We are seeking a part-time Registered Nurse to join the Swadlincote Community Nursing Team on a 12-month secondment. Flexible hours and working patterns may be considered for the right candidate.

Main duties of the job
Our service deliver's evidence-based care that supports patients to remain in their own homes.
This role offers the opportunity to work closely with general practice, therapy services and home visiting colleagues.
Assess, plan, implement and evaluate individualised patient care, promoting self-care wherever possible while maintaining privacy, dignity and clinical effectiveness.
Provide guidance and support to junior staff in delivering high-quality care, while actively assisting the District Nurse/ Senior Community Nurses in managing the healthcare environment.
Due to the nature of the role, the post holder must be able to travel across a wide geographical area, often with equipment, and have access to a suitable vehicle for business purposes

MAIN DUTIES AND RESPONSIBILITIES OF THE POST:
Organisational Management
1. The post holder will manage elective Booking for Consultants and Registrars working to clearly defined policies.
2. To be responsible for managing the patient waiting lists ensuring patients are dated within their waiting time breach dates.
3. To be responsible for organising operation dates with patients in strict order of the waiting list, taking into account case mi, instrument availability, age medication, surgeon etc.
4. To be responsible for managing the rebooking of patients if there are cancelled operations ensuring the relevant Trust policies are followed when re-booking patients (for example28 day rule).
5. In addition to own workload the post holder will be expected to provide cover for colleagues in the office during annual leave, sick leave and during busy periods where there is an imbalance of work.
Communication
1. Be responsible for contacting patients using persuasive and negotiating skills in agreeing a date for admission in accordance with the Patient Choice Policy.
2. Be responsible for providing excellent communication and information for patients coming in for surgery. Responsible for generating correct letters to patients explaining details of correct feeding instructions, information regarding medication, transport and sending appropriate information leaflets.
3. Liaise with the pre-assessment booking team to ensure pre-assessment appointments for patients booked in a timely fashion, so as not to delay surgery. Acting upon the outcome of the pre-assessment if patient deemed unfit to proceed.
4. Be responsible for all correspondence with patients, all telephone and face to face enquiries from patients and relatives using tact and diplomacy; analyse and resolve patient problems.
5. The post holder must be prepared to telephone patients/consultants about contentious issues, such as operations cancelled, re-arranging lists due to beds not available or breach issues.
Management of Information
1. The post holder must be fully conversant and competent in the features of patients administration system (PAS), and ClickView which supports all aspects of the management of the waiting lists.
2. The post holder must be conversant with government NHS guidelines and Trust Policy in all aspects of waiting list management.
3. Be responsible for data entry of TCI forms onto the PAS system within 3 working days of receipt.
4. Be responsible for intended procedure codes to be correctly entered onto the PAS system.
5. Be responsible for all relevant patient information, clinical and non-clinical to be recorded correctly onto the PAS system and to seek advice and information when this is not evident.
6. Be responsible for the quality and integrity of information concerning patients kept on PAS and all other patient information systems.

Join us for a unique opportunity to develop your clinical and leadership skills across the Yorkshire and Humberside Personality Disorder Partnership (YHPDP). The service has a focus on enabling progressive and trauma-informed ways of engaging people in difficult situations, to help improve the experience and outcomes for individuals who experience the world as profoundly unsafe (often associated with the diagnostic label of a 'personality disorder') and the staff working with them. Ideally, you will have experience of working within the criminal justice system or secure mental health care.
The YHPDP team includes psychologists, psychotherapists, occupational therapists, housing workers and probation staff. They are a regional service and part of Leeds 'Personality Disorder' services. The team work collaboratively with probation staff to develop risk management plans and support service users to seek the most appropriate treatment for their needs. The aim is to enable thoughtful, compassionate and therapeutic risk management delivered in partnership with colleagues from the Probation Service, with the ultimate aim of reducing reoffending, increasing psychological well-being and developing a workforce with the additional competencies for working effectively with this group. The team draw on an eclectic range of psychological theory to best enable non-psychological practitioners to understand behaviour which may often challenge working alliances.
Main duties of the job
To provide a highly specialist psychological service across the consultation and formulation (Core OM) strand of the Yorkshire and Humberside Personality Disorder Partnership. The YHPDP Core OM service provides a consultation and formulation service across the Yorkshire and Humberside region to support the implementation of the Offender Personality Disorderstrategy. The function of the role is:
To work across the team in a manner that is formulation-based, trauma-informed, and builds the capacity of the wider team to work in a psychologically- and trauma-informed way.
To provide line-management and/or clinical supervision to members in the multidisciplinary team, including Probation Practitioners, Assistant Psychologists or Trainees.
To work autonomously within professionalguidelines and the overall framework of the team's policies and procedures.
This post is a specific role within the team to provide the Core OM consultation service within the Offender Management in Custody model; providing consultation, formulation and workforce development opportunities to Prison Offender Managers and other staff in relation to their OPD caseload specifically within HMP Millsike and HMP Doncaster.
The YHPDP office is in Leeds there is an expectation that there will be some hybrid working, so the post holder may work remotely for some of the time each week. The post requires regular travel across the region, including to HMP Millsike and HMP Doncaster.

The post holder will be responsible for the administration of referrals into the Community response team using a variety of systems such as Microsoft TEAMs and SystmOne.
The role also involves collaborating with the whole of the Community Response Team which consists of Nurses, Therapists and Social Workers, all of which are working in an integrated team to provide acute discharge and admission avoidance solutions.

This exciting role is based within the Urgent Community Response Team.

It may come as a surprise, but only 22% of our funding comes from the government. End-of-life care is an essential service, not a luxury, yet it is not funded like other care services. To keep the Hospices services running, we need raise an additional £15,000 every day.

Job summary
Applications are invited for the role of Senior Clinical Fellow Doctor. This is a newly created post due to service re-design. Clinical Fellow posts are graded at the equivalent level to Resident Doctors in Psychiatry. You will be placed at the appropriate level based on your experience.
The post is suitable for doctors with a couple of years' experience in Psychiatry who hold MRCPsych and Section 12 approval and are seeking to gain further experience in General Adult Psychiatry, or to enhance their portfolio to gain entry to the training scheme.
Clinical Fellow posts are not recognised for training by the Postgraduate Dean, however, many of our Locally Employed Doctors successfully go on to join formal Training Schemes in psychiatry. You will have access to several training/educational opportunities including dedicated teaching sessions.
The post holder will not be expected to participate in an out of hours rota, however there may be opportunities to participate subject to agreement from your line manager and service manager.
Applicants will need to be GMC registered, with a right work in the UK, with at least 2 years of experience of management of mental health conditions under supervision. Section 12 approval is essential.
The post is for an initial period of 12 months, but this could be extended with mutual agreement.

Main duties of the job
Full participation in the multi-disciplinary team providing mental health services.
Under direct supervision of the team consultant, or experienced Registered MH Nurses (RMN) in the team, to provide clinical assessments and medical reviews of patients in crisis, including signposting and referral to other mental health services in the community.
Perform risk assessments and physical health assessments when required
Administrative tasks relevant to your clinical duties including medication prescribing, prescribing of discharge medications and other essential clinical documentations and communications with patients and other clinicians.
Outpatient clinics work under supervision, following up investigations, monitoring and documenting patient progress and response to interventions.
Facilitating discharge arrangements including writing discharge summaries
Liaison with medical and surgical wards in the acute hospital
Ensure regular and timely discussion of the clinical caseload with the supervising consultant.
Ensure effective and timely documentation and communication of clinical decision making and management plans.
To contribute to audit projects undertaken in the service and to attend appropriate audit meetings.

The Adult ADHD Service Line within Oxleas NHS Foundation Trust comprises two key services: the Adult ADHD Service and the South East London (SEL) ADHD Referral Triage Service. The Adult ADHD Service provides assessment, diagnosis, and treatment for adults with ADHD across Bromley, Bexley, and Greenwich, operating from an administrative base at Queen Marys Hospital, Sidcup, with a hybrid model of face-to-face and remote assessments. The service initiates and titrates ADHD medication, offering annual reviews or additional reviews as clinically indicated, before transferring stable care to GPs. The SEL ADHD Referral Triage Service delivers a centralised referral system for ADHD assessments across six SEL boroughs, ensuring equitable access and streamlined triage. Both services work collaboratively with GPs, Mental Health Hubs, and secondary care teams to manage complex cases, reduce waiting times, and maintain high standards of care in line with NICE guidance.

Maintaining good medical practice
During the placement you may choose to follow the Psychiatry Specialty Curriculum and maintain an up-to-date learning portfolio that you will bring to meetings with the trainer when asked to do so. As a minimum, this will be at the beginning, mid-term and the end of the placement.
Participation in undergraduate and postgraduate clinical teaching.
Participation in the training of other disciplines.
Taking part in continuing medical education.
The local MRCPsych teaching programme in held on the last Wednesday of each month and is held in the Memorial Hospital, Shooters Hill, Woolwich, London SE18 3RG.
Expected contribution to clinical governance and responsibility for setting and monitoring standards.
