas. Exercising leadership and judgement to ensure good clinical practises and high standards of care are maintained in order to meet service demand.
You will be working within an integrated neighbourhood and be involved with the current changes and developments within the organisation.Assess and develops students and junior staff to reach their potential by designing learning programmes and acting as a Mentor

Clinical Fellow - Acute Frailty Hub: Fixed term (12 months)
We are seeking a motivated Clinical Fellow to join our Frailty team, supporting the delivery of high-quality, patient-centred care for older adults.
The post holder will work closely with a team of Specialty Doctors and Consultants within a skilled multidisciplinary team. You will play a key role in the assessment, diagnosis, and management of frail and older patients presenting to the Emergency Department and subsequently admitted to the Frailty Same Day Emergency Care (SDEC) service and Frailty Decisions Unit.
This role offers an excellent opportunity to develop expertise in frailty medicine, with a strong focus on comprehensive geriatric assessment, early decision-making, and multidisciplinary working.
The primary aim of the service is to provide timely, safe, and effective care that supports patients to return to their usual place of residence as soon as possible, avoiding unnecessary hospital admission wherever appropriate.
Main duties of the job
Participate in Consultant-led ward rounds within the Frailty Decisions Unit.
Identify patients within the Emergency Department who are suitable for assessment and management within the Frailty Same Day Emergency Care (SDEC) service.
Undertake Comprehensive Geriatric Assessments (CGA) as part of a multidisciplinary team, under the supervision of the Consultant.
Deliver high-quality medical care in line with agreed clinical management plans.
Facilitate safe and timely discharge of patients, including completion of accurate and prompt clinical documentation and communication with General Practitioners to ensure continuity of care.
Ensure discharge plans are effectively coordinated with community health and social care services to support ongoing patient needs.
Support the training and supervision of Foundation Year (F1/F2) doctors under the guidance of Specialty Doctors and Consultants, and participate in teaching activities including Grand Rounds.
Participate in clinical audit, quality improvement, and governance activities as agreed with the Educational Supervisor.
Maintain compliance with data protection requirements and information governance standards.
Adhere to all relevant Trust policies, procedures, and clinical guidelines.
Undertake additional duties in response to clinical emergencies or unforeseen circumstances as required by Consultants or Specialty Doctors. Any such duties will be temporary and not of a sustained or regular nature.

We are recruiting ACPs on a substantive basis to join our established and developing Primary Care service within the Rotherham Urgent and Emergency Care Centre (UECC). This is an exciting opportunity to work as part of a well-supported MDT.
ACPs within our service work across an extended-hours rota, including evenings and weekends, with shifts running up to midnight.
This is a varied and rewarding clinical role. ACPs contribute across key areas of the service including Urgent Primary Care, GP Out of Hours and front-door streaming within the Emergency Department/UECC.
The role requires confident clinical assessment, decision-making and risk management, alongside appropriate escalation to GP or Emergency Department colleagues where required. You will work as an autonomous practitioner making appropriate referrals, initiating treatment and facilitating safe discharge, supported by critical thinking and an evidence-based approach.
We are looking for ACPs who are confident working with undifferentiated patients, demonstrate sound clinical judgement and are able to work effectively within a MDT. Applicants should hold an MSc in Advanced Clinical Practice, or equivalent and registered with either the NMC or HCPC.

Main duties of the job
This role involves providing high-quality clinical assessment, intervention and treatment planning for undifferentiated patients, including children presenting to the primary care stream of the UECC and or the GP Out of Hours service. You will work autonomously, making appropriate referrals or discharging patients safely while using advanced clinical judgement and an evidence-based approach.
You will be expected to work across services, delivering a wide range of consultation skills including:
- Front-door streaming
- Face-to-face consultations
- Telephone triage and consultations, including patients referred via NHS 111
- Booked clinic appointments
- Home visiting within the local area
